The management of tandem extracranial internal carotid artery and intracranial large vessel occlusion during endovascular thrombectomy (EVT) for acute ischemic stroke (AIS) has been under-investigated. We sought to investigate outcomes of AIS patients with tandem occlusion (TO) treated with carotid artery stenting (CAS) compared to those not treated with CAS (no-CAS) during EVT.We performed a cohort study using data from AIS patients enrolled in the Italian Registry of Endovascular Treatment in Acute Stroke. Outcomes were 3 months' mortality, functional outcome, complete and successful recanalization, any intracranial hemorrhage, parenchymal hematoma and symptomatic intracerebral hemorrhage.Among 466 AIS patients with TO, CAS patients were 122 and no-CAS patients were 226 (118 excluded). After adjustment for unbalanced variables, CAS was associated with a lower rate of 3 months' mortality (OR 0.407, 95% CI 0.171-0.969, p = 0.042). After adjustment for pre-defined variables, CAS was associated with a lower rate of 3 months' mortality (aOR 0.430, 95% CI 0.187-0.989, p = 0.047) and a higher rate of complete recanalization (aOR 1.986, 95% CI 1.121-3.518, p = 0.019), successful recanalization (aOR 2.433, 95% CI 1.263-4.686, p = 0.008) and parenchymal hematoma (aOR 2.876, 95% CI 1.173-7.050, p = 0.021). CAS was associated with lower 3 months mortality (OR 0.373, 95% CI 0.141-0.982, p = 0.046) and higher rates of successful recanalization (OR 2.082, 95% CI 1.099-3.942, p = 0.024) after adjustment for variables associated with 3 months' mortality and successful recanalization, respectively.Among AIS patients with TO, CAS during EVT was associated with a higher rate of successful reperfusion and a lower rate of 3 months' mortality.

The growing interest in emerging environmental crisis has increased the level of public attention and the willingness to take part in participatory science projects, commonly defined with the term “Citizen Science”. This phenomenon can be recorded globally in many national contexts, with a prevalence in anglophone and more industrialized countries. In the biodiversity sector, the growing demand for public involvement has been declined in many different ways, with solutions aimed at providing cognitive and participatory tools. Among these, one of the best known is the BioBlitz: a 24-hour event held in a specific place with the aim of listing as many living species as possible. This work presents the data collected during the first two BioBlitzes organized by the Maremma Natural History Museum in 2013 and 2014. Both were made in the area of the San Felice Oasis, near the terminal part of the San Leopoldo ditch. During the BioBlitz, data collected attested the presence of 616 different taxonomic entities, of which 507 were identified at the level of species and 22 at the level of subspecies. The rest were identified at higher taxonomic levels. Eleven alien species, one endemic species, 33 species protected by national and international laws and / or directives and 9 species at risk of extinction were identified.

The Maremma Natural History Museum is active in the field of Citizen Science at the Tuscan, national and international level. Among the founding bodies of the European Citizen Science Association (ECSA), it has promoted meetings for a national Citizen Science strategy in Italy, actively participated in the drafting of national guidelines on the subject and developed Citizen Science projects in Tuscany and other regions nearby. BioBlitzes are one of the best-known activities in the field of environmental Citizen Science. In May 2016, the Museum organized its fourth BioBlitz at the Accesa Lake Special Protection Area. On this occasion, data collected attested the presence of 784 different taxonomic entities, 678 of which were identified at the species level and 18 at the subspecies level. Nineteen alien species and two endemic species have been identified. Thirty-nine species protected by national or international laws and 13 endangered species have been detected. The area has been confirmed to be an important biodiversity hotspot; future utilization plans must be developed with due consideration for the fragility of the habitats close to the lake from the conservation point of view.

The threat to the survival of many species, caused by emerging ecological crises, has favored the development of a new environmental awareness. In recent decades, especially in the most industrialized nations, an increasing participation of citizens in the collection of scientific data has also been recorded. The active involvement of society in different aspects of science has been defined by the sociologist Alan Irwin as “Citizen Science”. Although environmental Citizen Science brings together the largest number of projects, the fields of application of this new way of doing science are innumerable. BioBlitz is one of the best-known examples of citizen science activities, aimed at monitoring the biodiversity of an area. The Maremma Natural History Museum, as part of its activities, regularly organizes BioBlitz on an annual basis, starting from 2013. During BioBlitz 2017 in the SPZ and SCZ “Monte Labbro and Alta Valle dellAlbegna” data were collected attesting the presence of 403 different taxonomic entities, of which 367 identified at the species level and 5 at the subspecies level. Six alien and 2 endemic species were identified. In terms of environmental legislation, 38 protected species and 7 endangered species were detected in the area. The most relevant aspect that emerges from the collected data is the consistent number of protected species. This confirms the environmental value of the area and the need for management and conservation plans for the entire site.

In recent decades, many natural history museums in Europe have integrated their traditional functions with new ways of interacting with visitors and local communities. In particular, the need to adapt their mission according to the changes in modern society has led them to develop initiatives aiming at an active involvement of the public in scientific research. This is one of the inspiring principles of Citizen Science. BioBlitz are among the most popular activities dedicated to biodiversity in this field. In line with the inspiring principles of this cultural revolution, since 2012 the Maremma Natural History Museum organized public participation initiatives for the collection of biodiversity data. We hereby report the main results of the BioBlitz 2018 organized by the Maremma Natural History Museum in the Talamone area, located in the southern portion of the Uccellina Hills. Four hundred and ninety eight taxa were sampled and identified, 447 of which at the level of species and 10 at the level of subspecies; the remaining refer to higher taxonomic levels. 31 protected species and 10 endangered species were found in the area; moreover, 6 alien species and one endemic species were detected. The main feature that distinguished BioBlitz 2018 to others conducted in previous years was the large number of participants. The data collected increased the scientific knowledge of the southern portion of the Maremma Regional Park, less known than other areas of the same protected area.

Contains fulltext : 219894.pdf (Publisher’s version ) (Closed access), OBJECTIVES: Carotid artery diameter responses to sympathetic stimulation, i.e., carotid artery reactivity (CAR), represent a novel test of vascular health and relates to cardiovascular disease (CVD)/risk. This study aims to understand the relationship between the increase in blood pressure and carotid artery diameter response during the CAR-test in healthy, middle-aged men. METHODS: Sample consisted of 40 normotensive men (aged 31-59 years) with no history of CVD of currently taking medication. Noninvasive ultrasound was used to measure carotid artery diameter during the cold pressor test (CPT), with CAR% being calculated as the relative change from baseline (%). Mean arterial pressure (MAP) was measured with beat-to-beat blood pressure recording. RESULTS: CAR% was 4.4 +/- 5.4%, peaking at 92 +/- 43 seconds. MAP increased from 88 +/- 9 mmHg to 110 +/- 15 mmHg, peaked at 112 +/- 38 seconds, which was significantly later than the diameter peak (P = 0.04). The correlation between resting MAP and CAR% was weak (r = 0.209 P = 0.197). Tertiles based on resting MAP or MAP-increase revealed no significant differences between groups in subject characteristics including age, body mass index, or CAR% (all P > 0.05). Subgroup analysis of individuals with carotid constriction (n = 6) vs. dilation (n = 34), revealed no significant difference in resting MAP or increase in MAP (P = 0.209 and 0.272, respectively). CONCLUSION: Our data suggest that the characteristic increase in MAP during the CPT does not mediate carotid artery vasomotion.

Archaeological evaluation - a total of 53 trenches, measuring 30m in length and 1.8m wide, were excavated. The evaluation recorded that, whilst much of the southern and western part of the site had limited potential for archaeological remains, archaeological activity was concentrated in the central and northeastern area of investigation, with a chronological range from the prehistoric to medieval and post-medieval periods. The evaluation recorded a circular segmented ring ditch, possibly later prehistoric in date, a sub-rectangular enclosure, a Romano-British field system which may have extended well into the post-Roman period and several circumscribed features such as pits and isolated linear ditches, likely to relate to the same chronological range.

- Abstract
We report 281 individuals carrying a pathogenic recurrent NF1 missense variant at p.Met1149, p.Arg1276, or p.Lys1423, representing three nontruncating NF1 hotspots in the University of Alabama at Birmingham (UAB) cohort, together identified in 1.8% of unrelated NF1 individuals. About 25% (95% confidence interval: 20.5-31.2%) of individuals heterozygous for a pathogenic NF1 p.Met1149, p.Arg1276, or p.Lys1423 missense variant had a Noonan-like phenotype, which is significantly more compared with the "classic" NF1-affected cohorts (all p < .0001). Furthermore, p.Arg1276 and p.Lys1423 pathogenic missense variants were associated with a high prevalence of cardiovascular abnormalities, including pulmonic stenosis (all p < .0001), while p.Arg1276 variants had a high prevalence of symptomatic spinal neurofibromas (p < .0001) compared with "classic" NF1-affected cohorts. However, p.Met1149-positive individuals had a mild phenotype, characterized mainly by pigmentary manifestations without externally visible plexiform neurofibromas, symptomatic spinal neurofibromas or symptomatic optic pathway gliomas. As up to 0.4% of unrelated individuals in the UAB cohort carries a p.Met1149 missense variant, this finding will contribute to more accurate stratification of a significant number of NF1 individuals. Although clinically relevant genotype-phenotype correlations are rare in NF1, each affecting only a small percentage of individuals, together they impact counseling and management of a significant number of the NF1 population.

- Abstract
Background and Purpose - As a reliable scoring system to detect the risk of symptomatic intracerebral hemorrhage after thrombectomy for ischemic stroke is not yet available, we developed a nomogram for predicting symptomatic intracerebral hemorrhage in patients with large vessel occlusion in the anterior circulation who received bridging of thrombectomy with intravenous thrombolysis (training set), and to validate the model by using a cohort of patients treated with direct thrombectomy (test set). Methods - We conducted a cohort study on prospectively collected data from 3714 patients enrolled in the IER (Italian Registry of Endovascular Stroke Treatment in Acute Stroke). Symptomatic intracerebral hemorrhage was defined as any type of intracerebral hemorrhage with increase of ≥4 National Institutes of Health Stroke Scale score points from baseline ≤24 hours or death. Based on multivariate logistic models, the nomogram was generated. We assessed the discriminative performance by using the area under the receiver operating characteristic curve. Results - National Institutes of Health Stroke Scale score, onset-to-end procedure time, age, unsuccessful recanalization, and Careggi collateral score composed the IER-SICH nomogram. After removing Careggi collateral score from the first model, a second model including Alberta Stroke Program Early CT Score was developed. The area under the receiver operating characteristic curve of the IER-SICH nomogram was 0.778 in the training set (n=492) and 0.709 in the test set (n=399). The area under the receiver operating characteristic curve of the second model was 0.733 in the training set (n=988) and 0.685 in the test set (n=779). Conclusions - The IER-SICH nomogram is the first model developed and validated for predicting symptomatic intracerebral hemorrhage after thrombectomy. It may provide indications on early identification of patients for more or less postprocedural intensive management.

Barn owl pellets were analyzed for comparing the small mammal fauna among three islands of Tuscan archipelago, Italy. Rarefaction analyses and diversity analyses were used on barn owl pellet samples at a total of 18 sites. Apodemus sylvaticus appeared the most abundant species in Elba and Capraia, and Mus musculus in Pianosa. Diversity profiles showed that Pianosa and Elba were very similar in terms of dominance and evenness profiles, whereas Capraia had higher dominance and lower evenness. Using our original data and literature review, we found that 5 species are found in Pianosa, 1 in Giannutri, 0 in Montecristo, 5 in Giglio, 7 in Elba, 6 in Capraia, and 1 in Gorgona. There was no effect of island area, human population size, or linear distance from mainland on species richness.

- Abstract
Analysis of 786 NF1 mutation-positive subjects with clinical diagnosis of neurofibromatosis type 1 (NF1) allowed to identify the heterozygous c.5425C>T missense variant (p.Arg1809Cys) in six (0.7%) unrelated probands (three familial and three sporadic cases), all exhibiting a mild form of disease. Detailed clinical characterization of these subjects and other eight affected relatives showed that all individuals had multiple cafè-au-lait spots, frequently associated with skinfold freckling, but absence of discrete cutaneous or plexiform neurofibromas, Lisch nodules, typical NF1 osseous lesions or symptomatic optic gliomas. Facial features in half of the individuals were suggestive of Noonan syndrome. Our finding and revision of the literature consistently indicate that the c.5425C>T change is associated with a distinctive, mild form of NF1, providing new data with direct impact on genetic counseling and patient management.

- Abstract
The article discusses a study which evaluated the quality of life in adult patient with anorectal malformations (ARM) and compared it to that experienced by a younger patient, with the new techniques available. Six of 66 patients subjected to posterior sagittal anorectoplasty (PSARP) presented with cloacae and 60 patients with ARM, with all patients undergoing the same workup including magnetic resonance imaging (MRI) diagnostics, evaluation for urinary and bowel incontinence and a urology screening. The study found that all patients whose age are 17 and above reported a good quality of life.

The clinical, neuropsychological and neuroradiological features of two patients affected by herpes simplex virus type 1 (HSV-1) encephalitis are described. An experimental study for the assessment of naming, recognition and description displayed in one patient a persistent significant impairment in naming living things. The other patient showed a failing "semantic memory" for the same categories, although a significant impairment emerged only for plants. In both patients, the late neuroradiological sequelae were localised mainly in the inferior and middle gyri of the left temporal lobe and in the left-side insula. In one patient, the right-side insula was also involved. The selective cerebral damage induced by HSV-1 is stressed and a correlation between the neuroradiological and neuropsychological findings is attempted. The stereotyped anatomical and neuropsychological changes lead to the belief that the virus may recognise, within the limbic system, particular cellular "strains" on the basis of their molecular specificity. - Abstract
To evaluate the role of anticoagulation therapy in carotid stenting with filter devices, we compared the activated clotting times (ACT) to angiographic filter patency and to the materials found inside the filters.29 endovascular treatments with filter cerebral protection in 27 patients affected by symptomatic internal carotid stenosis > 70% were considered. Angiographic findings, ACT and histopathologic specimens were recorded and statistically correlated.Satisfactory dilatation of the stenosis was always achieved with a complication rate of 3%. During the procedure nine filters caused temporary flow impairment. Histopathologic examination demonstrated material inside the filters in 86% of cases but this material was fibrin alone in 38% and plaque debris in 48%. Significant statistical correlation (p = 0.009) was found between low activated clotting time and occlusion of the filter.Distal protection filters can collect plaque fragments occurring during carotid stenting. A significant proportion of the debris found in the filters consisted of thrombotic material. Monitoring of heparin anticoagulation is recommended to prevent filter occlusion.

Le disrafie cranio-facciali costituiscono un importante capitolo della patologia malformativa. Sono lesioni rare, ma è sempre con un certo disagio che si affrontano i problemi etici, clinici e diagnostici che le accompagnano. Difficili e complesse le classificazioni proposte dai differenti autori.Fra le malformazioni più frequenti e suscettibili di correzione chirurgica efficace si sono esaminate in dettaglio l'iperteleorbitismo e le sindromi latero-facciali. Vengono valutate le possibilità di caratterizzazione morfologica e quantificazione biometrica offerte dalla TC con le recenti ricostruzioni 3D di superficie.Le informazioni ottenute con questa metodica risultano importanti per la comprensione della morfologia aberrante e per una corretta programmazione chirurgica.Il neuroradiologo ha gli strumenti idonei per la miglior comprensione della malformazione ma occorre sempre l'impegno di un «pacchetto» multidisciplinare per poter ridare a questi piccoli pazienti funzione, morfologia ed estetica.

Gli autori descrivono un caso di fistola carotido-cavernosa post-traumatica trattata per via endovascolare venosa. Il paziente era stato precedentemente sottoposto a molteplici interventi chirurgici con risultato insoddisfacente.La fistola è stata embolizzata con spirali e colla cianoacrilica, utilizzando l'approccio attraverso il seno petroso inferiore, ottenendone l'occlusione e il miglioramento della sintomatologia. L'embolizzazione per via venosa transpetrosa si è dimostrata tecnica efficace e priva di particolari difficoltà di esecuzione.
