5. Endoscopic and surgical treatment of non‐neoplastic proximal duodenal ulceration in dogs, and anatomical study of proximal duodenal vascularisation.

Abstract

Objectives: Proximal duodenal ulceration is often characterised by continuous bleeding, and treatment is challenging. The aims of this study were to investigate the role of vascularisation in proximal duodenal ulceration and describe clinical aspects, endoscopic features and treatment in dogs. Material and Methods: Polyurethane foam casts of gastroduodenal vessels were obtained from five dogs which had died from disorders unrelated to the digestive system. In addition, 12 dogs having proximal duodenal ulcers diagnosed by endoscopic examination were enrolled in a treatment trial. After the endoscopic diagnosis of a duodenal ulcer, all the dogs were treated medically and, in the absence of resolution, were subsequently treated by endoscopic electrocauterisation or by surgery. Results: A submucosal vascular network was evident in all the casts, with a prominent venous plexus seen exclusively in the first half inch of the duodenum. In clinical cases, on endoscopic examination, the duodenal ulcer was located at the proximal part of the duodenum, involving the mesenteric portion of the wall. The dogs not responding to medical treatment (6/12) were treated with endoscopic electrocauterisation, surgical coagulation or resection of the proximal duodenal portion. All the dogs survived until discharge, and the median survival time following discharge was 107.5 days. Clinical Significance: Based on the anatomical details highlighted in this study, the continuous bleeding observed in our patients may have been due to the prominent venous plexus evidenced at the level of the proximal duodenum. Abstract

Hernias of intervertebral discs are a common canine disease that is usually treated surgically. Recently, a histological scoring system for surgically removed canine intervertebral herniated discs has been developed by scoring the lesions of both the anulus fibrosus (AF) and the nucleus pulposus (NP). Since the proportion of AF and NP in the surgical samples may vary, depending on the surgical approach, the aim of this study is to grade separately the lesions in AF and NP and to modify the previously described scoring system adding three parameters: inflammation, mineralization and neovascularization. Possible association of the modified grading system with clinical parameters were statistically assessed. Herniated disc material was collected from 27 dogs. AF was present in 10/27 cases and was classified as grade 2 in 4 cases, grade 3 in 5 cases and grade 4 in 1 case. The NP was present in all 27 cases and was classified as grade 2 in 1 case, grade 3 in 5 cases, grade 4 in 9 cases and grade 5 in 12 cases. Abstract

Early treatment of inflammatory arthritis (IA) leads to reduced disease activity, reduced joint damage, decreased functional impairment and increased chance of remission. However, delay often occurs from referral to rheumatology appointment. This survey evaluated whether a preliminary triage carried out by healthcare workers without formal medical training could be effective in identifying patients with or without early IA.Patients were recruited during their first call to our centre, before their first visit. A simple questionnaire, including three questions and aimed at investigating the presence of sign and symptoms of IA was developed. The same survey was administered twice: the first time, during patient's first call to our centre (telephone survey), and the second time, during their first visit with the rheumatologist (Ambulatory visit survey). We compared the outcomes of the survey with the actual diagnosis made by the rheumatologist following standard medical examination.In total 484 patients were included in the study, and 34/484 (7.02%) were confirmed to have early IA. The telephone survey was able to detect the non-early IA patients in 99.5% of cases; the same result was reported for the ambulatory visit survey. The median time required to complete the questionnaire was 1 minute in both surveys.The adoption of a simple survey, also administered by non-medical personnel, may effectively contribute to the early detection of IA.

30. Clinical signs and MRI findings of bilateral porencephaly and caudal vermian hypoplasia in a young dog

Abstract

The present report describes a 4 months-old mixbreed female dog referred to the Veterinary Teaching Hospital of Bologna University for the suspicion of atypical seizure activity. The dog was adopted 40 days before from a kennel. After ten days, the owner noticed the occurrence of sudden-onset “attacks”, described as follows: sudden tilting of the head to the right, falling towards her right side, pathologic nystagmus and opisthotonus. After approximately 30 seconds, the dog recovered completely to a normal status. Episodes occurred two to three times per day. The owner reported also episodes of tail chasing and right circling lasting 10 to 15 minutes, which tended to stop once the patient was diverted. Physical evaluation showed an abnormal right deviation of the nasal structures. The neurological examination showed a normal mental status and behaviour, a mild right head tilt, vestibular ataxia, decreased postural reactions more pronounced on the left limbs, menace response absent on the left side and reduced contralaterally, and right ventral strabismus. Neuroanatomical localization of the lesion involved the central vestibular structures and the forebrain. Major differential diagnoses included as first anomalous disorders. CBC and serum biochemistry were within the normal range. The dog underwent magnetic resonance imaging (MRI) of the brain. The dog was imaged in sternal recumbency with a 0.22-T permanent magnet using a human knee coil. Sagittal, transverse, and dorsal plane images of the brain were acquired. Image sequences included T2- and T1-weighted images, and FLAIR. An area of low signal intensity was observed on T1-weighted images in the caudal cerebellar vermian region. The same region was characterized by a high signal intensity on T2-weighted images. These characteristics were consistent with the presence of a fluid-filled cavity in the region of the cerebellum, assumed to be due to cerebrospinal fluid accumulation. A second lesion with the same characteristics, connecting the lateral ventricle with the subarachnoid space, was observed in the left parietal lobe. A third, smaller, porencephalic area was noted in the right cerebral hemisphere. The final diagnosis, based on the clinical signs and MRI findings was congenital bilateral porencephaly and caudal vermian hypoplasia. The vestibular episodes were considered as possible partial-complex epileptic seizures and anticonvulsant therapy with Phenobarbital was instituted. The rate of recurrence of the seizures did not improve at starting dose. After an adjustments in the dosage based on serum Phenobarbital level, the episodes improved in frequency and entity. The non-progressive, atypical, clinical signs in our puppy fits very well with the multiple abnormalities documented on MRI, affecting both the cranial and the caudal fossa. Caudal fossa lesions are consistent with those reported in the Dandy-Walker Syndrome (DWS). DWS is a well known human congenital brain disorder which refers to a malformation complex characterized constantly by cerebellar vermis hypoplasia and enlargement of the fourth ventricle frequently associated to different abnormalities of other areas of the central nervous system and/or the heart, face, limbs, fingers and toes. In veterinary literature, these kinds of malformation have been sporadically reported in animals, including calves, foals, lambs, cats and dogs. The malformation is often incomplete in respect to human medicine, with varying degrees of vermian hypoplasia and differences in the severity of clinical signs. Forebrain MRI abnormalities in our puppy can be defined as asymmetric bilateral porencephaly. Porencephaly refers to the occurrence of single or multiple cystic cavities in the cerebrum, usually communicating with the lateral ventricles or subarachnoid space. 43. Effects of Glubran Acrylic Glue on the Subarachnoid Surface in Swine: Preliminary Findings

Abstract

Glubran2 is a cyanoacrylate-based synthetic glue modified by addition of a second monomer. Among its many advantages over traditional cyanoacrylate products, Glubran2 has a much lower thermal polymerization temperature than that of cyanoacrylates used for cutaneous application. For many years, the new glue has been widely used in neuroradiology endovascular treatments. Our study aimed to assess the use of Glubran2 to treat possible complications arising during the embolization of aneurysms or vascular malformations like AVM or fistulae. A complication encountered in some of these cases is rupture of the vessel wall by the microguide or perforation of the aneurysmal sac by the coils used for embolization. This complication could be overcome by injecting Glubran2 into the rupture point but this would put the glue directly in contact with the subarachnoid space and possibly reach adjacent brain tissue. The aim of this experimental study was to place the glue in direct contact with brain tissue to ascertain its immediate compatibility and tolerability in vivo and any ensuing tissue damage. Three swine underwent neurosurgical procedures and survived for different times after which they were slaughtered and their brains explanted and sent for macroscopic and microscopic investigation by the Pathological Anatomy Service. As a whole, Glubran2 was well tolerated in vivo and did not cause major damage in direct contact with brain tissue.
