Despite medical advances achieved recently, cardiovascular diseases are the maincause of morbidity and mortality in chronic kidney disease in the industrializedcountries. About half of deaths in these patients are due to a major cardiac event. While traditional risk factors are well identified, non-traditional risk factors have been highlighted. New pathophysiological pathways such as fibrosis and cardiac remodeling are being studied. Fibrosis and cardiac remodeling are particularly involved in heart failure with preserved ejection fraction.The main objective of this work is to observe early modifications of cardiac biomarkers during chronic kidney disease before dialysis. This work includes several studies allowing to answer to the main objective.In the first study, we proved the interest of using a clinical and biological tool in the prognostic assessment of major cardiac events and to prevent the complications of the chronic kidney disease. In the second work, we showed that an insulin-resistant and obese profile approaching metabolic syndrome exacerbates cardiac remodeling in rats with chronic renal failure. In this study, we were also able to propose that endothelio to mesenchymal transition phenomena at the origin of myofibroblasts could be involved in the process of cardiac fibrosis subsequent to chronic kidney disease. In the third experiment, we observed a discrete cardiac alteration after three months of renal insufficiency in rats made with an altered renal function induced at 12 weeks of life. In contrast, structural abnormalities of the heart are clearer when the renal insufficiency occurs at an earlier age (6 weeks of life), suggesting that cardiac alterations subsequent to a chronic kidney disease are more marked when the renal injury occurs in a young subject. Finally, all of this work contributes to the characterization of cardiac fibrosis and the functional impairment achieved during chronic renal failure., Malgré les progrès médicaux réalisés récemment, les pathologies cardio-vasculaires représentent les principales causes de morbidité et de mortalité chez le patient insuffisant rénal chronique. Environ 50% des décès chez ces patients sont dus à un évènement cardiaque majeur. Si des facteurs de risque traditionnels sont bien identifiés, des facteurs de risque non traditionnels ont été mis en évidence. De nouvelles voies physiopathologiques comme la fibrose et le remodelage cardiaque sont actuellement étudiées. La fibrose et le remodelage cardiaque sont notamment impliqués dans l’insuffisance cardiaque à fraction d’éjection préservée.L’hypothèse principale de ce travail est qu’il est possible d’observer des modifications précoces de marqueurs d’atteinte cardiaque au cours de l’insuffisance rénale chronique au stade de la pré-dialyse. Ce travail regroupe plusieurs études permettant de répondre à l’hypothèse principale. Dans la première étude, nous avons montré l’intérêt d’utiliser un score clinico-biologique dans l’évaluation pronostique des évènements cardiaques majeurs et permettant d’améliorer la prise en charge de la maladie rénale chronique et de prévenir ses complications.Dans la seconde, nous avons montré qu’un profil insulino-résistant et obèse se rapprochant du syndrome métabolique exacerbe le remodelage cardiaque chez les rats présentant une insuffisance rénale chronique. Dans cette étude, nous avons également évoqué des phénomènes de transition endothélio-mésenchymateuse à l’origine des myofibroblastes impliqués dans le processus de fibrose cardiaque dans la maladie rénale chronique. Dans la troisième étude, nous observons une discrète altération cardiaque après trois mois d’évolution de l’insuffisance rénale chronique chez des rats rendus insuffisants rénaux à 12 semaines de vie. En revanche, les anomalies de la structure cardiaque sont plus clairement présentes lorsque l’insuffisance rénale est initiée à un âge plus précoce (6 semaines de vie), donc que l’atteinte cardiaque consécutive à une insuffisance rénale chronique est plus marquée lorsque qu’elle survient chez un sujet jeune.Au final, l’ensemble de ces travaux apporte certains éléments permettant de répondre à la caractérisation de la fibrose cardiaque et l’atteinte fonctionnelle observée au cours de l’insuffisance rénale chronique.

Mindray BS480©, a multi-parametric and random-access clinical chemistry instrument, is suitable for medium-sized hospital applications. Large laboratories in hospital environments require high throughput non-emergency settings that could slow routine production lines. In addition, the possibility to adapt to different methodologies is of great convenience for improving the transfer from manual to automated applications. The aim of the study is to evaluate analytical performances and to draw a comparison between analyzers for most common clinical parameters under simulated routine conditions.Analytical performances in term of imprecision and comparison studies were performed between Mindray BS480© and reference analyzers such as Cobas 8000© for 12 routine parameters and ABX Pentra 400© for pyruvate and acetoacetate. Mindray BS480© usability, in terms of throughput and emergency sample handling, was also evaluated.Imprecision CVs for different analytes ranged from 0.7% to 7.9%, and the comparison study regression slope ranged from 0.74 to 1.22. Mindray workflow and emergency modes covered automated specifications.Results are considered significant for colorimetric, turbidimetric and ISE assays. Most performances were in line with Mindray and current recommendations. Mindray BS480© provides precise measurements for a variety of analytes. The possibility to adapt some methodologies is very useful, leading to a switch from manual methodology to automation.
