Abstract

Purpose: To evaluate the performance of the Fugo Blade in cutting cornea tissue in animals, both in vitro and in vivo. Methods: Comeal incisions were made on pig eyes using the Fugo Blade. The histologic characteristics of these cuts were then evaluated. Similar cuts were also performed in rabbit corneas in vivo and compared with cuts performed with mechanical blades. The rabbits were euthanized, and the corneas were obtained for histopathologic examination 14 days after surgery. Results: Slit-lamp examinations and histologic findings showed that the Fugo Blade can produce sharp cuts in the cornea with minimal tissue damage beyond the edges of the incisions. It cuts without resistance and permits normal wound healing. Conclusions: Although the Fugo Blade is currently approved by the Food and Drug Administration for anterior capsulotomy, our study demonstrates that the Fugo Blade can safely and quickly produce resistance-free cuts in corneal tissue in animals, opening additional avenues for use of this device in comeal surgery.

Abstract

PURPOSE. The authors report a case of fungal scleral keratitis and endophthalmitis as a complication of pterygium surgery. METHODS. Case presentation. RESULTS. A 46-year old woman underwent pterygium excision with topical use of intraoperative mitomycin C. By day 21 after excision, scleral melting was followed by fungal keratitis. Endophthalmitis ensued, which rapidly progressed, despite surgical and medical interventions. The eye was finally enucleated to prevent fungaemia. Pathology revealed a highly disorganized eye with disruption of scleral collagen in the area of scleral melting. CONCLUSIONS. Fungal keratitis and scleral melting are rare but devastating complications of pterygium surgery with adjuvant use of mitomycin C. (Eur J Ophthalmol 2009; 19: 478-80)

6. Aqueous Vascular Endothelial Growth Factor as a Predictor of Macular Thickening Following Cataract Surgery in Patients With Diabetes Mellitus

Abstract

Purpose To study associations between serum and aqueous vascular endothelial growth factor (VEGF) and insulin-like growth factor 1 (IGF-1) and macular edema measured with optical coherence tomography (OCT) following phacoemulsification in diabetic patients. Design Cohort study. Methods A pilot study of 36 consecutive diabetic patients undergoing planned phacoemulsification with IOL in 1 eye by one surgeon at the University of North Carolina consented to preoperative and postoperative OCT central subfield (CSF) thickness measurements and aqueous and blood samples for VEGF and IGF-1. Four patients with clinically significant macular edema (CSME) received laser preoperatively. Spearman-rank correlations were performed between growth factors and mean CSF or a clinically meaningful percent change in CSF (>11% of preoperative measurement) at 1 and 6 months postoperatively. Results There were no surgical complications or new cases of CSME following surgery. Mean aqueous VEGF in patients with retinopathy, determined preoperatively, increased with increasing level of severity. Patients with preoperative CSME also had severe or worse retinopathy and the greatest mean aqueous VEGF. Significant preoperative correlations existed between aqueous VEGF and more severe retinopathy whether CSME was present or absent ( r = 0.49; P = .007), and between aqueous VEGF and CSME ( r = 0.41; P = .029). At 1 month postoperative, aqueous VEGF was positively correlated with >11% change from preoperative CSF regardless of CSME status ( r = 0.47; P = .027). No noteworthy associations existed between CSF and IGF-1 values. Conclusions Aqueous VEGF was significantly positively associated with a clinically meaningful change in CSF in diabetic patients 1 month following cataract surgery. Accounting for preoperative CSF was important. Further study is indicated.

Abstract

Purpose To report 3 cases of myiasis with a singular subretinal track in the Northeastern part of United States. Design Noninterventional case series. Methods Three patients from Pennsylvania were referred for evaluation of a linear and serpiginous retinal scar. Funduscopic examination and fluorescein angiography were performed to assess the referred retinal lesion. Results Funduscopic examination showed a singular subretinal track from the superior periphery to either the posterior pole or inferior midperiphery in the study cases. Fluorescein angiography showed exudative retinal detachment in all 3 cases. Laser photocoagulation of the subretinal worm was performed in all reported cases. Conclusions Our case series represents a report of singular subretinal tracks presumably caused by the warble fly larva in the Northeastern part of United States.
