Purpose Bevacizumab (BZ) combined with first line chemotherapy (CC) has shown good clinical outcomes in metastatic colorectal cancer (mCRC). Overall survival (OS) and/or progression free survival in mCRC patients receiving BZ with or without 5FU-based CC is thought to be affected by clinical and morphological factor(s). Patients and methods We reviewed retrospective medical records of all consecutive mCRC patients treated with BZ with or without CC at tertiary care center between 2003 and 2009 out of which149 patients (m = 77, f = 72) were eligible. Results Our study population had a mean age at diagnosis of 63.5 years (SD = 11) with median follow-up period of 19.4 months. On initial radiological evaluation following BZ therapy, 56 patients (m = 31, f = 25) had complete or partial response categorized as "early responders." Remaining patients (m = 46, f = 47) who were either stable or showed progressive disease were categorized as "non-responders." Fifty percent among early responders and 60% among non-responders [relative risk (RR) 0.67 (95% confidence interval (CI), 0.43-1.06)] demonstrated disease progression on follow up. There was a slightly better OS among early responders compared to non-responders (median 21.5 months days versus 16.8 months, P = 0.07). Cox regression analysis suggested male sex (RR 0.65, 95% CI, 0.43-0.98), hematochezia (RR 0.63, 95% CI, 0.4-0.98), resectable primary tumor (RR 0.42, 95% CI, 0.24-0.72) and resectable metastatic mass (RR 0.32, 95% CI, 0.14-0.74) were found to be associated with longer OS. Abdominal pain (RR 1.76, 95% CI, 1.1-2.8), accompanying diabetes (RR 1.76, 95% CI, 1.09-2.85), and unexplained weight loss (RR 2.73, 95% CI, 1.73-4.29) were associated with poor OS. Conclusions Better OS among mCRC patients with resectable primary and metastatic tumors was seen. This is the first study to demonstrate slightly better outcome in males and negative influence of diabetes on outcome in mCRC treated with BZ.

Introduction: To conduct a statewide survey among Wisconsin-based dental providers evaluating current knowledgeability, attitudes and practice behaviors surrounding management of patients with diabetes/prediabetes in the dental setting. The study explored perceptions on feasibility, value, barriers, and current status of integrated care model (ICM) adoption by dental practicesMaterials and Methods: A 32-question paper-based survey was mailed to all licensed dentists and dental hygienists practicing in Wisconsin. The study was conducted over a 4 week period in 2019. The survey instrument was adapted from a previous validated survey and was expanded to include questions on ICM adoption. Content and validity analyses and beta testing were conducted prior to dissemination of the survey. Descriptive statistics and chi-square tests were applied for data analyses. Thematic analyses was performed on open-ended questions.Results: Survey response rate was 12% (N = 854/7,356) representing 41% dentists and 59% dental hygienists. While 68% reported educating patients on oral health-diabetes association, only 18% reported medical consultations to inform dental treatment, and “frequent” (22%) or “occasional” (40%), medical triage. Knowledge-based questions were correctly answered by >70% of participants. While 50% valued chair-side glycemic screening and 85% supported non-invasive chair-side screening to identify at-risk patients,>88% relied on patient-reported diabetic status. Barriers to ICM adoption included time investment (70%), patient activation/cooperation (62%), cost (50%), insurance coverage (50%), infrequent interdisciplinary communication (46%), lack of equipment (33%) and provider (31%).Conclusion: Low rates of ICM adoption, chair-side testing, medical consultation and triage, and need for educational curricula reform were identified.

This cross-sectional study sought to assess the current awareness, knowledge, and behavior regarding diabetes mellitus (DM) and periodontal disease (PD) association among a convenience sample of patients from a large Wisconsin-based integrated medical-dental health care organization serving largely rurally based communities. An anonymous 10-question survey was distributed at regional medical and dental centers of dental and medical clinics of a single health care institution over a 4-week period, to achieve a cross-sectional sampling of patients aged 18 to 80 years. Among 946 respondents, 616 were female. Patient-reported periodicity for dental visits was highest between 6 months and 1 year (56.4%). Conclusions and Practical Implications Patients identified and recommended incorporation of different types of oral health data for access via the PHP as vital to strengthening the communication between patients and dental professionals. Incorporating patient-identified oral health information in the PHP will inform strategies for improving patient engagement, strengthen patient-provider communication, and offer a venue for increasing oral health literacy and awareness.

This study sought to re-characterize trends and factors affecting electronic dental record (EDR) and technologies adoption by dental practices and the impact of the Health Information Technology for Economic and Clinical Health (HITECH) act on adoption rates through 2012. A 39-question survey was disseminated nationally over 3 months using a novel, statistically-modeled approach informed by early response rates to achieve a predetermined sample. EDR adoption rate for clinical support was 52%. Adoption rates were higher among: (1) younger dentists; (2) dentists ≤ 15 years in practice; (3) females; and (4) group practices. Top barriers to adoption were EDR cost/expense, cost-benefit ratio, electronic format conversion, and poor EDR usability. Awareness of the Federal HITECH incentive program was low. The rate of chairside computer implementation was 72%. Adoption of EDR in dental offices in the United States was higher in 2012 than electronic health record adoption rates in medical offices and was not driven by the HITECH program. Patient portal adoption among dental practices in the United States remained low.

Objective To effectively achieve a robust survey response rate in a timely manner, an alternative approach to survey distribution, informed by statistical modeling, was applied to efficiently and cost-effectively achieve the targeted rate of return. Design A prospective environmental scan surveying adoption of health information technology utilization within their practices was undertaken in a national pool of dental professionals (N=8000) using an alternative method of sampling. The piloted approach to rate of cohort sampling targeted a response rate of 400 completed surveys from among randomly targeted eligible providers who were contacted using replicated subsampling leveraging mailed surveys. Methods Two replicated subsample mailings (n=1000 surveys/mailings) were undertaken to project the true response rate and estimate the total number of surveys required to achieve the final target. Cost effectiveness and non-response bias analyses were performed. Results The final mailing required approximately 24% fewer mailings compared to targeting of the entire cohort, with a final survey capture exceeding the expected target. An estimated $5000 in cost savings was projected by applying the alternative approach. Non-response analyses found no evidence of bias relative to demographics, practice demographics, or topically-related survey questions. Conclusion The outcome of this pilot study suggests that this approach to survey studies will accomplish targeted enrollment in a cost effective manner. Future studies are needed to validate this approach in the context of other survey studies.

The goal of this project was to protect children while parents work in agriculture by improving off-farm services for children of migrant and seasonal farm workers. Large agricultural enterprises have policies forbidding children in the worksite. At the same time, their employees, who are trying to generate income, seek as many work hours as possible but often lack viable options for childcare services. As employers strive to increase their labor pool, and workers seek off-farm childcare, there is mutual interest in improving access to childcare services in agricultural regions dependent on large numbers of full-time and seasonal workers. This report describes the employers' perspectives on childcare needs of hired farm workers' families and their barriers and motivators to facilitating off-farm childcare services. Using descriptive survey research methodology, data were collected from a convenience sample of 102 agribusiness owners and Human Resource directors attending an agricultural conference regarding labor laws or personnel management. Results revealed significant differences for those companies employing more than 25 workers compared to their counterparts. Primary motivators for offering childcare as an employment benefit were improved employee morale, enhanced company reputation, and a more stable workforce. A major barrier was that half of large-scale enterprises lack guidance on how to provide childcare options for their workers. Survey results are being used to facilitate collaboration among employers, farm workers, and childcare providers to offer a safe, nurturing environment for children while their parents work in agriculture.

Background: Subclavian artery stenosis (SAS) is narrowing of the subclavian artery most commonly caused by atherosclerosis. It serves as a marker for cerebrovascular and myocardial ischemic events. Methods: A retrospective cohort study was conducted to determine the association of treatment via combination therapy (antiplatelet drug plus either by-pass surgery or percutaneous transluminal angioplasty (PTA) with or without stent implantation) versus antiplatelet drug therapy alone on cardiovascular events and all-cause mortality in Marshfield Clinic patients diagnosed with symptomatic SAS from January 1, 1995 to December 31, 2009. Results: Of the total 2153 cases, 100 patients were identified as eligible to be included in the study. Of these 100 patients that meet inclusion criteria, 30 underwent combination therapy while 70 were managed only with pharmacological drug treatment. Adverse cardiovascular events occurred in 5/30 (17%) of combination therapy patients compared to 28/70 (40%) of antiplatelet drug therapy only patients (p = 0.0355). Accordingly, all-cause mortality was higher (47%) in the antiplatelet drug therapy only group than the combination therapy group (13%) [hazard ratio = 3.45, p = 0.0218]. Conclusions: Preliminary findings in this pilot data set suggest that combination therapy (medications plus either surgical or interventional repair) of subclavian artery stenosis is associated with less cardiovascular adverse events and higher survival rates, benefits that extend beyond the target vessel. Prospective randomized studies with larger number of patients are needed to validate these findings.

Objective To identify risk factors associated with spontaneous recurrent epistaxis. Patients and Methods This was a retrospective cohort study assessing patients in the Marshfield Clinic system diagnosed as having epistaxis between January 1, 1991, and January 1, 2011. There were 461 cases with at least 2 episodes of spontaneous epistaxis within 3 years and 912 controls with only 1 episode in the same time frame. More than 50 potential risk factors were investigated, including demographic features, substance use, nasal anatomical abnormalities, nasal infectious and inflammatory processes, medical comorbidities, medications, and laboratory values. A Cox proportional hazards regression modeling approach was used to calculate hazard ratios of epistaxis recurrence. Results Traditional risk factors for epistaxis, including nasal perforation, nasal septum deviation, rhinitis, sinusitis, and upper respiratory tract infection, did not increase the risk of recurrence. Significant risk factors for recurrent epistaxis included congestive heart failure, diabetes mellitus, hypertension, and a history of anemia. Warfarin use increased the risk of recurrence, independent of international normalized ratio. Aspirin and clopidogrel were not found to increase the risk of recurrence. Few major adverse cardiovascular events were observed within 30 days of the first epistaxis event. Conclusion Congestive heart failure is an underappreciated risk factor for recurrent epistaxis. Hypertension and diabetes mellitus may induce atherosclerotic changes in the nasal vessels, making them friable and more at risk for bleeding. Patients with recurrent epistaxis may also be more susceptible to developing anemia. Physicians should promote antiplatelet and antithrombotic medication adherence despite an increased propensity for recurrent epistaxis to prevent major adverse cardiovascular events.

The objective of this study was to assess current knowledgeability, attitudes, and practice behaviors of primary care providers (PCPs) towards oral cancer screening. Applying a cross-sectional design, a 14-question survey was emailed to 307 PCPs practicing at a large, multi-specialty, rurally based healthcare system. Survey data were collected and managed using REDCap and analyzed applying descriptive statistics. A 20 % response rate (n = 61/307) was achieved for survey completion. Approximately 70 % of respondents were physicians, 16 % were nurse practitioners, and 13 % were physician assistants. Nearly 60 % of respondents were family medicine practitioners. Limited training surrounding oral cancer screening during medical training was reported by 64 %. Although 78 % of respondents reported never performing oral cancer screening on patients in their practice, >90 % answered knowledge-based questions correctly. Frequency rate for specialist referral for suspicious lesions by PCPs was 56 % “frequently”. Optimal periodicity for oral cancer screening on all patients selected by respondents was 61 % “annually”, 3 % “every 6 months”, 3 % “every visit”, 2 % “not at all”, and 31 % “unsure”. This study established a baseline surrounding current knowledgeability, practice patterns, and opinions of PCPs towards oral cancer screening at a single, large, regional healthcare system. In the absence of evidence-based support for population-based cancer screening, this study result suggests a need for better integration of oral cancer surveillance into the medical setting, supplemented by education and training with emphasis on assessment of high-risk patients to achieve early detection. Prospectively, larger studies are needed to validate these findings.

Meta-analysis is a powerful tool to estimate measures of associations or effects based on published or unpublished reports. However, problems exist in many meta-analyses, particularly related to study heterogeneity. This article proposes a way of concluding meta-analysis results using P values, taking heterogeneity into account. There is little published research focused on evaluating conclusiveness of summary results of reported meta-analyses. Generally, a P value is directly linked to the test statistic z=b/s(b) following a standard normal distribution with mean zero and unit variance, where b is an estimator of β and s(b) is the estimated standard error of b for any study included in a meta-analysis. This forms the basis of the proposed method for deriving overall test statistics and corresponding P values used for comparing results of meta-analyses. Two published meta-analyses were chosen and specific software was applied. Results are consistent with the two published meta-analysis reports in terms of P values for significance and direction of summary measure of treatment effect. This proposed method can be utilized to safeguard against improper conclusions of published meta-analyses due to heterogeneity. Exploring more sophisticated statistical methods for situations when the key assumption applied to this proposed method is violated could be pursued and could expand the scope of applications beyond this method.

Methods of ascertaining and defining asthma in epidemiologic research vary, and the extent of agreement between such measures is not clearly understood. Within a broader investigation of early-life farm exposures and risk of childhood asthma and other atopic conditions, the authors sought to generate and compare population-based estimates of asthma prevalence among farm children using several definitions, based on both parental report and medical chart review. The Marshfield Epidemiologic Study Area (MESA) is a geographically defined, population-based cohort that receives nearly all health care from Marshfield Clinic and affiliated institutions. The region includes about 2200 farms, and over 9500 children aged 5 to 17 years who were born in the region served as the study population. A stratified random sample of 1000 overselecting for likely farm resident children was drawn. Parents of 553 children completed the survey and gave permission to review medical records. Informative records were available for 531 (96%). A weighted analysis provided estimates for the full study population. Asthma ascertainment included parental reports of past asthma diagnosis, history of wheezing, and asthma medication use, as well as documentation of asthma diagnoses and medication use in the medical chart. Prevalence of asthma among farm children using a broad composite definition was 24.8%. Prevalence based on parental reports of a specific asthma diagnosis was 11.2%, whereas medical chart documentation of a past asthma diagnosis was found for 10.1% of farm children. Seventy-one percent of parental reports were validated in the charts, and 80% of chart-confirmed diagnoses were reported by the parent. Basing asthma prevalence for farm children on a history of asthma medication use gave a higher estimate than did history of a diagnosis. Of farm children, 19.2% reported wheezing or respiratory whistling, with 8.1% occurring in the past year. Observational research on asthma can be substantially influenced by ascertainment methods and case definitions. Although prevalence estimates based on a past asthma diagnosis were quantitatively similar for parental reports and chart confirmation, agreement on specific cases between the two sources was less than expected. Care should be taken to clearly describe asthma case definitions when reporting results of observational asthma research.

Irritable bowel syndrome (IBS) is a functional disorder of the gastrointestinal tract characterized by recurrent episodes of abdominal pain or discomfort along with changes in frequency or consistency of the stool in the absence of an organic etiology. The condition is heterogeneous, exhibiting variability in the frequency of symptoms reported within and between males and females.1 The pathophysiological mechanisms of IBS are not completely understood. Alterations in gut motility, visceral perception, and central processing of pain and motor function due to abnormalities in the enteric and central nervous system are believed to account for symptoms of IBS.2 The brain-gut axis and biopsychosocial model have been used to explain how intrinsic and extrinsic stimuli modulate disease expression.3,4 It is unknown whether IBS is primarily a disorder of abnormal perception to a normal stimulus, or a disorder of normal perception to an abnormal physiologic sensory stimulus. Since no structural abnormalities or biochemical markers characterize IBS, diagnosis is based on the presence of clinical symptoms.2 Symptom-based diagnostic criteria have been established to create uniformity in reporting and enhance diagnostic accuracy. All diagnostic criteria used to differentiate IBS from organic diseases are self-reported measures of abdominal pain and bowel habits (table 1 ▶).5–8 The diagnosis of IBS is confirmed by applying symptom-based criteria and pursuing further diagnostic evaluations to exclude organic diseases, as needed. Symptom-based criteria should be individualized, taking into account the patient’s age, associated conditions, duration of symptoms, previous diagnostic evaluations, severity of symptoms, travel history and lactose consumption. Criteria used to establish the diagnosis of IBS have evolved since the initial work of Manning et al5 in 1978 and reflect a better understanding of the symptomatology associated with this disease. The Rome I and II criteria reflect more specific clinical diagnostic standards than the Manning criteria.9 Thus, many patients previously diagnosed with IBS under the Manning criteria would not receive a diagnosis of IBS based on the most recent Rome criteria. However, if the more restrictive case definitions of the Rome I and II criteria are applied, patients with IBS may potentially be underdiagnosed.10,11 Previous studies evaluating case definitions were based on surveys and are limited by the diagnostic criteria used, the questions asked and the ethnicity and cultural background of the population sampled.9,10,12 In this retrospective study, we utilized data from medical records to evaluate the diagnosis of IBS in a population-based cohort within a clinical practice. The quality of medical record documentation of clinical symptoms of IBS was determined, and IBS diagnoses were validated based on the Manning, Rome I and Rome II criteria. These three alternative criteria used to diagnose IBS were compared in terms of biennial age- and gender-adjusted incidence rates beginning from January 1, 1993 through December 31, 2003 per 100,000 person-years. Percent concordance of each paired comparison of criteria-based IBS was also determined.

Objective: Emerging evidence-based information supports incorporation of point-of-care screening of individuals at-risk for undiagnosed medical conditions in dental settings. In this study, dental provider’s knowledgeability, attitude and behavior regarding chairside medical screening was assessed across nine federally qualified health center (FQHC)-operationalized dental centers integrated into a large Midwestern US healthcare organization to establish receptivity among regional dental providers. Further, this pilot effort informed the study design and survey tool for conducting a larger statewide study. Methods: Knowledgeability surrounding point-of-care screening, was piloted via a multiple-choice survey targeting 100 regional FQHC dental providers (dentists and dental hygienists) utilizing SurveyMonkey®. The survey captured provider demographics, medical knowledgeability, attitudes, and current practice patterns with conduct of chairside medical screening. Chi-square analysis compared responses based on the respondent’s gender, year of graduation and clinical role. Results: A 26% response rate was achieved. The 26 responding dental providers reported screening for hypertension, diabetes mellitus, prediabetes, obesity, chronic kidney disease, human papilloma virus, hepatitis, human immunodeficiency virus and adverse events of chemotherapy and radiotherapy, with a frequency of 88%, 37%, 8%, 42%, 21%, 29%, 25%, 25% and 37% respectively. Willingness to incorporate chairside screening into clinical practice that yielded immediate results was indicated by 57% of respondents. The most important factors associated with engagement of medical screening included time investment (92%), patient willingness (88%), liability (84%), insurance coverage (56%) and cost (52%). Conclusions: Overall, this pilot study indicated that dental providers’ attitude relative to chairside screening for medical conditions was favorable. While respondents indicated willingness to collect oral fluids for salivary diagnostics and blood pressure and height/ weight measurements, lack of willingness to utilize finger-stick approaches for assessing glycemic status was identified. A statewide study is currently being planned to establish a comprehensive understanding of barriers impeding adoption of chairside screening for medical conditions in a dental setting.

We describe phenotypic and genotypic traits of a group of methicillin-resistant Staphylococcus aureus (MRSA) clones that are either remnants of unsuccessful community-associated MRSA (CA-MRSA) clones or represent a transitional state with some yet-to-be-acquired characteristics of CA-MRSA. These rare strains ( n = 20) were identified during a 10-year period (1990-1999) from 13 unrelated health care facilities in Wisconsin. The isolates were recovered from patients in nosocomial or long-term chronic care facilities (60%) and outpatient settings (40%). Sixty percent ( n = 12) of the isolates were recovered from skin and soft tissue infections, whereas the remaining isolates ( n = 8) were from invasive infections. Ninety percent of isolates were susceptible to all antibiotic classes tested or resistant to erythromycin and clindamycin. Pulsed-field gel electrophoresis, multilocus sequence typing, and spa typing clustered these isolates into 8, 8, and 14 clonal groups, respectively. Eight plasmid profiles were represented in these strains. All four agr types were represented, with type IV being predominant (40%). All strains harbored subtypes of type IV staphylococcal cassette chromosome mec but lacked genes for the virulence factor Panton-Valentine leukocidin (PVL). The strains harbored one or more of the following toxin genes: sea , seb , sec , sed , see , seh , sej , sek , sel , seg , sei , sem , sen , and seo . Individual clonal groups maintained the same set of enterotoxin genes even though they were isolated over extended time periods, suggesting significant genomic stability. The potential role of PVL-carrying phages and plasmids in the success of CA-MRSA clones has been discussed.

Evaluation of current knowledgeability, attitudes, and practice behaviors of medical providers from a large health care system toward oral health was undertaken as a pilot effort to better understand and integrate oral health into the overall health care delivery.Invitations to complete a 28-question survey, designed in a web-based platform (SurveyMonkey(®)), were emailed to 1407 medical multispecialty physicians, residents, and nurses within the health system. The questionnaire included sections on provider demographics, oral health knowledgeability and attitudes, and current practice conducting oral health screenings.A 14% (n = 199/1407) response rate was achieved for survey completion. There were 16% who reported good coverage of oral/dental health topics in their medical training curriculum. Competency level was 30% for identifying tooth decay and oral pathology. There were 95% who reported never applying fluoride varnish in their practice, while80% answered knowledge-based questions correctly. Frequency rates for dental referral by the medical providers were 32% 'frequently' and 68% 'infrequently.' Perceptions of optimal frequency for conducting oral health assessment in their professional practices ranged from 69% indicating 'frequently' to 25% indicating 'infrequently.'Overall, positive attitudes were observed toward incorporation of oral health examination into medical practice. The study identified lack of oral health treatment and infrequent referral by medical providers to dental providers. Results support likelihood for acceptance of care models that incorporate a medical/dental team-based approach complemented by oral health training for medical providers to enhance holistic health care delivery. Limitations of this pilot study include potential selection bias and lack of generalizability beyond our institution; further studies are planned in additional settings statewide to validate findings.

More and more neonatal intensive care units (NICU) are utilizing commercially available web camera systems to facilitate family bonding with sick infants.1,2 The goal of camera services is to allow virtual visitation of the infant by family members and to enhance bonding. Parents have shown a positive attitude towards the camera service.1–3 These technologies need to be user friendly in order to minimally impact the workload of nursing staff.2 Poorly planned implementation could lead to underutilization of the camera service.2 The web-based camera service allows parents and other relatives to visualize the baby admitted in the NICU through a password protected system. The web camera is attached to the bedside of the baby; any baby admitted to the NICU can be accessed by the cameras following parental written consent. Each family is issued an individual password to login to the secure system. Families are able to share the password with their friends and relatives at their discretion. The nurses taking care of the baby are also responsible for checking that the camera’s focus is centered on the infant, and that it is switched off during procedures, physical assessment, and bathing, and that it is then switched back on afterward. There is a telephone at every infant bedside, so when parents call, the nursing staff are available to answer questions and concerns. Hawkes et al2 evaluated nursing staff perception prior to implementation of a web camera service in which a majority of nurses revealed a negative attitude; 72% of the nurses feared increased workload after the camera service started. Nurses were also concerned about security issues, an increased number of phone calls, maintaining the camera system, and the infant being seen in the view.2 Angel Eye developers, a camera company, interviewed nurses after the camera service was implemented. There were concerns regarding the design of the camera and ease of use.1 Rhoads et al1 were concerned that if the cameras were implemented haphazardly then there would be increasing resistance from the NICU staff. Piecuch et al4 noted that the increased frequency of phone calls by the video mothers compared to the control mothers might have caused distress to the nurses. There are limited data on change in workload after implementation of a camera service. It is important to understand the workload and stress factors of NICU nurses in order to prevent adverse events. Multiple factors like patient acuity, staffing and management factors, highly technical environment, death, family needs, and ongoing need for crisis intervention and decision making are implicated as stressors.5 Tucker et al6 noted that infant mortality in the NICU increased with an increase in nursing staff workload. Increased workload can, thus, adversely affect patient safety.7 Therefore, assessment of the impact of any new technology on nursing work load is essential. Filho et al8 examined the possible association between the intensity of staff workload and adverse events in the NICU like accidental extubation, obstruction of endotracheal tube, and accidental disconnection of ventilator circuit. Adverse events were noted frequently during their 6-month study period. The more infants cared for per nurse, the more likely was the occurrence of intermediate adverse events.8 We evaluated the implementation of a web camera service in our NICU. Our study hypothesis was that implementation of the web camera service would increase nursing workload. We studied this by having nurses document their time spent dealing directly with the camera system for each infant on camera during their shift. We also questioned nurses about time spent with families as a direct result of the camera. We attempted to identify determinants that might disrupt or facilitate the use of camera service among nursing staff. We further examined the parental perception and use of the camera service based on their interaction with the nursing staff.

Importance The long-term implications of hepatotoxic effects in patients with psoriasis remains uncharacterized, and a method is needed for the noninvasive monitoring of the development and progression of hepatic fibrosis in patients with psoriasis receiving long-term methotrexate therapy. Objective To evaluate if NASH FibroSure, a noninvasive test for nonalcoholic steatohepatitis (NASH) and hepatic fibrosis, can be used for patients with psoriasis to aid in determining eligibility for methotrexate sodium (MTX) therapy, monitor for the development of MTX-induced hepatotoxic effects, and monitor for worsening of hepatic fibrosis scores during MTX therapy. Design, Setting, and Participants A retrospective descriptive analysis was conducted among a cohort of patients with psoriasis treated with MTX who underwent NASH FibroSure testing between January 1, 2007, and December 31, 2013, at a dermatology referral center at a single institution. Data analysis was performed from January 1 to December 31, 2014. Main Outcomes and Measures NASH FibroSure risk scores suggesting the development and progression of hepatic fibrosis in patients with psoriasis receiving long-term MTX therapy. Results Included in the institutional experience portion of the study were 129 patients with psoriasis undergoing treatment with MTX, while 107 patients (57 women and 50 men; mean [SD] age, 83.3 [13.5] years) underwent NASH FibroSure testing during MTX therapy and were eligible for correlation analysis. Of the 129 patients with psoriasis undergoing treatment with MTX, 69 (53.5%) underwent NASH FibroSure testing prior to starting MTX; 19 of those patients (27.5%) had elevated fibrosis scores, and 54 (78.3%) had elevated steatosis scores. Among the 107 patients who underwent NASH FibroSure testing during MTX therapy, the cumulative MTX dose corresponded to a statistically significant association of a higher NASH FibroSure hepatic fibrosis score in women (Spearman ρ = 0.21; P = .02) but not in men (Spearman ρ = 0.17; P = .11). All patients in the cohort except 1 were managed without a liver biopsy. Conclusions and Relevance The patients with psoriasis in this study had a high prevalence of elevated hepatic steatosis scores. The NASH FibroSure test can be used to monitor changes in fibrosis score in patients with psoriasis receiving MTX. In a single-institution cohort, these results suggest that NASH FibroSure may be used, especially among female patients, to help monitor for risk of worsening fibrosis during MTX therapy.

Code teams respond to acute life threatening changes in a patient's status 24 hours a day, 7 days a week. If any variable, whether a medical skill or non-medical quality, is lacking, the effectiveness of a code team's resuscitation could be hindered. To improve the overall performance of our hospital's code team, we implemented an evidence-based quality improvement restructuring plan. The code team restructure, which occurred over a 3-month period, included a defined number of code team participants, clear identification of team members and their primary responsibilities and position relative to the patient, and initiation of team training events and surprise mock codes (simulations). Team member assessments of the restructured code team and its performance were collected through self-administered electronic questionnaires. Time-to-defibrillation, defined as the time the code was called until the start of defibrillation, was measured for each code using actual time recordings from code summary sheets. Significant improvements in team member confidence in the skills specific to their role and clarity in their role's position were identified. Smaller improvements were seen in team leadership and reduction in the amount of extra talking and noise during a code. The average time-to-defibrillation during real codes decreased each year since the code team restructure. This type of code team restructure resulted in improvements in several areas that impact the functioning of the team, as well as decreased the average time-to-defibrillation, making it beneficial to many, including the team members, medical institution, and patients.

PURPOSE: The goals of this study were to assess prospectively the impact of obesity, alcohol use, and smoking on total mortality and to test the etiologic hypothesis that subjects with two or more of these risk factors may experience an elevated risk of overall mortality. METHODS: Information on body mass index (BMI), alcohol intake, cigarette smoking, and other life-style factors was obtained from a cohort of 8006 Japanese-American men living in Hawaii. They were between 45 and 68 years of age at the initial examination (1965–1968). After 22 years of follow-up that included nearly 159,000 person-years of observation, 2667 deaths from all causes were identified. RESULTS: There was a significant quadratic (J-shaped) relation between BMI and overall mortality. A weaker J-shaped pattern in risk was also present for the intake of alcohol. A strong positive association was observed with pack-years of cigarette smoking. A synergistic interaction between BMI and alcohol was statistically significant ( P = 0.0017). Specifically, men who had the lowest body mass (BMI 2 ) and drank moderately to heavily (⩾25 oz/mo) experienced a 63% excess risk (relative risk, 1.63; 95% confidence interval; 1.33 – 1.99) compared to a reference group composed of men who had intermediate body mass (BMI, 21.21 – 26.30 kg/m 2 ) and drank occasionally to lightly (0.01 – 24.99 oz/mo). The increase in risk due to the interactive effect of low BMI and high alcohol intake was stronger (and statistically significant) than when each of these risk factors was considered separately (excess risk, 28% and 2%, respectively). There was no significant interaction for BMI and cigarette smoking, for alcohol and cigarette smoking, or for the three factors combined. CONCLUSIONS: The most important finding of this study was that, in addition to confirming that cigarette smoking could shorten life, extreme (high or low) BMI values and high alcohol consumption are each potentially harmful to health, but even more so if moderate or heavy drinking is concomitant with low body mass, a possible indicator for low intake of nutrients.

Abstract Nephrosclerosis, commonly found in subjects with hypertension and diabetes, is marked by hyalinization of arterioles and fibroplasia of small arteries in the renal cortex. Cardiovascular risk factors that predicted subsequent hyalinization of renal arterioles at autopsy were identified, using data from the Honolulu Heart Program, a prospective epidemiological study of cardiovascular disease (CVD) in Japanese-American men. Among 8006 participants at baseline, 1381 died between 1965 and 1982; 285 of these had a protocol autopsy, and 150 had assessments of arteriolar hyalinization from renal tissue. Subjects were categorized into four groups on the basis of the number of hyalinized arterioles per square centimeter of renal tissue, and CVD risk factor levels and proportions were compared across these groups with the use of general linear models and logistic regression. Multivariate assessment using logistic regression demonstrated that diastolic blood pressure (DBP) and glucose level were positively associated and alcohol intake was negatively associated with an elevated degree of renal arteriolar hyalinization, independent of other CVD risk factors. The odds ratios for elevated hyalinization associated with a 10–mm Hg increase in DBP, a 20-mg/dL increase in glucose level, and a 30-mL/d increase in alcohol intake were 1.97 (95% confidence interval [CI]=1.24-3.12), 1.23 (95% CI=1.07-1.41), and 0.24 (95% CI=0.11-0.55), respectively. Associations were similar when prevalent cases of CVD were excluded and when autopsy selection bias was taken into account. Renal arteriolar hyalinization was also more strongly associated with atherosclerosis in the larger cerebral vessels (Spearman's r =.59, P r =.16, P =.073) and aorta ( r =.24, P =.022). Hyalinization was significantly related to cardiovascular-renal mortality, and this association was accounted for by other CVD risk factors. These findings suggest that blood pressure, glucose level, and alcohol intake are independent predictors of hyalinization in renal arterioles and that this type of renal vasculopathy may be a marker for atherosclerosis in other vascular regions, particularly the cerebral vessels. The protective association involving alcohol and the possibility that renal arteriolar hyalinization may be an indicator of cerebral atherosclerosis may warrant investigation in other populations.

Background Research has shown that fish consumption limits damage to the lungs caused by cigarette smoking, possibly by the effects of fish on arachidonic acid metabolism. We explored this fish-smoking interaction using coronary heart disease (CHD) incidence and mortality as the outcome. Methods and Results The Honolulu Heart Program began in 1965 to follow a cohort of 8006 Japanese-American men aged 45 to 65 years who lived on Oahu, Hawaii, in 1965. Fish intake was measured at baseline by use of a questionnaire. For current smokers at baseline (n=3310) who reported low fish intake (30 cigarettes/d, respectively; trend test P P Conclusions Despite the findings of this investigation, the public health message for smokers continues to be to stop smoking. However, an interaction between fish intake and cigarette smoking is biologically plausible and deserves further investigation. The study of this phenomenon may shed light on the biological mechanisms by which cigarette smoking leads to CHD.

Validating genetic associations underlying adverse drug events (ADE) is daunting due to the relatively low incidence of these events, yet such studies hold high significance due to the substantial morbidity and mortality associated with event incidence. Paradoxically, these studies require large numbers of patients incurring incident events. Validation studies thus require careful examination of feasibility and study design to ensure their success. The current study explored feasibility of a study designed to validate a previously observed genetic association and incidence of thromboembolic events in women exposed to tamoxifen observed in a prior pilot study.1 Although tamoxifen has proven efficacy against all stages of breast cancer, venous thromboembolic event (VTE) complications, including deep venous thrombosis (DVT) and pulmonary embolism (PE), are uncommon but serious consequences of tamoxifen treatment with an estimated incidence of 1.7% to 8.4%.2,3,4 Genetic variability in estrogen-dependent signaling has been postulated in association with incidence of adverse events.3 Activity of tamoxifen is associated with competitive binding to estrogen receptor 1 and 2 (ER1, ER2), blocking ligand binding, thus resulting in inhibition of estrogen-mediated growth of estrogen-sensitive tumors.5 Alleged ER-mediated alteration of hepatic synthesis of hemostatic proteins that may increase risk of VTE has been postulated.6 We extended this hypothesis and proposed that tamoxifen-related VTE may occur due to polymorphisms in the ER1 and ER2 genes, impacting variably on hemostatic factor synthesis depending on which single nucleotide polymorphisms (SNPs) are encoded. Specifically, we posited functionally important SNPs in ER candidate genes that influence risk for DVT/PE in patients receiving tamoxifen for breast cancer treatment. We tested this hypothesis by comparing genotypes of SNPS occurring in ER 1 on previously banked DNA from women with breast cancer treated with tamoxifen who experienced VTE, and women drawn from the same population who experienced no VTE following exposure. Study findings demonstrated a tentative association of XbaI (rs9340799) genotype and the ER1 Xbal/PvuII haplotype (rs9340799 and rs2234693) (P=0.035) among subjects who developed VTE.1 These data remain to be validated in a larger cohort of subjects in addition to SNPs in three genes that demonstrated association with DVT previously reported by Bezemer et al,7 including the SERPINC1 antithrombin gene (rs2227589) (odds ratio [OR] 1.29; risk allele frequency 0.10), GP6 platelet collagen receptor (rs1613662) (OR=1.15; risk allele frequency 0.84), and CYP4V2 in the cytochrome P450 family 4 (rs13146272) (OR=1.24; risk allele frequency 0.64) (P≤0.05 for all three SNPs). The modest relative risk contributed by these SNPs was deemed important, because it represented a large population-attributable risk percentage due to their widespread distribution in the population. A dosage effect with increased ORs was further noted in homozygotes compared to heterozygotes.7 These SNPs were also explored in the present study to determine relative contribution of additional risk to the tamoxifen-exposed target population or potential for genetic interaction with ER1 genes XbaI/PvuII and/or other known risk factors. Assuming a 4% incidence of VTE among women in our population, projected enrollment of several hundred women with incident VTE post tamoxifen exposure was projected to conduct a powered validation study. The study tested feasibility of enrolling, phenotyping, and genotyping 20% of the total number of validated eligible cases and controls using the prospective randomized controlled trial design in order to project enrollment rates and assess probability of enrolling an adequate sample size into a prospective genetic association validation study with 80% power at 5% level of significance. Further, the study sought to (1) test proposed methodology for identification of subjects, (2) assess clinical data capture and prospective collection of DNA, (3) establish genotyping capacity of collected DNA, and (4) test preliminary analytical approaches to the sample data, although such analyses were under-powered. Study outcomes are summarized in this report, and importance of conducting feasibility in the context of designing validation studies of genetic association studies that examine adverse event prediction related to pharmacological treatment is highlighted.

Abstract The role of insulin in cardiovascular disease is uncertain, and studies in elderly or minority populations are infrequent. Fasting and 2-hour insulin concentrations and their cross-sectional associations with cardiovascular risk factors were examined in 3562 elderly (aged 71 to 93 years) Japanese American men from the Honolulu Heart Program who were reexamined between 1991 and 1993. Insulin distributions were skewed (mean and median: 16.8 and 12 μU/mL for fasting; 117.2 and 93 μU/mL for 2-hour); fasting but not 2-hour insulin levels declined significantly with age ( P P =.54, respectively). Factors most strongly correlated with insulin included measures of obesity, fat distribution, and levels of triglyceride, glucose ( r =.38 to r =.50 fasting, r =.21 to r= .27 2-hour), and HDL cholesterol ( r =−.41 and r =−.22, respectively). Other correlates included fibrinogen, hematocrit, heart rate, blood pressure, cigarettes per day (all positive), alcohol, physical activity, and forced vital capacity (negative). Associations were also evident across risk factor quintiles. Insulin levels were significantly elevated in men with hypertension and diabetes. In multiple linear regression analyses, log 10 fasting insulin was positively and independently associated with body mass index, triglycerides, glucose, fibrinogen, hematocrit, heart rate, diabetes, and hypertension and negatively associated with HDL cholesterol, physical activity, and forced vital capacity. The incidence rate of gastric cancer among men of Japanese ancestry living in Hawaii is about one-third as high as that of their counterparts living in Japan. Because of this difference, a prospective study was conducted to identify factors related to the development of gastric cancer in Hawaii. Eight thousand and six (8,006) men born from 1900-1919 were examined from 1965 to 1968 and followed for over 25 years. During this time, 250 incident cases of gastric cancer were identified. The study has found the following : 1) prior infection with Helicobacter pylori bacteria increased the risk for stomach cancer ; 2) cigarette smoking was positively associated with gastric cancer with age at which smoking started being an important risk factor ; 3) after taking cigarette smoking into account, alcohol intake was not related to stomach cancer risk ; 4) a low pepsinogen I level identified subjects at increased risk for the intestinal histologic type of gastric cancer ; 5) a low serum ferritin level was a marker for increased risk of stomach cancer ; 6) there was a weak indication that the intake of vegetables and fruits was inversely related to gastric cancer ; 7) there was no association of stomach cancer with levels of serum cholesterol, serum uric acid, serum micronutrients (retinol, β-carotene or α-tocopherol) or blood hematocrit ; 8) there was also no association of gastric cancer with body mass index or physical activity.

The Honolulu Heart Program continues to follow a cohort of Japanese-American men initially aged 45-68 years, of whom 4,000 had three acceptable measurements of forced expiratory volume in 1 second (FEV1) between 1965 and 1974 and were free of cardiovascular disease and cancer. The 6-year rate of change (slope) in FEV1 was calculated using a within-person linear regression method. Men were divided into tertiles based on the rate of change in FEV1. During 17 subsequent years of follow-up, 796 deaths occurred. The tertile with the greatest rate of decline in FEV1 (mean, -61 ml/year) had the highest age-adjusted total mortality rate (17.3/1,000 person-years), followed by rates of 13.2 for the middle tertile (mean, -25 ml/year) and 11.0 for men with the smallest change in FEV1 (mean, +9 ml/year) (test for trend, p0.0001). Using the Cox model, comparing the tertile with the smallest change in FEV1 as a reference group with the tertile with the greatest decline in FEV1, and after adjusting for age, hypertension, smoking, body mass index, alcohol intake, diabetes mellitus, and cholesterol, the authors found the relative risk (RR) for total mortality to be 1.48 (95% confidence interval (CI) 1.24-1.77). After stratification by smoking status, this association remained significant for past smokers (RR = 1.79, 95% CI 1.31-2.14), as well as for the low,or = 42 (RR = 1.46, 95% CI 1.05-2.03), and high,42 (RR = 1.56, 95% CI 1.20-2.02), pack-year groups. An increased risk was also present for current smokers (RR = 1.29), but it was of borderline significance (p = 0.08). No association was found among never smokers. These data suggest that the rate of decline in FEV1 is a predictor of total mortality among smokers.

Dental health insurance coverage in the United States is either nonexistent (Medicare and the uninsured), spotty (Medicaid) and limited (most employer-based private benefit plans). Perhaps as a result, dental health in the United States is not good. What public policy makers may not appreciate is that this may well be impacting medical care costs in a way that improved dental benefits would produce a substantial return to investment in expanded dental insurance coverage.

Articulation of medical and dental practices has been strongly called for based on the many oral-systemic connections. With the rapid development and adoption of electronic health records, the feasibility of integrating medical and dental patient data should be strongly considered. The objective of this study was to develop an initial understanding of the medical providers’ core dental information needs and opinion of integrated medical-dental electronic health record (iEHR) environment in their workflow. This was achieved by administering a 13 question survey to a group of 1,197 medical care providers employed by Marshfield Clinic in Wisconsin, United States. The survey received a response rate of 35%. The responses were analyzed based on provider ‘Role’ and ‘Specialty’. The majority of the respondents felt the need for patient’s dental information to coordinate or provide effective medical care. An integrated electronic health record environment could facilitate this holistic patient care approach.

The association of alcohol, diet, and other lifestyle factors with obstructive uropathy was investigated in a cohort of 6581 Japanese-American men, examined and interviewed from 1971 to 1975 in Hawaii. By studying this migrant population with its heterogeneous exposures, it increases the probabilities of identifying potential risk factors of this prostate disorder. After 17 years of follow-up, 846 incident cases of surgically treated obstructive uropathy were diagnosed with benign prostatic hyperplasia. Total alcohol intake was inversely associated with obstructive uropathy (P < 0.0001). The relative risk was 0.64 (95% confidence interval: 0.52-0.78) for men drinking at least 25 ounces of alcohol per month compared with nondrinkers. Among the 4 sources of alcohol, a significant inverse association was present for beer, wine, and sake, but not for spirits. Buddhist (vs. other) religion, rural (vs. urban) birthplace, and the presence of prostate symptoms were each associated with increased risk of obstructive uropathy, but no association was found with education, number of marriages, or cigarette smoking. Increased beef intake was weakly related to an increased risk (P = 0.047), while no association was found with the consumption of 32 other food items in the study.

It is not well understood why strains of community-associated methicillin-resistant Staphylococcus aureus (CA-MRSA), a major cause of skin and soft tissue infections, became successful so quickly, overtaking the place of methicillin-sensitive S. aureus (MSSA) in many communities. To evaluate the genetic basis of differences in their virulence traits, 293 S. aureus isolates consisting of three cohorts, genotypically defined clinical CA-MRSA ( n = 77), clinical MSSA ( n = 103), and nasal carriage MSSA ( n = 113), collected over a 19-year period in two Midwestern states in the United States, were (i) extensively genotyped and (ii) screened for 40 known virulence genes which included those for enterotoxins, leukocidins, hemolysins, and surface proteins and several newly identified putative toxin genes from the USA400 lineage of CA-MRSA. Genotypically, nasal carriage and clinical MSSA isolates were much more diverse than was the CA-MRSA group, which was found to be of USA400 lineage only. Virulence gene profiles of the three groups showed that CA-MRSA strains harbored significantly higher percentages (≥95%; P value, sea , sec , sec4 , seg2 , seh , sek , sel , sel2 , ear , ssl1 , lpl10 , lukSF-PV , lukD , lukE , and clfA genes than did the carriage and the clinical MSSA group (range, 0% to 58%). Genes of the enterotoxin gene cluster, seg , sei , sem , sen , and seo , were present in the clinical and carriage isolates but not in the CA-MRSA group. These results suggest that the presence of additional virulence factors in USA400 CA-MRSA strains compared to the nasal carriage and clinical MSSA strains probably contributed to their enhanced virulence.

Objective: Determine and compare the prevalence of known risk factors for cardiovascular disease among unselected individuals presenting with their first ever episode of atrial flutter (AFL) and atrial fibrillation (AF). Study Design and Setting: We evaluated 11 pre-selected clinical variables including age, sex, smoking history and other potential cardiac risk factors. Using the resources of the Marshfield Epidemiologic Study Area, a population-based database, all newly diagnosed cases of either AFL or AF in the region during a 4-year period were identified. Results: Among the 472 incident cases, 76 (16.1%) had AFL and 396 (83.9%) had AF. Compared to those with AF, subjects with AFL were more likely to have had a history of chronic obstructive pulmonary disease (25% vs. 12%, P = 0.006), heart failure (28% vs. 17%, P = 0.05), and smoking (49% vs. 37%, P = 0.06). Hypertension, on the other hand, was more common among individuals with AF (63% vs. 47%, P = 0.01). Conclusion: This study represents the first report to evaluate potential differences in the conditions associated with the development of AFL versus AF. Research into the mechanisms of atrial arrhythmogenesis may lead to improved preventive and therapeutic interventions.

BACKGROUND. The goals of this study were to measure the impact of cigarette smoking on cancer incidence and to determine the attributable risk of cancer due to smoking. METHODS. A cigarette smoking history was obtained from 8006 Japanese-American men examined from 1965 through 1968. After 22 years, 1389 incident cases of cancer were identified. There were 212 men with lung cancer; 202 with oral, esophageal, laryngeal, pancreatic, renal, ureteral, or bladder (oral-bladder) cancer; and 975 with cancer at other sites. RESULTS. Current smokers at time of examination had a higher incidence than nonsmokers for each of the three cancer site categories. Eighty-five percent of lung cancer cases diagnosed among current and never smokers can be attributed to cigarette smoking. The attributable risks were 46%, 16%, and 29%, respectively, for oral-bladder cancers, other cancers, and all cancers combined. In turn, the corresponding attributable risks were 60%, 26%, 13%, and 21% in comparing current smokers with past smokers. CONCLUSIONS. Current smokers can greatly reduce their risk of cancer, especially lung cancer, if they quit smoking.

Background. Helicobacter pylori are gram-negative spiral bacteria that are associated with chronic gastritis, a known precursor of gastric carcinoma. Persons at high risk for gastric carcinoma have been shown to have a high prevalence of H. pylori infection. Methods. We studied the relation of H. pylori infection and gastric carcinoma in a cohort of Japanese American men living in Hawaii. The 5908 men were enrolled and examined from 1967 to 1970. By 1989, 109 cases of pathologically confirmed gastric carcinoma had been identified. The stored serum of each patient with gastric carcinoma and of each matched control subject was tested for the presence of serum lgG antibody to H. pylori. Results. Ninety-four percent of the men with gastric carcinoma and 76 percent of the matched control subjects had a positive test for H. pylori antibodies, for an odds ratio of 6.0 (95 percent confidence interval, 2.1 to 17.3). Based on previous reports, it is uncertain whether serum cholesterol levels are inversely related to colon cancer risk. In this study, serum cholesterol levels were measured in 7926 Japanese-American men who were followed for over 20 years. Two hundred thirty-one incident cases of colon cancer and 97 cases of rectal cancer were identified. An increase in serum cholesterol levels was associated with a decrease in risk for colon cancer (P value for trend = .01) but not for rectal cancer. This association appeared stronger as the site of cancer moved from the sigmoid colon to the cecum. The data were further analyzed by interval from examination to diagnosis. The inverse association was present for colon cancer cases diagnosed within 10 years of examination (P value for trend less than .01), especially for cecum-ascending colon cancer cases (P less than .01). A similar inverse pattern was found for cecum-ascending colon cancer cases diagnosed after 10 years, but the association was not statistically significant. The results suggest that the preclinical effects of undiagnosed colon cancer contributed to the inverse association, but these effects do not entirely explain why the relationship with hypocholesterolemia was stronger in men who were subsequently diagnosed with right-sided colon cancer.

Hawaiian Japanese men (n = 432) who had undergone subtotal gastrectomy for peptic ulcers before 1971–1975 were followed up for detection of cancer development. They showed a significant increase in colon cancer risk ( P = 0.008) and lung cancer risk ( P = 0.002) compared with 6161 nongastrectomized men. The association with lung cancer persisted after adjustment for cigarette use ( P = 0.03). Alcohol consumption was associated with colonic cancer in this cohort, and gastrectomized men consumed more alcohol than nongastrectomized men; however, the association of gastrectomy with colon cancer persisted after adjustment for alcohol use ( P = 0.02). Gastrectomized men were lighter and had lower serum lipid levels than controls, suggesting that undernutrition might favor the development of some cancers. The type of gastroenteric anastomosis did not influence the cancer risk level in the colon or lung.
