Introduction: Children are at greatest risk for severe illness from Respiratory Syncytial Virus (RSV). The aim of the study was to find out the knowledge of RSV, practice and knowledge about vaccination during pregnancy and the willingness to accept vaccines against RSV during pregnancy in the future among mothers needs to be understood which would add up information for stakeholder and policy makers. Methods: A preformed Performa was used for face-to-face interview was conducted among 340 pregnant women who visited the Antenatal clinic from 15-Oct-2023 to 30-Nov-2023 in their second and third trimester. Socio-demographic characteristics, knowledge and the attitude concerning antenatal vaccination affecting the acceptance of RSV vaccine were evaluated from the interview. Results: The mean age was 28.4 years, with 310 (91.18%) already having at least one child. Six (1.76%) participants had previously heard about RSV, and 325 (95.59%) were aware of the problem caused by RSV after they were briefly explained about it in their local language. A total of 246 (72.35%) of the mothers expressed willingness to be vaccinated themselves rather than vaccinating their children if such an option existed. Only 2 (0.59%) participants were familiar with nasal vaccines, and only 18 (5.29%) believed in such vaccines being effective. Despite this, almost all participants 339 (99.71%) in the study demonstrated willingness to receive additional antenatal vaccines if approved for use in future. Conclusions: The study showed a limited understanding of RSV in children among pregnant women in Nepal. However, they are aware of the impact of bronchiolitis and expressed a strong willingness to undergo maternal vaccination against RSV.

Introduction The changes in the “uterocervical angle (UCA)” have been reported to play a crucial role in “spontaneous preterm birth (sPTB)”. Purpose The study aimed “to determine whether the UCA correlates with the risk of early 28-34 weeks and late 34-37 weeks preterm birth and assess the interobserver reproducibility of this measurement and evaluate its sensitivity as a predictor of sPTB”. Methods The prospective observational study was conducted on n=50 women with singleton pregnancies of gestational age 16 to 24 weeks. In all the patients, transvaginal sonography (TVS) was performed, and UCA was noted. The ultrasound machine software was utilized for sample size determination, with a set power of 90% and α-error at 0.05 and considering findings from prior studies. To discern the variation in predictive efficacy between these two measures, a minimum sample size of 75 patients was required. Results The mean age of study subjects was 28.16±2.90 years, ranging from 23 to 34 years. Obtuse UCA >95º was seen in 47 cases, out of which 46 had PTBs. A significant difference was found between UCA when compared between the preterm and term group (102.40±4.36 vs 97.67±4.87, P0.05). Transcranial direct current stimulation (tDCS) is a non-invasive and painless technique of brain neuromodulation that applies a low-intensity galvanic current to the scalp with the aim of stimulating specific areas of the brain. Preliminary investigations have indicated the potential therapeutic efficacy of multisession tDCS applied to the auditory cortex (AC) in the treatment of chronic tinnitus. The aim of this study was to explore the therapeutic effects of repeated sessions of bilateral tDCS targeting the AC on chronic tinnitus. A double-blinded randomized placebo-controlled trial was conducted on patients (n = 48) with chronic intractable tinnitus (>2 years duration). Participants were randomly allocated to two groups: one receiving tDCS (n = 26), with the anode/cathode placed over the left/right AC, and the other receiving a placebo treatment (n = 22). A 20 min daily session of 2 mA current was administered for five consecutive days per week over two consecutive weeks, employing 35 cm2 electrodes. Tinnitus handicap inventory (THI) scores, tinnitus loudness, and tinnitus distress were measured using a visual analogue scale (VAS), and were assessed before intervention, immediately after, and at one-month follow-up. Anodal tDCS significantly reduced THI from 72.93 ± 10.11 score to 46.40 ± 15.36 after the last session and 49.68 ± 14.49 at one-month follow-up in 18 out of 25 participants (p < 0.001). The risk ratio (RR) of presenting an improvement of ≥20 points in the THI after the last session was 10.8 in patients treated with tDCS. Statistically significant reductions were observed in distress VAS and loudness VAS (p < 0.001). No statistically significant differences in the control group were observed. Variables such as age, gender, duration of tinnitus, laterality of tinnitus, baseline THI scores, and baseline distress and loudness VAS scores did not demonstrate significant correlations with treatment response. Repeated sessions of bilateral AC tDCS may potentially serve as a therapeutic modality for chronic tinnitus.

Paroxysmal nocturnal hemoglobinuria can rarely present as cerebral ischemia and stroke due to arterial thrombosis. However, it should be considered in a young patient with bone marrow failure features, systemic thromboses, and hemolysis. The variants of paroxysmal nocturnal hemoglobinuria pose a diagnostic challenge and hence are important to recognize. We report a case of a 28-years-old female with Herlyn Werner Wunderlich Syndrome who presented with an ischemic cerebrovascular accident, pancytopenia, hemoglobinuria, and widespread abdominal thromboses suggestive of paroxysmal nocturnal hemoglobinuria. The patient was managed symptomatically and referred toa hematologist.

Phosphoinositides, the seven phosphorylated derivatives of phosphatidylinositol have emerged as regulators of key sub-cellular processes such as membrane transport, cytoskeletal function and plasma membrane signaling in eukaryotic cells. All of these processes are also present in the cells that constitute the nervous system of animals and in this setting too, these are likely to tune key aspects of cell biology in relation to the unique structure and function of neurons. Phosphoinositides metabolism and function are mediated by enzymes and proteins that are conserved in evolution, and analysis of knockouts of these in animal models implicate this signaling system in neural function. Most recently, with the advent of human genome analysis, mutations in genes encoding components of the phosphoinositide signaling pathway have been implicated in human diseases although the cell biological basis of disease phenotypes in many cases remains unclear. In this review we evaluate existing evidence for the involvement of phosphoinositide signaling in human nervous system diseases and discuss ways of enhancing our understanding of the role of this pathway in the human nervous system’s function in health and disease.

Radio Frequency Identification (RFID) networks usually require many tags along with readers and computation facilities. Those networks have limitations with respect to computing power and energy consumption. Thus, for saving energy and to make the best use of the resources, networks should operate and be able to recover in an efficient way. This will also reduce the energy expenditure of RFID readers. In this work, the RFID network life span will be enlarged through an energy-efficient cluster-based protocol used together with the Dragonfly algorithm. There are two stages in the processing of the clustering system: the cluster formation from the whole structure and the election of a cluster leader. After completing those procedures, the cluster leader controls the other nodes that are not leaders. The system works with a large energy node that provides an amount of energy while transmitting aggregated data near a base station.

A novel 1-formyl-2-pyrazoline was synthesized by reaction of an α,β- unsaturated ketone with hydrazine hydrate and formic acid. The structure of the title compound was established by UV, IR, 1H NMR, 13C NMR and microanalysis.

Reaction of dibenzalacetone with hydrazine hydrate and formic acid yielded a novel 2-pyrazoline 1, characterized by ESI-MS, FT-IR, UV, 1HNMR and 13CNMR data and microanalysis.
