Abstract

BACKGROUND AND OBJECTIVE: The prognosis in myelin oligodendrocyte glycoprotein (MOG) antibody-associated disease (MOGAD) is a matter of debate. Our aim was to assess the long-term outcomes of patients with MOGAD. METHODS: We retrospectively analysed the clinical and paraclinical data of patients from the French nationwide observatory study NOMADMUS who tested positive for MOG antibodies (MOG-IgG) and who had clinical follow-up of at least 8 years from their first episode. RESULTS: Sixty-one patients (median [range] age at onset 27 [3-69] years), with a median (mean; range) follow-up of 177 (212.8; 98-657) months, were included. Among 58 patients with a relapsing course, 26.3% relapsed in the first year after onset. Of the 61 patients, 90.2% experienced at least one episode of optic neuritis. At last visit, the median (mean; range) Expanded Disability Status Scale (EDSS) score was 1 (2.12; 0-7.5), 12.5% had an EDSS score ≥6 and 37.5% had an EDSS score ≥3. Of 51 patients with final visual acuity (VA) data available, 15.7% had VA ≤0.1 in at least one eye and 25.5% had VA ≤0.5 in at least one eye. Bilateral blindness (VA ≤0.1) was present in 5.9% of patients. Finally, 12.5% of patients presented bladder dysfunction requiring long-term urinary catheterization. No factor associated significantly with a final EDSS score ≥3 or with final VA ≤0.1 was found. CONCLUSION: Overall long-term favourable outcomes were achieved in a majority of our patients, but severe impairment, in particular visual damage, was not uncommon.

Abstract

A laser system composed of tunable lasers pumped by a copper vapor laser (Oxford Lasers Cu60) is described in this paper. The high resolution obtained with this system has allowed excitation of selective rotational levels of the 15 V and 10 V vibrational bands of the V 1B2 excited electronic state of the CS2 molecule in its vapor phase (∼100 mTorr). The rotational assignment of the excitation spectra was accomplished by observing the dispersed fluorescence. We show that it is not necessary to use a supersonic jet in order to assign the emission spectra of CS2. The goal of this work is to study the highly excited vibrational states of the ground electronic state of CS2 up to the first dissociation limit. For our purpose, there are two important consequences of the particular geometry of the 15 V excitation, which is just below and close to the bending potential barrier of the V 1B2 state. First, a very good Franck–Condon overlap in the excitation and a large Franck–Condon access to high vibrational states, as high as 20 000 cm-1, allow observation of the dispersed fluorescence through a high resolution monochromator with an OMA detector. This avoids the need for more complicated techniques, like SEP spectroscopy. Moreover, we show for the first time, that, in the V 1B2 excited state, a strong Coriolis interaction Q32 Q3 Jc couples the bending (0,3,0) and antisymmetric stretching levels (0,0,1). This breaking of the symmetric–antisymmetric selection rule gives access to the antisymetric stretching levels of the ground electronic state X Σ+g from the 15 V excitation. We also show that, below 12 000 cm-1, the vibrational couplings of CS2 can be described by a model of 2 degrees of freedom, which includes a strong 1:2 Fermi resonance and accidental resonant perturbations between adjacent polyads which is probably a first step in the transition to a chaotic regime in CS2. Abstract

In this paper we analyze the vibrational spectra of the Σ+g ground state of CS2, the experimental results of which have been described in a forth coming paper. We show that, up to 12 000 cm-1, CS2 can be described by a system of two degrees of freedom strongly coupled by a 1:2 type Fermi resonance. The corresponding vibrational spectra are refitted with the aid of only seven parameters. Analysis of the spectra by the statistical Fourier transform technique reveals stroboscopic effects between the symmetric stretching mode and the bending mode. The distinction between the ‘‘stroboscopic hole’’ due to these effects and the ‘‘correlation hole’’ due to nonintegrable terms in the Hamiltonian is discussed in detail. The study of the topology of the phase space of CS2 in the regular and chaotic cases is carried out in the basis described by a vibrational angular momentum which includes the Fermi resonance. We show the analogy between the localization of the wave packets of the eigenstates and the trajectories. We also show the destabilization of the trajectories due to a term in the Hamiltonian which couples neighboring polyads and which is a second Fermi resonance. Item does not contain fulltext, BACKGROUND & AIMS: Hepatitis E virus (HEV) has been associated with a number of neurological syndromes, but causality has not yet been established. The aim of this study was to explore the relationship between HEV and neurological illness by prospective HEV testing of patients presenting with acute non-traumatic neurological injury. METHODS: Four hundred and sixty-four consecutive patients presenting to hospital with acute non-traumatic neurological illnesses were tested for HEV by serology and PCR from four centres in the UK, France and the Netherlands. RESULTS: Eleven of 464 patients (2.4%) had evidence of current/recent HEV infection. Seven had HEV RNA identified in serum and four were diagnosed serologically. Neurological cases in which HEV infection was found included neuralgic amyotrophy (n=3, all PCR positive); cerebral ischemia or infarction (n=4); seizure (n=2); encephalitis (n=1); and an acute combined facial and vestibular neuropathy (n=1). None of these cases were clinically jaundiced and median ALT at presentation was 24IU/L (range 8-145). Cases of HEV-associated neuralgic amyotrophy were found in each of the participating countries: all were middle-aged males with bilateral involvement of the brachial plexus. CONCLUSIONS: In this cohort of patients with non-traumatic neurological injury, 2.4% had evidence of HEV infection. Symptoms of hepatitis were mild or absent and no patients were jaundiced. The cases of HEV-associated neuralgic amyotrophy had similarities with other HEV-associated cases described in a large retrospective study. This observation supports a causal relationship between HEV and neuralgic amyotrophy. To further understand the relevance of HEV infection in patients with acute neurological illnesses, case-control studies are warranted. Abstract

International audience; Most of classical light sources show a close similarity between their first and second order correlation functions (resp. g(1) and g(2)) functions. We present here the original coherence properties of a peculiar type of laser named modeless laser or Frequency Shifted Feedback (FSF) laser where the g(1) and g(2) functions show a different behaviour. We calculate and evidence experimentally the first and second order correlation functions of modeless lasers, through measurements of the homodyne beat signal and interferometric autocorrelation of a dye FSF laser at the output of a Michelson interferometer. Whereas the degree of first-order coherence vanishes beyond the coherence length of the FSF source, the degree of second-order coherence exhibits periodic revivals far beyond the coherence length, with a period equal to the cavity roundtrip time. Our observations are in good agreement with the theoretical treatment of Yatsenko et al. (Opt. Comm. 282 (2009) 300) [1].

Abstract

International audience; Here we present an interferometer where the fringe spacing is reduced by a factor of two and where the width of a fringe can be extremely narrow, leading to potential applications in very precise displacement measurement. Our interferometer is based on elementary aspects of atomic physics. The laser wavelength is tuned to an atomic transition (i.e. two-fold degenerate two level system). The laser field is initially polarized at 45Aring and sent to a polarizing beam splitter. Vertical and horizontal polarizations experience a relative phase shift and are recombined at the output. We present an experimental demonstration of such an interferometer by exciting a sodium vapour cell on the D1 line. The problem posed by the hyperfine structure is solved by exciting the whole Doppler-hyperfine transition by means of a dye frequency-shifted feedback laser delivering a 3 GHz broad and continuous spectrum. Both the lambda/2 periodicity and the anharmonic behaviour have been recorded on the fluorescence signal.

Abstract

1 The gastric mucosa of portal hypertensive rats exhibits important microvascular changes and a nitric oxide (NO)-dependent hyperemia. This study analyses whether portal hypertensive mucosa exhibits changes in its ability to withstand aggression. 2 Portal hypertension was induced by partial portal vein ligation (PPVL) or common bile duct ligation (CBDL) and gastric damage was induced by oral administration of ethanol or aspirin. Experiments were performed in conscious or anaesthetized rats and some animals were pre-treated with the NO-synthesis inhibitor L-NAME. 3 Conscious PPVL or CBDL rats showed an increased resistance to the damaging effects of ethanol. Oral administration of aspirin produced less gastric damage in PPVL conscious rats than in the control group. 4 The protective effects of portal hypertension were maintained in animals anaesthetized with ketamine and absent when pentobarbital was employed. 5 Pre-treatment with L-NAME restored the damaging effects of ethanol and aspirin in PPVL rats without modifying the level of damage in control animals. 6 Gastric bleeding induced by oral aspirin, as measured by the luminal release of Cr-51-labelled erythrocytes, was significantly greater in PPVL rats than in control animals. 7 Semi-quantitiative analysis by RT-PCR of the mRNA for endothelial NO-synthase (eNOS), neuronal NOS (nNOS) and inducible NOS (iNOS) levels showed that the expression of iNOS was slightly increased in both the gastric mucosa and smooth muscle of PPVL rats. No changes were observed in eNOS and nNOS expression. 8 Conscious portal hypertensive rats exhibit an enhanced resistance to acute gastric damage which is absent under the influence of some types of anaesthesia and seems related to an increased synthesis of nitric oxide. However, mucosal lesions in these animals show an augmented bleeding per area of injury.

Abstract

International audience; We describe the current status of the ELP-OA project in which we try to demonstrate in practice that it is possible to measure the tilt of a wave front using only a polychromatic laser guide star and no natural guide star. The first phase of ELP-OA, consisting of feasibility experiments, has recently been completed successfully. This paper provides an overview over the results of this first phase and over the continuation of the ELP-OA project.

Abstract

International audience; Adaptive optics at astronomical telescopes aims at correcting in real time the phase corrugations of incoming wavefronts caused by the turbulent atmosphere, as early proposed by Babcock. Measuring the phase errors requires a bright source, which is located within the isoplanatic patch of the program source. The probability that such a reference source exists is a function of the wavelength of the observation, of the required image quality (Strehl ratio), of the turbulence optical properties, and of the direction of the observation. Several papers have addressed the problem of the sky coverage as a function of these parameters (see e.g.: Le Louarn et al). It turns out that the sky coverage is disastrously low in particular in the short (visible) wavelength range where, unfortunately, the gain in spatial resolution brought by adaptive optics is the largest. Foy and Labeyrie have proposed to overcome this difficulty by creating an artificial point source in the sky in the direction of the observation relying on the backscattered light due to a laser beam. This laser guide star (hereafter referred to as LGS) can be bright enough to allow us to accurately measure the wavefront phase errors, except for two modes which are the piston (which is not relevant in this case) and the tilt. Pilkington has emphasized that the round trip time of the laser beam to the mesosphere, where the LGS is most often formed, is significantly shorter than the typical tilt coherence time; then the inverse-return- of-light principle causes deflections of the outgoing and the ingoing beams to cancel. The apparent direction of the LGS is independent of the tilt. Therefore the tilt cannot be measured only from the LGS. Until now, the way to overcome this difficulty has been to use a natural guide star to sense the tilt. Although the tilt is sensed through the entire telescope pupil, one cannot use a faint source because approximately equals 90% of the variance of the phase error is in the tilt. Therefore, correcting the tilt requires a higher accuracy of the measurements than for higher orders of the wavefront. Hence current adaptive optics devices coupled with a LGS face low sky coverage. Several methods have been proposed to get a partial or total sky coverage for the tilt, such as the dual adaptive optics concept, the elongation perspective method, or the polychromatic LGS (hereafter referred to as PLGS). We present here a progress report of the R&D; program Etoile Laser Polychromatique et Optique Adaptative (ELP-OA) carried out in France to develop the PLGS concept. After a short recall of the principles of the PLGS, we will review the goal of ELP-OA and the steps to get over to bring it into play.

Abstract

International audience; In previous experiments, a nearly complete vibrational spectrum of the CS2 molecule in the Σg+ state has been obtained using a laser induced fluorescence method. The statistical tests of the random matrix theory, applied on this spectrum, suggest a transition to quantum chaos. The aim of this letter is to show that these statistical results are biased because of a nonsingle excitation of the molecule. This is experimentally confirmed by high-resolution new experiments in supersonic jet, using the powerful laser chain of the french project for the isotopic separation of uranium by laser (SILVA project).

Abstract

The CD40/CD40 ligand (CD40L) pathway is involved in Crohn's disease (CD) pathogenesis. In the patients' circulation, soluble CD40L (sCD40L) levels are elevated and surface CD40L is increased in platelets and T cells, whereas in the intestine CD40 is overexpressed in the microvasculature and CD40L in platelets and T cells. The therapeutic effects of infliximab in CD are attributed to its systemic anti-TNF-alpha action, but because TNF-alpha modulates both CD40 and CD40L, we investigated whether infliximab affects the CD40/CD40L pathway in the intestine. Eighteen CD patients were evaluated before and after infliximab therapy. Plasma sCD40L was measured by ELISA and platelet and peripheral blood T cell (PBT) CD40L expression by flow cytometry. Microvascular CD40 and VCAM-1 expression were assessed in mucosal biopsies by immunohistochemistry and by flow cytometry in human intestinal microvascular endothelial cells (HIMEC). Cell cultures were performed in the presence and absence of infliximab. Infliximab treatment significantly reduced plasma sCD40L levels and eliminated CD40 and VCAM-1 from mucosal microvessels. In vitro infliximab prevented TNF-alpha-induced CD40 and VCAM-1 expression by HIMEC, and reduced PBT, but not platelet, surface CD40L expression and sCD40L release. In addition, infliximab decreased T cell-induced VCAM-1 expression in HIMEC by down-regulating CD40L in T cells and promoting T cells apoptosis. These findings point to a novel mechanism of action of infliximab, i.e., the disruption of CD40/CD40L-dependent cognate interactions between intestinal microvessels and T cells. Thus, in addition to neutralizing TNF-alpha and inducing T cell death, the therapeutic effects of infliximab in CD appear to be also mediated by inhibition of vascular inflammation in the gut.

35. Anatomical differences in responsiveness to vasoconstrictors in the mesenteric veins from normal and portal hypertensive rats

The present study evaluates the effects of pre-hepatic portal hypertension, induced in rats by partial portal vein ligation, on the responsiveness of rostral (proximal) and caudal (distal) rings from the mesenteric vein. The anatomical origin of the sample influenced the response to vasoconstrictors in sham-operated animals, and this pattern of reactivity was specifically modified in portal-ligated rats, In veins from sham-operated rats, contraction induced by a submaximal concentration of KCl (60 mM) was greater in proximal than in distal rings. Vasopressin and 5-hydroxytryptamine contracted mainly distal rings, methoxamine showed a greater effect on proximal rings, and endothelin-1 and angiotensin-II contracted vein rings independently of their anatomical origin. In veins from portal hypertensive rats, responses to KCl (60 mM) were increased in distal rings, and all rings exhibited enhanced reactivity to vasopressin and 5-hydroxyptyptamine as well as attenuation of the response to methoxamine. Responses to endothelin-1 were decreased in proximal vein rings from portal hypertensive rats whereas responses to angiotensin-II were not influenced by the anatomical origin, Incubation with atropine, propranolol or indomethacin, did not modify the responses to vasopressin and 5-hydroxytryptamine in tissues from either sham-operated or portal hypertensive animals. Likewise, the hyporeactivity to methoxamine and endothelin-1 in rings from portal hypertensive rats persisted in the presence of the nitric oxide inhibitor N-G-nitro-L-arginine methyl ester, These results suggest the physiological existence of anatomical differences in the responsiveness to vasoconstrictors throughout the mesenteric vein and that changes in the responsiveness of the mesenteric vein induced by portal hypertension are specific for each agonist and possibly result from individual variations at a receptor or post-receptor level.

Abstract

Administration of E. coli endotoxin (1 mg kg(-1), i.v.) abolished the acid response induced by the i.v. infusion of pentagastrin (8 mu g kg(-1) h(-1)) in the continuously perfused stomach of the anaesthetized rat. Local serosal application of tetrodotoxin (36 ng per rat) completely restored acid responses to pentagastrin in endotoxin-treated rats. However, pretreatment with atropine (0.5 mg kg(-1), s.c.), capsaicin (20, 30, and 50 mg kg(-1), s.c. 2 weeks before the study) or guanethidine (16 mg kg(-1), s.c. 3 and 16h before) did not influence the inhibitory effects of endotoxin. Continuous i.v. infusion with N-G-nitro arginine methyl ester (L-NAME, 10 mg kg(-1) h(-1)) restored the secretory responses to pentagastrin in endotoxin treated rats. The effects of L-NAME were reversed by L-arginine (100 mg kg(-1) h(-1) i.v.), but not by its enantiomer D-arginine (100 mg kg(-1) h(-1), i.v.). The secretory responses elicited by pentagastrin (10(-10)-10(-6) M) in the isolated lumen perfused stomach of the rat were not influenced by incubation (100 min) with endotoxin (10 mu g ml(-1)). These observations with tetrodotoxin indicate that inhibition of acid secretion by endotoxin in vivo involves neuronal activity, while inhibition of NO synthesis had a comparable inhibitory action. Activation of a systemic non-adrenergic non-cholinergic neuronal pathway involving NO could thus mediate the acute acid inhibitory effects of endotoxin.
