The field of organ transplantation is experiencing a transformative shift with the rise of Advanced Therapy Medicinal Products (ATMPs), which include gene therapies, somatic cell therapies, and tissue-engineered products. These therapies offer new, potentially curative treatments for longstanding medical challenges, impacting numerous patients. However, their adoption is hindered by complex regulatory frameworks, high production costs, and inconsistent access across Europe. The ESOT ATMP Task Force’s position paper analyzes these challenges from research to clinical application, advocating for a coordinated strategy to position Europe as a leader in ATMP development. It proposes specific actions such as streamlining regulatory pathways to accelerate approvals, boosting funding for ATMP research, and creating specialized facilities for development and implementation. The paper also highlights the critical roles of patient engagement and real-world evidence in optimizing clinical and regulatory practices.

Abstract

Little is known either about either physical activity patterns, or other lifestyle-related prevention measures in heart transplantation (HTx) recipients. The history of HTx started more than 50 years ago but there are still no guidelines or position papers highlighting the features of prevention and rehabilitation after HTx. The aims of this scientific statement are (i) to explain the importance of prevention and rehabilitation after HTx, and (ii) to promote the factors (modifiable/non-modifiable) that should be addressed after HTx to improve patients’ physical capacity, quality of life and survival. All HTx team members have their role to play in the care of these patients and multidisciplinary prevention and rehabilitation programmes designed for transplant recipients. HTx recipients are clearly not healthy disease-free subjects yet they also significantly differ from heart failure patients or those who are supported with mechanical circulatory support. Therefore, prevention and rehabilitation after HTx both need to be specifically tailored to this patient population and be multidisciplinary in nature. Prevention and rehabilitation programmes should be initiated early after HTx and continued during the entire post-transplant journey. This clinical consensus statement focuses on the importance and the characteristics of prevention and rehabilitation designed for HTx recipients.

While allograft rejection (AR) continues to threaten the success of cardiothoracic transplantation, lack of accurate and repeatable surveillance tools to diagnose AR is a major unmet need in the clinical management of cardiothoracic transplant recipients. Endomyocardial biopsy (EMB) and transbronchial biopsy (TBBx) have been the cornerstone of rejection monitoring since the field’s incipience, but both suffer from significant limitations, including poor concordance of biopsy interpretation among pathologists. In recent years, novel molecular tools for AR monitoring have emerged and their performance characteristics have been evaluated in multiple studies. An international working group convened by ESOT has reviewed the existing literature and provides a series of recommendations to guide the use of these biomarkers in clinical practice. While acknowledging some caveats, the group recognized that Gene-expression profiling and donor-derived cell-free DNA (dd-cfDNA) may be used to rule out rejection in heart transplant recipients, but they are not recommended for cardiac allograft vasculopathy screening. Other traditional biomarkers (NT-proBNP, BNP or troponin) do not have sufficient evidence to support their use to diagnose AR. Regarding lung transplant, dd-cfDNA could be used to rule out clinical rejection and infection, but its use to monitor treatment response is not recommended.

Abstract

Performance testing with the aim of generating an efficient and effective workload to identify performance issues is challenging. Many of the automated approaches mainly rely on analyzing system models, source code, or extracting the usage pattern of the system during the execution. However, such information and artifacts are not always available. Moreover, all the transactions within a generated workload do not impact the performance of the system the same way, a finely tuned workload could accomplish the test objective in an efficient way. Model-free reinforcement learning is widely used for finding the optimal behavior to accomplish an objective in many decision-making problems without relying on a model of the system. This paper proposes that if the optimal policy (way) for generating test workload to meet a test objective can be learned by a test agent, then efficient test automation would be possible without relying on system models or source code. We present a self-adaptive reinforcement learning-driven load testing agent, RELOAD, that learns the optimal policy for test workload generation and generates an effective workload efficiently to meet the test objective. Once the agent learns the optimal policy, it can reuse the learned policy in subsequent testing activities. Abstract

An effective perception system is a fundamental component for farming robots, as it enables them to properly perceive the surrounding environment and to carry out targeted operations. The most recent methods make use of state-of-the-art machine learning techniques to learn a valid model for the target task. However, those techniques need a large amount of labeled data for training. A recent approach to deal with this issue is data augmentation through Generative Adversarial Networks (GANs), where entire synthetic scenes are added to the training data, thus enlarging and diversifying their informative content. In this work, we propose an alternative solution with respect to the common data augmentation methods, applying it to the fundamental problem of crop/weed segmentation in precision farming. Starting from real images, we create semi-artificial samples by replacing the most relevant object classes (i.e., crop and weeds) with their synthesized counterparts. To do that, we employ a conditional GAN (cGAN), where the generative model is trained by conditioning the shape of the generated object. Moreover, in addition to RGB data, we take into account also near-infrared (NIR) information, generating four channel multi-spectral synthetic images. Quantitative experiments, carried out on three publicly available datasets, show that (i) our model is capable of generating realistic multi-spectral images of plants and (ii) the usage of such synthetic images in the training process improves the segmentation performance of state-of-the-art semantic segmentation convolutional networks.

Abstract

The application of autonomous robots in agriculture is gaining increasing popularity thanks to the high impact it may have on food security, sustainability, resource use efficiency, reduction of chemical treatments, and the optimization of human effort and yield. With this vision, the Flourish research project aimed to develop an adaptable robotic solution for precision farming that combines the aerial survey capabilities of small autonomous unmanned aerial vehicles (UAVs) with targeted intervention performed by multi-purpose unmanned ground vehicles (UGVs). This paper presents an overview of the scientific and technological advances and outcomes obtained in the project. We introduce multi-spectral perception algorithms and aerial and ground-based systems developed for monitoring crop density, weed pressure, crop nitrogen nutrition status, and to accurately classify and locate weeds. We then introduce the navigation and mapping systems tailored to our robots in the agricultural environment, as well as the modules for collaborative mapping. We finally present the ground intervention hardware, software solutions, and interfaces we implemented and tested in different field conditions and with different crops. Abstract

This work addresses the problem of coupling vision-based navigation systems for Unmanned Aerial Vehicles (UAVs) with robust obstacle avoidance capabilities. The former problem is solved by maximizing the visibility of the points of interest, while the latter is modeled by means of ellipsoidal repulsive areas. The whole problem is transcribed into an Optimal Control Problem (OCP), and solved in a few milliseconds by leveraging state-of-the-art numerical optimization. The resulting trajectories are well suited for reaching the specified goal location while avoiding obstacles with a safety margin and minimizing the probability of losing the route with the target of interest. Combining this technique with a proper ellipsoid shaping (i.e., by augmenting the shape proportionally with the obstacle velocity or with the obstacle detection uncertainties) results in a robust obstacle avoidance behavior. We validate our approach within extensive simulated experiments that show effective capabilities to satisfy all the constraints even in challenging conditions. We release with this paper the open source implementation.
