18. Comparative study of HoLEP in elderly patients with indwelling catheters: a retrospective dual-center study.

Abstract

Introduction: This study aimed to evaluate the safety and efficacy of HoLEP in patients aged > 85 years with indwelling catheter (IDC). Methods: We retrospectively analyzed our bicentric HoLEP database to identify consecutive patients with IDC and trial without catheter (TWOC) failure who underwent surgery between June 2012 and April 2020. Our primary focus was on the population over 85 years of age; Patients under 70 years of age were used as controls. We evaluated the rate of postoperative spontaneous micturition, adverse events and mortality, as well as catheterization duration, hospital stay, and rate of urinary incontinence. Results: In total, 144 patients were included, with 71 (49.3%) and 73 (50.8%) in the ≥ 85 and control group, respectively. The median [IQR] prostatic volume and Charlson score was 90 [70–130] vs. 90 [65–120] mL and 6 [5–7] vs. 3 [2–4] in the ≥ 85 and control groups, respectively. Postoperatively, the rates of major complications (2% vs. 3%) and transfusion (11% vs. 9%) were not significantly different. The median length of stay (2 [1–3] vs. 1 [1–2]; p = 0.03) and rate of TWOC failure during hospitalization (24% vs. 5%, p < 0.001) were significantly higher in the ≥ 85 group. At 3 months and one year follow-up visits, 0 and 10 patients (14.1%) in the ≥ 85 group and 0 and 2 patients (3%) in the control group died, respectively. One year after surgery, 69 (97%) and 73 (100%) (p = 0.2) patients in the ≥ 85 and control groups, respectively, could void spontaneously. Conclusion: HoLEP is a safe and effective procedure for elderly patients with indwelling catheters. At one year, 97% of patients achieved spontaneous voiding, while the urinary incontinence rate was 11%. Abstract

Purpose: To systematically evaluate and synthesize the existing literature on environmental aspects related to urological endoscopy procedures. Methods: A literature search was conducted through January 2024 using PubMed/Medline, and Web of Science databases. Preferred Reporting Items for Systematic Reviews and Meta-analysis (PRISMA) guidelines were followed to identify eligible studies. Results: The review reveals varying environmental footprints across procedures. In cystoscopy, studies suggest that disposable scopes result in decreased waste and water usage compared to reusable ones. However, life-cycle assessments did not conclusively highlight environmental superiority in favor of disposable devices. This discrepancy might dwell in the reprocessing methods used for the reusable cystoscopes that vary greatly among centers. For ureteroscopy, studies tend to show comparable carbon footprints for disposable and reusable scopes. Regarding transurethral resection of bladder tumors, a shift towards day-case surgery is associated with a substantial reduction in carbon emissions. Transurethral prostatic surgery assessments reveal significant discrepancies in the carbon footprints of disposable minimally invasive surgical devices, highlighting the need for recycling programs and manufacturing modifications. Conclusions: This review shed light on the necessity to develop strategies to reduce environmental impact such as individual assessments of single-use devices, optimizing reusable scope reprocessing, reconsidering the use of draping during cystoscopies, and minimizing hospital stays. This study serves as a call to industries to conduct similar audits and standardized studies for healthcare products and services, fostering evidence-based decisions to mitigate the environmental impact of urological endoscopy procedures. Abstract

Objectives Patients and Methods Results Conclusion To assess the comparative effectiveness of robot‐assisted radical cystectomy (RARC) with intracorporeal urinary diversion (ICUD) vs open radical cystectomy (ORC) for bladder cancer (BC).We conducted a real‐life monocentric study including all consecutive patients who underwent RARC with ICUD or ORC for BC at our institution from 2014 to 2023. Uni‐ and multivariable logistic and Cox regression analyses were used to compare perioperative, oncological and stricture outcomes between both groups by calculating odds (ORs) and hazard (HRs) ratios with their corresponding 95% confidence intervals (CIs), respectively.Overall, 316 patients underwent either RARC with ICUD (n = 228 [72.2%]) or ORC (n = 88 [27.8%]). The perioperative benefits of RARC vs ORC included decreased risks of major blood loss (OR 0.10, 95% CI 0.04–0.23; P < 0.001), perioperative transfusion (OR 0.30, 95% CI 0.16–0.57; P < 0.001), 90‐day major complications (OR 0.56, 95% CI 0.29–0.99; P = 0.04), and prolonged initial length of hospital stay (OR 0.20, 95% CI 0.09–0.35; P < 0.001), as well as more days alive and out of the hospital within 90 days of surgery (OR 2.56, 95% CI 1.46–4.6; P < 0.01). In addition, the use of RARC vs ORC was associated with a higher lymph node (LN) count (OR 3.35, 95% CI 1.83–6.30; P < 0.001), while there was no significant difference in recurrence‐free (HR 0.72, 95% CI 0.49–1.07; P = 0.1), cancer‐specific (HR 0.69, 95% CI 0.43–1.10; P = 0.1), overall (HR 0.76, 95% CI 0.47–1.20; P = 0.3) and uretero‐ileal stricture‐free (HR 1.18, 95% CI 0.62–2.25; P = 0.6) survival between both groups after a median (interquartile range) follow‐up of 42.3 (16.4–73.8) months.Our real‐world study supports the effectiveness of RARC with ICUD vs ORC for BC. We generally observed better perioperative outcomes, as well as similar oncological—except for higher LN count—and uretero‐ileal stricture outcomes after RARC with ICUD vs ORC. 40. Evaluation of patient satisfaction using the validated French version of the SSIPI questionnaire after inflatable penile implantation.

Abstract

Background: Penile prothesis (PP) is a recommended treatment for erectile dysfunction that is refractory to less invasive treatments, but there are few validated tools to assess patient satisfaction. Aim: The aim of this study was to assess patient satisfaction after PP implantation using the French Satisfaction Survey for Inflatable Penile Implant (SSIPI) questionnaire. Methods: Demographic, clinical, and perioperative data were collected from all consecutive patients who underwent PP implantation in our center between 2016 and 2021. The French SSIPI questionnaire was completed during a telephone call with each patient by an independent investigator. Outcomes: A good functional result was considered when the total SSIPI score was >48, corresponding to a score of >3 for each item. Results: The median global SSIPI score for the cohort was 66 (interquartile range [IQR], 60-73), and 53 (89.8%) patients were satisfied. The appearance of the penis with the PP was the item that had the lowest score for satisfaction (median score 23 [IQR, 19–26]), while the patients reported almost no pain (median score 10 [IQR, 9-10]). When patients with a total score of ≤64 were compared with those with a score of >64, PP size was significantly greater in the group with better functional results (P = .03). Clinical Implications: Assessment of patient satisfaction with a PP is important because this is the main criterion used to judge the success of surgery. Strengths and Limitations: To our knowledge, this is the first study to evaluate the satisfaction of patients with a PP using the French-validated version of SSIPI questionnaire. However, patients came from a single center and the population size was small.
