Abstract

Low back pain (LBP) is the leading cause of disability worldwide. Providing evidence-based practice (EBP) for patients with LBP is more cost-effective compared with non-EBP. To help health care professionals provide EBP, several clinical practice guidelines have been published. However, a relatively poor uptake of the guidelines has been identified across various countries. To enhance future implementation of EBP, the aim of this study was to explore barriers to using LBP guidelines in clinical practice. A qualitative constructivist grounded theory design was employed in order to gain an in-depth understanding of the barriers. Semi-structured interviews (+/− observations) of nine physiotherapists and nine chiropractors from primary care in the Central Denmark Region were conducted. Two key barriers were found to using guidelines in practice: (1) a scepticism due to doubts about validity and applicability of the guidelines, which emerged particularly among physiotherapists; and (2) a deep biomechanical professional identity, due to perceived role, interest, lack of skills, and patient preferences, which emerged particularly among chiropractors. For guidelines to be better implemented in practice, these key barriers must be addressed in a tailored strategy. Furthermore, this study showed a difference in barriers between the two professions. It is important that physiotherapists and chiropractors reflect on what constitutes their core task and professional identity if the implementation of the biopsychosocial model is to be successful. To overcome the barrier of scepticism towards guidelines, the applicability of the guidelines could be improved by elaborating on how the recommendations could be individualised. It is important to incorporate the biopsychosocial model into the programs of educational institutions and provide training to improve those skills in physiotherapists and chiropractors. Abstract

The quality of the academic workforce is influenced by multi-level interactionist factors yet there is limited research into these influencing factors. To investigate relationships between individual characteristics of Australian occupational therapy academics and work-related variables. An exploratory cross-sectional online survey collected demographic data and used standardized scales including the Turnover Intention Scale, 9-item Utrecht Work Engagement Scale, Work-Life Balance Scale, Role Overload Scale and Self-Defined Burnout Scale. Data were analysed using descriptive and inferential statistics. Academics who were more than five years in their current academic level were: significantly more likely to consider leaving their job; more often frustrated when not given opportunity to achieve their work-related goals; and less engaged. Those who had worked in higher education for longer than ten years were more likely to report role overload while participants working in higher education for less than five years were less likely to report burnout. To meet current and future demand for Australian occupational therapists, an engaged and supported academic workforce is needed. These findings suggest the need for attention to mentoring and career development particularly for those in academic levels longer than five years to increase work engagement and retention. Abstract

Background. Recent systematic reviews suggest that outpatient supervised and home-based rehabilitation exercise after total hip arthroplasty (THA) are equally effective. But there is a lack of knowledge concerning patients perspectives on rehabilitation after THA. Objectives. To explore patient-perceived facilitators and barriers to home-based rehabilitation exercise and general physical activity after THA. The aim of this study is to contribute to the scientific basis for future organization of rehabilitation after THA. Design. A qualitative study embedded in the trial: Pragmatic Home-based Exercise after Total Hip Arthroplasty, Silkeborg (PHETHAS-1), which was pre-registered on March 27, 2017 at Clinical Trials.gov (ID: NCT03109821 ). Methods. Twenty-two semi-structured individual interviews were conducted with patients who had undergone THA surgery. Thematic analysis was undertaken, and emergent themes and subthemes were categorized using the lens of critical psychology. Findings. Facilitating factors were related to Return to the well-known everyday life. A natural orientation toward general physical activity. Interaction with nature, and An obligation to health professionals and to oneself. Barriers were related to Pain. Boring exercises, and A feeling of distant rehabilitation. Conclusions. Our findings indicate that everyday life is of great importance to the participants and that they have a desire to return to living the way they did before their hips restricted their functioning. Home-based rehabilitation exercise can be used as a means to reach this goal. Contact with health professionals has a beneficial impact on participants adherence to home-based rehabilitation exercise because their participation is facilitating by being a part of a mandatory co-operation. All participants in this study favored general physical activity over home-based rehabilitation exercise.
