Abstract

Amblyopia is the most frequent cause of monocular vision loss. Transcranial Magnetic Stimulation (TMS) has been used to improve several vision parameters of the amblyopic eye in adulthood. This study is relevant in order to evaluate TMS effects and to raise awareness of the need for further research. Transcranial Magnetic Stimulation (TMS) is a neuromodulation technique capable of changing cortical excitability. In the last decade, it has been used to improve visual parameters in amblyopic patients. The main goal of this systematic review is to evaluate the influence of TMS in the amblyopic eye, in the visual parameters of amblyopic patients. Searches were done in PubMed and Embase databases, and a combined search strategy was performed using the following Mesh, EMBASE, and keywords: 'Amblyopia', 'Transcranial Magnetic Stimulation', and 'theta burst stimulation'. This review included randomised controlled studies, descriptive cases, and clinical case studies with adult amblyopes. All articles that had any of the following characteristics were excluded: children or animal studies, reviews, pathologies other than amblyopia, and other techniques rather than repetitive TMS (rTMS), or Theta Burst Stimulation (TBS). A total of 42 articles were found, of which only four studies (46 amblyopes) meet the criteria above. Three of the articles found significant improvement after one session of continuous TBS (cTBS) in visual parameters like visual acuity, contrast sensitivity, suppressive imbalance, and stereoacuity. One study found a significant visual improvement with 10 Hz rTMS. Only one stimulation-related dropout was reported. The few existing studies found in this review seem to show that through the usage of high-frequency rTMS and cTBS, it is possible to re-balance the eyes of an adult amblyope. However, despite the promising results, further research with larger randomised double-blind studies is needed for a better understanding of this process. [ABSTRACT FROM AUTHOR]

19. Evaluation of various membranes at different fluxes to enable large‐volume single‐use perfusion bioreactors.

Abstract

The growing demand for biological therapeutics has increased interest in large‐volume perfusion bioreactors, but the operation and scalability of perfusion membranes remain a challenge. This study evaluates perfusion cell culture performance and monoclonal antibody (mAb) productivity at various membrane fluxes (1.5–5 LMH), utilizing polyvinylidene difluoride (PVDF), polyethersulfone (PES), or polysulfone (PS) membranes in tangential flow filtration mode. At low flux, culture with PVDF membrane maintained higher cell culture growth, permeate titer (1.06–1.34 g/L) and sieving coefficients (≥83%) but showed lower permeate volumetric throughput and higher transmembrane pressure (TMP) (>1.50 psi) in the later part of the run compared to cultures with PES and PS membrane. However, as permeate flux increased, the total mass of product decreased by around 30% for cultures with PVDF membrane, while it remained consistent with PES and PS membrane, and at the highest flux studied, PES membrane generated 12% more product than PVDF membrane. This highlights that membrane selection for large‐volume perfusion bioreactors depends on the productivity and permeate flux required. Since operating large‐volume perfusion bioreactors at low flux would require several cell retention devices and a complex setup, PVDF membranes are suitable for low‐volume operations at low fluxes whereas PES membranes can be a desirable alternative for large‐volume higher demand products at higher fluxes. Abstract

A detailed case study of a Portuguese Islamic State network with strong connections to the United Kingdom sheds significant light on the foreign fighter recruitment pipeline between Europe and Syria. Several of the men involved in the network made two trips to Syria. On January 9, 2013, British authorities informed their Portuguese counterparts that at least two Portuguese citizens were involved in the (first) kidnapping of British journalist John Cantlie in Syria. That information opened a seven-year investigation, recently closed, in which the Portuguese Judiciary Police were able to follow in real time the preparations, contacts, and final travel to Syria of a group of Islamist extremists, including their wives and children, who would join what became the Islamic State. Investigators established how they financed themselves through the creation of fictitious applications for student loans and welfare benefits in the United Kingdom. They used that money to travel but also to recruit and help young British extremists make their way into Syria, using Lisbon as transit point. But investigators were not able to shut down the pipeline because foreign fighter travel would only become a crime under Portuguese law in 2015, underlining the importance of such legislation. Several members of the network are presumed dead, while others face trial in Portugal. The most senior alleged member of the network, Nero Saraiva, was transferred from Syria to Iraq where he could face trial or extradition to Portugal. 34. Longstanding effects of continuous theta burst stimulation in adult amblyopes.

Abstract

Continuous theta burst stimulation may be an important tool in the therapeutic management of amblyopia, when trying to correct the established neuronal imbalance. It is important to understand whether two sessions of continuous theta burst stimulation produce greater and longstanding changes in visual acuity and suppressive imbalance than one session of continuous theta burst stimulation We hypothesise that through the usage of continuous theta burst stimulation (cTBS) it is possible to change cortical excitability in a situation where visual impairment is present. We selected 22 adult amblyopes, 18 females and 4 males, with an age range of 20–59 years. They were randomised into two groups: group A with 10 amblyopes was submitted to one session of cTBS and group B with 12 amblyopes submitted to two sessions of cTBS. Visual acuity (VA) and suppressive imbalance (SI) were evaluated immediately before and after stimulation in both groups A and B. A follow-up was done in both groups. For both group A and B, the VA improvements were significant after cTBS (p = 0.005 and p = 0.003, respectively). Regarding SI, both group A and B had significant improvements after cTBS (p = 0.03 and p = 0.005, respectively). Comparing groups, A and B no significant differences were found with regard to the results obtained both for VA (p = 0.72) and SI (p = 0.24). However, significant differences were found between group A and B with regard to the duration of stimulation effect for VA (p = 0.049) and SI (p = 0.03). We conclude that two sessions of cTBS do not produce better results than one session of stimulation. However, it seems that two sessions of cTBS produce longstanding effects in VA and SI. Introduction: Evidence reveals that urgency7emergency nurses developed stress problems in the Context of a COVID-19 pandemic. Objectives: Study the relationship between the stress profile of urgency/emergency nurses in the COVID-19 context and professional an sociodemographic variables Methods: Quantitative, descriptive correlational study with a cross-sectional approach, with a sample of 355 national nurses who work in urgency/emergency situations, mostly female (51.1%) and with a mean age of 39.25±8.89 years. AN anonymous questionnaire was used, consisting of sociodemographic and professional questions (ad hoc) and the Stress Perception Scale (EPS), adapted for the Portuguese population by Pais Ribeiro & Marques (2009). Results: The majority is a female sample (51.0%), aged between 36-50 years (51.3%), married (48.7%) and with a degree (38.9%), working in the public sector (85.9%), in the North (35.8%), in non-designated services (22.3%), with time of professional experience between 11-20 years (40.8%), have a CTFP-IT (58.6%), practice a rotating schedule (86.8%), with ≤35 hours/week (75.2%) and with the professional category of “nurse” (59.4%). Conclusion: The average ranking values indicate a higher level of stress on the part of younger nurses (≤years), male, divorced, with a master’s degree, who work in the private sector, with more time of professional practice, followed by those who have between 21-30 years and those with 11-20 years of service and by nurses with a CIT; followed by those with CTFP-TRC and CCT. Nurses working in Lisbon and Tagus Valley region perceived a higher level of stress (p=0.001)., Introdução: As evidências revelam que os enfermeiros de urgência/emergência desenvolveram problemas de stress em contexto de pandemia por COVID-19. Results: The sample was mainly composed of women (51.1%), aged between 36-50 years (51.3%), with a university degree (38.9%), working in the public sector (85.9%), in the North (35.8%), between 11-20 years of professional experience (40.8%) and with the professional category of "nurse" (59.4%). Conclusion: As variáveis de caracterização profissional, revelaram que o título profissional, o serviço/unidade onde exercem funções e a localização geográfica, apresentam efeito significativo no perfil de Stress perceived (p < 0.005). In this context, the specialist nurses (p = 0.005) working in the SUP and OR (p = 0.002) of the Lisbon and Tagus Valley Region (p = 0.001) were those with the highest perception of Stress. On the other hand, generalist nurses (p = 0.005), who exercise functions at INEM (p = 0.002), and who exercise in the Autonomous Region of Madeira (p = 0.001) showed a lower Stress profile.

Abstract

Introduction: Evidence reveals that urgency7emergency nurses developed stress problems in the Context of a COVID-19 pandemic. Objectives: Study the relationship between the stress profile of urgency/emergency nurses in the COVID-19 context and professional an sociodemographic variables Methods: Quantitative, descriptive correlational study with a cross-sectional approach, with a sample of 355 national nurses who work in urgency/emergency situations, mostly female (51.1%) and with a mean age of 39.25±8.89 years. AN anonymous questionnaire was used, consisting of sociodemographic and professional questions (ad hoc) and the Stress Perception Scale (EPS), adapted for the Portuguese population by Pais Ribeiro & Marques (2009). Results: The majority is a female sample (51.0%), aged between 36-50 years (51.3%), married (48.7%) and with a degree (38.9%), working in the public sector (85.9%), in the North (35.8%), in non-designated services (22.3%), with time of professional experience between 11-20 years (40.8%), have a CTFP-IT (58.6%), practice a rotating schedule (86.8%), with ≤35 hours/week (75.2%) and with the professional category of “nurse” (59.4%). Conclusion: The average ranking values indicate a higher level of stress on the part of younger nurses (≤years), male, divorced, with a master’s degree, who work in the private sector, with more time of professional practice, followed by those who have between 21-30 years and those with 11-20 years of service and by nurses with a CIT; followed by those with CTFP-TRC and CCT. Nurses working in Lisbon and Tagus Valley region perceived a higher level of stress (p=0.001)., Introducción: La evidencia revela que las enfermeras de urgencia/emergencia desarrollaron problemas de estrés en el contexto de una pandemia de COVID-19. Resultados: La muestra es mayoritariamente femenina (51.0%), con edad entre 36-50 años (51.3%), casada (48.7%) y con título (38.9%), que trabaja en el sector público (85.9%), en la Norte (35.8%), en servicios no designados (22.3%), con tiempo de experiencia profesional entre 11-20 años (40.8%), tener un CTFP-IT (58.6%), pratican horario rotativo (86.8%), con ≤35 horas/semana (75.2%) y con categoría profesional de “enfermera” (59.4%) Conclusións: Los valores médios del ranking indican un mayor nível de estrés por parte de los enfermeros más jóvenes (≤35años), del sexo masculino, divorciados, con maestria, que trabajan en el sector privado, com más tiempo de ejercicio profesional, seguidos de los que tienen entre 21-30 años y por aquellos con 11-20 años de servicio y por enfermeros con CIT; seguidos por aquellos con CTFP-TRC y un CCT. 48. Longstanding effects of continuous theta burst stimulation in adult amblyopes

Abstract

Continuous theta burst stimulation may be an important tool in the therapeutic management of amblyopia, when trying to correct the established neuronal imbalance. It is important to understand whether two sessions of continuous theta burst stimulation produce greater and longstanding changes in visual acuity and suppressive imbalance than one session of continuous theta burst stimulation We hypothesise that through the usage of continuous theta burst stimulation (cTBS) it is possible to change cortical excitability in a situation where visual impairment is present. We selected 22 adult amblyopes, 18 females and 4 males, with an age range of 20–59 years. They were randomised into two groups: group A with 10 amblyopes was submitted to one session of cTBS and group B with 12 amblyopes submitted to two sessions of cTBS. Visual acuity (VA) and suppressive imbalance (SI) were evaluated immediately before and after stimulation in both groups A and B. A follow-up was done in both groups. For both group A and B, the VA improvements were significant after cTBS (p = 0.005 and p = 0.003, respectively). Regarding SI, both group A and B had significant improvements after cTBS (p = 0.03 and p = 0.005, respectively). Comparing groups, A and B no significant differences were found with regard to the results obtained both for VA (p = 0.72) and SI (p = 0.24). However, significant differences were found between group A and B with regard to the duration of stimulation effect for VA (p = 0.049) and SI (p = 0.03). We conclude that two sessions of cTBS do not produce better results than one session of stimulation. However, it seems that two sessions of cTBS produce longstanding effects in VA and SI.

Abstract

Background and Aims: Ambulatory surgery (AS) has increased rapidly in most countries since the 1990’s. There is an increasing effort by national healthcare systems to disseminate clinical information about this type of surgical environment to their populations. We conducted a public survey to understand what citizens think about AS and its organization in countries across the world. Methods: This prospective observational study used a survey presented on a tablet computer. Subjects were asked about their surgical experience and those submitted to AS were asked about their satisfaction rate. Subjects were also asked if they associate AS to a surgical programme and the level of information obtained from their National Health Services. Results: 400 citizens from 47 different countries were divided in six geographical areas, European (n=4, North, Central, East and South) and non-European (n=2, Developed and Non-Developed Countries / Emergent Economies). 51.0% reported they had heard of AS, and 29.3% had undergone an AS procedure. Those who had undergone AS, reported a high level of satisfaction with the procedure, with no differences in geographical areas (P=0.229). 90.5% would recommend AS to relatives and friends. Nevertheless, those interviewed wished to have more information related to AS from their National Health Authorities (NHA).
