What is known on this topic * Many youth and young adults with behavioral health needs drop out or experience significant disruption in continuity of care. * Discontinuation of behavioral [...], Objective: To develop and test predictive models of discontinuation of behavioral health service use within 12 months in transitional age youth with recent behavioral health service use. Data sources: Administrative claims for Medicaid beneficiaries aged 15-26 years in Connecticut. Study design: We compared the performance of a decision tree, random forest, and gradient boosting machine learning algorithms to logistic regression in predicting service discontinuation within 12 months among beneficiaries using behavioral health services. Data extraction: We identified 33,532 transitional age youth with >1 claim for a primary behavioral health diagnosis in 2016 and Medicaid enrollment of >11 months in 2016 and >11 months in 2017. Principal findings: Classification accuracy for identifying youth who discontinued behavioral health service use was highest for gradient boosting (80%, AUC = 0.86), decision tree (79%, AUC = 0.84), and random forest (79%, AUC = 0.86), as compared with logistic regression (71%, AUC = 0.71). Conclusions: Predictive models based on Medicaid claims can assist in identifying transitional age youth who are at risk of discontinuing from behavioral health care within 12 months, thus allowing for proactive assessment and outreach to promote continuity of care for younger persons who have behavioral health needs. Algorithms for guiding health care decisions have come under increasing scrutiny for being unfair to certain racial and ethnic groups. The authors describe their multistep process, using data from 3,465 individuals, to reduce racial and ethnic bias in an algorithm developed to identify state Medicaid beneficiaries experiencing homelessness and chronic health needs who were eligible for coordinated health care and housing supports. Through an iterative process of adjusting inputs, reviewing outputs with diverse stakeholders, and performing quality assurance, the authors developed an algorithm that achieved racial and ethnic parity in the selection of eligible Medicaid beneficiaries. 8. Connections Between Sub‐Cloud Coherent Updrafts and the Life Cycle of Maritime Shallow Cumulus Clouds in Large Eddy Simulation.

We develop a novel approach to detect cloud‐subcloud coupling during the cloud life cycle and analyze a large eddy simulation of marine shallow cumulus based on the Barbados oceanographic and meteorological experiment campaign. Our results demonstrate how the activity of sub‐cloud coherent updrafts (SCUs) affect the evolution of shallow cloud properties during their life cycles, from triggering to development, and through to dissipation. Most clouds (∼80%) $(\sim 80\%)$ are related to SCUs during their lifetime but not every SCU (∼20% ${\sim} 20\%$ for short‐lived ones) leads to cloud formation. The fastest growing SCUs in a relatively moist region are most likely to initiate clouds. The evolution of cloud base mass‐flux depends on cloud lifetime. Compared with short‐lived clouds, longer lived clouds have longer periods of development, even normalized by the full lifetime, and tend to increase their cloud base mass‐flux to a stronger maximum. This is consistent with the evolution of mass flux near the top of SCU, indicating that the development of clouds is closely related to the sub‐cloud activity. When the SCUs decay and detach from the lifting condensation level, the corresponding cloud base starts to rise, signifying the start of cloud dissipation, during which the cloud top lowers to approach the rising cloud base. Previous studies have described similar conceptual pieces of this relationship but here we provide a continuous framework to cover all the stages of cloud‐subcloud coupling. Our findings provide quantitative evidence to supplement the conceptual model of shallow cloud life cycle and is critical to improve the steady‐state assumption in parameterization. Plain Language Summary: The coupling between shallow trade wind cumulus clouds and sub‐cloud processes is not well understood and remains one of the roadblocks to improving our climate modeling capability. As model resolution increases, the steady‐state assumption adopted in conventional convection schemes no longer holds and the knowledge about the evolution of clouds is necessary to improve the parameterization. Observations have provided evidence of sub‐cloud coherent updrafts being the roots of shallow clouds, but it is currently not practical to continuously detect how the shallow clouds evolve alongside the sub‐cloud coherent structures. Our study takes a step forward to demonstrate how the sub‐cloud coherent structures interact with the shallow clouds. It is shown that the evolution of sub‐cloud coherent structures plays an important role from cloud initiation, development and through to dissipation. The comprehensive physical picture of the whole cloud life cycle uncovered in this study provides useful insights to improve the representation of cloud evolution in convection parameterizations with an explicit treatment of the cloud life cycle in the future. We examine the ability of two dynamic turbulence closure models to simulate the diurnal development of convection and the transition from dry to shallow cumuli and then to deep convection. The dynamic models are compared with the conventional Smagorinsky scheme at a range of cloud‐resolving and grey‐zone resolutions. The dynamic schemes include the Lagrangian‐averaged, scale‐dependent dynamic Smagorinsky model and a Lagrangian‐averaged, dynamic mixed model. The conventional Smagorinsky model fails to reproduce the shallow convection stage beyond the large‐eddy simulation regime, continuously building up the convective available potential energy that eventually leads to an unrealistic deep convection phase. The dynamic Smagorinsky model significantly improves the representation of shallow and deep convection; however, it exhibits issues similar to the conventional scheme at coarser resolutions. In contrast, the dynamic mixed model closely follows the large‐eddy simulation results across the range of sub‐kilometre simulations. This is achieved by the combined effect of an adaptive length scale and the inclusion of the Leonard terms, which can produce counter‐gradient fluxes through the backscatter of energy from the subgrid to the resolved scales and enable appropriate non‐local contributions. A further sensitivity test on the inclusion of the Leonard terms on all hydrometeor fluxes reveals the strong interaction between turbulent transport and microphysics and the possible need for further optimisation of the dynamic mixed model coefficients together with the microphysical representation. 17. Mesonic fluctuations in a nonlocal Nambu-Jona-Lasinio model

The effects of meson fluctuations are studied in a nonlocal generalization of the Nambu-Jona-Lasinio model, by including terms of next-to-leading order (NLO) in 1/N_c. In the model with only scalar and pseudoscalar interactions NLO contributions to the quark condensate are found to be very small. This is a result of cancellation between virtual mesons and Fock terms, which occurs for the parameter sets of most interest. In the quark self-energy, similar cancellations arise in the tadpole diagrams, although not in other NLO pieces which contribute at the \sim 25% level. The effects on pion properties are also found to be small. NLO contributions from real $\pi\pi$ intermediate states increase the sigma meson mass by $\sim 30%$. 20. Meson properties in an extended nonlocal NJL model

We consider a nonlocal version of the NJL model, based on a separable quark-quark interaction. The interaction is extended to include terms that bind vector and axial-vector mesons. The nonlocality means that no further regulator is required. Moreover the model is able to confine the quarks by generating a quark propagator without poles at real energies. Working in the ladder approximation, we calculate amplitudes in Euclidean space and discuss features of their continuation to Minkowski energies. Conserved currents are constructed and we demonstrate their consistency with various Ward identities. Various meson masses are calculated, along with their strong and electromagnetic decay amplitudes. We also calculate the electromagnetic form factor of the pion, as well as form factors associated with the processes gamma gamma^* to pi^0 and omega to pi^0 gamma^*. The decays $\rho^0\to 2\pi^+2\pi^-$ and $\rho^0\to 2\pi^0\pi^+\pi^-$ are studied using various effective Lagrangians for $\pi$ and $\rho$ (and in some cases $a_1$) mesons, all of which respect the approximate chiral symmetry of the strong interaction. 22. The sacrifice of good conscience : religion, ethics and guilt in the work of Wittgenstein, Levinas and Derrida

Emphasising Wittgenstein's naturalism, I argue the common misconception that his later work harbours something 'relativistic'. A closer reading of Wittgenstein reveals instead a unifying picture of human life grounded upon 'primitive' (pre-linguistic) behaviours. Turning to explicitly 'religious' themes I then argue that Wittgenstein's ethicalisation of religion is also naturalistic. Thus 'man' is described as a 'ceremonial animal', and belief in the immortal soul is said to relate to an experience of 'guilt' from which one feels there is no release. More broadly, Wittgenstein's reflections lead to a conception of religiosity beyond the reparative economics of eschatological hope. At this point Levinas's own ethicalisation of religion becomes pertinent. I argue that his work is principally concerned with existential guilt, or the question: 'Do I have a right to be?'. That one exists always at the expense of another is what Levinas's thinking hinges upon, echoing throughout his analyses of language, the 'face', 'vulnerability' and 'home', and similarly orienting his characterisation of religion as 'love without reward'. However, Levinas's work is deeply anti-naturalism in that the ethical relation represents a radical 'break' from the (alleged) egoism of natural instinct. I here contend that this view of the natural is mistaken. Indeed, Levinas's central themes are more easily explicated in broadly Wittgensteinian- naturalistic terms. Thus, while Levinas provides a supplement to the ethical terrain of Wittgenstein's later work, Wittgenstein in turn offers a corrective to Levinas's 'spiritualised' humanism. Having drawn upon Derrida's work throughout my argument, in the final chapter I turn explicitly to his recent remarks on the 'gift' and 'hospitality'. For in his cautious articulation of Levinas's 'gratuitous' ethics, Derrida demonstrates, not only why it is 'impossible', but why this 'impossibility' is constitutive of ethical life and necessary if the 'scandal of good conscience' is to be resisted.

A nonlocal version of the NJL model is investigated. It is based on a separable quark-quark interaction, as suggested by the instanton liquid picture of the QCD vacuum. The interaction is extended to include terms that bind vector and axial-vector mesons. The nonlocality means that no further regulator is required. Moreover the model is able to confine the quarks by generating a quark propagator without poles at real energies. Features of the continuation of amplitudes from Euclidean space to Minkowski energies are discussed. These features lead to restrictions on the model parameters as well as on the range of applicability of the model. Conserved currents are constructed, and their consistency with various Ward identities is demonstrated. In particular, the Gell-Mann-Oakes-Renner relation is derived both in the ladder approximation and at meson loop level. The importance of maintaining chiral symmetry in the calculations is stressed throughout. Calculations with the model are performed to all orders in momentum. Meson masses are determined, along with their strong and electromagnetic decay amplitudes. Also calculated are the electromagnetic form factor of the pion and form factors associated with the processes gamma gamma* --> pi0 and omega --> pi0 gamma*. The results are found to lead to a satisfactory phenomenology and demonstrate a possible dynamical origin for vector-meson dominance. In addition, the results produced at meson loop level validate the use of 1/Nc as an expansion parameter and indicate that a light and broad scalar state is inherent in models of the NJL type.

The UK Met Office has a 300‐m grid length numerical weather prediction (NWP) model running routinely over London and, in research mode, city‐scale hectometric grid length NWP has become commonplace. It is important to understand how moving from kilometre‐ to hectometre‐scale grid length NWP influences boundary‐layer vertical mixing. For a clear‐sky convective boundary layer (CBL) case study, using 55‐ and 100‐m grid length NWP, we demonstrate that CBL vertical mixing of passive scalar is almost fully resolved. Passive scalar converges near the surface after emission from an idealised pollution ground source representing city‐scale emissions, and is transported in updrafts preferentially into the upper boundary layer. Approximately 8 km downstream of the source edge, this causes 34%$$ 34\% $$ lower near‐surface concentrations compared with 1.5‐km grid length NWP, where vertical mixing is fully parameterised. This demonstrates that resolving ballistic‐type dispersion, which is not typically represented in NWP vertical mixing parameterisations, can have a leading‐order influence on city‐scale near‐surface pollution concentration. We present a simple analytical model that is able to capture diffusive and ballistic dispersion behaviour in terms of effective timescales. The timescale controlling how long it takes passive scalar to become well mixed in the CBL is ≈3$$ \approx 3 $$ times longer for 1.5‐km compared with 100‐ and 55‐m grid length NWP. Contains fulltext : 252214.pdf (Publisher’s version ) (Open Access) BACKGROUND: Geoeconomic variations in epidemiology, the practice of ventilation, and outcome in invasively ventilated intensive care unit (ICU) patients without acute respiratory distress syndrome (ARDS) remain unexplored. In this analysis we aim to address these gaps using individual patient data of four large observational studies. METHODS: In this pooled analysis we harmonised individual patient data from the ERICC, LUNG SAFE, PRoVENT, and PRoVENT-iMiC prospective observational studies, which were conducted from June, 2011, to December, 2018, in 534 ICUs in 54 countries. We used the 2016 World Bank classification to define two geoeconomic regions: middle-income countries (MICs) and high-income countries (HICs). ARDS was defined according to the Berlin criteria. Descriptive statistics were used to compare patients in MICs versus HICs. The primary outcome was the use of low tidal volume ventilation (LTVV) for the first 3 days of mechanical ventilation. Secondary outcomes were key ventilation parameters (tidal volume size, positive end-expiratory pressure, fraction of inspired oxygen, peak pressure, plateau pressure, driving pressure, and respiratory rate), patient characteristics, the risk for and actual development of acute respiratory distress syndrome after the first day of ventilation, duration of ventilation, ICU length of stay, and ICU mortality. FINDINGS: Of the 7608 patients included in the original studies, this analysis included 3852 patients without ARDS, of whom 2345 were from MICs and 1507 were from HICs. Patients in MICs were younger, shorter and with a slightly lower body-mass index, more often had diabetes and active cancer, but less often chronic obstructive pulmonary disease and heart failure than patients from HICs. Sequential organ failure assessment scores were similar in MICs and HICs. Use of LTVV in MICs and HICs was comparable (42·4% vs 44·2%; absolute difference -1·69 [-9·58 to 6·11] p=0·67; data available in 3174 [82%] of 3852 patients). The median applied positive end expiratory pressure was lower in MICs than in HICs (5 [IQR 5-8] vs 6 [5-8] cm H(2)O; p=0·0011). - Abstract
Extended day treatment (EDT) is an innovative intermediate-level service for children and adolescents with serious emotional and behavioral disorders delivered during the after school hours. This paper describes the core components of the EDT model of care within the context of statewide systems of care, including its core service components, levels of care, focus on evidence-based practices, workforce composition and staff training, and data collection and reporting mechanisms. Recommendations are provided for statewide implementation, followed by discussion of model development as an approach to systems reform for treating children and youth with emotional and behavioral disorders.
