BACKGROUND Registered nurses (RNs) frequently contract for safety with patients who are at risk for self-harm. It is important to determine if contracting for safety is an effective intervention. OBJECTIVE This study explores results of implementing a safety agreement tool, a form of contracting, in an acute psychiatric facility. STUDY DESIGN Two surveys were given to RNs after implementation of the safety agreement tool. The rate of self-harming incidents pre- and postimplementation was compared. RESULTS RNs were evenly divided in reporting if nurse-patient interactions were enhanced with safety agreements. However, RNs perceived increased patient responsibility, nurse contact, safety guidance, safety discussions, and time guidelines with the safety agreement use. The mean rate of self-harming incidents before was not significantly different from the mean rate after implementation of safety agreements. CONCLUSION Although implementation of the safety agreement tool did not result in statistically significant decreased levels of self-harm, RNs perceived numerous positive effects. Involving patients in groups has been identified as one way of optimizing patient treatment options while maximizing staff resources and containing costs. In this article, literature on the use of groups by nurses in an inpatient setting is reviewed, and the development of a program involving a psychoeducational group curriculum incorporating Orlando's nursing theory is described. The program was found to be very helpful in (a) providing structured content; (b) promoting greater comfort, active involvement, and learning by registered nurses and patients; and (c) demonstrating improved patient response. Four experiments were conducted to determine the effect of mat feeding strategy on the growth performance, removal, and mortality of pigs after weaning. In exp. 1, 1,392 weanling pigs (initially 7.0 kg; approximately 24 d of age) were randomly allotted to 1 of 2 mat feeding treatments (mat feed vs. no mat feed). Overall, mat fed pigs had a tendency for improved ADG (P = 0.065) and G:F (P = 0.060) compared to pigs not offered mat feed. Mat fed pigs had fewer removals (P = 0.013) compared to pigs not offered mat feed. In exp. 2, 2,912 weanling pigs (initially 5.5 kg; approximately 21 d of age) were randomly allotted to 1 of 2 mat feeding treatments (mat feed vs. no mat feed). Overall, no differences in growth performance were observed. However, mat fed pigs had decreased (P < 0.026) final body weights compared to pigs not offered mat feed. This may be related to removal rates as mat fed pigs had fewer removals (P = 0.026). In exp. 3, 3,264 weanling pigs (initially 5.5 kg; approximately 21 d of age) were randomly allotted to 1 of 4 treatments in a 2 × 2 factorial with main effects of diet form (pellet or crumble) and mat feeding (mat feed vs. no mat feed). No interactions between diet form and mat feeding were observed. No differences were observed in overall growth performance for the main effect of mat feeding. Pigs offered pelleted feed had decreased overall ADFI (feed disappearance from the feeder and feed placed on the mat; P = 0.013) and improved G:F (P < 0.001) compared to pigs offered crumble feed. No differences were observed in removals or mortalities for the main effect of mat feeding or diet form. In exp. 4, 3,227 weanling pigs (initially 5.1 kg; approximately 21 d of age) were randomly allotted to 1 of 3 treatments consisting of mat feeding small (3.2 mm) pellets, mat feeding large (12.7 mm) pellets, or no mat feeding. Overall, no differences were observed in ADG or G:F. Mat fed pigs had increased ADFI (P < 0.001) compared to pigs not offered mat feed. Given the shorter duration of this experiment, the extra feed provided with mat feeding had a greater impact on overall feed usage than exp. 1, 2, and 3. No differences were observed in removals or mortalities. When combining the removal and mortality data for the four experiments, mat fed pigs had fewer removals (P = 0.002) compared to pigs not offered mat feed. Stromal cell-derived factor 1 (SDF-1) is known to influence bone marrow stromal cell (BMSC) migration, osteogenic differentiation, and fracture healing. We hypothesize that SDF-1 mediates some of its effects on BMSCs through epigenetic regulation, specifically via microRNAs (miRNAs). MiRNAs are small non-coding RNAs that target specific mRNA and prevent their translation. We performed global miRNA analysis and determined several miRNAs were differentially expressed in response to SDF-1 treatment. Gene Expression Omnibus (GEO) dataset analysis showed that these miRNAs play an important role in osteogenic differentiation and fracture healing. KEGG and GO analysis indicated that SDF-1 dependent miRNAs changes affect multiple cellular pathways, including fatty acid biosynthesis, thyroid hormone signaling, and mucin-type O-glycan biosynthesis pathways. Furthermore, bioinformatics analysis showed several miRNAs target genes related to stem cell migration and differentiation. MicroRNAs (miRNAs) have recently come under scrutiny for their role in various age-related diseases. Similarly, cellular senescence has been linked to disease and aging. MicroRNAs and senescence likely play an intertwined role in driving these pathologic states. In this review, we present the connection between these two drivers of age-related disease concerning mesenchymal stem cells (MSCs). First, we summarize key miRNAs that are differentially expressed in MSCs and other musculoskeletal lineage cells during senescence and aging. Additionally, we also reviewed miRNAs that are regulated via traditional senescence-associated secretory phenotype (SASP) cytokines in MSC. Lastly, we summarize miRNAs that have been found to target components of the cell cycle arrest pathways inherently activated in senescence. This review attempts to highlight potential miRNA targets for regenerative medicine applications in age-related musculoskeletal disease., (Published by Elsevier Inc.)

Porcine circovirus associated disease (PCVAD) is a term used to describe the multi-factorial disease syndromes caused by porcine circovirus type 2 (PCV-2), which can be reproduced in an experimental setting through the co-infection of pigs with PCV-2 and porcine reproductive and respiratory syndrome virus (PRRSV). The resulting PCVAD-affected pigs represent a subpopulation within the co-infected group. In co-infection studies, the presence of increased microbiome diversity is linked to a reduction in clinical signs. In this study, fecal microbiota transplantation (FMT) was investigated as a means to prevent PCVAD in pigs co-infected with PRRSV and PCV-2d. The sources of the FMT material were high-parity sows with a documented history of high health status and robust litter characteristics. The analysis of the donated FMT material showed the absence of common pathogens along with the presence of diverse microbial phyla and families. One group of pigs ( n = 10) was administered the FMT while a control group ( n = 10) was administered a sterile mock-transplant. Over the 42-day post-infection period, the FMT group showed fewer PCVAD-affected pigs, as evidenced by a significant reduction in morbidity and mortality in transplanted pigs, along with increased antibody levels. Overall, this study provides evidence that FMT decreases the severity of clinical signs following co-infection with PRRSV and PCV-2 by reducing the prevalence of PCVAD.

Overview: Like most hospital inpatients, those with opioid use disorder (OUD) often experience acute pain during their hospital stay and may require opioid analgesics. Unfortunately, owing to clinicians' misconceptions about opioids and negative attitudes toward patients with OUD, such patients may be inadequately medicated and thus subjected to unrelieved pain and unnecessary suffering. This article reviews current literature on the topic of acute pain management for inpatients with OUD and dispels common myths about opioids and OUD.

Four experiments were conducted to investigate the effects of varying concentrations of supplemental vitamin D3 on pig growth, feed preference, serum 25-hydroxycholecalciferol [25(OH)D3] , and bone mineralization of nursing and weanling pigs. In Exp. 1, 270 pigs (1.71 ± 0.01 kg BW) were administered 1 of 3 oral vitamin D3 dosages (none, 40,000, or 80,000 IU vitamin D3) on d 1 or 2 of age. Increasing oral vitamin D3 increased serum 25(OH)D3 on d 10 and 20 (quadratic, P < 0.01) and d 30 (linear, P < 0.01). No differences were observed in ADG before weaning or for nursery ADG, ADFI, or G:F. Vitamin D3 concentration had no effect on bone ash concentration or bone histological traits evaluated on d 19 or 35. In Exp. 2, 398 barrows (initially 7 d of age) were used in a 2 × 2 split plot design to determine the influence of vitamin D3 before (none or 40,000 IU vitamin D3 in an oral dose) or after weaning (1,378 or 13,780 IU vitamin D3/kg in nursery diets from d 21 to 31 of age) in a 45-d trial. Before weaning (7 to 21 d of age), oral vitamin D3 dose did not influence growth but increased (P < 0.01) serum 25(OH)D3 at weaning (d 21) and tended (P = 0.08) to increase 25(OH)D3 on d 31. Increasing dietary vitamin D3 concentration from d 21 to 31 increased (P < 0.01) serum 25(OH)D3 on d 31. Neither the oral vitamin D3 dose nor nursery vitamin D3 supplements influenced nursery ADG, ADFI, or G:F. In Exp. 3, 864 pigs (initially 21 d of age) were allotted to 1 of 2 water solubilized vitamin D3 treatments (none or 16,516 IU/L vitamin D3 provided in the drinking water from d 0 to 10) in a 30-d study. Providing vitamin D3 increased serum 25(OH)D3 concentrations on d 10, 20, and 30; however, vitamin D3 supplementation did not affect overall (d 0 to 30) ADG, ADFI, or G:F. In Exp. 4, 72 pigs were used in a feed preference study consisting of 2 feed preference comparisons. Pigs did not differentiate diets containing either 1,378 or 13,780 IU vitamin D3/kg but consumed less (P < 0.01) of a diet containing 44,100 IU vitamin D3/kg compared with the diet containing 1,378 IU vitamin D3/kg. Overall, these studies demonstrate that supplementing vitamin D3 above basal concentrations used in these studies is effective at increasing circulating 25(OH)D3, but the supplement did not influence growth or bone mineralization. Also, concentrations of vitamin D3 of 44,100 IU/kg of the diet may negatively affect feed preference of nursery pigs.

Two experiments were conducted to evaluate the effect of nursery diet sources, porcine circovirustype 2 (PCV2) and Mycoplasma hyopneumoniae (M. hyo) vaccines, and vaccination timing on pig (Sus scrofa) performance. In Exp. 1, a total of 400 pigs (5.6 BW, 1.03 kg SD) were used in a 20-d study. Treatments were arranged in a 4 × 2 factorial in a blocked design (5 pigs/pen and10 pens/treatment), with main effects of diet manufacturing source (A, B, C, or D) and vaccination timing (d 0 or 8). On either d 0 (weaning) or 8, pigs received 2 vaccines (Circumvent PCV, Intervet/Schering-Plough Animal Health, Millsboro, DE; and RespiSure One, Pfizer Animal Health, New York, NY). A pre-determined amount of segregated early weaning (SEW) diet (0.45 kg/pig) was fed followed by a transition diet until d 8, and a common diet from d 8 to 20. Diet source affected (P < 0.001) ADG during the first 4 d and affected (P ≤ 0.02) ADG and ADFI from d 4 to 8. There were no differences (P ≥ 0.18) among diet sources once pigs were fed a common diet (d 8 to 20). Overall, diet source did not affect ADG; but ADFI tended (P = 0.06) to be decreased for pigs fed Diet C compared with those fed Diets A, B, and D. Pigs vaccinated on d 0 had decreased (P ≤ 0.01) ADG and ADFI (d 4 to 8 and d 0 to 8), resulting in lighter (P = 0.003) BW on d 8 than those of pigs not yet vaccinated (d 8). However, overall ADG was not affected by vaccination timing. In Exp. 2, 360 pigs (5.9 SD, 0.91 kg BW) were used in a 35-d trial to evaluate the effects of different vaccines. Treatments were arranged in a 3 by 2 factorial in a blocked design (5 pigs/pen and 12 pens/treatment). Main effects included PCV2 vaccine (none; CircoFLEX, Boehringer Ingelheim Vetmedica Inc., St. Joseph, MO; or Circumvent PCV); with or without M. hyo vaccine (RespiSure, Pfizer Animal Health, New York, NY). Overall, pigs vaccinated with Circumvent PCV had decreased (P < 0.02) ADG and ADFI compared with CircoFLEX-vaccinated or control pigs. On d 35, pigs vaccinated with Circumvent PCV weighed less (P < 0.01) than CircoFLEX-vaccinated or control pigs. RespiSure-vaccinated pigs had decreased (P ≤ 0.05) ADG compared with control pigs from d 14 to 21 and d 21 to 29. On d 35, RespiSure-vaccinated pigs tended (P = 0.06) to weigh and consume less than control pigs. These data indicate diet source and vaccination timing affects pig performance after weaning. Vaccination for PCV2 and M. hyo independently reduced ADG and ADFI, but the effect was product-dependent.

Purpose: The consequences of an anastomotic leak or disruption can be devastating, particularly in the colorectal surgery population. The purpose of this study was to evaluate and compare colon anastomoses with or without a collagen matrix buttress derived from bovine pericardium., Methods: A circular stapler was used to create colon-colon anastomoses in a canine model. Twenty animals underwent two anastomoses each: one buttressed with bovine pericardium, and one without any reinforcement. Staple lines were evaluated at Days 0, 3, 7, 14, 42, and 84. Three animals were killed at each time interval, and evaluation included bursting pressure, bursting location, and histology., Results: Colon segments with nonbuttressed anastomoses were more likely to burst at the staple line (63 percent), whereas buttressed anastomoses were more likely to burst at the adjacent intestine (74 percent; P=0.048). The burst pressure of nonbuttressed staple lines tended to be consistently, although not significantly, higher than the burst pressure of buttressed staple lines (P=0.651). At histologic analysis, the bovine pericardium buttress demonstrated an ability to allow cellular ingrowth at Day 3 and neovascularization at Day 7. There was no evidence of stenosis or infection., Conclusions: The use of a collagen matrix buttress in colorectal anastomoses was safe in a canine model. Our study indicates that true burst strength of the majority of buttressed anastomoses was greater than the adjacent intestine.

Arthritis is the leading chronic condition for older adults and the leading cause of disability in the United States. Providing holistic care to promote positive health perception and support healthy lifestyle decisions with these individuals is critical in attaining and maintaining quality of life. This study examined variables reflecting reactions to stress, lines of defense and resistance, and the basic core of humans in the context of the Neuman Systems Model in a convenience sample of 47 adults with a mean age of 70.4 years. The combined effects of the social, emotional, and physical impact of arthritis and additive or mediating effects of learned resourcefulness and spirituality on general health perception in older adults were investigated. Social, emotional, and physical impact together predicted health perception. The social impact of arthritis was a significant independent predictor for spirituality. Furthermore, spirituality and health perception were significantly correlated.

Bovine embryos were exposed to bovine viral diarrhea (BVD) virus in vitro. An uptake of BVD virus by the embryos could not be detected by several assay systems. A significant decrease in the titer of BVD virus was found to occur when the virus was incubated in saline solution + 5% goat serum or minimal essential medium + 5% goat serum for 24 hours at 37 C. Since there was significant inactivation of the BVD virus during the incubation period, lack of viral infectivity of the embryos may have been due to adverse effects of the experimental environmental conditions on the virus or the embryos or upon viral-embryo interaction.
