- Abstract
Background The association of childhood leukemia with traffic pollution was considered in a number of studies from 1989 onwards, with results not entirely consistent and little information regarding subtypes. Aim of the study We used the data of the Italian SETIL case-control on childhood leukemia to explore the risk by leukemia subtypes associated to exposure to vehicular traffic. Methods We included in the analyses 648 cases of childhood leukemia (565 Acute lymphoblastic–ALL and 80 Acute non lymphoblastic-AnLL) and 980 controls. Information on traffic exposure was collected from questionnaire interviews and from the geocoding of house addresses, for all periods of life of the children. Results We observed an increase in risk for AnLL, and at a lower extent for ALL, with indicators of exposure to traffic pollutants. In particular, the risk was associated to the report of closeness of the house to traffic lights and to the passage of trucks (OR: 1.76; 95% CI 1.03–3.01 for ALL and 6.35; 95% CI 2.59–15.6 for AnLL). The association was shown also in the analyses limited to AML and in the stratified analyses and in respect to the house in different period of life. Conclusions Results from the SETIL study provide some support to the association of traffic related exposure and risk for AnLL, but at a lesser extent for ALL. Our conclusion highlights the need for leukemia type specific analyses in future studies. Results support the need of controlling exposure from traffic pollution, even if knowledge is not complete.

20. Time-Frequency-Wavenumber Analysis of Surface Waves Using the Continuous Wavelet Transform

- Abstract
A modified approach to surface wave dispersion analysis using active sources is proposed. The method is based on continuous recordings, and uses the continuous wavelet transform to analyze the phase velocity dispersion of surface waves. This gives the possibility to accurately localize the phase information in time, and to isolate the most significant contribution of the surface waves. To extract the dispersion information, then, a hybrid technique is applied to the narrowband filtered seismic recordings. The technique combines the flexibility of the slant stack method in identifying waves that propagate in space and time, with the resolution of f-k approaches. This is particularly beneficial for higher mode identification in cases of high noise levels. To process the continuous wavelet transform, a new mother wavelet is presented and compared to the classical and widely used Morlet type. The proposed wavelet is obtained from a raised-cosine envelope function (Hanning type). The proposed approach is particularly suitable when using continuous recordings (e.g., from seismological-like equipment) since it does not require any hardware-based source triggering. This can be subsequently done with the proposed method. Estimation of the surface wave phase delay is performed in the frequency domain by means of a covariance matrix averaging procedure over successive wave field excitations. Thus, no record stacking is necessary in the time domain and a large number of consecutive shots can be used. This leads to a certain simplification of the field procedures. To demonstrate the effectiveness of the method, we tested it on synthetics as well on real field data. For the real case we also combine dispersion curves from ambient vibrations and active measurements

Integrated science curricula have been proposed since the late ‘70s with the aim of improving students’ scientific literacy. However, their adoption in classroom practice has been very limited. In Italy, only recently the official national guidelines have included the recommendation to adopt in the first two years of secondary school an integrated approach to science. In this study, we investigated teachers’ views about the integrated science curriculum proposed in the national guidelines. Four teachers (two biology, one chemistry, one physics) participated to the study and were asked about what could be the contents that can effectively be taught with and the didactical aims of an integrated science approach. The main contents identified by the teachers for an integrated approach were: nature of science, energy, gases, energy and matter flow in systems. The teachers identified also four general cross-cutting learning objectives: a) to familiarize students with the scientific knowledge as a whole, b) to provide students with tools and methodologies to understand real-world problems and act in a responsible and conscious way, c) to experience the scientific method and train students to learn “by discover”, d) to improve students’ views about Nature of Science. Implications of this study for the design of Integrated Science teaching-learning sequences are briefly discussed.

Objectives Leukaemia is the most common cancer in children, but its aetiology is still poorly understood. We tested the hypothesis that traffic-related air pollution is associated with paediatric leukaemia because of chronic exposure to several potential carcinogens. Methods The Italian SETIL study (Study on the aetiology of lymphohematopoietic malignancies in children) was conducted in 14 Italian regions. All incident cases of leukaemia in children aged ≤10 years from these regions (period 1998–2001) were eligible for enrolment. Two controls per case, matched on birth date, gender and region of residence were randomly selected from the local population registries. Exposure assessment at birth residence included traffic indicators (distance to main roads and length of main roads within 100 m) and estimates of pollutants concentrations (particulate matter -PM 2.5 and PM 10 - and gases -NO 2 and O 3 -) from national dispersion model and land use regression models. The association between the exposure variables and leukaemia was assessed by logistic regression analyses. Results Participation rates were 91.4% among cases and 69.2% in controls; 620 cases (544 acute lymphocytic and 76 acute non-lymphocytic leukaemia) and 957 controls were included. Overall, when considering the residence at birth, 35.6% of cases and 42.4% of controls lived along busy roads, and the mean annual PM 10 levels were 33.3 (SD=6.3) and 33.4 µg/m 3 (SD=6.5), respectively. No association was found, and all ORs, independent of the method of assessment and the exposure windows, were close to the null value. Conclusions Using various exposure assessment strategies, air pollution appears not to affect the incidence of childhood leukaemia.

Objective To investigate the clinical manifestations at diagnosis and during follow-up in patients with 22q11.2 deletion syndrome to better define the natural history of the disease. Study design A retrospective and prospective multicenter study was conducted with 228 patients in the context of the Italian Network for Primary Immunodeficiencies. Clinical diagnosis was confirmed by cytogenetic or molecular analysis. Results The cohort consisted of 112 males and 116 females; median age at diagnosis was 4 months (range 0 to 36 years 10 months). The diagnosis was made before 2 years of age in 71% of patients, predominantly related to the presence of heart anomalies and neonatal hypocalcemia. In patients diagnosed after 2 years of age, clinical features such as speech and language impairment, developmental delay, minor cardiac defects, recurrent infections, and facial features were the main elements leading to diagnosis. During follow-up (available for 172 patients), the frequency of autoimmune manifestations ( P = .015) and speech disorders ( P = .002) increased. After a median follow-up of 43 months, the survival probability was 0.92 at 15 years from diagnosis. Conclusions Our data show a delay in the diagnosis of 22q11.2 deletion syndrome with noncardiac symptoms. This study provides guidelines for pediatricians and specialists for early identification of cases that can be confirmed by genetic testing, which would permit the provision of appropriate clinical management.

Purpose: Neuroblastoma (NB) is the most common extra-cranial paediatric solid tumour. Incidence peaks in infancy, suggesting a role of in-utero and neonatal exposures but its aetiology is largely unknown. The aim of the present study is to evaluate the association between maternal characteristics and perinatal factors with the risk of NB, using data from the SETIL database. Methods: SETIL is a large Italian population-based case-control study established to evaluate several potential cancer risk factors in 0-10 year olds. Information about maternal characteristics, reproductive history, environmental and occupational exposures during pregnancy, as well as newborns' characteristics were obtained using a structured questionnaire. Extremely low frequency magnetic field (ELF-MF) home exposure was measured. The study included 1044 healthy controls and 153 NB cases, diagnosed between 1998 and 2001. Results: A twofold risk was associated to exposure in pregnancy to chemical products for domestic work and to hair dye. The risk associated with the latter was higher among 0-17 month old children (OR. =. 5.5, 95%CI: 1.0-29.3). Risk was increased for children whose mothers had suffered work related exposure in the preconception period to solvents (OR. =. 2.0 95%CI: 1.0-4.1) and in particular to aromatic hydrocarbons (OR. =. 9.2, 95%CI: 2.4-34.3). No association was observed with ELF-MF exposure. A higher risk was found among children with congenital malformations (OR. =. 4.9, 95%CI: 1.8-13.6) or neurofibromatosis (2 cases and 0 controls, p=. 0.016). Conclusions: Our study suggests maternal exposure to hair dyes and aromatic hydrocarbons plays a role and deserves further investigation. The association with congenital malformations might also be explained by over-diagnosis.External exposure, in particular during and before pregnancy might contribute to NB occurrence.

Umbilical cord blood (UCB) represents a source for hematopoietic stem cells (CB-HSC), and transplantation of cord blood has been part of clinical practice since more than 10 years (Lee et al., Blood 103, 2004). The UCB is usually collected by public or private cord blood banks, which store it for an unknown recipient for an undetermined time. The cryopreservation process, used for stem cell collection, is particularly critical for umbilical cord blood (Berz et al., Am J Hematol 82, 2007). The most used cryoprotectant agent (CPA) to avoid or reduce cell damage caused by ice crystals, is 10% dimethyl sulfoxide (Me2SO). Me2SO, while protecting the cells from membrane rupture, however has a toxic effect on them, depending on the temperature and the exposure time for both pre-freeze and post-thaw periods (Berz et al., Am J Hematol 82, 2007). Generation of oxygen-free radicals (ROS) is a major cause of cell damage during low-temperature storage (Katkova et al., Cryobiology 37 and Kearney, Burns 24, 1998). Cells counteract ROS accumulation through antioxidant enzymes, such as superoxide dismutase (SOD) and glutathione peroxidase (GPX). The addition of antioxidants to the conventional freezing medium improves the post-thaw cell recovery (Limaye et al., Stem Cell Res. 10, 2001). The purpose of this study is the improvement of cord blood HSC’s (CB-HSC) cryopreservation method by addition of a new isoform of recombinant manganese superoxide dismutase (rMnSOD), obtained from the native liposarcoma derived form (LSA-MnSOD) (Mancini et al., Int. J. Cancer 119, 2006). rMnSOD, while having the same enzymatic activity common to all SODs, differs for its higher molecular weight (30 kDa versus 24 kDa) due to the presence of an additional sequence, called leader peptide, not cleaved during its maturation. As demonstrated by Mancini et al. (2006), recombinant LSA-type MnSOD (rMnSOD) has a distinctive capacity to penetrate cells and converts ROS to H2O2. The latter is rapidly converted to O2 and H2O by catalase, thus resulting in a beneficial oxygenation of healthy cells. In this study, we isolated mononuclear cells by lymphoprep gradient tube (Pan Biotech, Germany) from cord blood after informed consent of healthy donors, received by the Cord blood Bank Ba.S.C.O of “Santobono Pausilipon” hospital of Naples. HSCs CD43+ were detected by flow cytometry and immunocytochemical methods at L.M., by using specific stem cell antibodies before and after the following experimental treatment. Cultured CB-HSCs were incubated with scalar concentrations from 1.5 µM to 0.015 µM of rMnSOD for 5 hours (with StemPro®-34 SFM, Life Technologies) and frozen using standard freezing protocol with 10% Me2SO. After thawing, CD34+ viable cells were counted by flow cytometry FACSCalibur II (Becton & Dickinson) by using BD Stem Cell Enumeration Kit (BD Biosciences). The results showed that the viability of CB-HSC CD34+ increased from 10% of the control to 45% by adding 0.15µM rMnSOD. Further experiments are progress to define the rMnSOD concentration able to obtain the greatest increase of post-thaw CB-HSC viability.

"High-dose methotrexate (MTX) is a key component of most treatment protocols for childhood and adolescent non-Hodgkin lymphoma (NHL). Recent studies have suggested that the toxicity of antifolate drugs, such as MTX, is affected by inherited single nucleotide polymorphisms (SNPs) in folate metabolizing genes. The aim of our study was to investigate the potential influence of the C677T and A1298C genetic variants of the methylenetetrahydrofolate reductase (MTHFR) gene on the clinical toxicity and efficacy of MTX in pediatric patients with NHL (n = 95) treated with therapeutic protocols Associazione Italiana Ematologia Oncologia Pediatrica (AIEOP) LNH-97 and EURO LB-02. We demonstrated that patients with the 677T genotype had an approximately six-fold greater risk of developing hematological toxicity compared with wild-type carriers, especially in the 1 g\/m2 treatment group (p = 0.01). Moreover, we identified a correlation between the risk of relapse and the T genotype: T carriers had reduced disease-free survival compared with wild-type patients (67% vs. 100%). Our data suggest a pharmacogenetic influence on the adverse effects of high-dose MTX in the 1 g\/m2 treatment group.. . "

- Abstract
Background The association of childhood leukemia with traffic pollution was considered in a number of studies from 1989 onwards, with results not entirely consistent and little information regarding subtypes. Aim of the study We used the data of the Italian SETIL case-control on childhood leukemia to explore the risk by leukemia subtypes associated to exposure to vehicular traffic. Methods We included in the analyses 648 cases of childhood leukemia (565 Acute lymphoblastic–ALL and 80 Acute non lymphoblastic-AnLL) and 980 controls. Information on traffic exposure was collected from questionnaire interviews and from the geocoding of house addresses, for all periods of life of the children. Results We observed an increase in risk for AnLL, and at a lower extent for ALL, with indicators of exposure to traffic pollutants. In particular, the risk was associated to the report of closeness of the house to traffic lights and to the passage of trucks (OR: 1.76; 95% CI 1.03–3.01 for ALL and 6.35; 95% CI 2.59–15.6 for AnLL). The association was shown also in the analyses limited to AML and in the stratified analyses and in respect to the house in different period of life. Conclusions Results from the SETIL study provide some support to the association of traffic related exposure and risk for AnLL, but at a lesser extent for ALL. Our conclusion highlights the need for leukemia type specific analyses in future studies. Results support the need of controlling exposure from traffic pollution, even if knowledge is not complete.

Background The aim of this study was to describe the patterns of marriage and parenthood in a cohort of childhood cancer survivors included in the Off-Therapy Registry maintained by the Italian Association of Pediatric Hematology and Oncology. Design and Methods We analyzed a cohort of 6,044 patients diagnosed with cancer between 1960 and 1998, while aged 0 to 14 years and who were 18 years old or older by December 2003. They were followed up through the regional vital statistics registers until death or the end of follow up (October 30, 2006), whichever occurred first, and their marital status and date of birth of their children were recorded. The cumulative probabilities of being married and having a first child were computed by gender and compared by tumor type within the cohort. Marriage and fertility rates (the latter defined as the number of live births per woman-year) were compared with those of the Italian population of the same age, gender, area of residence and calendar period by means of the observed to expected (O/E) ratios. Results During the follow-up period, 4,633 (77%) subjects had not married. The marriage O/E ratios were 0.56 (95% CI: 0.51-0.61) and 0.70 (95% CI: 0.65-0.76) among men and women, respectively. Overall, 263 men had 367 liveborn children, and 473 women had 697 liveborn children. The female fertility O/E ratio was 0.57 (95% CI: 0.53-0.62) overall, and 1.08 (95% CI: 0.99-1.17) when analyses were restricted to married/cohabiting women Conclusions Childhood cancer survivors are less likely to marry and to have children than the general population, confirming the life-long impact of their previous disease on their social behavior and choices. The inclusion of counseling in the strategies of management and long-term surveillance of childhood cancer patients could be beneficial to survivors as they approach adulthood. ©2011 Ferrata Storti Foundation.

Introduction: The health-related quality of life in X-linked agammaglobulinemia was investigated in 25 children and adolescents patients through the Italian version of Pediatric Quality of Life Inventory 4.0 Generic Core Scale for patients aged less then 18 years, comparing child perception to that of the parents and the physician's evaluation. The data were compared with the ones of 80 healthy controls and the literature data of a group of patients with rheumatic diseases. Discussion: The agammaglobulinemia subjects perceived a lower global quality of life than the healthy subjects, but significantly higher than the rheumatic diseases controls. The clinical relevance of health-related quality of life assessment in X-linked agammaglobulinemia pediatric patients is discussed. © 2008 Springer Science+Business Media, LLC.

Tertiary care centres, grouped in the Italian Association of Paediatric Haematology and Oncology (AIEOP) are unevenly distributed across the country. In an attempt to describe their perceived efficacy, we matched the residence and the location of the treatment centre in 18,441 patients aged ⩽15 years treated in the AIEOP network between 1989 and 2005. Overall, centres located in the central and southern regions were less appealing than those located in the North, although this trend decreased over the study period. Patients with solid tumours migrated more frequently than those with leukaemia or lymphoma. Information resulting from better knowledge of the non-random migrations for treatment of children with cancer will be useful to refine planning of the national paediatric haematology-oncology network with social and economic implications.

We describe the survival patterns of 10,791 Italian children (age 0-14) diagnosed with cancer during 1989-1998 and who were included in the hospital-based registry of the Italian Association of Paediatric Haematology and Oncology. Five-year cumulative survival percentages were 76% for lymphoproliferative disorders and 68% for solid tumours. Survival rates in 1994-1998 significantly improved for acute lymphocytic leukaemia (ALL), acute non-lymphocytic leukaemia, Hodgkin's lymphoma and Wilms' tumour. Gender and age were determinants of survival for some specific types of cancer. Girls with ALL and neuroblastoma exhibited a significant advantage (hazard ratio HR 0.72, 0.62-0.83) and disadvantage (HR 0.73, 0.59-0.90) over boys, respectively. Children with a Wilms' tumour diagnosed above age 3 had a worse prognosis than younger children (HR 2.3, 1.4-4.1). The persisting gender-related difference in survival rate for ALL requires understanding as to whether it is attributable to delays in the adoption of more recent therapeutic protocols, while the corresponding findings for Wilms' tumour and neuroblastoma deserve further biological interpretation.

- Abstract
The toxicity and efficacy of intrathecal liposomal cytarabine (LC) were evaluated in children with central nervous system (CNS) relapsed/refractory acute leukemia/lymphoma. Thirty patients (male:female ratio 21:9; median age 9.4 years) with CNS relapsed/resistant disease were treated with intrathecal LC at dosages adjusted for age. Twenty-seven (90%) patients simultaneously received systemic chemotherapy, including concurrent high-dose cytarabine or methotrexate in 21 (70%) cases. Of 28 patients evaluable for response, 25 patients (89%) achieved CNS complete remission and three (11%) partial remission. The median number of intrathecal LC administrations per patient was 4. The cerebrospinal fluid was cleared after a median of 3 intrathecal LC administrations. Neurological toxicity ≥ grade 3 occurred in four (13%) patients. No permanent sequelae were observed. The median overall survival was 20.9 months and the 5-year probability of survival was 46%. These encouraging data suggest that intrathecal LC is well tolerated and effective in children with relapsed/refractory CNS leukemia/lymphoma.

La rapamicina, antibiotico appartenente alla famiglia dei macrolidi, è classicamente usato come immunosoppressivo in quanto inibisce la risposta alle citochine. L'attività del farmaco è mediata dal legame con una immunofillina, FKBP12, formando un complesso che inibisce mTOR, una serin-treonin chinasi a valle di PI3k ed Akt cioè della via dei fosfoinositoli, che come è noto regola la proliferazione e la sopravvivenza delle cellule in risposta a citochine, mitogeni, aminoacidi. La via dei fosfoinositoli è ampiamente descritta essere coinvolta nella tumorigenesi e la rapamicina ha un riconosciuto effetto antitumorale nelle neoplasie che presentano una attivazione costitutiva di PI3k/Akt. Viste le proprietà di inibire sia l'at- tivazione linfocitaria che la sopravvivenza di cellule mali- gne, si è investigato se la rapamicina fosse in grado in vitro di esercitare una attività anti-tumorale nei confronti delle ALL pediatriche. A questo scopo sono state coltivate, in presenza di rapamicina a dosi scalari, cellule mononuclea- te separate su gradiente di Ficoll-Hypaque da 22 campio- ni di midollo osseo di pazienti con LLA (19 LLA all'esordio, 2 recidive e 1 refrattaria) e si è analizzata l'apoptosi con varie tecnologie. La percentuale di blasti nei campioni di cellule purificate era superiore al 90% eccetto che per la forma refrattaria in cui era pari al 59. 1%. L'apoptosi è stata dimostrata in 13 su 22 campioni (59. 0%) incubati con rapamicina. L'analisi del potenziale di membrana mitocon- driale rivelava una spiccata depolarizzazione già dopo 6 ore di coltura. Dopo 24 ore più del 70% delle cellule espo- nevano fosfatidilserina sulla membrana cellulare ed allo stesso tempo l'attività catalitica di caspasi 3 appariva aumentata del 170%. La rapamicina si dimostrava essere attiva in un range di concentrazione compreso tra 20 e 100 ng/ml. La risposta pro-apoptotica della rapamicina era dimostrata anche nei 2 campioni di LLA recidivata e in quella refrattaria ed in particolare in quest'ultima forma è stato possibile dimostrare la scarsa sensibilità all'azione apoptotica della rapamicina della quota midollare residua normale. Per verificare se l'azione antileucemica del macrolide potesse essere connessa all'inibizione della via dei fosfoinositoli si è analizzato se la wortmannina, uno specifico inibitore di PI3k, avesse lo stesso effetto apoptotico della rapamicina. L'analisi è stata effettuata su nove campioni sensibili alla rapamicina. In 6 casi la wortmannina induce- va apoptosi della stessa entità della rapamicina, in 3 casi l'effetto della wortmannina era poco apprezzabile o assente. In conclusione il nostro studio dimostra che la rapami- cina induce apoptosi dei blasti di LLA pediatrica, preservando la controparte midollare normale, con un meccani- smo che coinvolge, ma non solo, l'inibizione di PI3k/Akt e che richiede ulteriori approfondimenti.
