Abstract

This paper presents an acoustic echo canceler based on a U-Net convolutional neural network for single-talk and double-talk scenarios. U-Net networks have previously been used in the audio processing area for source separation problems because of their ability to reproduce the finest details of audio signals, but to our knowledge, this is the first time they have been used for acoustic echo cancellation (AEC). The U-Net hyperparameters have been optimized to obtain the best AEC performance, but using a reduced number of parameters to meet a latency restriction of 40 ms. The training and testing of our model have been carried out within the framework of the 'ICASSP 2021 AEC Challenge' organized by Microsoft. We have trained the optimized U-Net model with a synthetic dataset only (S-U-Net) and with a synthetic dataset and the single-talk set of a real dataset (SR-U-Net), both datasets were released for the challenge. The S-U-Net model presented better results for double-talk scenarios, thus their inferred near-end signals from the blind testset were submitted to the challenge. Abstract

Background: Although autoimmunity and hyperinflammation secondary to recombination activating gene (RAG) deficiency have been associated with delayed diagnosis and even death, our current understanding is limited primarily to small case series. Objective: Understand the frequency, severity, and treatment responsiveness of autoimmunity and hyperinflammation in RAG deficiency. Methods: In reviewing the literature and our own database, we identified 85 patients with RAG deficiency, reported between 2001 and 2016, and compiled the largest case series to date of 63 patients with prominent autoimmune and/or hyperinflammatory pathology. Results: Diagnosis of RAG deficiency was delayed a median of 5 years from the first clinical signs of immune dysregulation. Most patients (55.6%) presented with more than 1 autoimmune or hyperinflammatory complication, with the most common etiologies being cytopenias (84.1%), granulomas (23.8%), and inflammatory skin disorders (19.0%). Infections, including live viral vaccinations, closely preceded the onset of autoimmunity in 28.6% of cases. Autoimmune cytopenias had early onset (median, 1.9, 2.1, and 2.6 years for autoimmune hemolytic anemia, immune thrombocytopenia, and autoimmune neutropenia, respectively) and were refractory to intravenous immunoglobulin, steroids, and rituximab in most cases (64.7%, 73.7%, and 71.4% for autoimmune hemolytic anemia, immune thrombocytopenia, and autoimmune neutropenia, respectively). Evans syndrome specifically was associated with lack of response to first-line therapy. Treatment-refractory autoimmunity/hyperinflammation prompted hematopoietic stem cell transplantation in 20 patients. Conclusions: Autoimmunity/hyperinflammation can be a presenting sign of RAG deficiency and should prompt further evaluation. Multilineage cytopenias are often refractory to immunosuppressive treatment and may require hematopoietic cell transplantation for definitive management. © 2019 The Authors

Abstract

Objective: To develop and validate practical prognostic models for death at 14 days and for death or severe disability six months after traumatic brain injury. Design: Multivariable logistic regression to select variables that were independently associated with two patient outcomes. Two models designed: "basic" model (demographic and clinical variables only) and "CT" model (basic model plus results of computed tomography). The models were subsequently developed for high and low-middle income countries separately. Setting: Medical Research Council (MRC) CRASH Trial. Subjects: 10 008 patients with traumatic brain injury. Models externally validated in a cohort of 8509. Results: The basic model included four predictors: age, Glasgow coma scale, pupil reactivity, and the presence of major extracranial injury. The CT model also included the presence of petechial haemorrhages, obliteration of the third ventricle or basal cisterns, subarachnoid bleeding, midline shift, and non-evacuated haematoma. In the derivation sample the models showed excellent discrimination (C statistic above 0.80). The models showed good calibration graphically. The Hosmer-Lemeshow test also indicated good calibration, except for the CT model in low-middle income countries. External validation for unfavourable outcome at six months in high income countries showed that basic and CT models had good discrimination (C statistic 0.77 for both models) but poorer calibration. Conclusion: Simple prognostic models can be used to obtain valid predictions of relevant outcomes in patients with traumatic brain injury.

Abstract

Background Corticosteroids have been used to treat head injuries for more than 30 years. In 1997, findings of a systematic review suggested that these drugs reduce risk of death by 1-2%. The CRASH trial—a multicentre international collaboration—aimed to confirm or refute such an effect by recruiting 20 000 patients. In May, 2004, the data monitoring committee disclosed the unmasked results to the steering committee, which stopped recruitment. Methods 10 008 adults with head injury and a Glasgow coma score (GCS) of 14 or less within 8 h of injury were randomly allocated 48 h infusion of corticosteroids (methylprednisolone) or placebo. Primary outcomes were death within 2 weeks of injury and death or disability at 6 months. Prespecified subgroup analyses were based on injury severity (GCS) at randomisation and on time from injury to randomisation. Analysis was by intention to treat. Effects on outcomes within 2 weeks of randomisation are presented in this report. This study is registered as an International Standard Randomised Controlled Trial, number ISRCTN74459797. Findings Compared with placebo, the risk of death from all causes within 2 weeks was higher in the group allocated corticosteroids (1052 [21·1%] vs 893 [17·9%] deaths; relative risk 1·18 [95% CI 1·09-1·27]; p=0·0001). The relative increase in deaths due to corticosteroids did not differ by injury severity (p=0·22) or time since injury (p=0·05). Interpretation Our results show there is no reduction in mortality with methylprednisolone in the 2 weeks after head injury. The cause of the rise in risk of death within 2 weeks is unclear.

Abstract

The problem of designing joint power control and optimal beamforming (JPCOB) algorithms for the downlink of a coordinated multi-cell WCDMA system is considered throughout this paper. In this case, the JPCOB design is formulated as the problem of minimizing the total transmitted power in the coordinated multi-cell system, subject to a certain quality of service requirement for each user. In this paper, the performance of two JPCOB algorithms based on different beamforming approaches is compared over the coordinated multi-cell system. The first one, obtains local beamformers by means of the well-known virtual uplink-downlink duality. In contrast, the second algorithm implements multi-base beamformers, taking into account match filter equalizers at the receivers. Moreover, realistic system parameters, such as per-base station power constraints or the asynchronous nature of the signals arriving at the receivers, are taken into account. Simulation results show that the algorithm based on multi-base beamforming presents attractive properties, such as an inherent multi-base scheduling technique or a decreasing total transmitted power as the degree of coordination between base stations is increased. Abstract

The objectives of this study were to assess the immunolocalization of human osteopontin (OPN) in oral lesions and to identify human cell lines of oral squamous cell carcinoma (OSCC) origin that express OPN mRNA. OPN was localized using immunohistochemistry in the following oral specimens: normal epithelium (n=6), epithelial hyperplasia (n=4), epithelial dysplasia (n=28), carcinoma in situ (n=11) and squamous cell carcinoma (n=43). Cell lines UMSCC-1, MDA TU 138, MDA 686LN, SCC4, SCC9, SCC25, CAL 27 and MDA 1483 were characterized for OPN mRNA expression using Northern blotting. OPN was not detected in normal oral epithelium. Intracellular and intercellular immunoreactivity was seen in 75% of hyperplasias, 57% of dysplasias, 54% of carcinoma in situ and 67% of squamous cell carcinomas. UMSCC-1 expressed high levels of OPN mRNA. We conclude that OPN protein is detectable in premalignant and malignant lesions arising from oral epithelium. UMSCC-1 may be a useful cell line in which to conduct in vitro studies designed to clarify the role of OPN in OSCC. [ABSTRACT FROM AUTHOR]

Abstract

Three human gingival fibroblast cell lines were used to determine whether they could be induced by a synthetic RNA and super-induced by metabolic inhibitors to produce interferon (IFN-β). When established procedures were followed for human fetal or newborn skin fibroblast cell lines, the adult gingival fibroblasts produced comparable amounts of IFN-β. It was shown that the superinducers alone would not cause an IFN-β production response, and that the absence of serum in the production medium also inhibited the production of IFN-β. The effect of IFN-β on cell growth was carried out in T-flasks seeded with 105 HEp-2 cells. After one and two wk, the cells of triplicate control flasks and triplicate flasks containing various dilutions of the production media were enumerated to determine a cell multiplication inhibition (CMI) value. A correlation between the IFN-β content and the CMI effect, however, was not obtained, and it was concluded that other CMI agents, possibly more potent than the IFN-β, were being produced by the stimulated human gingival fibroblasts. Cell protein assays which gave a high ng/ cell protein content correlated with TEM micrographs which showed clusters of complex lysosomes, primarily in cells cultured in the IFN-β-containing nutrient. However, since commercial IFN-β initiated no such lysosomal response, it was further concluded that the complex lysosomes were due to CMI agent(s) other than IFN-β. [ABSTRACT FROM AUTHOR]

Phagocytosis by a human gingival epithelial-like cell line in tissue culture was studied using latex beads as a marker. Electron microscopic and chemical analyses revealed that the cells ingested the beads in a quantitative and reproducible manner. 40. Microfilaments in Human Cementoblasts and Periodontal Fibroblasts

Abstract

An electron microscopic surveywas carried out on the human periodontal ligament (PDL), including a part of the gingival connective tissue attached to extracted tooth roots (11 functioning premolars and 6 nonfunctioning third molars) in order to examine the characteristics of microfilaments (6 nm) in cementoblasts and PDL fibroblasts. Microfilaments which were grouped in bundles with semiperiodic dense nodes or in meshworks just beneath the cell membrane were seen predominantly in the cells characterized by their ultrastructurally immature appearance. These microfilaments were more commonly observed in third molar PDL than in premolar PDL and, in general, more conspicuous in cementoblasts than in fibroblasts. The significance of microfilaments in human PDL is discussed, particularly in relation to cell differentiation and morphogenesis.

41. Ultrastructural and Morphometric Analyses of Human Cementoblasts and Periodontal Fibroblasts

Abstract

In order to elucidate the cytologicalcharacteristics of human cementoblasts which distinguish them from periodontal fibroblasts, the periodontal ligament and gingival connective tissue attached to 37 extracted teeth from 27 patients (ages 10‐67) were analyzed by electron microscopy coupled with a morphometric procedure. The cementoblasts largely consisted of either immature or restingtypes of collagen‐producing cells (CPC), both of which were poor in the rough endoplasmic reticulum and Golgi complex; these organelles were well‐developed in the relatively less common activecementoblasts. The cementoblasts consistently revealed a higher mean‐volume‐density per tissue unit than the fibroblasts and sometimes were grouped into clusters with the formation of junctional apparatuses. The relative volume of glycogen particles per cytoplasm was significantly higher in the cementoblasts, whereas the rough endoplasmic reticulum was higher in the fibroblasts. From the present study, it is suggested that the cementoblasts are functionally less active CPC than the periodontal fibroblasts.
