Purpose. This study investigates the prognostic significance of tumor size and its effect on survival among patients undergoing gastrectomy and D2 lymph node dissection due to gastric cancer. Materials and Methods. The clinicopathological characteristics of 320 patients who were operated due to gastric cancer between November 2006 and September 2019 were assessed retrospectively, of which 271 were included in the present study. A receiver-operating characteristic curve (ROC) analysis was carried out to identify the tumor size cut-off value. Patients were divided into small-size and large-size tumor groups. Clinicopathological characteristics were assessed using Chi-square and Mann-Whitney U tests, while survival was assessed with a Kaplan-Meier log-rank test. Results. Introduction: Colorectal cancer (CRC) prognosis is typically determined based on clinical stage and histopathological findings, yet patients with the same stage and histological structure can exhibit varying survival outcomes. This highlights the need for additional prognostic biomarkers. Serum biomarkers are gaining increasing significance due to their affordability and accessibility. The albumin-alkaline phosphatase ratio (AAPR) has been associated with prognosis in hepatocellular and gastric cancers, but its role in CRC remains underexplored. Aim: This study aimed to evaluate the effect of the albumin-alkaline phosphatase ratio (AAPR) on the prognosis of patients with colorectal cancer (CRC). Material and method: Data from 358 patients who had undergone surgery for CRC were analyzed retrospectively to identify factors that could predict overall survival (OS). The Roc-Curve test was applied to determine the power of the preoperative AAPR in predicting mortality. Kaplan Meier and log-rank tests were used to examine the survival times of the patients. Results: Our findings revealed that an albumin-alkaline phosphatase cut-off ratio above 0.67 predicted mortality with a sensitivity of 17.54% and a specificity of 92.22%. Although patients with a lower AAPR exhibited a slightly shorter mean survival time compared to those above the cut-off value, this difference did not reach statistical significance (P = .112). Conclusions: The results of this study did not provide evidence to support the AAPR as a potential prognostic factor in patients with colorectal cancer. [ABSTRACT FROM AUTHOR]

Introduction: Minimal invasive surgery techniques are getting more popular by surgeons relying their advantages such as pain control, feasibility and increased affinity. Both techniques have similar prognostic influence regarding survival in colon cancer but increased versatility of laparoscopy in years shows more tendency of laparoscopy among surgeons. This study aims to evaluate of surgical site infection (SSI) rates between conventional and laparoscopic colon cancer procedures. Materials and Methods: Patients operated due to colon adenocarcinoma between 2018 and 2023 evaluated. Emergency, palliative or incomplete resections excluded. Demographic, pathologic, peroperative and postoperative records of patients evaluated. Patients seperated into groups by SSI occurance and surgical method choice. Results: SSI development was found higher in conventional surgery group (30.0% vs 11.6%; p=0.013). In comparison of patients by SSI development; only intraoperative Red Blood Concentrate (RBC) replacement founded to be higher in SSI (+) group (0±1 vs 0±1; p=0.002). All variables associated with SSI development were subjected to univariate regression analysis. It's shown that only conventional surgery choice was a indipendant risk factor for SSI development (OR: 3.489 (1.289 - 9.415); p=0.017). Conclusion: Laparoscopic colon surgery has better SSI rates than conventional colon surgery procedures. Our findings are similar with the general view on SSI ratio's between two surgical practices. Introduction: The main objective of this study is to present 10 Plummer-Vinson Syndrome cases treated and followed up at our clinic alongside cases in current literature. Materials and Methods: The cases of 10 patients with prospective records of Plummer-Vinson Syndrome treated and followed up in the Gastroenterological Surgery Clinic of the hospital were evaluated. Results: Seven (70%) of the patients were female, and three (30%) were male, with a mean age of 45±18. All the patients had a mean hemoglobin value of 8.4±0.94 g/dL and a mean erythrocyte volume level of 63±5.01 fL, and their ferritin levels were 6.5±5.42 ng/dL, which accounted for iron deficiency in the patients. With barium swallow studies before endoscopy, all patients were shown to have esophageal webs. All patients underwent endoscopic balloon dilatation under sedoanalgesia. Three cases of recurrence were observed, and those patients underwent the balloon dilatation process again. Squamous cell carcinoma in the distal esophagus was detected in one case in the 72nd month of follow-up. Conclusion: Endoscopic balloon dilatation together with oral iron replacement is a safe, simple, and efficient mode of treatment. As Plummer-Vinson Syndrome is regarded as a precancerous condition, endoscopic follow-up is required for subsequent treatment. [ABSTRACT FROM AUTHOR]

Introduction: The laparoscopic approach is preferred in colorectal cancer cases, yet concerns arise regarding the development of complications among the elderly patient population. This study aims to investigate the difference in the development of major complications between patients aged 65 and older undergoing laparoscopic procedures for colorectal cancer diagnoses and the younger patients. Materials and Methods: Between 2013 and 2023, records of patients who underwent laparoscopic surgery due to colorectal cancers at our center were extracted. Demographic characteristics, pathology data, complications developed during hospitalization, and lengths of stay were gathered from hospital records. Results: A total of 72 patients were included in the study, with 18 patients aged 65 and older and 54 patients aged younger than 65. When patients were evaluated based on the development of major and minor complications, all demographic and pathological characteristics were found to be similar. Only in the group of patients aged 65 and older, the length of hospital stay was found to be longer (p<0.001). In the multivariate analysis conducted, age was not found to be significant for the development of major complications (OR: 0.895 [0.246-3.264], p=0.897). Conclusion: This study has shown that being aged 65 or older or younger is not associated with the development of major complications in patients undergoing surgery for colorectal cancer. It has also demonstrated that laparoscopic surgical procedures can be safely used in patients aged 65 and older. Further studies with larger patient cohorts could provide more clarity on this topic. Introduction: Cholecystitis is associated with an increased risk of morbidity and mortality in patients under immunosuppressive therapy after lung transplantation. Therefore, cholecystectomy is recommended in patients with asymptomatic cholelithiasis who are candidates for lung transplantation. A pre-transplant prophylactic laparoscopic cholecystectomy approach may reduce this morbidity and mortality in lung transplant candidates with cholelithiasis. In this study, we aimed to examine our experience with laparoscopic cholecystectomy before lung transplantation. Materials and Methods: The data of eight lung transplant candidates who underwent prophylactic laparoscopic cholecystectomy for asymptomatic cholelithiasis were evaluated retrospectively. Results: Of the cases examined, 2 (25%) were female and 6 (75%) were male. The mean age was 34.5 for women and 44.16 for men. No postoperative morbidity or mortality was observed. Conclusion: Prophylactic laparoscopic cholecystectomy can be safely performed before transplantation in patients who are candidates for lung transplantation. [ABSTRACT FROM AUTHOR]

Introduction: Postoperative infections (POIs) are associated with prolonged postoperative recovery, delayed adjuvant therapy, psychological problems, and poor long-term outcomes. Aim: The study aims to cross-compare the ratio-based preoperative parameters to predict POIs in patients with D2 gastrectomy for gastric cancer. Materials and Methods: A retrospective cohort and single-center study evaluated the data of 293 patients who underwent curative gastrectomy between January 2007 and November 2019 in a tertiary hospital in Istanbul. A receiver operating characteristic (ROC) curve was used to assess the ability of laboratory values to predict clinically relevant POIs. The predictive capacity of the neutrophil-lymphocyte ratio (NLR), lymphocyte-monocyte ratio (LMR), platelet-lymphocyte ratio (PLR), and CRPalbumin ratio (CAR) was calculated by the area under the curve. Then, the cutoff points were determined for all 4 indexes. Results: POIs developed in 77 (26.2%) patients. Patients with POI had higher Charlson Comorbidity Index (CCI) scores and a longer length of hospital stay. ROC curve analysis revealed that NLR, LMR, and CAR were significantly effective in predicting POI, while PLR was ineffective. LMR was the best ability to predict POI. According to multivariate analysis, CCI score ≥ 3, NLR > 3.8, and LMR ≤ 2.34 were independent risk factors influencing POI. Conclusion: Preoperative LMR was most predictive for POI. Although CAR predicted the development of POI, it was not superior to LMR or NLR. PLR did not have any prediction for POI. Maggot debridement therapy (MDT) has been used for years in the treatment of chronic wounds and necrotic tissues. We report a case of subtotally amputated third toe that was treated with MDT after reattachment and developing complete necrosis. The necrotic toe was replaced with viable tissue and the wound healed completely after 2 weeks of MDT application. This case points out the regenerative effects of MDT besides its mechanical debridement effect on the necrotic tissue. [ABSTRACT FROM AUTHOR]

Introduction: In this study, we aimed to present our experience and findings in high-risk cardiac patients who were inserted percutaneous endoscopic gastrostomy (PEG) tube due to inability to take oral food, prolonged intravenous/nasogastric nutrition, or require long-term enteral feeding. Materials and Methods: A total of 64 patients were examined retrospectively who had PEG tube insertion between 2012 and 2020 in the intensive care unit or clinic by the gastrointestinal surgeon. All patients underwent cardiac surgery before feeding tube insertion. The necessity and short-term results for PEG were evaluated in this patient group. Results: A total of 64 patients underwent upper gastrointestinal endoscopy for insertion of a PEG tube. The procedure was successful in all patients and no complications were observed in the follow-up. Twenty-five (39.1%) of 64 patients were women. The mean age of the patients was 67.04 (±11.44) years. The number of patients for diabetes mellitus, hypertension, atrial fibrillation, cerebrovascular disease, chronic kidney disease, and chronic obstructive/restrictive pulmonary disease was 36 (56.3%), 53 (82.8%), 21 (32.8%), 19 (29.7%), 21 (32.8%), and 15 (23.4%), respectively. The mean hospital stay of the patients after cardiac surgery was 37.81 (±12.81) days, and the mean feeding from PEG tubes was 13.34 (±4.93) days. Conclusion: Patients with high-risk factors who have undergone cardiac surgery are more likely to need a PEG tube. This patient group should be evaluated well in the pre-operative period. Upper gastrointestinal endoscopy should be performed in patients with gastrointestinal symptoms.
