L’évolution de la prise en charge pharmacothérapeutique des patients atteints de cancer rend primordial le rôle du pharmacien officinal à travers sa prise en charge à l’officine mais aussi ses relations avec l’hôpital et les autres acteurs des soins primaires. L’objectif de ce travail est d’étudier cet accompagnement pharmacothérapeutique, et ce pour toutes voies d’administration confondues.

We present the development of new Polydimethylsiloxane (PDMS) chips, which are coupled with Love acoustic wave sensors to realize a detection cell of bio-organisms in liquid media. Three generations of biocompatible PDMS chips have been developed. Built-in thermistors allow a thermal control ({plus minus}0.05{degree sign}C). Unlike the usual assemblies, this chip is maintained by pressure on the sensor and not bonded welded on its surface. This technique makes it entirely removable and cleanable. Therefore, the surface of the sensor can be functionalised or regenerated. The realization of these chips is quick and inexpensive.

Five patients were successfully treated with a modified “on-top-plasty” technique, in which a finger stump is lengthened by transfer of an adjacent amputation stump with a reverse blood flow fingerstump. This technique can be performed in the acute phase or as a secondary procedure. A conventional on-top-plasty can be performed by transfer of a partially amputated index or ring finger to the “top” of the proximal phalanx of an amputated middle finger. Alternatively, the transferred part may be used in an intercalated fashion to reconstruct the middle phalanx, using a prosthesis to reconstruct the proximal interphalangeal joint. The results, complications and disadvantages of the technique are reported. We propose this procedure for the reconstruction of the middle ring finger when a free microneurovascular toe-to-hand transfer is contraindicated or refused by the patient.

To avoid a more proximal amputation at the distal part of the thigh, and when the knee joint is preserved, it is possible to lengthen short lower leg stumps. The authors report five cases in which the latissimus dorsirib flap was used to achieve a satisfactory functional prosthetic result. The bone segment is long enough to both lengthen the stump and allow its extremities to be firmly fixed to the tibia. Depending on the remaining tibia length, one or two ribs were included in the flap. The procedure allowed achievement of a 5cm to 9cm lengthening of the tibia. Bone healing time was 5 to 6 months and allowed prosthetic rehabilitation and ambulating 5 to 7 months after surgery. Final range of motion of the knee joint is compatible with normal ambulating, and the prosthesis is well tolerated. This procedure, which provides a large amount of skin, muscle, and bone, is very effective for reconstruction of short lower leg stumps. Plast. Reconstr. Surg.109 1013, 2002.

This article describes the authors' experience with the submental flap over the past 10 years. A brief review of the key points and some refinements in the operative technique are discussed. The results concern 31 patients with a mean age of 57 years. All flaps were pedicled except two. One case of composite flap with bone was used. The mean size of the flap was 11.8 × 5.5 cm, and the mean postoperative stay was 11.1 days. Complications encountered were one case of temporary palsy of the marginal mandibular branch of the facial nerve, one hematoma at the recipient site, and two cases of partial flap loss. Color and texture match were good. The authors believe this flap to have great clinical potential and to be a worthwhile addition to the existing surgical armamentarium. Plast. Reconstr. Three cases of distal thumb reconstruction with a reverse pedicled osteocutaneous flap taken from the dorso-ulnar aspect of the first metacarpal are presented. Even though the indications are rare, this flap is useful for the reconstruction of distal osteocutaneous defects of the thumb where more complex procedures are not feasible or considered as excessive. The submental island flap is a useful tool for facial reconstruction. It provides an excellent color and texture match for the face with a well-hidden donor site. The flap is simple and safe to raise. A wide range of applicability is allowed with the use of skin, the anterior belly of the digastric muscle, and the rim of the mandible. It is a valuable adjunct in the armamentarium of the reconstructive surgeon.

Twenty-nine hands were dissected in order to define the dorsal blood supply of the thumb, the first web and the index finger. The main objective was to determine if it is possible to create a local osteocutaneous flap to cover partial and complex tissue defects in the distal part of the thumb. We found that the thumb metacarpal could be used as a donor site in these situations, with either a radiodorsal pedicle or an ulnadorsal pedicle.

AbstractIn northeast Vietnam, the karst of Halong Bay is characterized by very active neotectonics. The directional distribution of fracturing of the calcareous rocks is characterized by the influence of two major fault zones: the Red River fault zone (N140) and the Tan-Lu fault zone (N050). Karst development was favoured by intense fracturing, according to these two major directions, and reactived during recent tectonics by a compressional regime with σ1 N070, followed by an extensional regimes with σ1 near to EW that led to significant vertical movement. These tectonics, coupled with intense erosion, led to genesis and evolution of the spectacular morphology of this peak karst. © Elsevier, Paris
