Accurate and timely detection, quantification, and attribution of methane emissions from Underground Gas Storage (UGS) facilities is essential for improving confidence in greenhouse gas inventories, enabling emission mitigation by facility operators, and supporting efforts to assess facility integrity and safety. We conducted multiple airborne surveys of the 12 active UGS facilities in California between January 2016 and November 2017 using advanced remote sensing and in situ observations of near-surface atmospheric methane (CH4). These measurements where combined with wind data to derive spatially and temporally resolved methane emission estimates for California UGS facilities and key components with spatial resolutions as small as 1-3 m and revisit intervals ranging from minutes to months. The study spanned normal operations, malfunctions, and maintenance activity from multiple facilities including the active phase of the Aliso Canyon blowout incident in 2016 and subsequent return to injection operations in summer 2017. We estimate that the net annual methane emissions from the UGS sector in California averaged between 11.0 3.8 GgCH4 yr-1 (remote sensing) and 12.3 3.8 GgCH4 yr-1 (in situ). Net annual methane emissions for the 7 facilities that reported emissions in 2016 were estimated between 9.0 3.2 GgCH4 yr-1 (remote sensing) and 9.5 3.2 GgCH4 yr-1 (in situ), in both cases around 5 times higher than reported. The majority of methane emissions from UGS facilities in this study are likely dominated by anomalous activity: higher than expected compressor loss and leaking bypass isolation valves. Significant variability was observed at different time-scales: daily compressor duty-cycles and infrequent but large emissions from compressor station blow-downs. This observed variability made comparison of remote sensing and in situ observations challenging given measurements were derived largely at different times, however, improved agreement occurred when comparing simultaneous measurements. Temporal variability in emissions remains one of the most challenging aspects of UGS emissions quantification, underscoring the need for more systematic and persistent methane monitoring.

In recent years, the tobacco- and/or nicotine-containing product (TNP) portfolio has expanded to include products such as e-vapor products, heated tobacco products, and nicotine pouches. Although scientific papers and regulatory requirements/recommendations related to the assessment of product use behavior and exposure to product use have been published for these novel TNPs, there is great diversity in the terminology used to characterize and assess these types of products. The aim of this paper is to define the terms and methods used for assessing product use behavior and exposure, with the objective to suggest a uniform application of terms used by scientists working in this field of research. This publication is the work product of a cross-industry work item commissioned by the Cooperation Centre for Scientific Research Relative to Tobacco (CORESTA) Product Use Behaviour and Biomarkers Subgroups.

Calvarial haemangiomas are benign, vascular tumours of the skull involving parietal and frontal bones. Mostly these lesions remain asymptomatic, and present with cosmetic deformity, headache, uncommon neurological symptoms and reported as case reports and case series. The radiological appearance can range from sessile growing intradiploically to globular and the lesions may extend outwards or inwards after eroding the outer and inner tables of the skull. “Sunburst appearance” and “Wagon-wheel sign” are classical radiological findings but the lesions may present simply as a lytic expansile or even sclerotic calvarial mass. Because of varied clinical presentation and atypical radiological characteristics, the final diagnosis can be clinched by histology only. In selected cases where these lesions are not cosmetically acceptable, en bloc resection with tumour free margins followed by cranioplasty is the treatment of choice. Most reports of calvarial haemangiomas in literature are in the form of case reports.

Breast cancer is a public health challenge globally as well as in India. Improving outcome and cure requires appropriate biomarker testing to assign risk and plan treatment. Because it is documented that significant ethnic and geographical variations in biological and genetic features exist worldwide, such biomarkers need to be validated and approved by authorities in the region where these are intended to be used. The use of western guidelines, appropriate for the Caucasian population, can lead to inappropriate overtreatment or undertreatment in Asia and India. A virtual meeting of domain experts discussed the published literature, real-world practical experience, and results of opinion poll involving 185 oncologists treating breast cancer across 58 cities of India. They arrived at a practical consensus recommendation statement to guide community oncologists in the management of hormone positive (HR-positive) Her2-negative early breast cancer (EBC). India has a majority (about 50%) of breast cancer patients who are diagnosed in the premenopausal stage (less than 50 years of age). The only currently available predictive test for HR-positive Her2-negative EBC that has been validated in Indian patients is CanAssist Breast. If this test gives a score indicative of low risk (< 15.5), adjuvant chemotherapy will not increase the chance of metastasis-free survival and should not be given. This is applicable even during the ongoing COVID-19 pandemic.

The present work focuses on investigating the effect of process variables (power, travel speed, powder flow rate) on microstructure and mechanical properties of Laser Metal Deposited (LMD) γ-TiAl thin walls. To this end, LMD technique was used to deposit γ-TiAl thin walls at different processing conditions. Microstructures of as-deposited samples were investigated using both optical and scanning electron microscopy. X-ray diffraction (XRD) technique was used to determine the phases present. Microhardness measurements were carried out along both longitudinal and build directions. Microstructural analysis of as-deposited samples revealed a fine lamellar structure comprising of γ and α2 phases. Colony size of 30–60 μm and lamellar spacing between 0.1 and 0.7 μm were observed. XRD analysis confirmed the presence of γ and α2 phases. Comparison of elemental analysis results on both powder and as-deposited samples revealed a negligible loss of Al and no oxygen pick up in the deposited thin walls. Hardness values were found to decrease with an increase in wall height, and hardness values increased marginally (5%) with an increase in travel speed. Further, 3D transient thermal analysis was also carried out to complement the LMD of thin walls in terms of melt pool dimensions and cooling rates. It was found that the melt pool depth (MPDc = 0.266 mm) is smaller at the centre than the edge (MPDe = 0.513 mm) of the wall. Cooling rates decrease with increase in the number of layers deposited.

Polycythemia vera is a myeloproliferative disorder caused by clonal expansion of erythroid precursors in the bone marrow commonly due to a mutation in the Janus kinase 2 (JAK2) gene located in the short arm of chromosome 9. Hyperviscosity of blood due to high hematocrit causes a low flow state that may predispose to infarct. These commonly occur in the supratentorial compartment. The case of a 46-year-old man who had an isolated cerebellar infarct with high hematocrit and hemoglobin levels and low serum erythropoietin levels is described. Further investigations eventually led to the unmasking of a JAK2 mutation-negative polycythemia vera.

Context: Dental enamel is the hardest and highly mineralized structure in human body. However, Developmental Enamel Defects (DEDs) may occur due to an interplay between multiple factors ranging from genetic inadequacy to environmental insults. Primary enamel defects provoke the local or systemic insults that the child might undergo pre-, peri- and post-natally. Several gene mutations and environmental factors, including systemic illnesses have already been identified that can permanently imprint enamel damage. The DED may appear as enamel hypoplasia or hypomineralization. Clinically, DED often presents problems of aesthetics and stained defects, tooth sensitivity, susceptibility to dental caries, erosion and tooth wear. Evidence Acquisition: An electronic search was conducted on PubMed, Cochrane, ScienceDirect and Clinical Key databases with the focus on articles published since 2000. The following keywords were applied: “Developmental Enamel Defect (DED)”, “Enamel hypoplasia”, and “Primary teeth”. Results: Managing the enamel defects involves early diagnosis and aesthetic rehabilitation of defective enamel, while maintaining its form and function. This should involve close cooperation between the paediatricians and the paediatric dentists, so that preventive regimens can be institutionalised at the earliest. Conclusions: Despite our understanding of DED, further research is required to establish accurate clinical diagnosis and successful treatment of such enamel defects.

Extralevator abdominoperineal excision (ELAPE) of the rectum offers wider circumferential margin and decreased rate of intraoperative tumour perforation. However, the need to change the position of the patient in between abdominal and perineal stages of the procedure and extended perineal resection result in increased morbidity and operative time. Evolving technique of laparoscopic transabdominal controlled division of levator ani muscles under direct vision could address these issues while providing all benefits of ELAPE for patients with low rectal cancers.

Hypoperfusion complex is an uncommon entity found on computed tomography (CT) of blunt trauma. It is more common in children compared to adults. Everyone should be aware of this entity to interact with clinicians to aid in triage and management of patients in view of poor prognosis. It is also important to prevent unnecessary laparotomies by confusing abdominal organ injuries. There are certain visceral and vascular findings described on abdominal CT that would alert the radiologist for this entity. This pictorial review should increase radiologists' awareness and recognition of the CT manifestations of hypoperfusion complex before an irreversible state of shock occurs in blunt abdominal trauma. Pharmacologically motivated natural product investigations have yielded a large variety of structurally unique lead compounds with interesting biomedical properties, but the natural roles of these molecules often remain unknown. In the present investigation, a series of benzyl substituted-flavone derivatives have been synthesized from the lead compounds and were screened against 1,1-diphenyl-2-picrylhydrazyl (DPPH) free radical scavenging and α-glucosidase inhibitory properties. The resulting activity profiles of these flavone derivatives were compared for degree of similarity to the profile of 1-3. Most of the synthesized derivatives displayed potent activities when compared to the parent compounds. Maximum potencies for DPPH free radical scavenging activity were observed only in compounds containing the 4-hydroxyl substitution and 3-methoxyl group on the phenyl ring. While the 2- and 4-hydroxyl group substitutions on the phenyl ring seem to be crucial for the intestinal α-glucosidase inhibitory activity. [ABSTRACT FROM AUTHOR]

The problem of data exchange between multiple nodes with storage and communication capabilities models several current multi-user communication problems like Coded Caching, Data Shuffling, Coded Computing, etc. The goal in such problems is to design communication schemes which accomplish the desired data exchange between the nodes with the optimal (minimum) amount of communication load. In this work, we present a converse to such a general data exchange problem. The expression of the converse depends only on the number of bits to be moved between different subsets of nodes, and does not assume anything further specific about the parameters in the problem. Specific problem formulations, such as those in Coded Caching, Coded Data Shuffling, and Coded Distributed Computing, can be seen as instances of this generic data exchange problem. Applying our generic converse, we can efficiently recover known important converses in these formulations. Further, for a generic coded caching problem with heterogeneous cache sizes at the clients with or without a central server, we obtain a new general converse, which subsumes some existing results. Finally we relate a “centralized” version of our bound to the known generalized independence number bound in index coding and discuss our bound’s tightness in this context.

Background: Addition of dexmedetomidine to ropivacaine for peripheral nerve blocks has shown to improve the efficacy of ropivacaine by prolonging the duration of analgesia. This study was undertaken to evaluate the effects of ropivacaine alone and in combination with dexmedetomidine in the axillary block. Materials and Methods: A total of 80 patients belonging to American Society of Anesthesiologists physical status I, II, and III, scheduled for elective forearm and/or hand surgeries were randomly allocated into one of the two groups to receive either 39 ml of 0.375% ropivacaine and 1 ml normal saline (Group R) or 39 ml of 0.375% ropivacaine and 1 μg/kg dexmedetomidine diluted to 1 ml with normal saline (Group RD). Results: There was a significant early the onset of sensory and the motor block in Group RD. Duration of sensory block in Group RD was 677.25 ± 99.64 min and in Group R was 494.38 ± 70.64 min and the difference was clinically significant (P < 0.001). Duration of motor block in Group RD was 712.88 ± 89.32 min and in Group R was 526.25 ± 70.229 min and was clinically significant. Duration of analgesia in Group RD was 764.38 ± 110.275 min and that in Group R was 576.88 ± 76.306 min and was clinically significant. There was a significant alteration in hemodynamics in Group RD when compared to Group R without any side effects. Conclusion: Dexmedetomidine as an adjuvant to ropivacaine provides quicker onset of anesthesia, longer duration of analgesia. It offers convenient, simple, effective mode of anesthesia, and postoperative analgesia for forearm and/or hand surgeries.

Erythema Induratum of Bazin (EIB) is a tuberculid showing lobular panniculitis. EIB is classified under cutaneous tuberculosis. A diagnosis of EIB is based on cutaneous characteristics, a positive mantoux test, evidence of tuberculosis and histopathological findings. We present a case of a 43-year-old lady with recurrent crops of nodules on legs. Mantoux test was positive. Histopathology revealed lobular panniculitis, epithelioid cells, lymphocytes and giant cells surrounding foci of caseous necrosis which was suggestive of erythema induratum. Her lesions responded to antitubercular therapy (ATT).

Spinal arteriovenous fistulas are rare entities. They often present with congestive myelopathy but are infrequently diagnosed as the cause of the patients′ symptoms. Only one such case has been described previously in Indian literature. We describe one such case who presented to us after a gap of 3 years since symptom onset and following a failed laminectomy where the cause was later diagnosed to be an intradural fistula in the filum terminale fed by the anterior spinal artery and review the available literature.

Facetal cysts are usually encountered as incidental radiological findings in spinal imaging studies. Only rarely can neurological symptoms be attributed to them. These cysts are lined by vascularized synovial tissue. There are few reports in literature of hemorrhagic transformation in these cysts with sudden increase in size precipitating symptoms acutely. We report one such case where the existence of a hitherto undiagnosed cyst was unmasked by the haemorrhage. There is a need to be aware of this complication in patients with untreated or incidentally diagnosed cysts so that any sudden neurological deterioration can be dealt with promptly.

Aims : The application of a paint-on die spacer onto the dies prior to the fabrication of cast crowns is an acceptable procedure to improve the fit of the restoration. The primary purpose of this study was to evaluate the variation in thickness of different brands of die spacer based on the effect of storage time and application of additional layers. Materials and Methods: Dies were duplicated from a master model of ivorine teeth, with a full ceramic preparation. Dies obtained were then painted with one, two, and three coats of each brand of die-spacers separately. These dies were embedded in die stone and sectioned bucco-lingually. The same procedure was done after three and six months. The thickness of the paint on die spacer was measured at five points on the die using an optical microscope, and the data were statistically analyzed. Results and Conclusion : There was a definite variation in the thickness of the die spacer with all the three brands and at various points on the die. Two coat thicknesses were found to be in the range of tolerance of 20-40 microns. Thickness at occlusal groove was noted to be the maximum with least at occlusoaxial line angles. Thickness also showed a very significant increase when bottles were stored for a period of three to six months and then applied.

Lumbar puncture in the presence of an intracranial tumor with raised intracranial pressure is known to have catastrophic consequences due to herniation of intracranial contents through the tentorial hiatus or foramen magnum. There are relatively few case reports about the same sequence of events when lumbar puncture is performed below the level of a complete spinal block. The mechanism of such deterioration is also subject to conjecture as the spinal cord (unlike the uncus or cerebellar tonsils) is tethered by the dentate ligament and roots on either side, and is hence less mobile. We present one such case of spinal coning and review the available literature.

Takayasu′s arteritis (TA) is rare, chronic progressive, pan-endarteritis involving the aorta and its main branches, with a specific predilection for young Asian women. Anaesthesia for TA patients is complicated by their severe uncontrolled hypertension, extreme arterial blood pressure differentials, aortic regurgitation (AR), end-organ dysfunction, stenosis/aneurysms of major blood vessels and difficulties encountered in monitoring arterial blood pressure. We present the usefulness of ultrasound during anaesthetic management of a 35-year-old woman posted for emergency caesarean section due to intra-uterine growth retardation, foetal tachycardia in active labour, who was already diagnosed to have TA along with moderate AR and uncontrolled hypertension, using epidural technique. The use of intra-operative doppler helped resolve the initial dilemma about the diagnosis and treatment of the differential blood pressure between the affected and the normal upper limb in the absence of prior arteriogram.

The present research work is focussed to develop an intelligent system to establish the input-output relationship utilizing forward and reverse mappings of artificial neural networks. Forward mapping aims at predicting the density and secondary dendrite arm spacing (SDAS) from the known set of squeeze cast process parameters such as time delay, pressure duration, squeezes pressure, pouring temperature, and die temperature. An attempt is also made to meet the industrial requirements of developing the reverse model to predict the recommended squeeze cast parameters for the desired density and SDAS. Two different neural network based approaches have been proposed to carry out the said task, namely, back propagation neural network (BPNN) and genetic algorithm neural network (GA-NN). The batch mode of training is employed for both supervised learning networks and requires huge training data. The requirement of huge training data is generated artificially at random using regression equation derived through real experiments carried out earlier by the same authors. The performances of BPNN and GA-NN models are compared among themselves with those of regression for ten test cases. The results show that both models are capable of making better predictions and the models can be effectively used in shop floor in selection of most influential parameters for the desired outputs.

Papillary carcinoma is the most common malignant tumor of the thyroid, especially in countries with adequate or excess iodine in diet. Many studies indicate that a sizable number of papillary cancer cases occur in a setting of chronic thyroiditis. But the tumor that arises more frequently in thyroiditis is malignant lymphoma. We report a rare association of papillary carcinoma of thyroid in an elderly lady with adult T-cell lymphoma/leukemia. Fine needle aspiration of the thyroid, neck nodes and evaluation of the bone marrow and peripheral blood helped in the diagnosis of papillary cancer coexisting with adult T-cell lymphoma/leukemia.

Auraptene is being investigated for its chemopreventive effects in many models of cancer including skin, colon, prostate, and breast. Many mechanisms of action including anti-inflammatory, antiproliferative, and antiapoptotic effects are being suggested for the chemopreventive properties of auraptene. We have previously shown in the N-methylnitrosourea induced mammary carcinogenesis model that dietary auraptene (500 ppm) significantly delayed tumor latency. The delay in time to tumor corresponded with a significant reduction in cyclin D1 protein expression in the tumors. Since cyclin D1 is a major regulator of cell cycle, we further studied the effects of auraptene on cell cycle and the genes related to cell cycle in MCF-7 cells. Here we show that auraptene significantly inhibited IGF-1 stimulated S phase of cell cycle in MCF-7 cells and significantly changed the transcription of many genes involved in cell cycle.

The addition of lithiated allenyl sulfones to nonracemic sulfinimines furnished sulfinamides possessing the allenyl sulfonyl moiety with excellent diastereoselectivity. The formed products are transformed into different heterocycles, including the synthesis of pyrroline, pyran, and functionalized benzopyran.

Introduction: The physical modification of cartilage grafts during rhinoplasty risks chondrocyte death at the margins where the tissue is cut. This study compares chondrocyte viability between diced, scaled, and pate samples in human models, and further computes percent chondrocyte viability as a function of sequential dicing size in a computational model., Methods: Septal cartilage from 11 individuals was prepared as follows: diced (1 mm cubic), scaled (shaved to <1 mm thickness ~ translucent), pate (0.02 g of scraped cartilage surface), positive control (2 × 2 mm diced), and negative control (2 × 2 mm diced soaked in 70% EtOH). Viability analysis was performed using Live/Dead assay™ and confocal microscopy. Numerical simulation of cartilage dicing in 0.05 mm increments was performed using MATLAB assuming 250 chondrocytes/mm 3 with each average chondrocyte size of 65 μm 2 ., Results: Chondrocyte viability was similar between 1 mm diced cartilage, scaled cartilage, and positive control samples (p > 0.05). Conversely, pate samples had significantly less viability compared to positive controls, diced samples, and scaled samples (all p < 0.01 after Bonferroni correction). Pate samples had similar chondrocyte viability compared to negative controls (p = 0.36). On computational modeling, cartilage viability decreased to 50% as the diced sample was cut from 1 mm edge length to 0.7-0.8 mm. Similarly, cartilage viability decreased to 26% at 0.55-0.65 mm, 11% at 0.4-0.5 mm, and <5% at <0.4 mm edge length., Conclusion: Modifying septal cartilage grafts into 1 mm diced or scaled samples maintains ideal chondrocyte viability whereas pate preparations result in significant chondrocyte death.
