- Abstract
Objective: To develop and validate practical prognostic models for death at 14 days and for death or severe disability six months after traumatic brain injury. Design: Multivariable logistic regression to select variables that were independently associated with two patient outcomes. Two models designed: "basic" model (demographic and clinical variables only) and "CT" model (basic model plus results of computed tomography). The models were subsequently developed for high and low-middle income countries separately. Setting: Medical Research Council (MRC) CRASH Trial. Subjects: 10 008 patients with traumatic brain injury. Models externally validated in a cohort of 8509. Results: The basic model included four predictors: age, Glasgow coma scale, pupil reactivity, and the presence of major extracranial injury. The CT model also included the presence of petechial haemorrhages, obliteration of the third ventricle or basal cisterns, subarachnoid bleeding, midline shift, and non-evacuated haematoma. In the derivation sample the models showed excellent discrimination (C statistic above 0.80). The models showed good calibration graphically. The Hosmer-Lemeshow test also indicated good calibration, except for the CT model in low-middle income countries. External validation for unfavourable outcome at six months in high income countries showed that basic and CT models had good discrimination (C statistic 0.77 for both models) but poorer calibration. Conclusion: Simple prognostic models can be used to obtain valid predictions of relevant outcomes in patients with traumatic brain injury.

- Abstract
Closely linked to the concepts of growth and globalisation is the increasing recognition being given to the importance of the workplace as a site of learning. The value of the book rests in the fact that this framework does not have its foundations in just one academic discipline, but rather provides a range of perspectives drawn from psychology, social psychology, sociology and critical theory. The purpose is to move away from a unidimensional understanding of workplaces and workplace learning to provide a different standpoint from which to better appreciate the diversity and complexity of learning in the workplace.

- Abstract
The contexts in which work is performed and socially positioned are undergoing considerable change. Though these changes may take on different forms in different work contexts, there is general agreement that they are the consequence of the impact of globalisation and the gradual evolution in the ways that workers perceive their engagement with paid and unpaid work activities. The global marketplace is being shaped by factors such as the reintegration of Europe and the emergence of new economies in the developing nations of Asia and South America that bring with them new forms of cooperation and competition as well as by advances in information and communication technologies that result in rapid flows of information and financial capital. These factors have resulted in widespread organisational change as organisations face the daily manifestations and impacts of these changes. There is no disagreement that change is inevitable, growth in such periods of change, however remains optional. Only those organisations that 'grow' as a result of change will remain, not only to survive, but to thrive, in the ever-evolving global marketplace.

- Abstract
Efforts to develop and define education and training models that are responsive to the needs of the high performance workplace have involved interventions that are well beyond formal instruction derived from psychological research on human learning. Current literature on human learning argues that such instructional interventions are limited and often considered fragmented - they tend to focus on specific items of decontextualised knowledge (see Lave 1991; Scribner, 1986). Whilst some current models are more comprehensive ans systematic than in the past, they nevertheless reflect a view of developing professional expertise that is prescriptive and deterministic. Present models may be useful for many types of knowledge and skills development; however their potential for developing skilled and knowledgeable performance needed in a contingent and dynamic work environment is far from optimal. The emergent model based on a shared cognition or community of learners, expands our understanding of learning but it is still prescriptive and narrow in that it undervalues the contribution of the individuals' cognition in human learning and performance (Lave & Wagner, 1991). To explore the emerging nature of workplace knowledge and skills and their significance for education and training, this chapter will firstly discuss the multi-dimensional nature of emerging workplace contexts and the associated knowledge base. Secondly, the changing views about human learning processes will be discussed in the context of a proposed model for examining the complex nature of workplace knowledge.

- Abstract
Background Corticosteroids have been used to treat head injuries for more than 30 years. In 1997, findings of a systematic review suggested that these drugs reduce risk of death by 1-2%. The CRASH trial—a multicentre international collaboration—aimed to confirm or refute such an effect by recruiting 20 000 patients. In May, 2004, the data monitoring committee disclosed the unmasked results to the steering committee, which stopped recruitment. Methods 10 008 adults with head injury and a Glasgow coma score (GCS) of 14 or less within 8 h of injury were randomly allocated 48 h infusion of corticosteroids (methylprednisolone) or placebo. Primary outcomes were death within 2 weeks of injury and death or disability at 6 months. Prespecified subgroup analyses were based on injury severity (GCS) at randomisation and on time from injury to randomisation. Analysis was by intention to treat. Effects on outcomes within 2 weeks of randomisation are presented in this report. This study is registered as an International Standard Randomised Controlled Trial, number ISRCTN74459797. Findings Compared with placebo, the risk of death from all causes within 2 weeks was higher in the group allocated corticosteroids (1052 [21·1%] vs 893 [17·9%] deaths; relative risk 1·18 [95% CI 1·09-1·27]; p=0·0001). The relative increase in deaths due to corticosteroids did not differ by injury severity (p=0·22) or time since injury (p=0·05). Interpretation Our results show there is no reduction in mortality with methylprednisolone in the 2 weeks after head injury. The cause of the rise in risk of death within 2 weeks is unclear.

- Abstract
Contemporary lifespan development models of adaptive development have been applied to the workforce to examine characteristics of the ageing employee. Few studies have examined adaptive development in terms of worker perceptions of workplace, or their learning and development issues. This study used the recently developed Revised Learning and Development Survey to investigate employee selection and engagement of learning and development goals, opportunities for learning and development at work, and constraints to learning and development at work. Demographic and career goal variables were tested amongst a sample of private hospital employees, almost all of whom were nurses. Workers under 45 years of age perceived greater opportunities for training and development than more mature aged workers. Age and physical demands interacted such that physical demands of work were associated with lower engagement in learning and development goals in mature aged workers. The opposite was observed amongst younger workers. Engagement in learning and development goals at work predicted goals associated with an intention to decrease work hours or change jobs to a different industry when opportunities to learn via work tasks were limited. At the same time limited opportunities for training and development and perceptions of constraints to development at work predicted the intention to change jobs. Results indicate consideration must be paid to employee perceptions in the workplace in relation to goals. They may be important factors in designing strategies to retain workers.
