Abstract

International audience; The goal of this paper is to present new comparative evidence on heart attack care in 17 countries showing that changes in medical treatments are universal, but have differed greatly. We have collected a large body of comparable information that show how countries differ in treatment rates and why these differences are relatively marked. Countries appear to differ systematically in the time at which intensive cardiac procedures began to be widely used and in the rate of growth of the procedures. Our results show that differences in treatment rates are greatest for expensive medical technologies. Also strict financing limits and regulatory policies have affected the adoption of intensive technologies. These differences may have important economic and health consequences.

We present new comparative evidence on heart attack care in seventeen countries showing that technological change-changes in medical treatments that affect the quality and cost of care-is universal but has differed greatly around the world. Differences in treatment rates are greatest for costly medical technologies , where strict financing limits and other policies to restrict adoption of intensive technologies have been associated with divergences in medical practices over time. Countries appear to differ systematically in the time at which intensive cardiac procedures began to be widely used and in the rate of growth of the procedures. The differences appear to be related to economic and regulatory incentives of the health care systems and may have important economic and health consequences. H ea lt h c ar e is an i ndu st ry that is becoming part of a global economy. Biomedical knowledge and technologies are already "global" in the sense that leading physicians in all developed countries read the same journals and electronic reviews and participate in international consortia to encourage best practices. For the most part, the same drugs and devices are available worldwide, at least within a few years of each other. Leading clinical researchers and experts collaborate internationally; leading drug and biotechnology firms think globally. As worldwide policy interest in quality of care continues to rise, international comparisons of health care systems are becoming common. Yet health care also remains a local industry, with care delivered by physicians influenced by their local peers, practice settings, and health care policies.

11. Knowledge, attitudes and practices towards pandemic influenza among cases, close contacts, and healthcare workers in tropical Singapore: A cross-sectional survey

Abstract

Background. Effective influenza pandemic management requires understanding of the factors influencing behavioral changes. We aim to determine the differences in knowledge, attitudes and practices in various different cohorts and explore the pertinent factors that influenced behavior in tropical Singapore. Methods. We performed a cross-sectional knowledge, attitudes and practices survey in the Singapore military from mid-August to early-October 2009, among 3054 personnel in four exposure groups - laboratory-confirmed H1N1-2009 cases, close contacts of cases, healthcare workers, and general personnel. Results. 1063 (34.8%) participants responded. The mean age was 21.4 (SE 0.2) years old. Close contacts had the highest knowledge score (71.7%, p = 0.004) while cases had the highest practice scores (58.8%, p < 0.001). There was a strong correlation between knowledge and practice scores (r = 0.27, p < 0.01) and knowledge and attitudes scores (r = 0.21, p < 0.01). The significant predictors of higher practice scores were higher knowledge scores (p < 0.001), Malay ethnicity (p < 0.001), exposure group (p < 0.05) and lower education level (p < 0.05). The significant predictors for higher attitudes scores were Malay ethnicity (p = 0.014) and higher knowledge scores (p < 0.001). The significant predictor for higher knowledge score was being a contact (p = 0.007). Conclusion. Knowledge is a significant influence on attitudes and practices in a pandemic, and personal experience influences practice behaviors. Efforts should be targeted at educating the general population to improve practices in the current pandemic, as well as for future epidemics.

15. Curcumins-Rich Curry Diet and Pulmonary Function in Asian Older Adults.

Abstract

Background: Research on the effects of dietary nutrients on respiratory health in human populations have not investigated curcumin, a potent anti-oxidant and anti-inflammatory compound present principally in turmeric used in large amounts in Asian curry meals. Objectives: To examine the association of curry intake with pulmonary function among smokers and non-smokers. Design: The frequency of curry intake, respiratory risk factors and spirometry were measured in a population-based study of 2,478 Chinese older adults aged 55 and above in the Singapore Longitudinal Ageing Studies. Results: Curry intake (at least once monthly) was significantly associated with better FEV1 (b = 0.045±0.018, p = 0.011) and FEV1/FVC (b = 1.14±0.52, p = 0.029) in multivariate analyses that controlled simultaneously for gender, age, height, heightsquared, smoking, occupational exposure and asthma/COPD history and other dietary or supplementary intakes. Increasing levels of curry intake ('never or rarely', 'occasional', 'often', 'very often') were associated with higher mean adjusted FEV1 (p for linear trend = 0.001) and FEV1/FVC% (p for linear trend = 0.048). Significant effect modifications were observed for FEV1 (curry" smoking interaction, p = 0.028) and FEV1/FVC% (curry"smoking interaction, p = 0.05). There were significantly larger differences in FEV1 and FEV1/FVC% between curry intake and non-curry intake especially among current and past smokers. The mean adjusted FEV1 associated with curry intake was 9.2% higher among current smokers, 10.3% higher among past smokers, and 1.5% higher among non-smokers. Conclusion: The possible role of curcumins in protecting the pulmonary function of smokers should be investigated in further clinical studies. [ABSTRACT FROM AUTHOR]
