Abstract

L'intérêt pour la prédiction du développement ultérieur de l'enfant, à partir de la période périnatale, n'est pas récent. Le pari de la science actuelle est qu'en comprenant les mécanismes, les processus et les facteurs qui sont impliqués dans cette prédiction, nous pouvons mieux soutenir le développement de nos enfants. Au-delà des stratégies d'intervention fondée sur la bonne volonté, les travaux en cours nous permettent de croire que la prévention réalisée en période périnatale aurait, en partie, cette incidence espérée. Par l'entremise de cet ouvrage pluridisciplinaire, nous souhaitons outiller les personnes intervenantes, étudiantes et professionnelles à propos des facteurs de risque périnataux et des interventions préventives qui y sont associés. Nous espérons ainsi pouvoir mobiliser des efforts collectifs à tous les niveaux pour que nos actions envers les enfants puissent se réaliser à la hauteur de nos possibilités et de nos ambitions.

Abstract

This book provides an overview of the physiology of pregnancy and the pathophysiology of common pregnancy complications. Using this information, options are discussed for the early detection and prevention of these complications. Chapter 1 outlines the maternal adaptation to pregnancy, which enables the fetus to grow, mature and thrive without compromising maternal health. Chapter 2 describes the pregnancy course in healthy women, who develop a common pregnancy complication, such as a placental syndrome, preterm birth or gestational diabetes. These complications are often superimposed on a latent defect in the cardiovascular, renal, immunologic and/or metabolic functions. Chapters 3 and 4 discuss pregnancy in women with a chronic disease that often affects one of these functions. Finally, chapter 5 uses the information provided in the preceding chapters to discuss the options for early detection and/or prevention of common pregnancy complications.The emphasis of this book is on the pathophysiology of a complicated pregnancy rather than on its management. It explains the sequence of events that characterizes the development of common pregnancy complications. This insight is expected to assist in the decision making and counseling by midwives and obstetricians of pregnant women, who are at increased risk of these complications.

Doody Rating : 3 stars : This book has been written and edited by obstetricians with a vast clinical experience in managing high risk pregnancies including pregnancies with multiple risk factors and often presenting late with complications. The topics have been highly selected and there are 29 chapters which cover the entire spectrum of high-risk pregnancy. The chapters are well laid down starting from prenatal diagnosis through to obstetric and medical disorders in pregnancy and labor and ending with a chapter on Drugs in Obstetrics. The individual chapters provide extensive knowledge needed in that particular topic. The scan pictures, photographs of actual patients and that using models to illustrate the mechanism of labor and the technical procedures of labor and delivery are very clear and worth commending. The latest information available in the literature from various clinical guidelines, cochrane data base and meta-analysis has been incorporated into each chapter. The summary, recommendations and key points which are given at the end of each chapter are commendable and make reading and remembering the important facts easy compared to standard textbooks. The references are up-to-date and the latest references up to the time of publication are provided. Navigating through each chapter has been made easy as every chapter has the same layout. The contents are of a high standard, which is applicable for those who are training to be specialists in obstetrics and gynecology as well as practicing clinicians. This book should be with in the libraries of the hospitals and medical schools and the clinicians office.

Abstract

Maternal health, the topic of the CWP Forum on which this report is based, is a vital concern for all societies around the world. The wellbeing of pregnant women and mothers, and the care they receive before, during, and after delivery, is integral to the health of every mother and child, and to the families and communities to which they belong. Yet while significant progress has been made in improving maternal health over the last several decades, in recent years the rate of progress in a number of countries has slowed considerably or even regressed. Meanwhile, gaps in access to high quality maternal health care – not only between regions and countries, but also within countries – are growing. On January 29, 2019, CWP invited approximately 35 experts from inside and outside of Columbia University who represent a range of substantive and institutional perspectives, to deepen our understanding of why it has proven so challenging to achieve global health targets for maternal health, discuss the root causes of the problem, and identify ways in which we might substantially improve maternal health outcomes. Out of nearly 20 ideas proposed and described in this report, three will now be developed further by CWP as potential projects, based on the recommendations of experts at the Forum. Many of the remaining proposals will be pursued through other channels and as they are, CWP will work to foster their development and track their impact.

Abstract

Introduction Neonatal mortality associated with preterm birth can be reduced with antenatal corticosteroids (ACS), yet 36.0 weeks GA. In phase 1, UH measurements were collected prospectively in the Democratic Republic of Congo, India and Pakistan, using distinct tapes to address variation across regions and ethnicities. In phase 2, we tested accuracy in 250 pregnant women with known GA from early ultrasound enrolled at prenatal clinics in Argentina, India, Pakistan and Zambia. Providers masked to the ultrasound GA measured UH. Receiver operating characteristics (ROC) analysis was conducted. Results 1,029 pregnant women were enrolled. In all countries the tapes were most effective identifying pregnancies between 20.0–35.6 weeks, compared to the other GAs. The ROC areas under the curves and 95% confidence intervals were: Argentina 0.69 (0.63, 0.74); Zambia 0.72 (0.66, 0.78), India 0.84 (0.80, 0.89), and Pakistan 0.83 (0.78, 0.87). The sensitivity and specificity (and 95% confidence intervals) for identifying pregnancies between 20.0–35.6 weeks, respectively, were: Argentina 87% (82%–92%) and 51% (42%–61%); Zambia 91% (86%–95%) and 50% (40%–60%); India 78% (71%–85%) and 89% (83%–94%); Pakistan 63% (55%–70%) and 94% (89%–99%). Conclusions We observed moderate-good accuracy identifying pregnancies ≤35.6 weeks gestation, with potential usefulness at the community level in low-middle income countries to facilitate the preterm identification and interventions to reduce preterm neonatal mortality. Further research is needed to validate these findings on a population basis.

34. Maternal Early Pregnancy Serum Metabolomics Profile and Abnormal Vaginal Bleeding as Predictors of Placental Abruption: A Prospective Study

Abstract

Background & Objective Placental abruption, an ischemic placental disorder, complicates about 1 in 100 pregnancies, and is an important cause of maternal and perinatal morbidity and mortality worldwide. Metabolomics holds promise for improving the phenotyping, prediction and understanding of pathophysiologic mechanisms of complex clinical disorders including abruption. We sought to evaluate maternal early pregnancy pre-diagnostic serum metabolic profiles and abnormal vaginal bleeding as predictors of abruption later in pregnancy. Methods Maternal serum was collected in early pregnancy (mean 16 weeks, range 15 to 22 weeks) from 51 abruption cases and 51 controls. Quantitative targeted metabolic profiles of serum were acquired using electrospray ionization liquid chromatography-mass spectrometry (ESI-LC-MS/MS) and the Absolute IDQ® p180 kit. Maternal sociodemographic characteristics and reproductive history were abstracted from medical records. Stepwise logistic regression models were developed to evaluate the extent to which metabolites aid in the prediction of abruption. We evaluated the predictive performance of the set of selected metabolites using a receiver operating characteristics (ROC) curve analysis and area under the curve (AUC). Results Early pregnancy vaginal bleeding, dodecanoylcarnitine/dodecenoylcarnitine (C12 / C12:1), and phosphatidylcholine acyl-alkyl C 38:1 (PC ae C38:1) strongly predict abruption risk. The AUC for these metabolites alone was 0.68, for early pregnancy vaginal bleeding alone was 0.65, and combined the AUC improved to 0.75 with the addition of quantitative metabolite data (P = 0.003). Conclusion Metabolomic profiles of early pregnancy maternal serum samples in addition to the clinical symptom, vaginal bleeding, may serve as important markers for the prediction of abruption. Larger studies are necessary to corroborate and validate these findings in other cohorts.
