Abstract

Although the adult and pediatric respiratory divisions at the University of Sherbrooke represent relatively recent additions to the fields of respiratory medicine and critical care, both divisions have made numerous significant contributions in clinical medicine, undergraduate teaching, graduate training and research at the local, national and international levels. The current article aims to summarize the history surrounding the many dedicated individuals and the milestones marking the implementation and expansion of state-of-the-art clinical, training and research programs in respiratory medicine at the Université de Sherbrooke. Abstract

Gastroesophageal refluxes (GERs) are universal in newborns and may induce deleterious consequences, especially in preemies. We have previously shown that nasal continuous positive airway pressure (nCPAP) inhibits GERs in full-term lambs, complementing similar results in adult humans. The effect of high-flow nasal cannula (HFNC) on GERs, however, remains unknown. This study aimed to assess the effects of nCPAP and HFNC on GERs in our preterm lamb model. Eleven preterm lambs born 14 days before full-term (∼34 wk of human gestation) were studied. Esophageal pH-impedancemetry and polysomnography recordings were performed simultaneously for 6 h under three randomly ordered conditions: nCPAP 6 cmH2O, HFNC 7 L/min, and no respiratory support (control). The indexes (/h) of GERs and air-containing swallows were analyzed during each condition. The states of alertness and cardiorespiratory events were also analyzed during the identified GERs. nCPAP significantly decreased the GER index compared with control and HFNC during both quiet wakefulness and nonrapid eye movement sleep, whereas HFNC did not alter the GER index. In addition, nCPAP significantly increased the air-containing swallow index compared with control and HFNC. No significant differences were observed between the tested conditions for GER-related cardiorespiratory events, which were nevertheless rarely observed. Similar to full-term lambs, nCPAP strongly inhibited GERs in the preterm lamb, despite an increase in air-containing swallows. In contrast, HFNC did not impact GERs. Our results are clinically relevant when discussing the use of CPAP in preemies with GERs. NEW & NOTEWORTHY: Nasal CPAP inhibited gastroesophageal refluxes in preterm lambs, despite increasing air-containing swallows. High-flow nasal cannula did not impact GERs. Abstract

Introduction: Preterm infants have immature vital lower airway protective reflexes. Therefore, oral feeding or gastroesophageal refluxes (GER) can be associated, in certain infants, with serious complications such as laryngeal penetrations, pulmonary aspirations and acute cardiorespiratory events (bradycardias, apneas, oxygen desaturations). Nasal respiratory support (NRS), such as nasal continuous positive airway pressure (nCPAP) and high-flow nasal cannula (HFNC), is routinely used in preterm infants. However, the impact of NRS on these protective mechanisms of the lower airways remains largely unknown in this population. Project 1: Some teams fear that NRS might disrupt sucking–swallowing–breathing coordination and could induce severe cardiorespiratory events. Standardized bottle-feeding was assessed under video-fluoroscopy with and without induced tachypnea using our unique preterm lamb model. The article published in Front. Physiol. (DOI: 10.3389/fphys.2021.785086) shows that NRS do not impact the safety or efficiency of bottle-feeding or sucking–swallowing–breathing coordination in preterm lambs, even in the presence of tachypnea. Project 2: Significant cardiorespiratory events can be triggered in preterm infants during laryngeal chemoreflexes (LCRs) and esophageal reflexes. We previously showed that nCPAP blunted the cardiorespiratory inhibition induced by LCRs. The article published in Pediatr. Res. (DOI: 10.1038/s41390-023-02883-w) shows that nCPAP 6 cmH2O decreases the cardiorespiratory inhibition induced with LCRs, but not with esophageal reflexes in preterm lambs. This blunting effect is less marked with HFNC 7 L/min. Project 3: Nasal CPAP was previously shown to strongly inhibit GER in full-term lambs as well as in adult humans. 14. Activité des muscles abdominaux sous ventilation mécanique nasale chez des agneaux prématurés sans détresse respiratoire

Abstract

La ventilation mécanique nasale, très utilisée chez le prématuré, doit être synchronisée avec l’activité des muscles respiratoires pour assurer une ventilation pulmonaire optimale. Une mauvaise synchronisation patient-ventilateur, ou la présence d’efforts respiratoires importants, sont considérées comme néfastes. L’impact de la contraction des muscles expiratoires lors de ventilation mécanique nasale reste inconnu. Nous faisons l’hypothèse que les muscles abdominaux peuvent être responsables d’une asynchronie patient-ventilateur et qu’ils seraient plus souvent actifs en aide inspiratoire nasale qu’en Neuro-Asservissement de la Ventilation Assistée nasale. L’objectif de notre étude est de décrire et comparer l’activité des ABD lors de AIn et de NAVAn chez les agneaux prématurés sans détresse respiratoire. L’activité électrique des ABD en AIn et NAVAn à 15/4 cmH2O a été enregistrée chez 7 agneaux prématurés (terme 133/147 jours) en prenant en compte les stades de conscience. Nous avons analysé l’activité électrique des ABD lors de : 1) la ventilation de base; 2) de mouvements corporels; 3) d’évènements d’autodéclenchement et de cyclage retardé; 4) d’une augmentation de la pression de fin d’expiration; 5) en hypercapnie imposée – à des niveaux correspondant à une hypercapnie permissive, possible seulement en Ain. Une activité phasique expiratoire des muscles abdominaux a été observée en ventilation de base dans 1 (1, 5) % [médiane (Q1, Q3)] en AIn et 2 (1, 6) % en NAVAn des cycles respiratoires, lorsque tous les stades de conscience sont confondus (p = 0.9). Aucun effet des stades de conscience n’a été observé (p ≥ 0.6). Nos analyses ont montré que les mouvements corporels entrainent une perturbation de la ventilation mécanique de 6 (5, 12) s en AIn et 11 (9, 14) s en NAVAn (p = 0.4). L’activité des abdominaux a été observée seulement 8/123 fois lors d’autodéclenchements et 14/266 fois lors de cyclages retardés. Aucune augmentation significative n’a été observée lors d, Nasal mechanical ventilation, widely used in premature infants, must be synchronised with the respiratory muscle activity to ensure optimal pulmonary ventilation. Poor patientventilator synchronisation, and/or the presence of increased respiratory efforts, may be harmful. The presence of expiratory muscle activity during nasal mechanical ventilation remains largely unknown. We hypothesized that the abdominal (ABD) muscles are responsible for patient-ventilator asynchrony and more often active in nasal Pressure Support Ventilation (nPSV) than in nasal Neurally Adjusted Ventilatory Assist (nNAVA). The goal of this study is to describe and compare abdominal muscle activity during nPSV and nNAVA in preterm lambs without respiratory distress. Seven preterm lambs born at 133 days (normal term = 147 days) were studied during nPSV and nNAVA 15/4 cmH2O across the sleep states. Electrical activity of the abdominal muscles (EAabd) was recorded during: 1) baseline ventilation; 2) body movements; 3) imposed epochs of autotriggering and delayed cycling; 4) increased end expiratory pressure; 5) imposed hypercapnia—at levels corresponding to permissive hypercapnia, possible in nPSV only. Phasic expiratory EAabd was observed during baseline ventilation in 1 (1, 5) % [median (Q1, Q3)] of respiratory cycles during nPSV and 2 (1, 6) % during nNAVA, all sleep states taken together (p = 0.9). No effect of sleep states was noted (p ≥ 0.6). Body movements lead to a disruption of mechanical insufflations for 6 (5, 12) s in nPSV vs. 11 (9, 14) s in nNAVA (p = 0.4). Phasic expiratory EAabd was observed only 8/123 times during autotriggering and 14/266 times during delayed cycling events. No significant increase was observed when increasing PEEP to 6 cmH2O (p = 0.8). Finally, the presence of phasic expiratory EMGabd significantly increased in hypercapnia 41 (25, 53) % vs. normocapnia 0 (0, 14) % (p = 0.03). 22. Ovine model of congenital chest wall and spine deformity: From birth to 3 months follow‐up.

Abstract

Background: The evolution and treatment of lung alterations related to congenital spine and chest wall deformities (CWD) are poorly understood. Most animal models of CWD created postnatally were not evaluated for respiratory function. The goal of our study was to evaluate the effects of a CWD induced in utero on lung growth and function in an ovine model. Methods: A CWD was induced in utero at 70–75 days of gestation in 14 ovine fetuses by resection of the 7th and 8th left ribs. Each non‐operated twin fetus was taken as control. Respiratory mechanics was studied postnatally in the first week and at 1, 2, and 3 months. Post‐mortem respiratory mechanics and lung histomorphometry were also assessed at 3 months. Results: Eight out of 14 CWD lambs (57%) and 14 control lambs survived the postnatal period. One severe and five mild deformities were induced. At birth, inspiratory capacity (25 vs. 32 mL/kg in controls), and dynamic (1.4 vs. 1.8 mL/cmH2O/kg), and static (2.0 vs. 2.5 mL/cmH2O/kg) respiratory system compliances were decreased in CWD lambs. Apart from a slight decrease in inspiratory capacity at 1 month of life, no other differences were observed in respiratory mechanics measured in vivo thereafter. Postmortem measurements found a significant decrease in lung compliance—for each lung and for both lungs taken together—in CWD lambs. No differences in lung histology were detected at 3 months in CWD animals compared to controls. Conclusions: Our study is the first to assess the effects of a prenatally induced CWD on lung development and function from birth to 3 months in an ovine model. Our results show no significant differences in lung histomorphometry at 3 months in CWD lambs compared to controls. Resolution at 1 month of the alterations in respiratory mechanics present at birth may be related to the challenge in inducing severe deformities. [ABSTRACT FROM AUTHOR]
