Background Increasing evidence suggests that infection with the nonpathogenic human pegivirus type 1 (HPgV-1) exerts a clinical benefit in human immunodeficiency virus (HIV) patients, which could be attributable to immunomodulatory effects. Whether this impact can be extrapolated to kidney transplantation (KT) remains largely unknown. Methods We measured plasma HPgV-1 RNA by real-time polymerase chain reaction targeting the 5 ' untranslated region at various points (pretransplantation, day 7, months 1, 3, 6, and 12) in 199 KT recipients. Study outcomes included posttransplant serious infection, immunosuppression-related adverse event (opportunistic infection and/or de novo cancer), and acute graft rejection. Results HPgV-1 infection was demonstrated in 52 (26.1%) patients, with rates increasing from 14.7% at baseline to 19.1% by month 12 (p-value = .071). De novo infection occurred in 13.8% of patients with no detectable HPgV-1 RNA before transplantation. Double-organ (liver-kidney or kidney-pancreas) transplantation (odds ratio [OR]: 5.62; 95% confidence interval [CI]: 1.52-20.82) and donation after brain death (OR: 2.21; 95% CI: 1.00-4.88) were associated with posttransplant HPgV-1 infection, whereas pretransplant hypertension was protective (OR: 0.23; 95% CI: 0.09-0.55). There were no significant differences in the incidence of study outcomes according to HPgV-1 status. Plasma HPgV-1 RNA levels at different points did not significantly differ between patients that subsequently developed outcomes and those remaining free from these events. No correlation between HPgV-1 RNA and immune parameters or torque teno virus DNA load was observed either. Conclusion Unlike patients living with HIV, HPgV-1 infection does not seem to influence patient or graft outcomes after KT.

Background. The recurrence of proteinuria after kidney transplantation (KT) is a characteristic complication of focal segmental glomerulosclerosis (FSGS). It has been suggested that pre-emptive rituximab might prevent recurrences in patients at risk, but there is no agreement about which factors might help to identify such patients. Methods. We studied 93 kidney transplants with biopsy-proven idiopathic FSGS in order to analyse if preventive rituximab treatment could decrease recurrences in patients at risk. Results. Fifteen patients (16.1%) presented a recurrence after KT, but when we restricted the analysis to the 34 patients presenting nephrotic syndrome at primary disease onset, the recurrence diagnosis rate increased to 44.1%. All patients with recurrence had complete nephrotic syndrome at the time of diagnosis. After multivariate adjustment, the only significant risk factor for recurrence was the presence of complete nephrotic syndrome at diagnosis. Twelve of the 34 patients at risk for recurrence received rituximab at the time of transplantation. Clinical and analytical characteristics were similar in all patients at risk. The number of recurrences was similar among treated (50%) and non-treated patients (40.9%). Conclusions. Nephrotic syndrome with hypoalbuminaemia at diagnosis is the most important feature to identify patients at risk of recurrence. Our data suggest that pre-emptive rituximab is not effective to prevent FSGS recurrences.

BACKGROUND: The magnitude and kinetics of severe acute respiratory syndrome coronavirus 2-specific cell-mediated immunity (SARS-CoV-2-CMI) in kidney transplant (KT) recipients remain largely unknown. METHODS: We enumerated SARS-CoV-2-specific interferon-gamma-producing CD69+ CD4+ and CD8+ T cells at months 4 and 6 from the diagnosis of coronavirus disease 2019 (COVID-19) in 21 KT recipients by intracellular cytokine staining. Overlapping peptides encompassing the SARS-CoV-2 spike (S) glycoprotein N-terminal 1- to 643-amino acid sequence and the membrane protein were used as stimulus. SARS-CoV-2 IgG antibodies targeting the S1 protein were assessed by ELISA at month 6. RESULTS: Detectable (=0.1%) SARS-CoV-2-specific CD4+ T-cell response was found in 57.1% and 47.4% of patients at months 4 and 6. Corresponding rates for CD8+ T cells were 19.0% and 42.1%, respectively. Absolute SARS-CoV-2-specific T-cell counts increased from month 4 to month 6 in CD8+ (P=0.086) but not CD4+ subsets (P=0.349). Four of 10 patients with any detectable response at month 4 had lost SARS-CoV-2-CMI by month 6, whereas 5 of 9 patients mounted SARS-CoV-2-CMI within this period. All but 2 patients (89.5%) tested positive for SARS-CoV-2 IgG. Patients lacking detectable SARS-CoV-2-specific CD4+ response by month 6 were more likely to be under tacrolimus (100.0% versus 66.7%; P=0.087) and to have received tocilizumab for the previous COVID-19 episode (40.0% versus 0.0%; P=0.087). CONCLUSIONS: Although still exploratory and limited by small sample size, the present study suggests that a substantial proportion of KT recipients exhibited detectable SARS-CoV-2-CMI after 6 months from COVID-19 diagnosis.

Background & Aims: The development of direct-acting antiviral agents (DAAs) is a major step forward in the treatment of hepatitis C (HCV). The aims of the study were to evaluate the efficacy and tolerability of DAAs in kidney transplant (KT) recipients. Methods: Hepa-C is a Spanish registry of patients treated with DAAs in which clinical, virological and analytical data were prospectively included. We report on the data from 103 KT recipients who received DAAs. Results: The most commonly used DAAs combinations were sofosbuvir/ledipasvir (n = 59, 57%) and sofosbuvir + daclatasvir (n = 18, 17%). Ribavirin was used in 41% of patients. Sustained viral response after 12 weeks (SVR12) rate was 98%. Grade 2 or 3 anemia appeared in 14 (33%) of patients receiving ribavirin and in 9 (15%) without (p = 0.03). There were three episodes of acute humoral graft rejection. No patient discontinued therapy due to adverse events. Importantly, 57 (55%) patients required immunosuppression dose adjustment. Overall, there were no statistically significant differences in the mean level of serum creatinine, eGFR and proteinuria before and after treatment. Nonetheless, seventeen (16%) patients experienced renal dysfunction (increase in serum creatinine >25%) during antiviral therapy, of whom 65% were cirrhotic in comparison with only 29% cirrhotic patients who did not develop significant renal dysfunction (p = 0.004). Conclusions: Antiviral therapy with DAAs was highly efficacious and safe in KT recipients. Nevertheless, a non-negligible number of patients, most of them cirrhotic, developed mild allograft dys-function and a significant proportion of patients required immunosuppression dose adjustment, warranting a close follow-up during therapy. Lay summary: Infection by hepatitis C virus is often found in kidney transplant patients and its presence increases mortality and graft failure. We investigated the efficacy and safety of the new direct-acting hepatitis C antivirals in this population, in which previous information is scarce. Our data shows that, as occurs in the non-transplant setting, new anti-HCV antivirals are highly efficacious kidney transplant patients. Overall, this therapy is also quite safe, although worsening of renal function is observed in 16% of patients warranting a close follow-up observation of graft function during antiviral therapy. Fundamento: En el año 2010 Euskadi puso en marcha la «Estrategia para afrontar el reto de la cronicidad» tratando de reorientar las organizaciones sanitarias hacia modelos más integrados que proporcionaran una respuesta de calidad y más eficiente al paciente crónico. Se han creado cuatro Organizaciones Sanitarias Integradas (OSI) que suponen la integración en una única organización de los dos niveles asistenciales: atención primaria y especializada. El objetivo del estudio es conocer el grado de desarrollo alcanzado por las cuatro OSI en su estrategia de gestión integrada de la cronicidad tomando como referente de la evaluación el Chronic Care Model. Material y métodos: Autoevaluación en las cuatros OSI con la herramienta IEMAC por parte del equipo directivo de cada organización. Resultados: La puntuación promedio es de 16 puntos sobre 100, lo que implica que las organizaciones están emprendiendo planes de acción en el ámbito de la gestión de la atención integrada, pero con un despliegue limitado y sin que se puedan aportar evidencias de evaluación de los resultados de las acciones puestas en marcha. La dimensión mejor valorada fue «Organización del Sistema de Salud», mientras que la menos valorada fue "Salud Comunitaria". Conclusiones: Las OSI son motores del cambio para lograr una atención integral a la cronicidad. Los aspectos relacionados con la integración de recursos comunitarios y la evaluación de resultados constituyen dos de las áreas de mejora para desplegar el conjunto de intervenciones propuestas en IEMAC. Los cambios organizativos que supone el CCM requieren de períodos de tiempo superiores a dos años. Background: In 2010 the Basque Government launched its "Strategy for tackling the challenge of chronicity in the Basque Country", in order to transform healthcare organisations into more integrated care models, with the aim of improving quality and efficiency in chronicity management. Four Integrated Healthcare Organisations (IHOs) were created to unify primary and specialised care into one single organisation. The aim of this study is to measure the degree of readiness of these IHOs to cope with chronicity, using the Chronic Care Model (CCM) as a reference. Material and methods: Self-assessment processes using ARCHO (Assessment of Readiness for Chronicity in Health Care Organisations) were carried out in four IHOs by the management teams. Results: The average score was 16 out of 100 points, which signals that healthcare organisations are undertaking action plans in the realm of integrated care, but with limited deployment and without a systematic process for evaluating outcomes. The dimension that ranks best is "Organization of the health system", while "Community health" has the lowest ranking. Conclusions: IHOs are the key for achieving integrated care for chronic illnesses. Integration of community resources and evaluation of results are two of the fields that need to be improved in order to achieve the set of interventions proposed in ARCHO. The organisational changes involved in the CCM require periods longer than two years.

Acute kidney injury (AKI) in the intensive care unit (ICU) is associated with an enhanced mortality. The Acute Dialysis Quality Initiative group has proposed the RIFLE (Risk–Injury–Failure–Loss–ESRD) classification to standardize the approach to AKI. This study was performed to estimate the AKI incidence in ICU patients in northeastern Italy and describe clinical characteristics and outcomes of patients with AKI on the basis of their RIFLE class. A prospective multicenter observational study was performed of patients who fulfilled AKI criteria in 19 ICU in northeastern Italy. Data were analyzed using multivariate logistic regression and survival curve analysis. Of 2164 ICU patients who were admitted during the study period, 234 (10.8%; 95% confidence interval 9.5 to 12.1%) developed AKI; 19% were classified as risk (R), 35% as injury (I), and 46% as failure (F). Preexisting kidney disease was present in 36.8%. The most common causes of AKI were prerenal causes (38.9%) and sepsis (25.6%). At diagnosis of AKI, median serum creatinine and urine output were 2.0 mg/dl and 1100 ml/d, respectively. ICU mortality was 49.5% in class F, 29.3% in I, and 20% in R. Independent risk factors for mortality included RIFLE class, sepsis, and need for renal replacement therapy, whereas a postsurgical cause of AKI, exposure to nephrotoxins, higher serum creatinine, and urine output were associated with lower mortality risk. In this study, AKI incidence in the ICU was between 9 and 12%, with 3.3% of ICU patients requiring renal replacement therapy. Sepsis was a significant contributing factor. Overall mortality was between 30 and 42%, and was highest among those in RIFLE class F.

El objetivo del presente trabajo fue estimar la variabilidad y las relaciones filogenéticas entre 6 razas de ganado criollo colombiano, cebú Brahman y la española Pirenaica, mediante el uso de 12 marcadores moleculares tipo microsatélite. El número promedio de alelos fue de 11.58 con los valores más altos para los marcadores HEL 13 y ETH 10. La heterocigocidad promedio de todas las razas fue de 0.7 y el coeficiente de endogamia fue de 0.097, con los valores más altos para la raza Romosinuano. Los mayores valores de diversidad se presentaron en la raza Casanareño. La distancia genética entre las razas criollas colombianas y la raza española Pirenaica fue amplia (0.7-1.7) indicando posiblemente poca intervención de esta raza en el origen de las razas criollas. En general, las mayores distancias genéticas se presentaron entre las razas de origen Bos taurus (razas criollas colombianas y Pirenaica) con respecto a la Bos índicus (Brahman). - Abstract
The World Health Organization defines emerging and re-emerging infectious diseases as "that appeared in some population for the first time or have previously existed, but quickly increase the incidence in the geographical area". Tuberculosis (Tb) is one of the most deadly reemerging diseases. The discovery of isoniazid and other drugs initially led to the effective cure of Tb, emptying and dismantling sanatoriums control systems of Public Health in developed countries; its reemergence has been driven by immune deficiencies of people with HIV, which greatly increased the risk of latent infection with Mycobacterium tuberculosis progressing to active disease and is transmitted to others; the inappropriate use of antituberculosis therapy also complicates the problem, leading to the emergence and spread of drug-resistant and multidrug-resistant strains. Although Tb can affect all people, regardless of their social status, there are risk groups such as chronically immunosuppressed, being transplanted kidney "the forgotten group" by the need for drug therapy to prevent and treat rejection of their grafts, they live predisposed to infection with catastrophic natural evolution outside not be diagnosed and treated early, and the fact of facilitating the re-emergence and persistence of the disease as endemic in Mexico. - Abstract
INTRODUCTION: Severe sepsis and septic shock are common problems in the intensive care unit and carry a high mortality. Endotoxin, one of the principal components on the outer membrane of gram-negative bacteria, is considered important to their pathogenesis. Polymyxin B bound and immobilized to polystyrene fibers (PMX-F) is a medical device that aims to remove circulating endotoxin by adsorption, theoretically preventing the progression of the biological cascade of sepsis. We performed a systematic review to describe the effect in septic patients of direct hemoperfusion with PMX-F on outcomes of blood pressure, use of vasoactive drugs, oxygenation, and mortality reported in published studies. METHODS: We searched PubMed, the Cochrane Collaboration Database, and bibliographies of retrieved articles and consulted with experts to identify relevant studies. Prospective and retrospective observational studies, pre- and post-intervention design, and randomized controlled trials were included. Three authors reviewed all citations. We identified a total of 28 publications - 9 randomized controlled trials, 7 non-randomized parallel studies, and 12 pre-post design studies - that reported at least one of the specified outcome measures (pooled sample size, 1,425 patients: 978 PMX-F and 447 conventional medical therapy). RESULTS: Overall, mean arterial pressure (MAP) increased by 19 mm Hg (95% confidence interval [CI], 15 to 22 mm Hg; p < 0.001), representing a 26% mean increase in MAP (range, 14% to 42%), whereas dopamine/dobutamine dose decreased by 1.8 microg/kg per minute (95% CI, 0.4 to 3.3 microg/kg per minute; p = 0.01) after PMX-F. There was significant intertrial heterogeneity for these outcomes (p < 0.001), which became non-significant when analysis was stratified for baseline MAP.
