Purpose: The purpose of this paper is to examine the way "moderation" is defined and operationalised at UK universities. It is hoped that this investigation provides pointers for modifications in university documentation and practices, as well as indicates possible areas for future research. Design/methodology/approach: This paper begins with a review of relevant scholarly literature, first tracing the history of the concept of moderation (essentially meaning the avoidance of extremes) in western thought and then proceeding to show how moderation is understood and operationalised in UK universities. Relevant documentation from 10 UK universities, all in the public domain, is analysed to show both commonalities and differences in definition and operationalisation of moderation. Findings: This paper shows that universities differ in their understanding of the scope of moderation, with some seeing it as covering the evaluation of draft assessment artifacts. It is also noted that the distinction between moderation and marking is not always expressed in university documentation in ways that distinguish between the two with maximal clarity. Research limitations/implications: Limitations include the relatively small sample of documents examined. Conversely, ancient universities (e.g. Edinburgh), long-established universities (e.g. Manchester) and 1992 and more recent universities (e.g. Manchester Metropolitan University and Suffolk) are contained in the sample, so moderation practices from across the sector are included. Practical implications: The main findings are that some universities see moderation as including drafting, redrafting and approval of assessment artifacts, whereas others do not. In addition, although all universities stress that moderation and marking are separate activities, some documentation discusses both the activities in tandem; thus, undermining the contention that they are discrete. Both these findings have implications for UK university documentation in terms of both document structure and precise wording. Originality/value: The approach taken, in which a sample of publicly available university documents is scrutinised and evaluated, casts a new light on understandings of "moderation", which is a term and concept that may not always be examined critically by lecturers and quality assurance professionals involved in higher education.

Purpose: Human papillomavirus (HPV) causes an estimated 300,000 high grade cervical dysplasias and 36,000 preventable cancers each year in the United States alone. Despite having a safe, effective and long lasting vaccine since 2006, the rate of uptake has been suboptimal, particularly in rural areas. In 2020, Utah ranked near last in teenage HPV vaccination rates with rural areas trailing urban areas by about 18 percent. In this study, we identified factors that affect the intent of rural Utah parents to vaccinate their children against HPV. Methods: A survey was distributed electronically to Utah residents in rural areas. Recruitment was carried out through targeted advertising, community organizations, and professional survey panels. The survey was open from Nov. 15, 2022 to April 15, 2023. A total of 410 respondents were used for analysis. Survey results were analyzed using exploratory factor analysis, confirmatory factor analysis, and structural equation modeling. Findings: Distance to care was shown to negatively influence direct intent to vaccinate, while trust in government, general vaccine attitudes, and HPV knowledge positively influence intent to vaccinate. It was found that religious practice decreased vaccine hesitancy while cautious sexual attitudes, distance to care, and general negative vaccine attitudes increased vaccine hesitancy. Background: Seasonal influenza vaccination rates are very low among teenagers. Objectives: We used publicly available data from the NIS-Teen annual national immunization survey to explore factors that influence the likelihood of a teen receiving their seasonal flu shot. Methods: Traditional stepwise multivariable regression was used in tandem with machine learning to determine the predictive factors in teen vaccine uptake. Results and Conclusions: Age was the largest predictor, with older teens being much less likely to be vaccinated than younger teens (97.48% compared to 41.71%, p < 0.0001). Provider participation in government programs such as Vaccines for Children and the state vaccine registry positively impacts vaccine uptake (p < 0.0001). Identifying as non-Hispanic Black was a small, negative predictor of teen vaccine uptake (78.18% unvaccinated compared to 73.78% of White teens, p < 0.0001). The state quartile for COVID-19 vaccine uptake also strongly predicted flu vaccine uptake, with the upper quartile of state COVID-19 vaccine uptake being significantly more likely to also get vaccinated for influenza (76.96%, 74.94%, 74.55%, and 72.97%, p < 0.0001). Other significant factors are the number of providers, education of the mother, poverty status, and having a mixed provider facility type. Additionally, the multivariable regression analysis revealed little difference in the predictive factors of vaccine uptake between pre- and post-pandemic datasets. [ABSTRACT FROM AUTHOR]

Acquired immunodeficiency syndrome (AIDS) occurs when HIV depletes CD4+ helper T cells. Some patients develop AIDS slowly or not at all, and are termed long-term non-progressors (LTNP), and while mutations in the HIV-1 Viral Protein R (vpr) gene such as R77Q are associated with LTNP, mechanisms for this correlation are unclear. This study examines the induction of apoptosis, cell cycle arrest, and pro-inflammatory cytokine release in the HUT78 T cell line following infection with replication-competent wild-type strain NL4-3, the R77Q mutant, or a vpr Null mutant. Our results show a significant enhancement of apoptosis and G2 cell cycle arrest in HUT78 cells infected with R77Q, but not with WT NL4-3 or the vpr Null strain. Conversely, HUT78 cells infected with the WT virus show higher levels of necrosis. We also detected lower TNF and IL-6 release after infection with R77Q vs. WT. The apoptotic phenotype was also seen in the CEM cell line and in primary CD4+ T cells. Protein expression of the R77Q vpr variant was low compared to WT vpr, but expression levels alone cannot explain these phenotypes because the Null virus did not show apoptosis or G2 arrest. These results suggest that R77Q triggers a non-inflammatory apoptotic pathway that attenuates inflammation, possibly contributing to LTNP. [ABSTRACT FROM AUTHOR]

Background/Objectives: Systemic lupus erythematosus (lupus) and B-cell lymphoma (lymphoma) co-occur at higher-than-expected rates and primarily depend on B cells for their pathology. These observations implicate shared inflammation-related B cell molecular mechanisms as a potential cause of co-occurrence. Methods: We consequently implemented a novel Immune Imbalance Transcriptomics (IIT) algorithm and applied IIT to lupus, lymphoma, and healthy B cell RNA-sequencing (RNA-seq) data to find shared and contrasting mechanisms that are potential therapeutic targets. Results: We observed 7143 significantly dysregulated genes in both lupus and lymphoma. Of those genes, we found 5137 to have a significant immune imbalance, defined as a significant dysregulation by both diseases, as analyzed by IIT. Gene Ontology (GO) term and pathway enrichment of the IIT genes yielded immune-related "Neutrophil Degranulation" and "Adaptive Immune System", which validates that the IIT algorithm isolates biologically relevant genes in immunity and inflammation. We found that 344 IIT gene products are known targets for established and/or repurposed drugs. Among our results, we found 48 known and 296 novel lupus targets, along with 151 known and 193 novel lymphoma targets. Known disease drug targets in our IIT results further validate that IIT isolates genes with disease-relevant mechanisms. Conclusions: We anticipate the IIT algorithm, together with the shared and contrasting gene mechanisms uncovered here, will contribute to the development of immune-related therapeutic options for lupus and lymphoma patients. The doctoral viva voce examination has existed in Britain since the PhD (or DPhil) was introduced at Oxford University almost a hundred years ago. However, despite some recent research studies, it seems that the viva remains somewhat under-researched and that viva voce examinations continue to take place according to largely unchanged and unchallenged procedures. This paper presents evidence collected via questionnaire and interview from a sample of UK academics working in education departments at 16 universities in England. It is shown that individuals within the sample hold contrasting views about the purpose, value, and degree of reliability associated with oral assessment at doctoral level. The paper critically examines eight aspects of the viva's design, purpose, conduct, and outcomes, under such headings as "examiner judgement," "examiner behaviour/attitudes" and "the viva as opportunity." The question of whether examiners should have the power to waive the viva under certain circumstances is also addressed. Rather than attempting to provide definitive answers to such questions, the paper highlights a series of inter-related issues which seem problematic. The author's intention is to spark further discussion in academia about the viva's current and possible future content, conduct, and purposes.

This paper summarises and then critiques some of the key arguments and conclusions provided by Wellington who argues that it would be sensible to give up the search for the "inner essence" of "doctorateness" (a term referring to a common quality of doctoral candidates or of their written dissertations). It is argued here that progress towards description of doctorateness has in fact already been made, and it is proposed that this can be built on by promoting further discussion focusing on issues such as whether doctorateness should be seen as residing in doctoral theses, doctoral candidates, or both; and by recognising the polysemous nature of concepts like "originality" and "new knowledge" which often arise in university documentation relating to work at doctoral level.

Enormous distances across the vast South Pacific hinder student access to the main Fiji campus of the regional tertiary education provider, the University of the South Pacific (USP). Fortunately, USP has been a pioneer in distance education (DE) and promotes multi-modal delivery of programmes. Geography has embraced DE, but doubts remain about comparability of various modes of study. This paper offers a diachronic examination of final grade performance of undergraduate physical geography students learning via three different modes. Data collected over 10 years on the pass rates of greater than 1300 students are analysed. Various influences on student achievement are highlighted. (Contains 9 notes, 7 tables, and 2 figures.)

Purpose: The aims of this paper are twofold: first, to engage with the definition of quality as "excellence" and to show why this could be regarded as unhelpful and misleading; and, second, to suggest some factors which contribute to a "cultural divide" between quality assurance specialists in universities and their colleagues who are full-time academics. In both cases the paper seeks to raise and explore these issues because their resolution may suggest ways forward for quality assurance in higher education. Design/methodology/approach: The paper is generally based on consideration and critical evaluation of published work relevant to the two issues mentioned previously. However, other forms of evidence are drawn into the argument. Notably, lexical data from the British National Corpus are examined in order to substantiate points about the semantics of the word "quality". Findings: The paper finds, on the grounds of both lexical semantics and consideration of scholarly literature on quality assurance in higher education, that it is unhelpful to understand the term "quality" as equivalent to "excellence". It also identifies possible reasons why a "cultural divide" exists between university lecturers and quality assurance specialists. Originality/value: The paper should be of interest to both quality assurance specialists and lecturers in universities. It offers logical, language-based reasons why "quality" should not be regarded as "excellence" and goes on to relate this to the notion of "quality enhancement". Preliminary suggestions are also made about means through which the "cultural divide" between academics and quality assurance specialists might be narrowed, to the potential benefit of universities seen as both complex entrepreneurial organizations, and academic communities. In an article published a little over a decade ago (Betteridge, 2009), the journalist Ian Betteridge offered some scathing comments about a piece published a few days earlier in TechCrunch by Erick Schonfeld (Schonfeld, 2009). Amongst other things, Betteridge suggested that the headline concerned ('Did Last.fm Just Hand Over User Listening Data to the RIAA?') was 'a great demonstration of my maxim that any headline which ends in a question mark can be answered by the word "no".' Readers of English Today will realise immediately that this 'maxim' cannot possibly be watertight as expressed by Betteridge, since only polar questions can be answered with a 'yes' or 'no'. For example, WH-questions (Quirk & Greenbaum, 1973: 196) such as 'Who opened my letter?' and 'How long have you been waiting?' obviously cannot be responded to in any sensible way with a simple 'yes' or 'no'. Furthermore, it is not difficult to find media headlines taking the form of non-polar questions: for example, "What would a no-deal Brexit mean for business?" (O'Dwyer et.al., 2020). The pharmacological treatment of systemic lupus erythematosus (SLE) aims to decrease disease activity, progression, systemic compromise, and mortality. Among the pharmacological alternatives, there are chemically synthesized drugs whose efficacy has been evaluated, but which have the potential to generate adverse events that may compromise adherence and response to treatment. Therapy selection and monitoring will depend on patient characteristics and the safety profile of each drug. The aim of this review is to provide a comprehensive understanding of the most important synthetic drugs used in the treatment of SLE, including the current treatment options (mycophenolate mofetil, azathioprine, and cyclophosphamide), review their mechanism of action, efficacy, safety, and, most importantly, provide monitoring parameters that should be considered while the patient is receiving the pharmacotherapy.
