We report the case of a 60-year-old patient whose computed tomography revealed multiple pleural foci that were classified as potentially malignant. After revealing traumatic splenectomy in the patient's history, the differential diagnosis of splenosis was considered and a 99m-techentium heat-damaged autologous red blood cells scintigraphy performed. This conventional method can be used to reliably make an exact diagnosis avoiding more expensive or invasive methods.

Generalized lymphatic anomaly (GLA) is a congenital malformation of the lymphatic vessels that is often diagnosed in early childhood. Owing to the rarity and heterogeneity of its clinical course, GLA is frequently misdiagnosed, especially in adults. A 67-year-old man was incidentally found to have bone and pleural lesions. Multiple bone lesions detected on magnetic resonance images were mistaken for malignancy, and pathological evaluation led to the diagnosis of GLA. GLA should be considered in the differential diagnosis of multiple bone lesions, and a proactive biopsy to confirm the diagnosis may help avoid unnecessary invasive procedures.

The goal of the work described here was to compare high-frequency contrast-enhanced ultrasound (HF-CEUS) and conventional high-frequency ultrasound (HFU) with respect to performance and safety during ultrasound-guided biopsy of pleural lesions. We performed a retrospective study on patients with pleural lesions who received the puncture biopsy under the guidance of conventional HFU or HF-CEUS between August 2018 and August 2021. These patients received either a conventional HF-U (HF-U group) or HF-CEUS (HF-CEUS group) examination. Clinical characteristics, pathological results, ultrasonic images and complications were compared between these two groups. A total of 144 patients were enrolled, with 70 in the HFU group and 74 in the HF-ECUS group. Except for the time required for ultrasonic localization (p < 0.05), there were no significant differences in clinical characteristics between the two groups. The success rate of biopsy in the HF-CEUS group was higher than that in the HFU group (93.2% vs. 81.4%, p < 0.05). There were significant differences between the two groups in terms of measurements of pleural thickness, necrotic areas, large blood vessels and lesion boundaries (p < 0.05). The complication rates were 2.7% and 12.9% in the HF-CEUS and HFU groups, respectively, with a significant difference (p < 0.05). Compared with conventional HFU, the HF-CEUS-guided pleural biopsy had a better success rate and fewer complications. HF-CEUS could facilitate the biopsy in patients with pleural lesions. In 330 cases combined pleural effusion, CEUS guided biopsy increased the diagnostic yield from 60.30% (199 /330) to 98.36% (325 /330) in all cases (p < 0.05), from 15.56% (14/90) to 94.44% (85/90) in malignant lesions (p < 0.01) and from 77.08% (185/240) to 100% (240/240) in infectious lesions (p < 0.05). Cervical lymphadenectomy in thyroid cancer is a frequent intervention with around 20,000 operations per year in Germany. Some of the complications are identical with those in thyroid surgery, such as recurrent laryngeal nerve palsy and hypoparathyroidism as well as postoperative bleeding and wound infection. Specific complications of lateral lymphadenectomy are lesions of the accessory, phrenic and hypoglossal nerves, the sympathetic trunk and cervical plexus as well as lesions of the salivary glands and lymphatic vessels, in particular the thoracic duct. Most of these complications are rare with a frequency of less than 1%. Profound knowledge of the anatomy and a meticulous dissection technique make a decisive contribution to minimizing these complications. [ABSTRACT FROM AUTHOR]

Objective: To evaluate the efficacy, safety and feasibility of Ultrasound guided Sure-Cut Needle biopsy for peripheral lung and mediastinal nodules, as an out-patient procedure. Study Design: A Prospective Interventional study. Place and Duration: At Pulmonology-OPD, Gulab Devi Teaching Hospital Lahore from 1st October, 2016 to 28th February, 2019. Methodology: Peripheral lung, mediastinal or pleural lesions measuring greater than 2.0cm by ultrasound, abutting the visceral pleura were biopsied with Sure-Cut needle, gauge-16 under real-time ultrasonography with 3.5--5.5 MHZ machine in out-patient department. Adequacy of procedure was confirmed after receiving the histopathology report. All complications encountered during or after procedure were recorded. Results: Out of 179 procedures, 98.32% samples were adequate and among them 63.63% cases were malignant while 36.37%patients were with non-malignant etiology. The biopsy-induced complications observed were biopsy site pain (10.05%) and mild ooze at puncture site in 6.70% patients. No life threatening complication or table-death encountered. Conclusion: Ultrasound guided biopsy of peripheral lung and mediastinal lesions, greater than 2.0cm, has an excellent efficacy, safety and feasibility when performed as an out-patient-procedure. An alternative method that can be utilized is sampling of pleural lesion through convex probe endobronchial ultrasound (CEBUS) either through the central large airways or from esophagus if the lesions are in proximity. We present our center’s experience in diagnosing pleural lesion using C-EBUS in 4 patients along with review of the literature.

Abstract: 18-Fluoredeoxyglucose position emission tomography and computed tomography (18F-FDG PET/CT) scanning has been widely used in the assessment of malignancy. We report here an increased 18F-FDG uptake in pleural nodules in the PET/CT scan images of a patient having recurrent fever and chest pain. Further studies with bacterial culture and histopathology of biopsy confirmed the lesions as nocardial infection. To our knowledge, it is the first report of PET /CT findings in pleural multiple irregular nodules with nocardial infection. Our study suggests that a combination of FDG-PET/CT scanning and pathological study may be considered in distinguishing uncommon benign lesions in the lung. [Copyright &y& Elsevier]

Background: Ultrasound (US)-guided percutaneous needle biopsy is a useful diagnostic technique with short examination time and real-time monitoring at the bedside. However, there are only a few studies that report on thoracic lesions, whereas the computed tomography (CT)-guided biopsy is well established. There is also limited data comparing US- and CT-guided biopsy. We aimed to clarify the efficacy and safety of US-guided biopsy for thoracic lesions adjacent to the chest wall., Methods: We retrospectively enrolled consecutive patients who underwent US- or CT-guided percutaneous biopsies for thoracic lesions adjacent to the chest wall between April 2012 and December 2017. Clinical characteristics, lesion size, lesion-pleura contact arc length (LPCAL), diagnostic rate, and complications were compared between the 2 groups., Results: This study enrolled 61 US-guided and 70 CT-guided biopsies. No significant difference was found in age or sex. The lesion size and LPCAL in the US-guided group were significantly larger than those in the CT-guided group (P<0.0001). The diagnostic rate was marginally higher in the US-guided group (93.4%) than in the CT-guided group (84.3%) (P=0.101). When the median cut-off of the LPCAL was defined as 40 mm in all cases, the diagnostic rate for lesion size >40 mm was significantly higher in the US-guided group than in the CT-guided group (P=0.009). Complication rates were significantly lower in the US-guided group (3.3%) than in the CT-guided group (24.3%) (P<0.001)., Conclusions: US-guided percutaneous needle biopsy for thoracic lesions adjacent to the chest wall is a feasible technique compared with CT-guided biopsy because of its higher diagnostic rate with a longer LPCAL and reduced complications., Competing Interests: Conflicts of Interest: The authors have no conflicts of interest to declare.

A computed tomography scanner first, then a magnetic resonance imaging were performed for chest pain in a 24-year-old woman allowed to find out a 5-cm long and 2-cm large right pleural tumour close to the rachis (T9 and T10) and spindle-shaped. This patient was a smoker and reported a fall down the stairs a few weeks ago. A scan-guided biopsy was decided and microscopic examination revealed a fibrous tissue in which were entrapped regular and non-suspicious alveolar glands.
