- Abstract
Oral rehabilitation with complete dentures is one of the most popular and traditional prosthetic treatment options for edentulous patients with systemic, anatomical and/or financial limitations, the advancement of digital technologies and the development of new dental materials, the manufacture of digital complete dentures has become a reality and has become popular for optimizing clinical and laboratory activity. Edentulism generates the loss of oral function, interfering with chewing, aesthetics, phonetics and social life, the prosthetic rehabilitation of these cases reduces systemic health problems and improves the quality of life. This clinical case report demonstrates the clinical workflow of oral rehabilitation with maxillary complete dentures using CAD software, additive manufacturing 3D printing with biocompatible resins. Advantages of CAD/CAM technology: reduction in clinical care time, costs with materials, laboratory phases, costs with prosthesis laboratory, reduction of clinical consultations, better biomechanical qualities and adaptation of prostheses, this method does not require a high investment from the dental surgeon with equipment and software. Rehabilitative treatment was performed according to the needs of the case. The installation of the upper complete denture presented retention, stability, occlusion, phonetic, and aesthetics, restoring balance to the stomatognathic system, providing quality of life. The aim was to review the literature and discuss the occlusal concepts in oral rehabilitation with complete dentures, emphasizing the advantages and needs of each occlusal scheme. A search was carried out on the databases PubMed, SciELO, Google Scholar and in textbooks, using "Dental Occlusion"; "Dental Occlusion Balanced"; "Canino guidance"; "Denture complete"; and "Denture design" as keywords and the Boolean operators AND, OR, and NOT. Among the occlusal schemes available for prosthetic planning, the bilateral balanced occlusion (BBO) and the canine guided occlusion (CGO) were the most studied and discussed in the disocclusion of complete dentures. Studies indicate the type of occlusion can interfere with muscle activity of the stomatognathic system, masticatory efficiency, bone resorption, retention and stability of removable dentures, and patient satisfaction. In the literature examined, the recommendation of the CGO as the preferred occlusal scheme in the planning of complete dentures was noticeable. The results of the studies indicate less muscle activity during mastication, better retention, and stability of the total denture. It is simpler and easier to obtain this occlusal scheme than the BBO. CGO benefits phonetics; it is aesthetically preferred by patients; and, in terms of quality of life, self-perception of masticatory and occlusal strength, has shown to be satisfactory. We conclude that CGO can be recommended as a favorable occlusal scheme for oral rehabilitation with complete dentures. [ABSTRACT FROM AUTHOR]

Objective: To conduct a cross-sectional analysis of the sociodemographic and clinical profile of edentulous bimaxillary patients and their habits of using old complete dentures (CD). Subsequently, the participants were rehabilitated with complete dentures, in three years (2017-2019), in three municipalities located in the state of Pernambuco, - Brazil, through a university extension program. Methods: We evaluated a total of 58 participants through a questionnaire with 73 questions. Data collected were used for quantitative assessment, and the level of significance was set at 0.05. Results: Most of the participants were elderly patients with an average total edentulous time of 24.3 ± 15.3 years, and 22.14 ± 14.22 years at the maxilla and mandible, respectively. In addition, there was a high prevalence of users who had bimaxillary prostheses (74%). In 57.1% of the sample, there was a predominance of brushing with toothpaste, 42.9% used soap/detergent, sodium hypochlorite solution, and (or) sodium bicarbonate as adjuvants in cleaning. Moreover, 66.6% of the patients needed a clinical intervention for CD repairs. About the harmful habits, the elderly showed a less frequent habit of sleeping with their prostheses compared to the adults (p ≤ 0.001). Conclusion: Most of the participants were female with an average age of 63 years. They presented an average of 20 years of edentulousness (maxilla and mandible) and the extended use of the dentures (> 10 years), in addition to the habit of continuous use. Abstract Introduction The most used material for the preparation of the baseplates is the acrylic resin, but it can present distortions. Objective To evaluate preparation technique, region and storage time that presents less maladaptation of the base when made with self-cured acrylic resin. Material and method Models were created in gypsum type III simulating edentulous maxilla, as divided into 3 groups (n = 10): GC (control group) thermopolymerizable acrylic resin; G1 - manual adaptation technique and G2 - drip technique. For the measurements, silicone by condensation of light consistency that was interposed between base and model was used. With a hydraulic press, 50 kg pressure was applied leading the base of the model. The obtained mold was measured in the palate, canine and molar regions with a digital caliper at the following times: immediately after the base polymerization, at 24, 48, 72, 96 hours and one week. The results were submitted to statistical analysis. Result G1 presented maladaptation of 0.43 mm ± 0.10, while G2 obtained 0.39 mm ± 0.11. The lowest maladaptation occurred in the CG. The palate region presented greater maladaptation (0.52 ± 0.07) and the canine region, the lowest (CD = 0.27 mm ± 0.07 and CE = 0.27 ± 0.09); There was no statistically significant difference for storage times. Conclusion G2 presented lower values than G1, with no statistically significant difference. The palate region presented greater maladaptation, followed by molars and canines. The bases continued to maladaptation the model after the immediate polymerization, with no statistically significant difference.

Individuals with different functions such as the functions of the stomatognathic system (SE), such as decreased chewing capacity, phonetic alteration and aesthetic impairment. The prosthetic oral rehabilitation assumes a function of great importance for the patient with prognosis, being able to be paramount for general health and quality of life. Rehabilitation as functions of SE and reinserting the individual into the social environment. Objective: to establish a prosthetic technique performed with no intention to promote the prognosis of a person over a series of years after the creation of a conventional molding technique. Methods: This is a case report, reporting a volunteer patient who needs a new total prosthesis. Clinical and laboratory stepswere used. Results: It as found to improve the retention, stability and aesthetics of the device as an improvement in patient occlusion. Conclusion: An issue that maybe easier to correct on a large scale in the chewing and phonation functions is greater than the disadvantages to dental discrepancy of the inter-arch size present in prognostic patients. Background: Traditionally, a series of steps have been carried out to make a complete denture, which are still essential despite technological advances. However, in some patients it is difficult to adapt new dentures. Objective: To describe a simplified technique for the duplication of total dentures using thermo-cured acrylic. This technique seeks to preserve intraoral mechanoreceptors and muscle adaptation that a patient has managed for years with the old dentures. Case description: A patient with severe bone resorption in both jaws needed to change 20-plus-yearold dentures. Duplicators similar to 3i® were made to obtain copies similar to the patient's dentures. Simultaneous impressions of both jaws with closed mouth were taken. The definitive thermo-cured dentures similar to the patient's old ones maintained neurosensory physiology and the patient did not feel the change. Conclusions: Conservation of the sensory receptors such as cold, pressure, heat and muscle activity is essential for success in cases of denture duplication. The author has around 2500 cases of denture duplication in a more than-20-year practice. [ABSTRACT FROM AUTHOR]

Introduction: overdentures are removable, muco-implantsupported, complete dentures, stabilized by elements installed on residual roots or implants. A variety of docking systems have been used to support overdentures, including bar-clip systems, balls, magnets and telescoping crowns. Objective: to evaluate, through a literature review, the overdentures retained by bar-clip system, exposing a correct planning, as well as describing protocols of making such devices. Methods: a literature review was carried out in the databases of the Latin American and Caribbean Center on Health Sciences Information (LILACS), Medline, Brazilian Dentistry Bibliography (BBO), the virtual library (SCIELO) and the collection of books from the University of Passo Fundo/RS (UPF). Literature Review: the bar-clip system consists of a plastic bar for casting or already metal and a retention clip. In some situations, the clip-on system is more suitable than other systems, although it has limitations in certain clinical scenarios. The bar-clip system allows considerable retention and stability, resulting in a restoration of the masticatory function, greater safety and great patient satisfaction. Final considerations: most of the studies showed an improvement in the quality of life related to the oral health of the patients after treatment with overdenture compared to the conventional total removable prosthesis. Introduction: the odontological records are the documents kept during the treatment. It offers data about both the patient's anamnesis and theirs pre-existing conditions, as well as the planning and all the work already done. Objective: identify, by way of analysis of the records, all information obtained about the patient's profiles and the work offered in dental prosthetics in a center dedicated to odontological specialties in a city in the countryside of Bahia State both in 2016 and 2017. Methodology: this study refers to a retrospective observation of cross-sectional cuts, descriptive and analytic, with the purpose of making a full analysis of the patients' records of the patients treated at the Centro de Especialidades Odontológicas na Bahia (Center of Odontological Specialities at Bahia). Results: the results showed a higher frequency of female, age from 61 to 65 years, with systemic pathologies. The upper and lower total edentulism was more frequent, and the mean time to complete rehabilitation was 6 months. Conclusion: the analysis of the odontological records is the most efficient way of obtaining all the needed information of the users and the quality of the services provided. A file well-filled offers to the professional every detail of each patient, ensures an excellent and tailored clinical work. [ABSTRACT FROM AUTHOR]

Objetivo: Este trabalho tratou sobre a avaliação da presença de Disfunção Temporomandibular (DTM) em pacientes portadores de prótese total superior com redução de Dimensão Vertical de Oclusão (DVO). A DTM abrange muitos problemas clínicos na região orofacial, tais como, dor, ruídos articulares, fadiga, bruxismo e zumbidos. Diversos são os fatores predisponentes e perpetuantes da DTM, sendo que as considerações anatômicas, que envolvem as discrepâncias inter-arcos e relações oclusais são as que podem se relacionar mais diretamente à prótese total e em consequência a esta pesquisa. Metodologia: Para a avaliação da presença de DTM, foi utilizado o questionário de Fonseca (1992) e para determinação da perda de DVO os testes métricos e fonéticos. Resultados e Conclusões: a amostra constituiu-se de 20 pacientes, após a coleta dos dados e análise dos resultados, a prevalência obtida em relação ao índice de DTM foi: pacientes com índice ausente 20%, índice leve 40%, índice moderado 25%, índice severo 15%, sendo que a somatória destes dois últimos são aqueles considerados portadores de DTM, ou seja, a prevalência de DTM nesta população foi de 40%.

A qualidade de vida relacionada à saúde bucal (QVRSB) demonstra o impacto de um tratamento protético na qualidade de vida do paciente, segundo sua própria perspectiva. Este estudo clínico pareado avaliou, por meio de diferentes instrumentos, a QVRSB de idosos edêntulos após a inserção de novas próteses totais (PTs) convencionais e overdentures mandibulares retidas por implante único (OMRIs). A QVRSB de 22 idosos portadores de PTs antigas e desajustadas foi avaliada por aplicação do Oral Health Impact Profile (OHIP-EDENT), Dental Impact on Daily Living (DIDL) e Geriatric Oral Health Assessment Index (GOHAI). A seguir, novas PTs convencionais foram instaladas, e após 2 meses de uso a QVRSB foi novamente mensurada. Posteriormente, os idosos receberam um único implante na região de sínfise mentoniana, e após a osseointegração as novas PTs convencionais foram convertidas em OMRIs. - Abstract
The oral cavity has a great influence on the quality of life, both biologically and psychologically and socially, through improvements in self - esteem, self - expression, socialism and satisfactory facial aesthetics. However, the index of total toothlessness in the population is very high, which can generate discomfort in living with society. The objective of this study was to evaluate the satisfaction of users of total dentures made in the last 5 years in the. In the city of Quixadá - CE. It was a descriptive research, aiming to analyze the quality of the total dentures, being applied questionnaires of alternative questions in 10 patients with questions involving the degree of satisfaction regarding the occlusion, final result of the total prosthesis and hygiene care. The results showed that the degree of satisfaction of the patients was very high and that the hygiene of their prostheses was considered satisfactory, since it was performed frequently three times a day. It was concluded that the patients demonstrated satisfaction with the use of their prosthetic devices, considering improvement in functionality, esthetics, self-expression and increased self-esteem. Aim: To perform an integrative analysis of the most up to date scientific literature on attitudes towards dental prosthesis hygiene and oral hygiene in the elderly who use complete dentures in Brazil. Methodology: The bibliographic search was carried through the Scientific Electronic Library Online - SciELO and Google Scholar databases, using the following two combined controlled descriptors "oral hygiene", "complete prosthesis", "oral health", "geriatric dentistry" with the operator Boolean “AND”. Peer-reviewed research articles were included. The Cochrane Handbook guidelines were followed, when applicable. Results: The research resulted in finding 21 articles, of which 15 were excluded. The integrative review was structured with 6 articles for data extraction and critical and descriptive evaluation. Conclusion: The studies in this review suggested that elderly Brazilians have many oral health problems and a low level of knowledge about oral and prosthesis hygiene. An important gap in practical research was identified in the literature, with regard to oral care in elderly populations using complete dentures. Objetivo: realizar un análisis integrador de la literatura científica más actual sobre las actitudes hacia la higiene de la prótesis dental y la higiene bucal de ancianos portadores de prótesis completa en Brasil. Metodología: Investigación bibliográfica realizada en las bases de datos Scientific Electronic Library Online – SciELO y Google Scholar, utilizando los descriptores controlados “higiene bucal”, “dentadura completa”, “salud bucal”, “odontología geriátrica”, combinados dos a dos con el operador booleano “Y”. Se incluyeron artículos de investigación revisados por pares. Se siguieron las pautas del Manual Cochrane cuando correspondía. Resultados: Las búsquedas acumularon 21 artículos, de los cuales 15 fueron excluidos. La revisión integradora se estructuró con 6 artículos para extracción de datos y evaluación crítica y descriptiva. Conclusión: Los estudios de esta revisión sugieren que los ancianos brasileños tienen muchos problemas de salud bucal y poco conocimiento sobre higiene bucal y prótesis dental. Se identificó un vacío importante en la literatura de investigación práctica sobre el cuidado bucal en poblaciones de ancianos con prótesis dentales completa. Objetivo: Realizar uma análise integrativa da literatura científica mais atual sobre as atitudes frente à higiene da prótese e à higiene oral de idosos que usam prótese total no Brasil. A avaliação mecânica de tal situação clínica é de extrema relevância, a fim de se estabelecer protocolos mais seguros durante esta fase inicial da implantodontia. O objetivo deste estudo foi realizar uma análise biomecânica pelo método dos elementos finitos 3D, da distribuição de tensões durante a fase provisória de reabilitação com implantes osseointegrados simulando uma situação clínica de protocolo/overdenture inferior sobre 4 implantes do tipo cone morse, na fase de cicatrização. Para este estudo foram confeccionados 6 modelos 3D, sob carregamento em 2 direções (vertical e oblíquo em relação ao plano oclusal), simulando osso tipo II, variando a altura do tecido gengival (2 mm e 3 mm) e a espessura do material reembasador (0 mm - sem material, 2 mm e 4 mm), sendo utilizado um cicatrizador compatível com a altura + 0,5 mm para cada situação gengival. A posição dos 4 implantes cone morse (UNITITE, UCM 3510N, SIN) foi sempre a mesma, nas posições perdidas dos dentes 46, 43, 33 e 36. Para a altura gengival de 2 mm foi utilizado cicatrizador de 2,5 mm de altura (UNITITE, CIMU 4525, SIN) e para a altura gengival de 3 mm, o cicatrizador de 3,5 mm de altura (UNITITE, CIMU 4535, SIN). Os modelos utilizados foram: (G2C2,5-SR) altura gengival de 2mm, cicatrizador de 2,5mm, sem material reembasador; (G2C2,5-R2) altura gengival de 2mm, cicatrizador de 2,5mm, com soft rreembasador de 2mm; (G2C2,5-R4) altura gengival de 2mm, cicatrizador de 2,5mm, com material reembasador de 4mm; (G3C3,5-SR) altura gengival de 3mm, cicatrizador de 3,5mm, sem material reembasador; (G3C3,5-R2) altura gengival de 3mm, cicatrizador de 3,5mm, e reembasador de 2mm e (G3C3,5-R4) altura gengival de 3mm, cicatrizador de 3,5mm e soft reembasador de 4mm. A metodologia para modelagem se baseou em um escaneamento de superfície de uma prótese total inferior montada, sob um modelo inferior padrão, e de seu modelo de assentamento. As geometrias dos implantes (CM 3,5x10mm) e de seus cicatrizadores foram obtidas por simplificação do desenho dos mesmos no programa SolidWorks e no programa Rhinoceros 3D. O software utilizado para análise foi o ANSYS 17.0. A visualização dos resultados foi qualitativa através de mapas de deslocamento, von Mises e tensão máxima principal. As regiões com maiores deslocamentos foram as dos implantes mais anteriores (#33 e #43), sendo os do modelo 6 submetidos a forças verticais os que apresentaram maiores deslocamentos, próximos a 0,05 mm. A força oblíqua, de forma geral, tendeu a sobrecarregar mais os implantes do lado onde as cargas foram aplicadas. No tecido ósseo foi possível observar que o osso cortical se mostrou com mais concentração de tensão de tração que o osso trabecular. A utilização do material reembasador gerou menores tensões transmitidas para o tecido gengival, concentrando estas tensões principalmente no material reembasador. A não utilização de material reembasador aumenta as concentrações de tensões nos implantes/cicatrizadores, independentemente de sua espessura ou da espessura gengival. Concluiu-se que o uso de reembasador soft diminuiu as tensões nos implantes/cicatrizadores e tecido gengival; o aumento da espessura do material reembasador não influenciou na diminuição das tensões aos tecidos de suporte e tecidos gengivais mais espessos foram favoráveis em diminuir as tensões transmitidas ao tecido ósseo, mas concentram mais tensões nos implantes/cicatrizadores. During the period of osseointegration of implants in an oral rehabilitation with a protocol or overdenturetype prosthesis, a conventional temporary complete denture can be used and coated with soft reline materials, which aim to reduce the stress to the bone-implant interface. The mechanical assessment of such a clinical situation is extremely relevant in order to establish safer protocols during this initial phase of implantology. The objective of this study was to perform a biomechanical analysis by the 3D finite element method, of the stress distribution during the provisional phase of rehabilitation with osseointegrated implants simulating a clinical situation of protocol/lower overdenture on 4 cone morse implants, in the healing phase. For this study, 6 3D models were made, under loading in 2 directions (vertical and oblique in relation to the occlusal plane), simulating type II bone, varying the height of the gingival tissue (2 mm and 3 mm) and the thickness of the reline material (0 mm - without material, 2 mm and 4 mm), using a healer compatible with the height + 0.5 mm for each gingival situation. The position of the 4 cone morse implants (UNITITE, UCM 3510N, SIN) was always the same, in the missing positions of teeth 46, 43, 33 and 36. For the 2 mm gingival height, a 2.5 mm high healer was used. (UNITITE, CIMU 4525, SIN) and for a gingival height of 3 mm, the healer with a height of 3.5 mm (UNITITE, CIMU 4535, SIN). The models used were: (G2H2,5-WS) 2mm gingival height, 2.5mm healing, without soft material; (G2H2,5-S2) 2mm gingival height, 2.5mm healing, with 2mm reline; (G2H2,5-S4) 2mm gingival height, 2.5mm healing, with 4mm reline; (G3H3,5-WS) gingival height of 3mm, healing of 3.5mm, without soft material; (G3H3,5-S2) 3mm gingival height, 3.5mm healing, and 2mm reline and (G3H3,5-S4) gingival height of 3mm, healing of 3.5mm and reliner of 4mm. The methodology for modeling was based on a surface scan of a complete lower denture assembled, under a standard lower model, and its seating model. The geometries of the implants (CM 3.5x10mm) and their healers were obtained by simplifying their design in the SolidWorks program and in the Rhinoceros 3D program. The software used for analysis was ANSYS 17.0. The visualization of the results was qualitative through displacement maps, von Mises and maximum principal stress. The regions with the greatest displacements were those of the most anterior implants (#33 and #43), and those of model 6 submitted to vertical forces presented the greatest displacements, close to 0.05 mm. The oblique force, in general, tended to place more stress on the implants on the side where the loads were applied. In the bone tissue, it was possible to observe that the cortical bone showed a higher concentration of tensile stress than the trabecular bone. The use of soft material generated lower tensions transmitted to the gingival tissue, concentrating these tensions mainly in the soft material. The non-use of soft material increases stress concentrations on implants/healers, regardless of their thickness or gingival thickness. It was concluded that the use of a soft reliner reduced the stresses on the implants/healers and gingival tissue; the increase in the thickness of the reline material did not influence the reduction of tensions to the supporting tissues and thicker gingival tissues were favorable in reducing the tensions transmitted to the bone tissue, but concentrated more tensions in the implants/healing agents. - Abstract
The objective of this study was to demonstrate the importance of oral rehabilitation with total superior prosthesis and partial removable lower prosthesis, based on a study on the Combination Syndrome or Kelly Syndrome. In these cases, most patients today do not tolerate the situation of being edentulous, as this situation interferes with mastication, aesthetics, phonetics and social interaction. A case report was presented where the patient attended the USS dental clinic complaining that she was not satisfied with her prosthesis and that she was aesthetically discomforted and from an adequate care regained her self-esteem and returned to smiling. - Abstract
Introduction: the dental total prosthesis is an artificial organ that should promote health and quality of life. The way of use, conservation and hygiene is fundamental for the success and longevity of it. One tool that seems to be effective in detecting and disseminating this type of information is activities of University extension. Objectives: to analyze the level of knowledge about the use of complete sets of dentures and their conservation in a low-income population through a cross-sectional epidemiological study. Methods: This research had a convenience sample of 37 users of complete sets of dentures. Data was collected using a questionnaire applied by two trained examiners. The data were interpreted via descriptive and statistical analysis (tests of Kruskal-wallis e Kolmogorov -- Smirnov/ p = 0,05). Results: The average income of the sample was R$ 668.00. The average time of denture use was 7.3 years; 70.27% of the patients sleep with their dentures, and 83.78% of them sanitize the dentures with a toothbrush and toothpaste. Conclusion: Regarding the variables association, we noticed tendencies yet to be explored in the habits of the complete denture wearer. Moreover, we conclude that the population's level of knowledge on the subject needs to be improved. Thereby, once these conditions are detected and transmitted to government officials, the extension project through the research tool will be able to assist government's course of action regarding oral health. Introdução: A disfunção temporomandibular (DTM) representa uma das principais causas de dor na região orofacial. A dimensão vertical de oclusão (DVO) refere-se à posição vertical da mandíbula em relação à maxila e seu restabelecimento correto é essencial nas reabilitações protéticas, pois variações deste parâmetro podem influenciar negativamente os quadros de DTM. Objetivo: Avaliar a presença de disfunção temporomandibular em pacientes portadores de prótese total e sua possível associação com a redução de dimensão vertical de oclusão. Material e Métodos: A amostra foi composta de 31 pacientes voluntários, com idades entre 42 e 87 anos, de ambos os sexos, sendo portadores de prótese total, atendidos na Clínica de Prótese Total Removível da Faculdade de Odontologia da Universidade Federal de Juiz de Fora. A análise da redução de DVO foi realizada através dos testes métrico, estético e fonético e o diagnóstico de DTM foi estabelecido a partir do Eixo I do RDC/TMD (Research Diagnostic Criteria for Temporomandibular Disorders). Os dados foram tabulados e submetidos à análise estatística descritiva no programa pelo SPSS (Statistical Package for the Social Science) versão 15.0 para Windows. Para comparação das variáveis categóricas entre os grupos de DTM foi utilizado o teste Qui-quadrado com nível de significância admitido de 5% (p0,05).

El objetivo de este estudio fue comparar la resistencia al impacto de dos acrílicos convencionales y dos de alto impacto para bases protésicas. Materiales y métodos: se realizaron 40 barras en cera para base para ser enmufladas de acuerdo con especificaciones de cuatro marcas de acrílicos (n=10), Lucitone199®, Masterdent acrylic resin, Veracril alto impacto® y convencional. La resistencia impacto se evaluó mediante la prueba de Charpy (Máquina ITC-XJU-22), con masa de péndulo de 0.237 Kg y fuerza de impacto de 1J. se realizó análisis estadístico por ANOVA a una vía y prueba de tukey. Resultados: los valores de resistencia al impacto fueron 16.5, 10.8, 9.3 y 5.9 KJ/m2 para Lucitone199®, Veracril alto impacto®, Veracril convencional® y Masterdent acrylic resin. Conclusiones: con las limitaciones de esta investigación, los acrílicos de alto impacto se pueden considerar como una alternativa más viable para la elaboración de prótesis totales ante los acrílicos convencionales, en busca de disminuir el riesgo de fracturas por accidentes o cargas repentinas de impacto. Abstract Introduction and objective: acrylic resins are the ideal materials for the elaboration of dentures and removable prostheses, although the falls or blows make them susceptible to fractures. Knowing the properties and limitations of acrylics in our environment, allows us to carry out treatments with the best materials. The objective of this study was to compare the impact resistance of two conventional and two high impact acrylics for prosthetic bases. Materials and methods: 40 bars were made in base-wax and processed in muffles according to specifications of four trademarks of acrylics (n=10), Lucitone199®, Masterdent acrylic resin®, Veracril alto impacto® and convencional®. The impact resistance was evaluated by the Charpy test (Machine ITC-XJU-22), with a pendulum mass of 0.237 Kg and an impact force of 1J. One-way ANOVA and tukey test were performed for statistical analysis was performed. Results: the impact resistance values were 16.5, 10.8, 9.3 and 5.9 KJ / m2 for Lucitone199®, Veracril alto impacto®, Veracril convencional® and Masterdent acrylic resin®. Conclusions: with the limitations of this investigation, high impact acrylics can be considered as a more viable alternative for the elaboration of dentures compared to conventional acrylics, in order to reduce the risk of fractures due to accidents or sudden impact loads. Conhecer as propriedades e limitações dos acrílicos em nosso ambiente, permite realizar tratamentos com os melhores materiais.. Objective: To assess whether the CAD/CAM technology is superior to the conventional technique for making Tissue Born total dentures. Methodology: This review followed the PRISMA recommendations. A systematic review was carried out by two calibrated reviewers in the Medline-PubMed, Scopus, Scielo, Lilacs, Web of Science and Cochrane Library databases, using the descriptors: “Complete denture”, “Complete dentures”, “Total denture”, “Total dentures”, “Complete prosthesis”, “Complete prosthesis”, “Total prosthesis”, “Total prosthesis”, “Computer Aided Design”, “Computer Aided Manufacturing”, “CAD/CAM”, ''Benefits” and “Advantage”. Only in vitro studies comparing CAD/CAM technology (printing or milling) with conventional manufacturing were included, and studies without a control group and prostheses on implants were excluded. Results: The strategy used resulted in 410 articles. Of these, 11 met the inclusion criteria. After reading in full, only 6 were included in the systematic review. Despite requiring a high investment and professional training, the selected studies demonstrated the following advantages of the CAD/CAM system over the conventional one: less monomer release and better adaptation of the dentures Conclusion: The manufacturing process of total dentures through the system CAD/CAM proved to be advantageous compared to the conventional method, the main one being the lower release of monomer, accompanied by better biomechanical qualities and marginal adaptation, despite its high cost. Objetivo: Evaluar si la tecnología CAD / CAM es superior a la técnica convencional para hacer dentaduras postizas totales con soporte de moco. Metodología: Esta revisión siguió las recomendaciones de PRISMA. Dos revisores calibrados realizaron una revisión sistemática en las bases de datos Medline-PubMed, Scopus, Scielo, Lilacs, Web of Science y Cochrane Library, utilizando los descriptores: “Dentadura completa”, “Dentadura completa”, “Dentadura total”, “Prótesis total”, “Prótesis completa”, “Prótesis completa”, “Prótesis total”, “Prótesis total”, “Diseño asistido por computadora”, “Fabricación asistida por computadora”, “CAD / CAM”, “Beneficios” y “Ventaja”. Solo se incluyeron los estudios in vitro que comparaban la tecnología CAD / CAM (impresión o fresado) con la fabricación convencional, y se excluyeron los estudios sin grupo control y prótesis sobre implantes. Resultados: La estrategia utilizada resultó en 410 artículos. De estos, 11 cumplieron los criterios de inclusión. Después de la lectura completa, solo se incluyeron 6 en la revisión sistemática. A pesar de requerir una alta inversión y formación profesional, los estudios seleccionados demostraron las siguientes ventajas del sistema CAD / CAM sobre el convencional: menor liberación de monómero y mejor adaptación de las prótesis Conclusión: El proceso de fabricación de prótesis totales mediante el sistema CAD / La CAM demostró ser ventajosa en comparación con el método convencional, siendo la principal la menor liberación de monómero, acompañada de mejores cualidades biomecánicas y adaptación marginal, a pesar de su elevado coste. Materiais e métodos: Usuários de próteses totais que usavam o mesmo par de próteses por mais do que 5 anos foram incluídos (sem disfunção temporomandibular) (n=15). Avaliações de qualidade de vida (Oral Health Impact Profile for Edentulous – OHIP-EDENT), força máxima de mordida, e eletromiografia dos músculos masseter e temporal foram realizadas em 5 tempos diferentes (T1 – próteses antigas originais; T2 – após 18 dias de uso da prótese mandibular reembasada; T3 – após 18 dias do restabelecimento da dimensão vertical de oclusão; e T4 e T5 – 30 e 100 dias de uso de novas próteses totais. 38. Influence of quality of total prostheses on the prevalence of temporomandibular disorder

Introduction Temporomandibular disorder (TMD) involves the mastication and temporomandibular articulation muscles. Completely toothless patients with prostheses should be thoroughly assessed since edentulism causes damage that can lead to TMD onset. Objective This study aims to assess the effect of the technical quality of total prostheses (TPs) on TMD occurrence in patients with bimaxillary TPs. Material and method 154 patients responded to a questionnaire to obtain personal data, information regarding prostheses, anamnestic index for TMD, and the prosthesis quality index. Result Of the 154 patients examined, 64 (41.6%) had no TMD, 61 (39.6%) had mild TMD, 23 (14.9%) moderate, and only 6 (3.9%) showed severe TMD. As for the quality of total prosthesis, 78 (50.6%) completely toothless individuals had satisfactory TPs while 76 (49.4) presented unsatisfactory prostheses. Regarding the time using total prosthesis, around 50% of individuals with up to 10 years use presented some degree of TMD and 70% of individuals with over 10 years presented TMD. Conclusion Considering the methodology used and the population studied, we conclude that the quality of the TPs did not influence the incidence of TMD. Resumo Introdução A disfunção temporomandibular (DTM) é uma desordem que envolve os músculos da mastigação e da articulação temporomandibular. Pacientes desdentados totais e que utilizam protéses requerem uma avaliação criteriosa, já que o edentulismo leva a prejuízos que podem contribuir para o aparecimento de DTM. Objetivo O objetivo deste presente estudo foi avaliar o efeito da qualidade técnica das próteses totais (PTs) na incidência de DTM em pacientes portadores de PTs bi-maxilares. Material e método Foi aplicado um questionário a 154 pacientes contendo os dados pessoais, o histórico das próteses, o índice anamnésico para DTM e o índice de qualidade de prótese. Resultado Dos 154 pacientes examinados, 64 (41,6%) estavam livres de DTM, 61 (39,6%) possuíam DTM leve, 23 (14,9%), moderada e apenas 6 (3,9%) mostravam DTM severa. Quanto à qualidade da prótese total, 78 (50,6%) desdentados totais portavam PTs satisfatórias e 76 (49,4) estavam com próteses insatisfatórias. Quanto ao tempo de uso da prótese total, cerca de 50% dos indivíduos com até 10 anos de uso apresentaram algum grau de DTM e 70% dos indivíduos com mais de 10 anos acusaram a presença de DTM. Conclusão Considerando-se a metodologia empregada e a população estudada, pôde-se concluir que a qualidade das PTs não influenciou na presença de DTM.

Numerous pathogens, including SARS-CoV-2, can remain viable on surfaces over days, which favors cross-contamination. Preventive measures are essential to prevent infections and control the spread of COVID-19. Fox ruler is an essential device in the clinical planning of complete dentures and the contact with the patient’s oral fluids during handling is unavoidable. Despite its importance, the conventional polycarbonate Fox ruler is easily damaged in the sterilization process. Therefore, the purpose of this article is to propose the use of a disposable Fox ruler. The confection is carried out in a simple way with wooden toothpicks joined by cyanoacrylate glue, following the same shape as the conventional one, but with the additional advantage of being made in an individual size according to the upper edge. The proposed disposable Fox ruler makes it possible to obtain the parallelism of the upper wax roller with the bipupillary line and with the Camper plane. Therefore, the disposable device described in this article was able to perform the same functions as the conventional one, in a practical way, without risks of cross contamination and with low cost.

A prótese total da articulação coxofemoral representa uma das técnicas mais aceitas, nos Estados Unidos e na Europa, para o tratamento cirúrgico da displasia coxofemoral severa em cães. Entretanto, ainda é pouco difundida e estudada no Brasil. No presente trabalho, foi utilizada prótese confeccionada no Brasil, com o objetivo de avaliar a sua aplicação, evolução pós-operatória e complicações associadas. Foram utilizados dez cães, sadios e adultos. Todos foram submetidos ao procedimento de prótese total cimentada da articulação esquerda utilizando-se um componente femoral de cromo-cobalto, cabeça fixa, e uma cúpula acetabular de polietileno de alta densidade. As avaliações clínicas e radiográficas foram realizadas no pré-operatório e aos 30, 60, 90, 120 e 150 dias após a intervenção cirúrgica. A prótese total utilizada proporcionou um bom resultado funcional ao membro operado, em 80% dos animais. As principais complicações relacionadas foram a luxação protética e a soltura do componente acetabular.Total hip prosthesis is one of the most accepted methods used in the United States and Europe for the treatment of severe hip dysplasia in dogs. However, there are few studies with the technique and the procedure is still not well established in Brazil. A prosthesis made in Brazil was used. The purpose of this study was to evaluate its application and determine the postoperative finds and complications. Ten mature healthy dogs were used. A fixed-head cemented total hip prosthesis was applied on the left leg of the dogs. A chrome-cobalt femoral stem and high density polyetylene acetabular cup was used. Clinical and radiographic evaluations were performed before surgery and again at 30, 60, 90, 120 e 150 days post operation. Proper weight bearing was noticed on the operated limb in the majority of the dogs. The main complications were the dislocation and loosening of the acetabular cup.

42. Impact of residual ridge anatomy on masticatory efficiency of conventional complete denture users

Objective: The aim of this cross-sectional study was to evaluate the anatomical characteristics of edentulous residual ridges in stone casts of complete dentures users and assess correlations with masticatory efficiency. Methods: The sample included 74 study casts, of which 37 were higher and 37 bottom. Measurements were performed using a drypoint compass and a transparent ruler. Arches and ridges of maxillae and mandibles were classified into small, medium, and large. Masticatory efficiency was obtained by the sieve method with the old prostheses and 3 months after placement of new prostheses. The correlations between the anatomical variables of the ridges and arches (height, width and size) and masticatory efficiency were verified by the Spearman correlation test. Significant differences in masticatory efficiencies were assessed by the Wilcoxon test. Results: People with larger maxillary alveolar ridge presented better masticatory performance with old and new dentures. There was no correlation between maxillary ridge width and size with masticatory efficiency, but a correlation was found between maxillary arch width and masticatory efficiency evaluated with the old dentures. Conclusions: With the new dentures, the correlation had a tendency for significance. For the mandible, no correlation was found between height, width, and size of the ridge or with width, length, and size of the arch and masticatory efficiency. In addition, there was no significant difference between masticatory efficiencies evaluated with the old and new prostheses. For the maxilla, ridge height and arch width influence masticatory efficiency. For the mandible, the anatomy had little influence on masticatory performance. RESUMO Objetivo: Este estudo transversal objetivou avaliar as características anatômicas de rebordos residuais de usuários de próteses totais em gesso pedra e avaliar correlações com a eficiência mastigatória. Métodos: A amostra incluiu 74 modelos de estudo, sendo 37 superiores e 37 inferiores. As medidas foram realizadas com um compasso de ponta seca e régua transparente. Os arcos e rebordos da maxila e mandíbula foram classificados em pequenos, médios e grandes. A eficiência mastigatória foi obtida pelo método da peneira com as próteses antigas e 3 meses após a instalação das próteses novas. As correlações entre as variáveis anatômicas dos rebordos e arcos (altura, largura e tamanho) e a eficiência mastigatória foram verificadas pelo teste Spearman. Diferenças significativas nas eficiências mastigatórias foram avaliadas pelo teste de Wilcoxon. Resultados: Pessoas com crista alveolar maxilar alta apresentaram melhor desempenho mastigatório com próteses novas e antigas. Não houve correlação entre a largura e o tamanho da crista maxilar com a eficiência mastigatória, mas foi encontrada uma correlação entre a largura do arco maxilar e a eficiência mastigatória avaliadas com as próteses antigas. Para a mandíbula, não foi encontrada correlação entre altura, largura e tamanho da crista ou com largura, comprimento e tamanho do arco e eficiência mastigatória. Além disso, não houve diferença significativa entre as eficiências mastigatórias avaliadas com as próteses antigas e novas. Conclusão: Para a maxila, a altura da crista e a largura do arco influenciam a eficiência mastigatória. Para a mandíbula, a anatomia teve pouca influência no desempenho mastigatório.

43. Estudo comparativo de tratamento da estomatite protética pelo reembasamento ou substituição das próteses totais

Este estudo clínico buscou comparar duas formas de tratamento para pacientes que fossem portadores de próteses totais superiores e apresentassem clinicamente a Estomatite Protética. Foram selecionados 30 pacientes divididos em dois grupos. O grupo I foi constituído por 18 pacientes que tiveram suas próteses substituídas por novas. O grupo II foi constituído por 12 pacientes onde foi realizado o reembasamento das próteses totais em uso. Os pacientes foram acompanhados por um período de 8 semanas. Em ambos os grupos foi solicitado aos pacientes que removessem a prótese à noite para a desinfecção com solução de hipoclorito de sódio. Os resultados mostraram a cura da Estomatite Protética num prazo de 4 semanas numa proporção de 90%, para os dois grupos sem diferença estatisticamente significante. Foi possível concluir que, tanto a substituição da prótese como o seu reembasamento associados à higiene e a desinfecção mostraram-se eficazes no controle da Estomatite Protética., This clinic survey aimed at comparing two different treatments for patients using full upper denture and having clinical Denture Stomatitis diagnosis. Thirty patients were selected and divided into two groups. Group I included 18 patients, whose dentures were replaced by new ones. Group II included 12 patients, whose full dentures currently used were relined. Patients were followed-up during 8 weeks. Patients of both groups were required to remove their dentures at night to be disinfected with Sodium Hypochlorite solution. The results observed show Denture Stomatitis was cured within 4 weeks in 90% the cases for both groups, showing no significant statistical differences. It may be concluded that both dentures replacement and their filling, associated to hygiene and disinfection show efficacy in Denture Stomatitis control.

Embora a reabilitação oral com recurso a implantes se apresente como, a solução mais promissora devido às inúmeras vantagens que apresentam. Muitos pacientes não reúnem as condições necessárias para este tipo de reabilitação. E, como tal, a prótese total removível torna-se o tratamento possível para estes pacientes. A reabilitação oral com próteses totais removíveis, é muitas vezes associada a problemas como, estabilidade e retenção, tornando-se um desafio constante para os médicos dentistas. Assim, a técnica de confeção de próteses totais pela técnica de sucção, surge com o principal objetivo de aumentar o grau de estabilidade e retenção. O efeito de sucção é gerado através do selamento periférico total da prótese pela mucosa móvel. E, para que seja possível aplicar este conceito na prática clínica é fundamental a compreensão de todo o mecanismo envolvente. A importância da história clínica, impressões preliminares e definitivas e ainda a montagem dos dentes são definidas como os principais critérios de sucesso para esta técnica de reabilitação oral. Portanto, esta revisão tem como objetivo principal, abordar todo o conceito teórico da técnica de sucção assim como, descrever as etapas clínicas necessárias para a sua confeção. The prevalence of edentulism varies between different regions and countries around the world. The most commonly reported problems include chewing function, phonetics, aesthetics, and psychological problems. To solve these problems, fixed or removable solutions can be presented. Although oral rehabilitation using implants is presented as the most promising solution due to the numerous advantages they present, many patients do not meet the necessary conditions for this type of rehabilitation. Therefore, a full removable denture becomes the possible treatment for these patients. Oral rehabilitation with removable dentures is often associated with problems such as stability and retention, which are a constant challenge for dentists. Thus, the technique of making complete dentures using the suction technique emerges with the main goal of increasing the degree of stability and retention. The suction effect is generated through the total peripheral sealage of the prosthesis by the mobile mucosa. And, in order to apply this concept in clinical practice, it is essential to understand the entire surrounding mechanism. The importance of the clinical history, preliminary and definitive impressions and also the assembly of the teeth are defined as the main success criteria for this oral rehabilitation technique. Therefore, this review's main objective is to address the entire theoretical concept of the suction technique as well as describe the clinical steps necessary for its making.

Dissertação para obtenção do grau de Mestre no Instituto Universitário Egas Moniz Ao longo dos anos, vários conceitos e teorias têm emergido sobre os diferentes tipos de esquemas oclusais, com o objetivo de obter o conceito do que será uma oclusão ideal. O estabelecimento de uma oclusão equilibrada e harmoniosa com as estruturas do aparelho mastigatório torna-se o objetivo primordial na confeção e montagem da prótese total. Fatores como a performance mastigatória, a eficiência muscular, a sintomatologia dolorosa da Articulação Temporomandibular (ATM), a saúde periodontal e por fim, a estética condicionam a escolha do padrão de oclusão adotado em prótese total. Dos vários esquemas oclusais sugeridos na literatura, há predominância de três, sendo estes: oclusão balanceada bilateral, oclusão lingualizada e oclusão lateral por guia canina. Sendo que o mais empregue, atualmente, é a oclusão balanceada bilateral. No entanto, a oclusão lingualizada apresenta várias vantagens, em relação aos outros tipos de esquemas oclusais. O objetivo do presente estudo consistiu na revisão das vantagens e desvantagens da oclusão lingualizada comparativamente à oclusão balanceada bilateral, bem como as suas indicações. Concluiu-se que apesar da prótese total montada em Oclusão Balanceada Bilateral apresentar geralmente bons resultados quanto à estabilidade e retenção da prótese, assim como quanto às funções estética e fonética; a Oclusão Lingualizada parece apresentar em algumas circunstâncias clínicas, tais como reabsorções ósseas extremas, melhores resultados a longo prazo. O estudo realizado nesta monografia realça a importância de um bom diagnóstico prévio na elaboração do planeamento da prótese total e sugere a necessidade da realização de mais estudos clínicos e protocolos que facilitem ao clínico a escolha de um esquema oclusal adequado a cada paciente. Over the years, several concepts and theories have emerged about the different types of occlusal schemes, with the aim of achieving the concept of an ideal occlusion. The establishment of a balanced and harmonious occlusion with the structures of the masticatory apparatus becomes the primary objective in the fabrication and fitting of the total prosthesis. Factors such as masticatory performance, muscle efficiency, TMJ pain, periodontal health and, finally, esthetics condition the choice of occlusion pattern adopted in total prosthesis. Of the various occlusal patterns suggested in the literature, three predominate: bilateral balanced occlusion, lingualized occlusion and lateral occlusion by canine guide. Currently, the most widely used is the bilateral balanced occlusion. However, the lingualized occlusion has several advantages over the other types of occlusal schemes. The aim of the present study was to review the advantages and disadvantages of lingualized occlusion compared to balanced bilateral occlusion, as well as its indications. It was concluded that despite the total denture mounted in Bilateral Balanced Occlusion generally present good results in terms of stability and retention of the prosthesis, as well as in terms of aesthetic and phonetic functions; Lingualized Occlusion seems to present in some clinical circumstances, such as extreme bone resorptions, better long-term results. The study carried out in this monograph highlights the importance of a good prior diagnosis in planning the complete denture and suggests the need to carry out more clinical studies and protocols that make it easier for the clinician to choose an appropriate occlusal scheme for each patient.

This study investigated, through computerized electrognathographic evaluations (K6-I Diagnostic System, Myotronics-Noromed Inc., Tukwila, USA), the mandibular movement pattern of 16 patients rehabilitated with complete dentures presenting no symptoms of stomatognathic functional alterations. The patients were instructed to wear an intra-oral appliance for occlusal plane coverage over their usual superior denture and were then rehabilitated with new dentures preserving a free-way space of 3 mm. After sixty days, the occlusal vertical dimension was increased and the modified inferior dentures were used for another 60 days. The obtained results were submitted to the Tukey Test and to the Friedman test, depending on which variable was under consideration, both at a significance level of 5%. The data revealed a significant decrease in free-way space when the first and the last evaluations were compared. No significant differences were found during opening and closing. It was concluded that the presence of a free-way space at the end of the treatment confirms the importance of its existence for maintaining the balance of the masticatory system, assuming the occurrence of a postural repositioning.O objetivo deste estudo foi avaliar por meio de eletrognatografia computadorizada (K6-I Diagnostic System, Myotronics-Noromed Inc., Tukwila, EUA) o padrão dos movimentos mandibulares de 16 pacientes reabilitados com dentaduras completas, livres de alterações funcionais do sistema estomatognático. Os pacientes foram instruídos a utilizar um aparelho intra-oral de cobertura oclusal plana adaptado na prótese usual superior e então reabilitados com novas próteses preservando a existência de um espaço funcional livre de 3 mm. Após 60 dias promoveu-se um aumento da dimensão vertical de oclusão e as próteses inferiores assim modificadas foram utilizadas por mais 60 dias. Os resultados foram submetidos ao Teste de Tukey e ao Teste de Friedman, dependendo da variável em estudo, ambos com 5% de significância. Constatou-se decréscimo significante do espaço funcional livre quando compararam-se a primeira e a última avaliações. Não foram encontradas diferenças durante abertura e fechamento. Concluiu-se que a presença de um espaço funcional livre ao final do tratamento vem comprovar a importância de sua existência na manutenção do equilíbrio do sistema mastigatório, admitindo-se que ocorre o reposicionamento postural.

50. Efficacy of biofilm disclosing agent and of three brushes in the control of complete denture cleansing Eficácia de um evidenciador de biofilme e de três escovas no controle da higiene de próteses totais

OBJECTIVE: This report evaluated the efficacy of three brushes and one biofilm disclosing agent in complete denture cleansing. METHODS: Twenty-seven wearers of maxillary dentures were distributed into three groups and received different brushes: Oral B40, conventional toothbrush (Oral B); Denture, denture-specific brush (Condor); Johnson & Johnson, denture-specific brush (Johnson & Johnson). The 60-day experimental period was divided into two techniques: I - brushing (brush associated with a paste - Dentu Creme, Dentco) three times a day; II - brushing and daily application of 1% neutral red on the denture internal surface. Biofilm quantification was carried out weekly and the areas with dye biofilm were obtained by means of Image Tool 2.02 software. RESULTS: Biofilm removal was more effective during Technique II (Wilcoxon test: p=0.01) for the three groups of brushes. When the brushes were compared in Technique I, the Kruskal Wallis test indicated statistical difference between Denture X Johnson & Johnson and Denture X Oral B40, in which the Denture was more efficient. For Technique II, there was no statistical difference between brushes (p>0.05). CONCLUSION: The disclosed application promoted more efficacy on biofilm removal, regardless of the brush used. Denture (Condor) was more efficient than the other brushes during Technique I.Este trabalho avaliou três escovas e um evidenciador de biofilme na higiene de próteses totais. Vinte e sete usuários de próteses totais superiores foram distribuídos em três grupos que receberam diferentes escovas: Oral B40 para dentes naturais (Oral B); Denture para dentadura (Condor) e Johnson & Johnson para dentadura (Johnson & Johnson). O período experimental de 60 dias foi dividido em 2 Técnicas: I - utilização das escovas associadas a um dentifrício (Dentu-Creme, Dentco) três vezes ao dia; II - escovação e aplicação diária do evidenciador Vermelho neutro a 1% na superfície interna da prótese total. A quantificação do biofilme foi realizada semanalmente e as áreas com biofilme corado foram medidas com o auxílio do software Image Tool 2.02. A remoção do biofilme foi mais efetiva durante a Técnica II (teste de Wilcoxon: p=0.01) nos três grupos. Comparando as escovas, na Técnica I o teste de Kruskal Wallis indicou diferença significante (p=0.05) entre Denture X Johnson & Johnson e Denture X OralB 40, sendo a Denture a mais eficaz. Na Técnica II, as escovas não apresentaram diferença estatisticamente significante (p>0,05). A aplicação do evidenciador promoveu maior eficácia na remoção do biofilme, independentemente da escova utilizada. Na comparação das escovas na Técnica I, a escova Denture foi a mais eficiente.
