Abstract: Deep polarization surveys at low radio frequencies are key to cosmic magnetism studies: Larger catalogs of polarized extragalactic sources and increased precision on Faraday rotation measures (RMs) make it possible to probe the magneto-ionic medium along the lines of sight of the sources and to construct denser RM grids. In a first paper, we presented a search for polarized sources in deep observations of the 25 square degree area of the European Large Area ISO Survey North 1 (ELAIS-N1) field with the LOw Frequency ARray (LOFAR) in the range 114.9 to 177.4 MHz. In this paper, we investigate the properties of the polarized radio galaxies and use the catalog to produce an RM grid of the field. After identifying the host galaxies and collecting redshift information, we characterized the radio galaxies in terms of their radio morphologies, rest frame radio luminosities, and linear sizes. We calculated residual rotation measures (RRMs) by removing the Galactic RM and studied the variation in the RRMs with redshift and degree of polarization. We produced an RRM grid of the field and compared the positions of the polarized sources with those of galaxy clusters and superclusters. The radio galaxies show a variety of morphologies, including diffuse emission; Fanaroff Riley type II sources make up about half of the sample. Using available multiband catalogs, we found redshifts for the hosts of all polarized sources in the range of 0.06 to 1.9. Polarized emission is detected mainly from large radio galaxies. The RRM values have a median close to zero, and they appear to be independent of redshift and degree of polarization. The sources in the lines of sight of clusters of galaxies and of a supercluster are indistinguishable in their polarization and RRM properties from the population of sources that are not behind these structures., Comment: 15 pages, 7 figures

Abstract: The aim of this study is to probe the sub-mJy polarized source population with LOFAR. We present the method used to stack LOFAR polarization datasets, the resulting catalog of polarized sources, and the derived polarized source counts. The ELAIS-N1 field was selected for a polarimetric study at 114.9-177.4 MHz. A total area of 25 deg2 was imaged at 6"- resolution in the Stokes Q and U parameters. Alignment of polarization angles was done both in frequency and in Faraday space before stacking datasets from 19 eight-hour-long epochs. A search for polarized sources was carried out in the final, stacked dataset, and the properties of the detected sources were examined. The depolarization level of sources known to be polarized at 1.4 GHz was quantified. A one-sigma noise level of 19 {\mu}Jy/beam was reached in the central part of the field after stacking. Twenty-five polarized sources were detected above 8\sigma, five of which had not been detected in polarization at any other radio frequencies before. Seven additional polarized components were found by lowering the threshold to 6\sigma at positions corresponding to sources known to be polarized at 1.4 GHz. In two radio galaxies, polarization was detected from both radio lobes, so the final number of associated radio continuum sources is 31. The detected sources are weakly polarized, with a median degree of polarization of 1.75% for the sample of sources detected in polarized emission. The sources previously detected in polarization at 1.4 GHz are significantly depolarized at 150 MHz. The catalog is used to derive the polarized source counts at 150 MHz. Abstract: We present deep polarimetric observations of the European Large Area ISO Survey-North 1 (ELAIS-N1) field using the Low Frequency Array (LOFAR) at 114.9-177.4 MHz. The ELAIS-N1 field is part of the LOFAR Two-metre Sky Survey deep fields data release I. For six eight-hour observing epochs, we align the polarization angles and stack the 20"-resolution Stokes $Q$, $U$-parameter data cubes. This produces a 16 deg$^2$ image with 1$\sigma_{\rm QU}$ sensitivity of 26 $\mu$Jy/beam in the central area. In this paper, we demonstrate the feasibility of the stacking technique, and we generate a catalog of polarized sources in ELAIS-N1 and their associated Faraday rotation measures (RMs). While in a single-epoch observation we detect three polarized sources, this number increases by a factor of about three when we consider the stacked data, with a total of ten sources. This yields a surface density of polarized sources of one per 1.6 deg$^2$. The Stokes $I$ images of three of the ten detected polarized sources have morphologies resembling those of FR I radio galaxies. This represents a greater fraction of this type of source than previously found, which suggests that more sensitive observations may help with their detection., Comment: This paper is part of the 1st data release of the LoTSS Deep Fields. Abstract: Background: Current management practices and outcomes in weaning from invasive mechanical ventilation are poorly understood. We aimed to describe the epidemiology, management, timings, risk for failure, and outcomes of weaning in patients requiring at least 2 days of invasive mechanical ventilation. Methods: WEAN SAFE was an international, multicentre, prospective, observational cohort study done in 481 intensive care units in 50 countries. Eligible participants were older than 16 years, admitted to a participating intensive care unit, and receiving mechanical ventilation for 2 calendar days or longer. We defined weaning initiation as the first attempt to separate a patient from the ventilator, successful weaning as no reintubation or death within 7 days of extubation, and weaning eligibility criteria based on positive end-expiratory pressure, fractional concentration of oxygen in inspired air, and vasopressors. The primary outcome was the proportion of patients successfully weaned at 90 days. Key secondary outcomes included weaning duration, timing of weaning events, factors associated with weaning delay and weaning failure, and hospital outcomes. This study is registered with ClinicalTrials.gov, NCT03255109. Findings: Between Oct 4, 2017, and June 25, 2018, 10 232 patients were screened for eligibility, of whom 5869 were enrolled. 4523 (77·1%) patients underwent at least one separation attempt and 3817 (65·0%) patients were successfully weaned from ventilation at day 90. 237 (4·0%) patients were transferred before any separation attempt, 153 (2·6%) were transferred after at least one separation attempt and not successfully weaned, and 1662 (28·3%) died while invasively ventilated. The median time from fulfilling weaning eligibility criteria to first separation attempt was 1 day (IQR 0–4), and 1013 (22·4%) patients had a delay in initiating first separation of 5 or more days. To date, the collected evidence on the efficacy and tolerability of pharmacological prevention therapies raises more doubts than hopes. Abstract: In developed countries, the largest share of food is wasted at the household level. Household food waste results from a complex interaction between economic factors, well-established routines, and social norms. To explain this interaction, we propose a simple model of waste behavior where the individual and social economic costs generated by wasting are counterbalanced by the security and status generated through acquiring excess food, thus causing a social dilemma. This trade-off is mediated by social capital, which measures the intensity with which each individual within a community evaluates the negative effects of waste. We test this model's hypotheses using a 2016 dataset of food behaviors and opinions of Italian households, which we merge with variables known to elicit the local level of social capital. We find individual food waste levels to be negatively related with social capital. Contrastingly, status concerns with respect to food and the lack of organizational abilities are both more prevalent in low social capital areas, and are related to increased food waste. This relationship is mediated by income.

Abstract: The outbreak of the COVID-19 pandemic has led to a disruption of surgical care. The aim of this multi-centric, retrospective study was to evaluate the impact of the pandemic on surgical activity for thyroid disease among the Italian Units of Endocrine Surgery. Three phases of the pandemic were identified based on the epidemiological situation and the public measures adopted from the Italian Government (1st phase: from 9th March to 3rd May 2020; 2nd phase: from 4th May to 14th June; 3rd phase: from 15th June to 31st). The patients operated upon during these phases were compared to those who underwent surgery during the same period of the previous year. Overall, 3892 patients from 28 Italian endocrine surgical units were included in the study, 1478 (38%) operated upon during COVID-19 pandemic, and 2414 (62%) during the corresponding period of 2019. The decrease in the number of operations was by 64.8%, 44.7% and 5.1% during the three phases of COVID-19 pandemic, compared to 2019, respectively. During the first and the second phases, the surgical activity was dedicated mainly to oncological patients. No differences in post-operative complications were noted between the two periods. Oncological activity for thyroid cancer was adequately maintained during the COVID-19 pandemic.

Abstract: In this work, the synthesis of the cannabinoid receptor 1 neutral antagonists 8-chloro-1-(2,4-dichlorophenyl)-N-piperidin-1-yl-4,5-dihydrobenzo-1H-6-oxa-cyclohepta[1,2-c] pyrazole-3-carboxamide 1a and its deaza N-cyclohexyl analogue 1b has led to a deepening of the structure-activity studies of this class of compounds. A series of novel 4,5-dihydrobenzo-oxa-cycloheptapyrazoles analogues of 1a,b, derivatives 1c-j, was synthesized, and their affinity towards cannabinoid receptors was determined. Representative terms were evaluated using in vitro tests and isolated organ assays. Among the derivatives, 1d and 1e resulted in the most potent CB1 receptor ligands (KiCB1 = 35 nM and 21.70 nM, respectively). Interestingly, both in vitro tests and isolated organ assays evidenced CB1 antagonist activity for the majority of the new compounds, excluding compound 1e, which showed a CB1 partial agonist behaviour. CB1 antagonist activity of 1b was further confirmed by a mouse gastrointestinal transit assay. Significant activity of the new CB1 antagonists towards food intake was showed by preliminary acute assays, evidencing the potentiality of these new derivatives in the treatment of obesity.

Abstract: Acute pyelonephritis (aPN) is defined as a severe form of urinary tract infection. Despite its severity and the high incidence in the community setting, there is no consensus on the optimal duration of treatment. The aim was to compare effectiveness and tolerability of short-versus long-course treatment with the same antibiotic agent in patients with aPN. We included 4 RCTs (439 participants). Short antibiotic treatment lasted from 4 to 14 days and long treatment from 7 to 42 days but was at least 2 days longer than the corresponding short-course. At the end of treatment, we did not find any significant differences in clinical success [risk ratio (RR) 1.01; 95% confidence interval (CI), 0.96-1.07, moderate quality evidence] as well as in microbiological success (RR 0.99; 95% CI, 0.92-1.07, very low-quality evidence). At 4-6 weeks after the end of treatment there were no significant differences in clinical relapses (RR 1.20,95% CI 0.43-3.30, very low-quality evidence) and re-infection of other germs (RR 2.40; 95% CI, 0.68-8.49, very low-quality evidence), even if short-term therapy seemed to have more risk of recurrences (RR 2.39, 95% CI 1.19-4.83, very low quality of evidence). The incidence of any adverse effect seemed to be lower with the short-term therapy, though the results are not statistically significant (RR 0.63, 95% CI 0.39-1.02, low quality evidence). Short-term treatment for aPN seems to be equivalent to long-term treatment in terms of clinical and microbiological success at the end of treatment or tolerability. The only relevant difference is the frequency of recurrence of the same biological germ up to 4-6 weeks after the end of treatment, which is significantly higher with the short-term therapy

Abstract: Purpose – Agri-food supply chains are facing a number of challenges, which cause inefficiencies resulting in the waste of natural and economic resources, and in negative environmental and social impacts. To deal with such inefficiencies, food supply chain actors search for economically viable innovations to prevent and reduce food waste. This paper analyses the factors that affect the decision of supply chain operators to adopt innovations to reduce food waste. Design/methodology/approach – The analysis has been carried out using a four step approach that included: 1) a systematic literature review to identify economic factors affecting the decision to adopt these innovations; 2) an inventory of food waste drivers and reduction possibilities along food supply chains through innovations; 3) a mapping of the results of step 1 and 2; 4) deriving conclusions regarding the factors affecting the adoption of a specific innovation to reduce and prevent food waste. Practical implications – This study analyses the drivers of food waste generation together with the factors affecting the decision to adopt innovations to reduce food waste, and provides solutions to supply chain operators to prevent and reduce food waste through different types of innovations. Originality/value – Literature did not addressed systematically innovation aiming at the reduction of food waste yet. This paper provides a novel view on the problem of food waste reduction by means of innovation, by linking factors affecting the decision to innovate with food waste drivers.

Abstract: In the field of cardiovascular diseases, elevated levels of serum uric acid (UA) reflect a marked activation of the xanthine oxidase pathway with increase in free radicals production; it is often associated with an inflammatory state, oxygen consumption and endothelial dysfunction. All these associations have been also confirmed in heart failure (HF) but the pathophysiological role of UA in this setting is not well understood. The aim of this study was to evaluate the prognostic role of UA in outpatients enrolled in the Italian Registry of Congestive Heart Failure (IN-CHF).All patients met the European Society of Cardiology (ESC) criteria for diagnosis of HF. We considered patients with complete clinical data and UA level available at the baseline and at 1-year follow-up. The study population was composed of 877 patients aged 63 ± 12 years. One-year mortality was 10.8% and dead patients had a higher level of UA than survivors (7.1 mg dl⁻¹ vs 6.6 mg dl⁻¹, p0.0207). In multivariable full model of analysis, UA did not result in an independent predictor of death in overall population, but only in patients with low body mass index (BMI) (≤22 kg m⁻²) (hazard ratio (HR): 2.38, 95% confidence interval (CI) 1.36-4.18). In this subgroup, a statistically significant gradual relationship between UA and survival was detected starting from values higher than 8 mg dl⁻¹.Elevated level of UA is not an independent predictor of mortality in chronic HF, but it markedly worsens outcome if associated with low level of BMI. This association is likely an indicator of chronic inflammatory and catabolic state.

Abstract: Aims. The European Federation of Psychiatric Trainees (EFPT), founded in London in 1992, is an independent international federation of psychiatric trainees' national associations. The EFPT is engaged in several actions in order to pursue its general aims to promote high standards of quality of psychiatric training and promote the collaboration and networking between European psychiatric trainees. Methods. Member associations constitute the general assembly that meets annually during the European Forum of Psychiatric Trainees. During the Forum, working groups are created and the activities for the next year are planned. Results. Regular activities of the EFPT include the production of statements expressing the standpoint of trainees on educational issues, the conduction of research projects, the creation of new national trainees' associations, the facilitation of networking among European psychiatric trainees and the organization an annual meeting (EFPT Forum). The last EFPT Forum has been organized in Sorrento (Italy) on May 2012. Psychiatric trainees from 37 European countries have registered to this event. Discussion. EFPT activities constitute a opportunity for psychiatric trainees to directly participate in the improvement of their psychiatric training and to establish a network with European colleagues that will contribute to their professonal development.

Abstract: The transcription factor, nuclear factor erythroid 2 p45-related factor 2 (Nrf2), acts as a sensor of oxidative or electrophilic stresses and plays a pivotal role in redox homeostasis. Oxidative or electrophilic agents cause a conformational change in the Nrf2 inhibitory protein Keap1 inducing the nuclear translocation of the transcription factor which, through its binding to the antioxidant/electrophilic response element (ARE/EpRE), regulates the expression of antioxidant and detoxifying genes such as heme oxygenase 1 (HO-1). Nrf2 and HO-1 are frequently upregulated in different types of tumours and correlate with tumour progression, aggressiveness, resistance to therapy, and poor prognosis. This review focuses on the Nrf2/HO-1 stress response mechanism as a promising target for anticancer treatment which is able to overcome resistance to therapies.

Abstract: RAGE is a multiligand receptor able to bind advanced glycation end-products (AGEs), amphoterin, calgranulins, and amyloid-beta peptides, identified in many tissues and cells, including neurons. RAGE stimulation induces the generation of reactive oxygen species (ROS) mainly through the activity of NADPH oxidases. In neuronal cells, RAGE-induced ROS generation is able to favor cell survival and differentiation or to induce death through the imbalance of redox state. The dual nature of RAGE signaling in neurons depends not only on the intensity of RAGE activation but also on the ability of RAGE-bearing cells to adapt to ROS generation. In this review we highlight these aspects of RAGE signaling regulation in neuronal cells.

Abstract: Aims. To assess in a sample of people with mental disorders: 1) fear of crime and perceived insecurity; 2) the association between fear of crime and insecurity; 3) the incidence of crimes. Methods. Twenty-four Italian mental health centres have been invited to participate in the study from the network of the Early Career Psychiatrists' Committee of the Italian Psychiatric Association. In each participating centre, the first 20 patients consecutively accessing the mental health centre between February and April 2011 have been recruited. All patients have been assessed using validated assessment tools. Results. The final sample consists of 426 patients. They are mostly female (70.1%), with a mean age of 45 years (±13.5), and with a good level of education. Fifty-two percent of patients have a diagnosis of mood disorders, and 37.8% on anxiety spectrum disorders. About half of the sample declares that the most prominent feeling toward life is uncertainty. Almost all patients report to have at least one big fear, with the most frequently report being: 1) loss or death of a loved one (41.2%); 2) financial constraint (28.4%); 3) physical or mental health problems (26.5%). Discussion and conclusions. Our results show the presence of a common sense of uncertainty among patients, probably as a result of the historical moment we are facing. It is reasonable that this attitude toward life can have a detrimental impact on patients' psychological and physical wellbeing, contributing to high levels of distress. Further studies are needed in order to clarify the possible relationship between fears, uncertainty and mental disorders

The fauna, collected from the La Valute Nord (VALN) section includes five species. Monograptus belophorus and Monoclimacis flumendosae are reported for the first time in the Carnic Alps. The graptolite fauna allows to date the section to the belophorus Zone (Sheinwoodian, Wenlock).

Abstract: This study sought to prospectively assess which factors were related to short-term worsening heart failure (HF) leading to or not to hospital admission, in long-term outpatients followed by cardiologists. The subsequent decision-making process was also analyzed. The study population consisted of 2,701 outpatients enrolled in the registry of the Italian Network on Congestive Heart Failure (IN-CHF) and followed by 133 cardiology centers (19% of all existing Italian cardiology centers). Clinical and follow-up data were collected by local trained clinicians; 215 patients (8%) had short-term decompensation (on average 2 months after the index outpatient visit). Multivariate analysis showed that previous hospitalization, long duration of symptoms, ischemic etiology, atrial fibrillation, higher functional class (New York Heart Association classification III to IV), higher heart rate, and low systolic blood pressure were independently associated with HF destabilization. Poor compliance (21%) and infection (12%) were the most frequent precipitating factors, but a precipitating factor was not identified in 40% of the patients. Poor compliance was more common in women, but no other clinical characteristics emerged as being related with a specific precipitating factor. Fifty-seven percent of the patients with a short-term recurrence of worsening HF required hospital admission; infusion treatment with inotropes and/or vasodilators was necessary in 19% of them. Long-term therapy was changed in 48% of the patients. Thus, in ambulatory HF patients, short-term worsening HF can be predicted according to the clinical characteristics on an outpatient basis. Nearly 1/3 of precipitating factors can be prevented. Patient education and avoidance of inappropriate treatment may reduce the number of relapses.

Abstract: Objectives: The role of atrial fibrillation (AF) in older patients with heart failure (HF) is controversial because many variables seem to influence their outcome. We investigated the predictivity of AF in 3 age groups of outpatients with HF. Methods: We analyzed 8,178 outpatients enrolled in the Italian Network on Congestive Heart Failure Registry with HF diagnosed according to the European Society of Cardiology criteria. A trained cardiologist established the diagnosis of AF and HF at the entry visit at each center. We stratified the population into 3 age groups, as follows: group A, ≤65 years; group B, 66–75 years, and group C, >75 years. Results: Group A was composed of 4,261 patients, 683 with AF (16.0%); in group B there were 2,651 patients, 638 with AF (24.1%), and group C was composed of 1,266 patients, 412 with AF (32.5%). The 1-year mortality rate was higher in AF patients in all groups. In a multivariate model, AF remained an independent risk factor for death in groups A and B, but not in group C [group A: hazard ratio (HR) 1.42, 95% confidence interval (CI) 1.10–1.81; group B: HR 1.29, 95% CI 1.00–1.67; group C: HR 1.05, 95% CI 0.78–1.43]. Conclusion: The prevalence of AF increased with age and was associated with a higher mortality rate. However, AF independently predicted all-cause mortality only in patients aged ≤75 years.
