In the workup of follicular lymphoma (FL), bone marrow biopsy (BMB) assessment is a key component of FLIPI and FLIPI2, the most widely used outcome scores. During the previous decade, several studies explored the role of FDG-PET/CT for detecting nodal and extranodal disease, with only one large study comparing both techniques.The aim of our study was to evaluate the diagnostic accuracy and the prognostic impact of both procedures in a retrospective cohort of 299 FL patients with both tests performed at diagnosis. In order to avoid a collinearity bias, FLIPI2 was deconstructed in its founding parameters, and the bone marrow involvement (BMI) parameter separately included as: a positive BMB, a positive PET/CT, the combined "PET/CT and BMB positive" or "PET/CT or BMB positive". These variables were also confronted independently with the POD24 in 233 patients treated with intensive regimens.In the total cohort, bone marrow was involved in 124 and 60 patients by BMB and PET/CT, respectively. In terms of overall survival, age 60 y.o. and the combined "PET/CT or BMB positive" achieved statistical independence as a prognostic factor. In patients treated with an intensive regimen, only the combined "PET/CT or BMB positive" added prognostic value for a shorter overall survival, when confronted with the POD24.Our results show that in FL both BMB and PET/CT should be considered at diagnosis, as their combined assessment provides independent prognostic value in the context of the most widely use clinical scores.

Introducción y objetivo: La cirugía radioguiada del hiperparatiroidismo (HPT) ha evolucionado en los últimos años, gracias a la aparición de nuevas herramientas intraoperatorias y al perfeccionamiento de las técnicas pre-operatorias. El objetivo de este trabajo es realizar una revisión actualizada del papel del PET-CT con colina como técnica preoperatoria, así como de las herramientas intraoperatorias, en la radiolocalización de adenomas de paratiroides. Síntesis: El PET-CT con colina es una técnica preoperatoria, alternativa a la gammagrafía con 99mTc-MIBI, para la localización pre-quirúrgica del HPT, en casos en que las pruebas convencionales sean negativas o dudosas. Esta técnica ha demostrado buenos resultados en dos metaanálisis publicados recientemente. La localización radioguiada mediante sonda gamma continúa siendo la técnica intraoperatoria de elección en los casos con glándulas paratiroides que muestran captación significativa de 99mTc-MIBI. La gammacámara portátil es una herramienta adyuvante a la sonda, que permite realizar una imagen intraoperatoria y asegurar la completa resección de los especímenes. El “free hand SPECT” ya se está aplicando en la cirugía radioguiada de paratiroides, pero aún su uso no está extendido, probablemente debido a que no se ha demostrado la superioridad de esta técnica frente al uso de la sonda y la gammacámara portátil. La radiolocalización de lesiones ocultas (ROLL) mediante inyección de 99mTc-MAA y la inyección de semillas de 125I son dos alternativas para la localización radioguiada de paratiroides 99mTc-MIBI negativas, y ya están siendo empleadas con éxito. Los trazadores híbridos, empleados ya en la cirugía radioguiada del ganglio centinela, aún no se han aplicado en el hiperparatiroidismo. Conclusiones: El PET-CT con colina es una herramienta exitosa y cada vez más utilizada en la localización pre-operatoria del HPT. Aunque la sonda gammadetectora sigue siendo de elección en la cirugía del HPT, tanto por sus buenos resultados, como por la gran experiencia acumulada y bajo coste; la adición de la gammacámara portátil podría ser de ayuda en casos concretos y para asegurar la exéresis completa de los especímenes. La inyección de semillas con 125I y de 99mTc-MAA, guiada por ecografía, es de utilidad en casos 99mTc-MIBI negativos. La “free hand SPECT” y los trazadores híbridos aún requieren más experiencia en su aplicación a la cirugía del HPT para demostrar sus posibles ventajas frente al resto de herramientas utilizadas.

El tratamiento del carcinoma diferenciado de tiroides (CDT) con radioyodo (RAI) se viene realizando durante más de 70 años. Sin embargo, no existe consenso en cuanto a indicaciones, dosis y otros aspectos relacionados con el cuidado de los pacientes. Es más, en los últimos años se ha cuestionado la necesidad de terapia adicional con RAI en los pacientes con carcinoma diferenciado de tiroides (CDT) tras la cirugía, como lo ha hecho la guía ATA 2015. Cuestionamiento que no ha sido suscrito por las sociedades Europea y Americana de Medicina Nuclear (EANM y SNMIM, respectivamente), lo que ha dado lugar a se produzcan encuentros (Encuentros de La Martinica) entre la EANM, la SNMIM y las Sociedades Americana (ATA) y Europea de Tiroides (ETA). Se han celebrado dos encuentros, en 2018 y 2019, en los que se establecieron unos principios (Principios de La Martinica). Estos principios y los resultados de las publicaciones más recientes, en cuanto a la potencialidad de la terapia con RAI en pacientes con CDT, van a ser objeto de desarrollo en este trabajo para llevar a cabo una actualización del tratamiento del CDT con RAI.

Material and methods Thirty-six cases and 140 locations were studied. PET/CT, ceCT and serum CA-125 was conducted in all cases. Nineteen cases were requested for restaging, 17 for suspected relapse. We compared ceCT and PET/CT, assessed by histopathology or radiological follow-up, calculating sensitivity (S) and positive predictive value (PPV) by cases and lesions. We evaluated the correlation between size, number, uptake of the lesions and CA-125. We conducted survival analysis, using ROC curves to calculate the optimal cut-off of SUVmax for survival prediction. We checked whether PET/CT modify the therapeutic attitude vs. conventional imaging. Results PET/CT and ceCT were concordant in 12 cases: 11 positives (30 lesions), all confirmed. There was 1 FN. In the 24 non-concordant, PET/CT was positive in 19 (97 lesions); ceCT in 21 (59 lesions); 54% of the lesions were concordant. Overall, PET/CT detected 127 lesions, with S = 97% and PPV = 100%. ceCT detected 89 lesions, with S = 61% and PPV = 90%. No significant correlation was found between CA-125 and the other parameters. PET/CT detected 10 positive cases, with normal CA-125. PET/CT modified therapeutic management in 15 cases. Significant differences were found in survival with SUVmax = 11.8 Conclusions PET/CT plays an important role in ovarian cancer relapse, with sensitivity and PPV higher than ceCT, modified therapeutic management in up to 42% of cases, and could be a valuable tool for predicting survival.

espanolObjetivo Evaluar el 18F-FDG-PET/TC en la sospecha de recidiva del cancer de ovario con pruebas de imagen negativas/no concluyentes y en la reestadificacion de recurrencia potencialmente resecable. Material y metodos Estudiamos 36 casos y 140 localizaciones. Se realizaron PET/TC, TC con contraste i.v. y CA-125 en todos los casos: reestadificacion (19), sospecha de recidiva (17). Comparamos TC y PET/TC, valorados con histopatologia y seguimiento radiologico, calculando sensibilidad y valor predictivo positivo (VPP) por casos y lesiones. Evaluamos la correlacion entre tamano, numero y captacion de las lesiones y el CA-125. Realizamos un analisis de supervivencia, utilizando curvas ROC para calcular el cut off optimo de SUVmax para prediccion de supervivencia. Comprobamos si la PET/TC modifica la actitud terapeutica vs. imagen convencional. Resultados PET/TC y TC fueron concordantes en 12 casos: 11 positivos (39 lesiones), todos confirmados. Hubo un FN. En los 24 no concordantes, PET/TC fue positivo en 19 (97 lesiones); TC en 21 (59 lesiones); 54% de las lesiones fueron concordantes. Globalmente, PET/TC detecto 127 lesiones (sensibilidad = 97% y VPP = 100%) y TC 89 (sensibilidad = 61% y VPP = 90%). No se encontro correlacion significativa entre CA-125 y los otros parametros. La PET/TC detecto 10 casos positivos, con CA-125 normal. Modifico el manejo terapeutico en 15 casos. Se encontraron diferencias significativas en supervivencia (SUVmax = 11,8). Conclusiones La PET/TC juega un importante papel en la recurrencia del cancer de ovario, con sensibilidad y VPP mayores que la TC, modifico el manejo terapeutico en un 42% de casos y podria ser una herramienta util para prediccion de supervivencia. Introducción y objetivo: Los incidentalomas parotídeos detectados mediante PET/TC con 18F-FDG de cuerpo entero se definen como focos hipermetabólicos incidentales e imprevistos presentes en las glándulas parótidas, en ausencia de lesión parotídea conocida, descubiertos al realizar una PET/TC por cualquier otra causa. EL objetivo principal de este estudio es eterminar la prevalencia de incidentalomas parotídeos en pacientes a los que se realizó una PET/TC con 18F-FDG corporal por cualquier otra causa. Método: Se realizó una búsqueda sistemática en las bases de datos PubMed, WoS y EMBASE, así como otras fuentes de datos, de artículos que incluían series retrospectivas de pacientes sometidos a PET/TC18F-FDG de cuerpo entero por otra causa y que presentaban captaciones incidentales en parótida. Se realizó estudio descriptivo de las variables recogidas de la muestra. Se realizó metanálisis para evaluar la prevalencia de captaciones parotídeas incidentales en toda la muestra. Resultados: Se seleccionaron 20 artículos que cumplían criterios de inclusión para estudio cualitativo; de los cuales se realizó estudio cuantitativo de 19. Los estudios suman un total de 151603 pacientes en los que se realizaron 154441 PET/TC 18F-FDG. En 859 pacientes (0,57% de los pacientes, IC 95% 0.53, 0.61%) hubo al menos una captación incidental en las glándulas parótidas. De los 859 pacientes iniciales, se estudiaron 537 pacientes, en los que se registró un total de 560 captaciones de 18F-FDG en las glándulas parótidas (0,36% de las exploraciones, IC 95% 0.34, 0.39%); el resto de los pacientes no pudieron ser estudiados por diferentes causas. Respecto a la distribución de las captaciones, fueron más frecuentes las captaciones únicas focales (82,7%, IC 95% 79.3, 85.6%) y las captaciones unilaterales (93,6%, IC 95% 91.2, 95.3%). La mayoría de los estudios histológicos resultaron benignos (50,4%, IC 95% 46.2, 54.5%). Según este estudio, el valor medio del SUVmax no es útil para diferenciar captaciones malignas de benignas ni de captaciones no tumorales. Según el metanálisis, la prevalencia de pacientes con al menos una captación incidental, respecto del total de exploraciones PET/TC 18F-FDG realizadas, es de 0,96% (IC 95% 0.005, 0.014%). Conclusiones: La prevalencia de incidentalomas parotídeos en pacientes en los que se realizó una PET/TC con 18F-FDG corporal por cualquier otra causa, excluyendo los tumores primarios de parótida, es de 0,96%.

El síndrome de burnout es una respuesta del estrés que se produce en el individuo por su interacción negativa con la organización, factores como la excesiva carga laboral y la pobreza del rol que cumple dentro de la misma, llevan a que el individuo trate o genere estrategias que le permitan contrarrestar el estrés que podría generarse. La presente investigación tiene como objetivo principal determinar la influencia existente entre el síndrome de burnout y la responsabilidad organizacional del personal militar de la FAE, con el fin de proponer estrategias de mejora que permitan un mejor desenvolvimiento del personal en las diferentes actividades que se realiza en la institución para así lograr mejores resultados teniendo como base fundamental un firme compromiso con la institución.Se aplicó una investigación descriptiva, aplicada, con un enfoque cuantitativo, no experimental, descriptivo, correlacional y de corte transversal. La población obtenida fue de 6720 personas y como muestra se aplicó una encuesta a 363 militares en servicio activo. Se obtuvo como resultados que el cansancio emocional tiene una relación significativa con todas las dimensiones del compromiso organizacional. Se llegó a la conclusión que el síndrome de burnout es inversamente proporcional al compromiso organizacional, lo que significa que, si los niveles de síndrome de burnout aumentan, el compromiso organizacional disminuye y en viceversa.

This study was carried out with the objective to identify the relationships between the management of human talent, emotional intelligence and the performance of teachers in a higher education institution; as well as the differences between groups, according to their sociodemographic characteristics. This research used a quantitative approach; a three-section questionnaire was used as a research instrument, the first one made it possible to evaluate the management of human talent; in the second one, the Trait Emotional Intelligence Questionnaire (TEIQue-SF) was used to measure the level of emotional intelligence; and in the third one, demographic questions were asked. For the performance of professors, the results of the professor’s evaluation were used. The sample consisted on 338 teachers (237 men, 101 women) selected at random. The results show the existence of a low and linear relationship between the management of human talent and emotional intelligence, as well as between the management of human talent and performance at work. Differences between groups were also identified in some aspects of the management of the human talent, emotional intelligence, and professors' performance with respect to demographic factors such as gender, age, level of education and teaching experience.

El financiamiento crediticio es un factor fundamental para emprender cualquier actividad económica, Ecuador es considerado uno de los países con más microemprendiemientos a nivel de América Latina en donde más del 80% de empresas constituidas pertenecen a este sector, por su parte en el Cantón Mejía de todos los negocios censados el 91% son microempresas. Sin embargo, las condiciones de financiamiento que se aplica no son las más favorables del mercado, una de sus principales alternativas es el microcrédito cuyas características son elevadas tasas de interés y periodos de corto plazo. El desarrollo de la investigación se basa en el objetivo de analizar si una determinada alternativa de financiamiento influye de manera significativa en el desarrollo económico de la microempresa del sector comercio al por menor del Cantón Mejía. La presente investigación se desarrolló describiendo la situación actual del microempresario; fundamentos teóricos, de referencia, conceptuales; una recolección de datos mediante una técnica documental y de campo; para el tratamiento de información se trabajó con el software estadístico SPSS Statistics 25, se utilizó la estadística inferencial para probar la hipótesis a través de análisis bivariados con pruebas Chi cuadrado y una prueba de correlación de Spearman para medir la relación entre las variables; finalmente se elaboró como propuesta de mejora una guía de financiamiento destinada al microempresario para brindarle información que le permita tomar mejores decisiones en materia financiera.

Cystic echinococcosis is a zoonotic disease caused by the cestode Echinococcus granulosus. The activity of the cysts is assessed through the WHO-IWGE standardized classification based on ultrasound features. However, viability of the cysts is not always concordant with the activity assessed by ultrasound. The aim of the present study is to describe the metabolic activity of cysts in patients with cystic echinococcosis through FDG-PET.Prospective observational study where adult patients diagnosed of cystic echinococcosis were offered to undergo FDG PET/CT before treatment onset. Demographic, clinical, radiological, and histopathological information was collected from all patients.Sixteen patients were included, 50% were male, and age ranged from 18 to 85 years. Most of the patients had liver involvement, and all patients had CE3, CE4 or CE5 stage of the WHO-IWGE classification. Only one patient (CE5) had an increasedIn our pilot study, we did not find the correlation between the FDG PET/CT imaging and the cystic echinococcosis cyst bioactivity.

[EN]Introduction and objective: The aggressive histological subtypes of thyroid cancer are less common and have a worse prognosis than well –differentiated ones (DTC). The most important aggressive subtypes are Hürtle cell carcinoma (CHH), anaplasic and poorly differentiated carcinoma. In this review, we are going to evaluate the role of PET-CT, especially with 18F-FDG, in these aggressive histological lines, as well as in aggressive subtypes of DTC (tall cells, diffuse sclerosing…) and in radioiodine-refractory differentiated thyroid carcinoma. Synthesis: Although the main indication for 18F-FDG PET in thyroid cancer is suspected recurrence, in patients with DTC with elevated serum thyroglobulin and radioiodine-negative whole-body scan (WBS), this technique has increasing applications in DTC. Likewise, 18F-FDG PET is also an efficient diagnostic tool in the study of anaplastic carcinoma, poorly differentiated and other aggressive lines It is recommended, in current clinical guidelines, as part of the initial staging in anaplastic, poorly differentiated, and invasive HCC. And it is increasingly used in other indications such as prognostic assessment, evaluation of treatment response, and even during the follow-up of high-risk patients. The Hodgkin lymphoma (HL) genomic landscape is hardly known due to the scarcity of tumour cells in the tissue. Liquid biopsy employing circulating tumour DNA (ctDNA) can emerge as an alternative tool for non-invasive genotyping. By using a custom next generation sequencing (NGS) panel in combination with unique molecule identifiers, we aimed to identify somatic variants in the ctDNA of 60 HL at diagnosis. A total of 277 variants were detected in 36 of the 49 samples (73·5%) with a good quality ctDNA sample. The median number of variants detected per patient was five (range 1–23) with a median variant allele frequency of 4·2% (0·84–28%). Genotyping revealed somatic variants in the following genes: SOCS1 (28%), IGLL5 (26%), TNFAIP3 (23%), GNA13 (23%), STAT6 (21%) and B2M (19%). Moreover, several poor prognosis features (high LDH, low serum albumin, B-symptoms, IPI ≥ 3 or at an advanced stage) were related to significantly higher amounts of ctDNA. Variant detection in ctDNA by NGS is a feasible approach to depict the genetic features of HL patients at diagnosis. En la actualidad, en el Ecuador existen muchas cooperativas de ahorro y crédito, las mismas que han ido suplantando a las entidades bancarias, por cuanto estas llegan a muchos más sectores de la población, ofreciendo financiamiento confiable y efectivo. Las cooperativas han sido divididas por la Superintendencia de Economía Popular y Solidaria en segmentos, caracterizadas de acuerdo al valor de los activos que refleje la empresa y sus posibles costos de agencia. El objetivo de la presente investigación es analizar los procesos de crédito y su impacto en las instituciones financieras tales como las COAC. La investigación que se aplicó fue descriptiva, de campo, documental, exploratoria con un diseño no experimental, a través de las cuales se analizaron los procesos crediticios y su incidencia en los costos de agencia en las COAC en el segmento dos de la provincia de Pichincha, cantón Quito. Se realizó una encuesta a directivos y empleados de los departamentos de crédito de seis Cooperativas de Ahorro y Crédito. Dando como resultado que las cooperativas evalúan con frecuencia el riesgo de crédito y los costos de agencia, lo cual es importante porque permite medir el comportamiento financiero de las entidades, determinando factores negativos que pudieran ser corregidos en su momento. Se llegó a la conclusión que se debe contar con un equipo de trabajo altamente calificado ya que el ineficiente manejo en el riesgo crediticio afecta significativamente la rotación de los activos y por consiguiente disminución de la rentabilidad.

SummaryIntroductionThe location of the pathological parathyroid glands in hyperparathyroidism is usually carried out by means of 99mTc-sestamibi scintigraphy, which increases its precision by adding the ultrasound examination. The non-localization of the parathyroid glands increases the difficulties for surgical removal. To increase the detection of pathological glands, other radioactive tracers are used, such as methionine, fluorocholine or 18F-flurpiridaz.ObjectiveTo establish if PET / CT with 18-Fluorocholine in patients with hyperparathyroidism increases the number of uptakes compared to the 99mTc-sestamibi scan.MethodSystematic review and meta-analysis. Two subgroups were analyzed. Subgroup 1: trials comparing both techniques as an initial exploration. Thirteen studies including 1131 examinations were selected (596 PET / CT with 18-Fluorocholine vs. 535 scintigraphy with 99mTc-sestamibi). Meta-analysis was performed following the random effects model and the odds ratio was calculated. Subgroup 2: studies that include 18-Fluorocholine as a rescue examination in patients with a previous negative study with a 99mTc-sestamibi scan. 17 articles including 412 examinations with 359 patients in which there was at least one uptake were selected. Meta-analysis of the prevalence of the number of patients in whom there was at least one uptake was performed using the random effects model.ResultsSubgroup 1: The number of patients in which at least one uptake occurs is significantly higher with the 18-Fluorocholine examinations (OR 4.264, 95% CI 2.400-7.577). Subgroup 2: the prevalence of uptake among patients with previous negative MIBI studies was 0.90 [0.87, 0.94]. The probability of detection of both techniques in this group reaches 0.98. Publication bias in the meta-analyzes is low.Discussion18-Fluorocholine protocols provide higher precision, clearer images, with faster acquisition as well as being readily available for most PET / CT centers.ConclusionThe PET / CT study with 18-Fluorocholine can be recommended as a study for the location of pathological parathyroid glands after studies with negative MIBI.

Introduction and objective: Nuclear Medicine has a relevant role in the study of thyroid and parathyroid gland pathology. The aim of this work is to review the imaging techniques available nowadays in Nuclear Medicine, to study this pathology. Synthesis: Thyroid scinthigraphy is, even today, a useful tool in the study of thyroid pathology, such us in the thyroid nodule or in congenital hypothyroidism. SPECT-CT, compared to whole body scintigraphy (WBS) with radioiodine, improve the diagnosis and change the therapeutic management in patients with differentiated thyroid cancer, at the same time that, added to the parathyroid scintigraphy, it helps planning the surgery, especially in ectopic adenomas. PET-CT with FDG and other tracers has a relevant role in several indications and histology types of thyroid cancer, although mainly in the suspicion of recurrence. In addition, Fluorocholine PET-CT is an excellent alternative, with high successful localization rate, in cases with primary hyperparathyroidism (pHPT) and negative conventional techniques. Conclusions: With technical advances in the area of Nuclear Medicine, and particularly with hybrid imaging, the study of thyroid pathology has improved, especially in thyroid cancer, as well as pre-surgical localization of pHPT.

RESUMEN: Introducción y objetivos: La PET/TC es una técnica cuyas indicaciones en Oncología se encuentran en expansión y el cáncer de tiroides es una de ellas. Método: Revisión narrativa. Comentarios: En el presente artículo se revisan las indicaciones establecidas hasta la fecha, así como aquellos campos en los que son necesarias investigaciones adicionales que permitan establecer la utilidad de esta técnica en los distintos tipos de patología maligna tiroidea.

El tratamiento de la patología con radioyodo (RAI) se viene realizando desde hace más de 7 décadas. Sin embargo, no existe consenso en cuanto a indicaciones, dosis y otros aspectos relacionados con el cuidado de los pacientes. La razón de ello es la ausencia de ensayos clínicos prospectivos bien diseñados para resolver estos interrogantes en cuanto al tratamiento con 131I, a pesar de la alta prevalencia de las enfermedades tiroideas. El tratamiento con 131I está indicado en el tratamiento del hipertiroidismo, producido por la enfermedad de Graves, por el adenoma tóxico y por el bocio multinodular tóxico; del bocio multinodular no tóxico y del carcinoma diferenciado de tiroides (CDT). El objetivo del tratamiento con RAI en caso del hipertiroidismo y del bocio multinodular no tóxico es disminuir la función tiroidea y/o disminuir el volumen de la glándula tiroides. En el caso del CDT, los objetivos de la administración de RAI tras la cirugía son la ablación de los restos tiroideos, el tratamiento adyuvante de la enfermedad microscópica sospechada no confirmada y el tratamiento de la enfermedad persistente loco-regional o metastásica. A la espera de los resultados de ensayos clínicos actualmente en: marcha, el tratamiento con 131I está justificado no solo en los pacientes de alto riesgo, sino también en los pacientes de riesgo bajo (T > 1 cm) e intermedio.

This article belongs to the Special Issue Personalized Medicine: Recent Progress in Cancer Therapy., The biology and clinical impact of bone marrow (BM) infiltration in patients with diffuse large B-cell lymphoma (DLBCL) remains unclear in the rituximab era. We retrospectively analyzed 232 patients diagnosed with DLBCL at our center between 1999 and 2014. Concordant-presence of large cells similar to those of the lymph node biopsy- and discordant-infiltration by small cells forming lymphoid aggregates, lacking cytological atypia-BM infiltration was defined by histological criteria and further characterized by flow cytometry (FCM). Cell of origin (COO) was determined using Hans’ algorithm. For the clonal relationship between tumor and discordant BM, the VDJH rearrangement was analyzed. Survival analyses were restricted to 189 patients treated with rituximab and chemotherapy. Thirty-six (16%) had concordant, and 37 (16%) discordant BM infiltration. FCM described different indolent lymphomas among discordant cases, clonally related with DLBCL in 10/13 available samples. Median follow-up was 58 months. 5-year-progression-free survival (PFS) for non-infiltrated, discordant and concordant groups was 68%, 65% and 30%, respectively (p < 0.001). Combining COO and BM infiltration, patients with discordant BM and non-germinal center B-cell COO also had decreased 5-year-PFS (41.9%). In multivariate analysis, concordant BM had an independent effect on PFS (HR 2.5, p = 0.01). Five-year cumulative incidence of central nervous system (CNS) relapse was 21%, 4% and 1% in concordant, discordant and non-infiltrated groups, respectively (p < 0.001). RESUMEN: Introducción y objetivo: La Medicina Nuclear tiene un papel destacado en el estudio de la patología de tiroides y paratiroides. El objetivo de este trabajo es realizar una revisión de las técnicas de imagen disponibles actualmente en Medicina Nuclear, para el estudio de dicha patología. Síntesis: La gammagrafía de tiroides sigue siendo una técnica útil en el estudio de la patología tiroidea, como, por ejemplo, en el nódulo tiroideo o el hipotiroidismo congénito. El SPECT-CT mejora el diagnóstico, frente al rastreo corporal total (RCT) con radioyodo, en pacientes con cáncer diferenciado de tiroides y cambia el manejo terapéutico, a la vez que, añadido a la gammagrafía de paratiroides, ayuda a la planificación de la cirugía, sobre todo en adenomas ectópicos. El PET-CT con 18F-FDG y con otros trazadores, tiene un papel relevante en varias indicaciones y tipos histológicos de cáncer de tiroides, aunque principalmente en la sospecha de recidiva. Así mismo, el PET-CT con fluorocolina es una alternativa excelente, con cifras elevadas de localización exitosa, en los casos de hiperparatiroidismo primario (HPTp) con pruebas convencionales negativas. Conclusiones: Con los avances técnicos de los últimos años en el ámbito de la Medicina Nuclear y, particularmente, con la imagen híbrida, se ha mejorado el estudio de la patología tiroidea, fundamentalmente del cáncer de tiroides, así como la localización prequirúrgica del HPTp.

El sector agrícola es sustancial para el desarrollo de la producción nacional en el Ecuador, es así como el acceso a los microcréditos que ofrecen las diferentes entidades financieras locales les permite a estos pequeños productores conseguir recursos necesarios para realizar inversiones en sus cultivos y en diferentes actividades económicas que realicen. Sin embargo, el acceso a estos microcréditos en su mayoría no refleja el mejoramiento de la producción agrícola rural doméstica, ya sea por el mal uso o el desconocimiento total del crédito, influyendo en su calidad de vida y creando a las cooperativas que otorgaron estos créditos una cartera vencida alta. Esta investigación tiene como objetivo analizar la aplicación de los microcréditos otorgados por las entidades financieras adscritas a la Cooperativa de Ahorro y Crédito Rural Sierra Norte en el sector agrícola rural. La metodología aplicada fue de carácter cuantitativo, exploratoria, no experimental, dando como resultado que el otorgamiento de crédito a los socios no es analizado con profundidad, pues no ofrecen charlas, talleres o capacitaciones que les permita influir en el socio para que cancelen dicho microcrédito. Las conclusiones de la presente investigación son que la cooperativa carece de estrategias que lleven a los empleados a persistir al socio deudor que cancele los valores pendientes, debido a que es los asesores de crédito y cobro mantienen relaciones familiares o amistosas con los socios deudores, y confían más de su palabra, que del perfil de crediticio del deudor.

La Gestión de Riesgo Empresarial, conocida como Enterprise Risk Management (ERM) se ha relacionado con la performance y el riesgo que asumen las empresas. Las compañías que usan ERM deben mejorar la comprensión de los riesgos a nivel agregado, mejorando la eficiencia del capital invertido, la rentabilidad, la mejora de la comunicación interna y de la reducción de las asimetrías informativas. El objetivo ha sido mostrar las características administrativas y el nivel de riesgo en las organizaciones. Además, estudiar el mejoramiento de dicha gestión sobre el rendimiento de las empresas y su impacto sobre la estabilidad financiera. Se ha utilizado el método hipotético-deductivo, con enfoque analítico, fundamentadas a partir de diferentes planteamientos teóricos y estudios empíricos previos. Los resultados obtenidos constatan que el riesgo al que se exponen las empresas depende de forma relevante de las decisiones que toman las personas implicadas en la gestión. Las conclusiones que se obtuvieron fueron que las empresas familiares se interesan en menor medida por implementar ERM salvo cuando los accionistas disponen de un mayor control de la empresa, en cuyo caso estarán más motivados a implementar sistemas de gestión del riesgo.

La presente investigación presenta un análisis del complemento al estudio del diagnóstico de laactividad inmobiliaria en la ciudad de Quito, habiendo analizado varios parámetros que fueron devital importancia para estructurar una propuesta metodológica para planificar y gerenciar proyectosinmobiliarios, analizamos a los proyectos en sus 5 estructuras: técnico, administrativo, legal, comercial,y financiero.Cada una de las estructuras tiene su particularidad y es necesario mitigar cualquier tipo de riesgoinherente a la actividad inmobiliaria, que el proyecto se desarrolle con la menorprobabilidad de un fracaso, es así que se logró establecer parámetros de medición bajo los cuales sepueden considerar equilibrios que un proyecto inmobiliario debe cumplir.Finalmente se realizó un análisis financiero integral, la composición de las estructuras de costos de unproyecto y el análisis de sus fuentes de financiamiento, se planteó un caso de estudio para analizar elcomportamiento de un desarrollo inmobiliario con datos reales bajo diferentes escenarios que son losque se aplican en el día a día y que se dan en el sector de la construcción, todos estos análisis fuerondesarrollados no como un análisis de teoría, al contrario se aplicó la teoría en casos de la vida real, coneste análisis tenemos la capacidad de tomar la mejor decisión al momento de evaluar un proyecto y unaguía muy práctica para aquellos inversionistas que desean invertir en la construcción.

RESUMEN Introducción: la bronquiolitis es la infección respiratoria baja más común en los infantes. Objetivo: determinar los costos hospitalarios en pacientes con bronquiolitis ingresados en el Hospital Pediátrico Docente Gral. Luis A. Milanés Tamayo Método: se realizó un estudio descriptivo, retrospectivo, transversal, de costos hospitalarios, en 289 pacientes ingresados con diagnóstico de bronquiolitis, período julio - octubre del 2015 en el Hospital Pediátrico Docente Luis A. Milanés Tamayo de Bayamo. Los datos recogidos se llevaron a base de datos, se analizaron a través de estadísticos descriptivos, con empleo de frecuencias absolutas y relativas, programa SPSS 22 para Windows. Resultados: predominaron los menores de un año (98.2 %); el 4,8 % ingresó directamente en UCIP, el 95,2% ingresó en sala de respiratorio, de ellos el 82.4 % permaneció en el servicio de respiratorio y el 12, 8% de los pacientes se trasladó posteriormente a la unidad de cuidados intensivos pediátricos (UCIP). Todos los pacientes ingresados en servicio de respiratorio recibieron inicialmente tratamiento sintomático, este y la oxigenoterapia precoz fueron los más utilizados. El costo total por medicamentos, ascendió a $826.00, el sulfato de magnesio, salbutamol solución y la vancomicina fueron los que más incidieron. Los rayos X de tórax y los estudios microbiológicos fueron los complementarios que más costos aportaron. Conclusiones: el costo final por hospitalización fue bajo, la mayoría de los pacientes tuvieron una estadía hospitalaria menor de 72 horas, y todos los pacientes evolucionaron satisfactoriamente. ABSTRACT Introduction: bronchiolitis is the most common lower respiratory infection in Infants. Objective: to determine hospital costs in patients with bronchiolitis admitted to the Pediatric Teaching Hospital Luis A. Milanés Tamayo Method: a descriptive, retrospective and cross-sectional study of 289 patients admitted with the diagnosis of acute bronchiolitis in the period from July to October of 2015 at the Pediatric General Teaching Hospital Luis A. Milanés Tamayo of Bayamo, was carried out. Collected data were entered into a database, and analyzed using SPSS 22 statistical software for Windows. Relative and absolute frequencies were used. Results: the age group of children under one-year-old predominated with 98.2 %. 4, 8% were admitted directly to PICU. 95.2 % of patients were admitted to respiratory service, 82, 4% of them, remained at this service, and 12, 8% were later transferred to the pediatric intensive care unit (PICU). All patients received symptomatic treatment and early oxygen therapy on admission, those were the most used therapies. The overall cost for drugs reached $ 826.00; magnesium sulfate, salbutamol solution and vancomycin were the medication that affected most. Chest X-rays and microbiological studies were among the most cost-effective adjuncts. Conclusions: the final cost for hospitalization was low, most of the patients Were admitted for less than 72 hours, and all patients had a favorable outcome.

The aim of this work was to review the current recommendations for staging and response assessment of patients with Hodgkin lymphoma (HL) and non-Hodgkin lymphoma (NHL) in routine clinical practice after chemotherapy and/or stem cell transplantation. A five-point scale (5-PS) from the First International Workshop on PET in Lymphoma in Deauville, France, in 2009, was recommended as the standard tool to score imaging to assess treatment response in patients with lymphoma using 18F-Fluorodeoxyglucose (FDG) PET/CT. Following the recommendations of the 11th and 12th International Conferences on Malignant Lymphoma held in Lugano (Switzerland), in 2011 and 2013, respectively, a consensus (the so-called Lugano Classification) was reached regarding the use of PET/CT for staging and response assessment in FDG-avid lymphomas. As a result, 18F-FDG PET/CT was formally incorporated into standard staging for FDG-avid lymphomas. A bone marrow biopsy is no longer indicated for the routine staging of HL and most diffuse large B-cell lymphomas. PET/CT will be used to assess response in FDG-avid histologies using the 5-point scale. The recent introduction of biological agents with immune mechanisms requires flexibility in interpretations of the Lugano criteria due to tumour flare or a pseudo-progression effect produced by these agents. Provisional criteria have been proposed (Lymphoma Response to Immunomodulatory Therapy Criteria) with the introduction of the term ‘Indeterminate Response’ in order to identify this phenomenon until confirmed as flare/pseudoprogression or true progression. All these recommendations will improve evaluations of patients with lymphoma, and allow comparison of results from clinical practice and trials.

Introduction: Mediastinal paragangliomas are low incidence tumors that arise from neural crest. Its diferential diagnosis include several malignant diseases, so its correct characterization is mandatory for an adecuate therapeutic management. 111In-Pentetreotide scintigraphy/SPECT-CT can detect paraganglioma, mainly in those cases of atypical or unsuspected location, and may have a significant role in follow-up of those patients diagnosed with multifocal and familiar paraganglioma. Case: Here we report a case of an extremely rare middle mediastinal paraganglioma, detected by scintigraphy and SPECT-CT with 111In-Pentetreotide in a patient diagnosed of multifocal and family history of paraganglioma. Surgery was carried out by median sternotomy and extracorporeal circulation. The pathological examination of the surgical specimen showed a para-aortic low-grade tumor, positive for chromogranin and synaptophysin, cytokeratin AE1-AE3 negative and Ki67 lower than 5%, compatible with paraganglioma. Currently, the patient is tumor free, under clinical monitoring. Conclusions: 111In- Pentetreotide scan proved to be a helpful diagnostic method because of its potential to explore full body, so It allows us to locate unsuspected and atypical location paragangliomas. This finding suggests that patients diagnosed with multifocal head and neck paraganglioma should undergo periodical follow-up with 111In-Pentetreotide scan to detect unsuspected paraganglioma.

Context: Outcomes of patients with transformed lymphoma have historically been poor, prompting many centres to include stem-cell transplant (SCT) as part of the treatment strategy. However, several studies in the rituximab era suggest that survival may be more favourable than previously recognized, questioning the need for such intensive therapies. Objective: We analyzed the efficacy of SCT in transformed lymphomas at a single reference Transplant Unit. Design: We collected data from 51 patients; the indication was a diagnosis of diffuse large B-cell lymphoma (DLBCL) transformed from an indolent lymphoma, from 1995 to 2018. Of them, 36 received an auto-SCT and 15 an allo-SCT. Results: Median age was 60 (range, 40-69) and 52 (range, 35-65) years for patients receiving auto-SCT and allo-SCT, respectively. Patients in the allo-SCT group had been more intensively pretreated, as 13 (87%) patients received >3 lines of treatment before SCT and 13 (87%) underwent a previous auto-SCT, whereas patients in the auto-SCT group were pretreated with a median of 2 (range, 1 to 4) lines of treatment. Regarding disease status at transplant, most patients (78% and 73% in the auto and allo-SCT groups, respectively) were transplanted in complete remission (CR). All auto-SCT patients received BEAM as conditioning regimen, whereas patients in the allo-SCT group received several fludarabine-based combinations. Transplant-related mortality (TRM) was 3.2% and 27% in the auto and allo-SCT groups, respectively. With a median follow-up of 61 and 50 months,4-year overall survival (OS) and progression-free-survival (PFS) were 87% and 76%, for the auto-SCT patients, and 52% and 56%, respectively, for the allo-SCT patients. In the auto-SCT group, 12 (33%) patients experienced disease progression and 7 (19.5%) died (5 [13.9%] from lymphoma), whereas in the allo-SCT group, only 2 (13%) patients had disease progression but 4 (27%) died due to transplant-related causes. Conclusions: SCT is a potentially curative option for patients with transformed lymphoma, since more than a half of patients are long-term survivors. Progression is the major cause of failure in auto-SCT whereas allo-SCT patients died because of toxicity. Efforts need to be made to identify patients who do not benefit from auto-SCT to avoid toxicity.

© The Author(s) 2019., Follicular lymphoma (FL) is a heterogeneous disease whose pathogenesis remains partially unknown. Around 20% of FL patients experience early progression or treatment-refractory disease and 2–3% of patients per year experience histological transformation (HT) into a more aggressive lymphoma (tFL). Here, we evaluate the immunoglobulin heavy chain variable (IGHV) gene usage and mutational status in 187 FL cases to assess its impact on clinical outcome and histological transformation. The IGHV gene repertoire was remarkably biased in FL. The IGHV4-34 (14%), IGHV3-23 (14%), IGHV3-48 (10%), IGHV3-30 (9%) and IGHV3-21 (7%) genes accounted for more than half of the whole cohort. IGHV3-48 was overrepresented in cases of tFL (19%) compared with non-transformed FL at 5 years (5%, P = 0.05).
