Introduction: Since the beginning of the COVID-19 pandemic, nurses have been on the frontline providing care for patients with COVID-19. Caring for patients with COVID-19 can be a rewarding experience for nurses, but research also suggests that nurses experience numerous challenges on the frontline., Objectives: This study aims to explore the experiences of frontline nurses caring for patients with COVID-19 in Nigeria., Methods: Ten nurse volunteers working in a COVID-19 isolation center were purposively recruited. Data were collected with a semi-structured interview guide, and a template analysis approach was used to analyze the transcribed interviews., Results: The participants volunteered at the isolation center for safety reasons, professional gain, and concern for humanity. Working at the isolation center was accompanied by changes in working hours, work dynamics, care context and care tools. These changes resulted in personal, professional, and work-related challenges for nurses. However, team spirit, positive patient outcomes, gratitude and family support helped the nurses cope with the challenges., Conclusions: This research highlighted that working on the frontline of COVID-19 is associated with multiple and complex challenges that can impact nurses' personal and professional life. Background/objectives: Healthcare systems' adoption and sustenance of successful transitional care models (TCMs) have been limited by cost-prohibitive resource needs. Cost-effective TCMs that improve patient outcomes are needed to promote adoption by healthcare systems and sustainability. This study evaluated the effectiveness of a TCM utilizing community health workers (CHWs) in reducing inappropriate healthcare utilization and costs., Design: A cohort study with a pre-post intervention evaluation of the intervention group., Setting: A 953-bed academic urban safety-net hospital., Participants: Eligible participants (N = 154) were hospitalized or had repeated emergency room (ER) visits, identified to be at high risk for readmission., Intervention: Promotion of self-management skills acquisition and care coordination by CHWs achieved through predischarge interdisciplinary team meetings, regular home visits and phone contact, accompaniment to primary care physicians' (PCP) appointments, support with transportation, medications, and self-management education., Measurements: Outcome measures were readmissions, ER visits, and PCP establishment., Results: Mean age of participants was 67, 65% were male, 92% African American. There was a significant reduction in overall number of readmissions (Z = 9.6, p < 0.001), also observed at 30-day (Z = 5.5, p < 0.001), 3-month (Z = 4.3, p < 0.001), 6-month (Z = 4.0, p = 0.001), and 1-year (Z = 5.4, p < 0.001) post-intervention. There was a significant reduction in the overall number of ER visits (Z = 5.5, p < 0.001), also seen at 3-month (Z = 3.3, p < 0.001), 6-month (Z = 3.0, p < 0.001), and 1-year (Z = 4.0, p < 0.001) intervals. Care with a PCP was established in 86.6% of participants. Utilization costs were significantly lower post-intervention ($11,530,376.39 vs $4,017,493.17, p < 0.001)., Conclusion: Use of CHWs during transitions of care may be a cost-effective approach to reducing healthcare utilization and costs and may promote adoption and sustainability within healthcare systems., (© 2021 The American Geriatrics Society.)

Background: Meticillin-resistant Staphylococcus aureus (MRSA) is a leading cause of healthcare-associated infection in the neonatal intensive care unit (NICU). Decolonization may eliminate bacterial reservoirs that drive MRSA transmission., Aim: To measure the association between colonization pressure from decolonized and non-decolonized neonates and MRSA acquisition to inform use of this strategy for control of endemic MRSA., Methods: An eight-year retrospective cohort study was conducted in a level-4 NICU that used active surveillance cultures and decolonization for MRSA control. Weekly colonization pressure exposures were defined as the number of patient-days of concurrent admission with treated (decolonized) and untreated (non-decolonized) MRSA carriers in the preceding seven days. Poisson regression was used to estimate risk of incident MRSA colonization associated with colonization pressure exposures. The population-attributable fraction was calculated to assess the proportion of overall unit MRSA incidence attributable to treated or untreated patients in this setting., Findings: Every person-day increase in exposure to an untreated MRSA carrier was associated with a 6% increase in MRSA acquisition risk [relative risk (RR): 1.06; 95% confidence interval (CI): 1.01-1.11]. Risk of acquisition was not influenced by exposure to treated, isolated MRSA carriers (RR: 1.01; 95% CI: 0.98-1.04). In the context of this MRSA control programme, 22% (95% CI: 4.0-37) of MRSA acquisition could be attributed to exposures to untreated MRSA carriers., Conclusion: Untreated MRSA carriers were an important reservoir for transmission. Decolonized patients on contact isolation posed no detectable transmission threat, supporting the hypothesis that decolonization may reduce patient-to-patient transmission.
