Introdução: Os Serviços de Urgência são a principal porta de entrada no SNS e pela facil acessibilidade e capacidade de resposta aos mais diversificados problemas, muitos são aqueles que recorrem ao SU por situações não urgentes, constituindo-se como uma preocupação nacional e mundial. Um dos grupos populacionais com maior numero de idas ao SU, é o dos idosos com 80 e mais anos, dado que as implicações associadas ao envelhecimento, (INE, PORDATA, 2022), os tornam assíduos e grande consumidores de serviços de saúde. Objetivos: Caraterizar o perfil clínico do utilizador, não urgente, do SU - a Pessoa com 80 e mais anos. Material e Métodos: Estudo de natureza quantitativa, com alvo nas Pessoas com 80 e mais anos, que durante o ano civil pré-pandemia recorreram ao SU de um Centro Hospitalar da Zona Centro de Portugal. Foram incluídas todas as Pessoas com 80 e mais anos, a quem, após Triagem de Manchester (TM) (GPT, 2018) foi atribuída uma das prioridades consideradas não urgentes (Verde, Azul ou branco) e excluídas aquelas em que a causa do recurso ao SU foi por: queda ou acidente e ainda aquelas que aquando da alta/destino após o episódio de urgência, se verificou: internamento, óbito ou transferência para outro hospital, a amostra ficou assim constituida por 2618 episódios, correspondendo a 88.6% dos incluídos. A amostra foi constituída através da amostragem aleatória estratificada. Resultados: Prevalece o género feminino (58.1%) com uma média de idades de 85.7 anos (dp=4.317), 12 referem-se a centenários, 64.7% triados como menos urgente (verde), admitidos no turno da manhã (71.6%), o dia da semana com maior afluência foi a 2.ª feira (18.1%) e de menor o domingo (9.2%), nos meses de julho e agosto foram ultrapassados os 10%. Os tempos médios de demora, desde a admissão à TM foi de 14’, para a TM de 2’, desde a TM à 1.ª observação médica aproximou-se das 2 horas (1 hora e 50’), e os tempos médios por episódio aproximaram-se das 6 horas (5 horas 40’), sendo que 33.6% ultrapassou as 6 horas. Conclusões: O perfil apurado, denota que o utilizador, não urgente, com oitenta e mais anos no SU, carece de recusos acrescidos e diferenciados pela comunidade, de modo a manter a saúde e abolir a necessidade de ir ao S.Urgência.

Flavylium compounds include a large variety of natural compound classes, namely, anthocyanins, 3-deoxyanthocyanidins, auronidins, and their respective aglycones as well as anthocyanin-derived pigments (e.g., pyranoanthocyanins, anthocyanin-flavan-3-ol dimers). During the past few decades, there has been increasing interest among chemists in synthesizing different flavylium compounds that mimic natural structures but with different substitution patterns that present a variety of spectroscopic characteristics in view of their applications in different industrial fields. This Review provides an overview of the chemistry of flavylium-based compounds, in particular, the synthetic and enzymatic approaches and mechanisms reported in the literature for obtaining different classes of pigments, their physical-chemical properties in relation to their pH-dependent equilibria network, and their chemical and enzymatic degradation. The development of flavylium-based systems is also described throughout this Review for emergent applications to explore some of the physical-chemical properties of the multistate of species generated by these compounds. publishersversion published

A molecule (flavylium cation) generates a sequence of other different molecules by means of external stimuli. The reversibility of the system allows for the flavylium cation to be recovered by other external stimuli, completing one cycle. Differently from supramolecular chemistry, molecular metamorphosis is not a bottom-up approach. All events occur at the bottom. The procedures to characterize the kinetics and thermodynamics of the cycles are summarized. They are based on direct pH jumps (addition of a base to the flavylium cation) and reverse pH jumps (addition of an acid to equilibrated solutions at higher pH values). Stopped flow is an indispensable tool to characterize these systems. The following metamorphic cycles will be described to illustrate the concept: (i) introducing the flavanone in the metamorphic system and illustrating the concept of a timer at the molecular level; (ii) response of the flavylium-based metamorphosis to light inputs and the write-lock-read-unlock-erase molecular system; (iii) a one-way cycle of direct-reverse pH jumps; (iv) interconversion of the flavylium cation with 2,2′-spirobis[chromene] derivatives; (v) 6,8 A-ring substituent rearrangements. publishersversion published
