Introduction Internet-based self-help interventions have the potential to help people address their emotional needs at relatively low costs. However, if the system does not offer optimal functions, it could reduce end-user adherence and satisfaction with treatment and compromise the effectiveness of the program. This study evaluated the usability of an Internet-based self-help intervention for emotional self-management among the general population of Italy during the COVID-19 pandemic. Methods A balanced sex-age sample of 10 individuals who met the inclusion criteria were consecutively recruited online. The think-aloud testing method, the system usability scale and an ad hoc semi-structured interview were used to determine the overall system usability. Quantitative data were analyzed using descriptive statistics and qualitative data were analyzed using thematic analysis. However, there are several issues which need to be resolved before translation of zinc oxide nanoparticles into medical use, including lack of suitable biocompatible dispersion protocols and a better understanding being needed of the mechanism of their selective cytotoxic action. Methods: Nanoparticle dose affecting cell viability was evaluated in a model of proliferating cells both experimentally and mathematically. The key issue of selective toxicity of ZnO NPs toward proliferating cells was addressed by experiments using a biological model of noncancerous cells, ie, mesenchymal stem cells before and after cell differentiation to the osteogenic lineage.Results: In this paper, we report a biocompatible protocol for preparation of stable aqueous solutions of monodispersed zinc oxide nanoparticles. We found that the threshold of intracellular ZnO NP concentration required to induce cell death in proliferating cells is 0.4 ± 0.02 mM. Finally, flow cytometry analysis revealed that the threshold dose of zinc oxide nanoparticles was lethal to proliferating pluripotent mesenchymal stem cells but exhibited negligible cytotoxic effects to osteogenically differentiated mesenchymal stem cells.Conclusion: Results confirm the ZnO NP selective cytotoxic action on rapidly proliferating cells, whether benign or malignant.Keywords: zinc oxide nanoparticles, dispersion, selective cytotoxicity, mesenchymal stem cells

Objectives The main objective of the study is to investigate the short-term efficacy of Acceptance and Commitment Therapy (ACT) on the simultaneous modification of biological indicators of risk and psychological well-being in patients with coronary heart disease attending cardiac rehabilitation (CR).Design This was a two-arm randomised controlled trial comparing a brief, manualised, ACT-based intervention with usual care (UC).Setting The study was conducted in an outpatient CR unit in Italy. Data collection took place from January 2016 to July 2017.Participants Ninety-two patients were enrolled and randomised, following an unbalanced randomisation ratio of 2:1 to the ACT group (n=59) and the control group (n=33). Eighty-five patients completed the ACT (n=54) and the UC (n=31) interventions and were analysed.Interventions The control group received UC, a 6 weeks multidisciplinary outpatient CR programme, encompassing exercise training, educational counselling and medical examinations. The experimental group, in addition to UC, participated in the Acceptance and Commitment Therapy on HEART disease (ACTonHEART) intervention encompassing three group sessions based on ACT.Outcomes The primary outcomes were Low Density Lipoproteins (LDL)cholesterol, resting systolic blood pressure, body mass index (BMI) and psychological well-being measured by the Psychological General Well-Being Index (PGWBI). Outcome measures were assessed at baseline and at the end of CR.Results Based on linear mixed models, no significant group × time interaction was observed for either the primary outcomes (β, 95% CI: PGWBI =−1.13, –6.40 to –4.14; LDL cholesterol =−2.13, –11.02 to –6.76; systolic blood pressure =−0.50, –10.76 to –9.76; diastolic blood pressure =−2.73, –10.12 to –4.65; BMI =−0.16, –1.83 to –1.51, all p values >0.05) or the secondary outcomes (all p values >0.05). A significant time effect was found for the PGWBI total (beta=4.72; p=0.03).Conclusions Although analyses revealed null findings, the results can inform the design of future ACT-based CR interventions and can help researchers to strike a balance between the idealised implementation of an ACT intervention and the structural limitations of existing CR programmes.Trial registration number NCT01909102.

Abstract Background The Three Factor Eating Questionnaire-Revised 18 (TFEQ-R-18) is an extensively used questionnaire to measure three transdiagnostic features of eating behavior: cognitive restraint, uncontrolled eating, and emotional eating. Objective This research aims to investigate the psychometric properties of the Italian version of the TFEQ-R-18 in three large community samples. Method Cross-sectional research designs were employed. In Study 1 (N = 537), an exploratory graph analysis (EGA) was used to examine item clustering within the TFEQ-R-18. In Study 2 (N = 645), a confirmatory factor analysis (CFA) was conducted to test its structural validity. In Study 3 (N = 346), a MANOVA was employed assessing mean differences across eating disorders (e.g., anorexia nervosa, bulimia nervosa, binge eating disorder). Results In Study 1, the EGA accurately identified the three original dimensions of the TFEQ-R-18. Study 2 showed that the Italian TFEQ-R-18 has good fit indexes (CFI = 0.989, RMSEA = 0.064; 90% CI [0.058, 0.070], SRMR = 0.062), and possesses robust psychometric properties. Study 3 reveals distinct, statistically significant differences among eating disorders. Conclusion The TFEQ-R-18 proves to be a concise and precise tool for measuring transdiagnostic eating behaviors. Its applicability in the Italian context, supported by robust psychometric properties, suggests its utility for both research and clinical purposes. The findings affirm its potential to inform interventions aimed at enhancing psychological health. Level of evidence Level V, descriptive study.

The evaluation of time and frequency domain measures of coupling and causality relies on the parametric representation of linear multivariate processes. The study of temporal dependencies among time series is based on the identification of a Vector Autoregressive model. This procedure is pursued through the definition of a regression problem solved by means of Ordinary Least Squares (OLS) estimator. However, its accuracy is strongly influenced by the lack of data points and a stable solution is not always guaranteed. To overcome this issue, it is possible to use penalized regression techniques. The aim of this work is to compare the behavior of OLS with different penalized regression methods used for a connectivity analysis in different experimental conditions. Bias, accuracy in the reconstruction of network structure and computational time were used for this purpose. Different penalized regressions were tested by means of simulated data implementing different ground-truth networks under different amounts of data samples available. Then, the approaches were applied to real electroencephalographic signals (EEG) recorded from a healthy volunteer performing a motor imagery task. Penalized regressions outperform OLS in simulation settings when few data samples are available. The application on real EEG data showed how it is possible to use features extracted from brain networks for discriminating between two tasks even in conditions of data paucity. Penalized regression techniques can be used for brain connectivity estimation and can be exploited for the computation of all the connectivity estimators based on linearity assumption overcoming the limitations imposed by the classical OLS.

As the worldwide population gets increasingly aged, in-home telemedicine and mobile-health solutions represent promising services to promote active and independent aging and to contribute to a paradigm shift towards patient-centric healthcare. In this work, we present ACTA (Advanced Cognitive Training for Aging), a prototype mobile-health solution to provide advanced cognitive training for senior citizens with mild cognitive impairments. We disclose here the conceptualization of ACTA as the integration of two promising rehabilitation strategies: the "Nudge theory", from the cognitive domain, and the neurofeedback, from the neuroscience domain. Moreover, in ACTA we exploit the most advanced machine learning techniques to deliver customized and fully adaptive support to the elderly, while training in an ecological environment. ACTA represents the next-step beyond SENIOR, an earlier mobile-health project for cognitive training based on Nudge theory, currently ongoing in Lombardy Region. Beyond SENIOR, ACTA represents a highly-usable, accessible, low-cost, new-generation mobile-health solution to promote independent aging and effective motor-cognitive training support, while empowering the elderly in their own aging.

27. Usability study of SOSteniamoci: An internet-based intervention platform to support informal caregivers in Italy

Background Providing informal care can be experienced as stressful and lead to caregiver burden. Internet-based interventions, a specific form of eHealth, have proven to be a good option to support informal caregivers. SOSteniamoci, an internet-based intervention already tested in Lithuania, was translated and adapted for Italian caregivers. Objective As many novel eHealth solutions have been rejected by end-users due to usability problems, we aimed to evaluate the usability of the adapted platform, using a computer-based prototype. Methods The following methods and metrics were applied: 1. task analysis, using audio and video recordings that included three usability metrics: task completion rate, frequency of errors, and frequency of help requests; 2. the system usability scale (SUS); and 3. a semi-structured interview to collect additional data about the system's design and overall satisfaction. Results Ten informal caregivers (60% female; age M = 47.8, SD = 15.21) provided insights and suggestions for increasing the usability of the platform. The platform was considered satisfactory, with a mean score on the SUS of 75 (SD = 13.07) out of 100. The task analysis measurements highlighted difficulties in how to log in to the platform, understanding what the intervention is about, and texting the therapist. The same difficulties were also mentioned during the post-experience interview. Thus, improvements were subsequently made to enhance users’ experience when navigating the platform. Finally, the platform overall was found to be intuitive and friendly, and the contents were appreciated. Conclusion To maintain participants’ engagement and prevent drop-out, it is crucial to test the usability of internet-based interventions. This paper proposes a lexicographic Deep Reinforcement Learning (DeepRL)-based approach to chance-constrained Markov Decision Processes, in which the controller seeks to ensure that the probability of satisfying the constraint is above a given threshold. Standard DeepRL approaches require i) the constraints to be included as additional weighted terms in the cost function, in a multi-objective fashion, and ii) the tuning of the introduced weights during the training phase of the Deep Neural Network (DNN) according to the probability thresholds. The proposed approach, instead, requires to separately train one constraint-free DNN and one DNN associated to each constraint and then, at each time-step, to select which DNN to use depending on the system observed state. The presented solution does not require any hyper-parameter tuning besides the standard DNN ones, even if the probability thresholds changes. 6G will exploit satellite, aerial and terrestrial platforms jointly to improve radio access capability and to unlock the support of on-demand edge cloud services in the three dimensional space (3D) by incorporating Mobile Edge Computing (MEC) functionalities on aerial platforms and low orbit satellites. This will extend the MEC support to devices and network elements in the sky and will forge a space borne MEC enabling intelligent personalized and distributed on demand services. 3D end users will experience the impression of being surrounded by a distributed computer fulfilling their requests in apparently zero latency. In this paper, we consider an architecture providing communication, computation, and caching (C3) services on demand, anytime and everywhere in 3D space, building on the integration of conventional ground (terrestrial) base stations and flying (non-terrestrial) nodes. Given the complexity of the overall network, the C3 resources and the management of the aerial devices need to be jointly orchestrated via AI-based algorithms, exploiting virtualized networks functions dynamically deployed in a distributed manner across terrestrial and non-terrestrial nodes.

Despite the increasing importance of innovative medications and bariatric surgery for the treatment of obesity, lifestyle interventions (diet and physical activity) remain the first-line therapy for this disease. The use of digital devices in healthcare aims to respond to the patient's needs, in order to make obesity treatment more accessible, so our study aims to assess the safety and efficacy of a Digital Therapy for Obesity App (DTxO) for achieving weight loss and its maintenance in patients affected with obesity undergoing an experimental non-pharmacological treatment. Here we present the study protocol of a prospective, multicenter, pragmatic, randomized, double-arm, placebo-controlled, parallel, single-blind study on obese patients who will be treated with a new digital therapy to obtain an improvement in their disease condition through the application of different simultaneous strategies (a dietary regimen and personalized advice program, a tailored physical exercise program, a cognitive–behavioural assessment and program, alerts and reminders, dedicated section on prescribed drugs intake, and chat and online visits with clinical professionals). We believe that DTxO will offer a promising intervention channel and self-regulation tool holding the potentiality to decrease treatment burden and treat more patients thanks to the partial replacement of traditional medical consultation with digital or telephone management, improving self- engagement and reducing the high demands the “obesity pandemic” for both patients and national health services in terms of time, cost, and effort. ObjectiveThis scoping review aims to provide an accessible summary of available evidence on the efficacy of psychological couple-based interventions among patients with heart disease and their partners focusing on specific aspects and strategies by assessing different emotional and physical cardiac-related outcome measures.MethodsA literature search was performed in PubMed, Scopus, Medline, PsycINFO, and Web of Science databases using the keywords “heart diseases” and “couple-based intervention.” A literature search using systematic methods was applied. Data were extracted to address the review aims and were presented as a narrative synthesis.ResultsThe database search produced 11 studies. Psychological couple-based interventions varied in terms of the type of intervention, personnel, format (group or individual, phone or in person), number of sessions, and duration. Most of the contributions also lacked adequate details on the training of professionals, the contents of the interventions, and the theoretical models on which they were based. Finally, although partners were involved in all the treatment, in most studies, the psychological strategies and outcomes were focused on the patient.ConclusionThe variability of the psychological couple-based interventions of included studies represents a challenge in summarizing the existing literature. Regarding their impact, psychological interventions for patients with cardiovascular disease and their partners were found to moderately improve patients’ and partners’ outcomes.

This paper presents a Federated Learning (FL) algorithm that allows the decentralization of all FL solutions that employ a model-averaging procedure. The proposed algorithm proves to be capable of attaining faster convergence rates and no performance loss against the starting centralized FL implementation with a reduced communication overhead compared to existing consensus-based and centralized solutions. To this end, a Multi-Hop consensus protocol, originally presented in the scope of dynamical system consensus theory, leveraging on standard Lyapunov stability discussions, has been proposed to assure that all federation clients share the same average model employing only information obtained from their ${m}$ -step neighbours. Experimental results on different communication topologies and the MNIST and MedMNIST v2 datasets validate the algorithm properties demonstrating a performance drop, compared with centralized FL setting, of about 1%.

- Abstract
Background: The association between volume, complications and pathological outcomes is still under debate regarding colorectal cancer surgery. The aim of the study was to assess the association between centre volume and severe complications, mortality, less-than-radical oncologic surgery, and indications for neoadjuvant therapy. Methods: Retrospective analysis of 16,883 colorectal cancer cases from 80 centres (2018-2021). Outcomes: 30-day mortality; Clavien-Dindo grade >2 complications; removal of ≥ 12 lymph nodes; non-radical resection; neoadjuvant therapy. Quartiles of hospital volumes were classified as LOW, MEDIUM, HIGH, and VERY HIGH. Independent predictors, both overall and for rectal cancer, were evaluated using logistic regression including age, gender, AJCC stage and cancer site. Results: LOW-volume centres reported a higher rate of severe postoperative complications (OR 1.50, 95% c.i. 1.15-1.096, P = 0.003). The rate of ≥ 12 lymph nodes removed in LOW-volume (OR 0.68, 95% c.i. 0.56-0.85, P < 0.001) and MEDIUM-volume (OR 0.72, 95% c.i. 0.62-0.83, P < 0.001) centres was lower than in VERY HIGH-volume centres. Of the 4676 rectal cancer patients, the rate of ≥ 12 lymph nodes removed was lower in LOW-volume than in VERY HIGH-volume centres (OR 0.57, 95% c.i. 0.41-0.80, P = 0.001). A lower rate of neoadjuvant chemoradiation was associated with HIGH (OR 0.66, 95% c.i. 0.56-0.77, P < 0.001), MEDIUM (OR 0.75, 95% c.i. 0.60-0.92, P = 0.006), and LOW (OR 0.70, 95% c.i. 0.52-0.94, P = 0.019) volume centres (vs. VERY HIGH). Conclusion: Colorectal cancer surgery in low-volume centres is at higher risk of suboptimal management, poor postoperative outcomes, and less-than-adequate oncologic resections. Centralisation of rectal cancer cases should be taken into consideration to optimise the outcomes.

- Abstract
In order to avoid that many senior citizens with Mild Cognitive Impairment (MCI) could become chronic patients with a low level of functioning in many cognitive, behavioral and emotional domains, some technological solutions have been improved to better enhance less compromised healthy lifestyle, providing real-time clinically-based suggestions and reducing some dysfunctional behaviors. Moreover another critical problem is the overweight and obesity that can affect many people in Western but also Eastern regions. In order to cope with MCI and obesity, mhealth monitor-ing solutions (smart-watches based and wearable based) have been devel-oped. The major aim of this paper is to show the rationale behind the SENIOR Project, a Cariplo funded project that will develop and deliver wearable monitoring solutions with tailored nudging feedbacks for patients with obesity an MCI. Scientific background, rationale, aims, outcomes measured and innovations are discussed.

- Abstract
Background: The Three Factor Eating Questionnaire-Revised 18 (TFEQ-R-18) is an extensively used questionnaire to measure three transdiagnostic features of eating behavior: cognitive restraint, uncontrolled eating, and emotional eating. Objective: This research aims to investigate the psychometric properties of the Italian version of the TFEQ-R-18 in three large community samples. Method: Cross-sectional research designs were employed. In Study 1 (N = 537), an exploratory graph analysis (EGA) was used to examine item clustering within the TFEQ-R-18. In Study 2 (N = 645), a confirmatory factor analysis (CFA) was conducted to test its structural validity. In Study 3 (N = 346), a MANOVA was employed assessing mean differences across eating disorders (e.g., anorexia nervosa, bulimia nervosa, binge eating disorder). Results: In Study 1, the EGA accurately identified the three original dimensions of the TFEQ-R-18. Study 2 showed that the Italian TFEQ-R-18 has good fit indexes (CFI = 0.989, RMSEA = 0.064; 90% CI [0.058, 0.070], SRMR = 0.062), and possesses robust psychometric properties. Study 3 reveals distinct, statistically significant differences among eating disorders. Conclusion: The TFEQ-R-18 proves to be a concise and precise tool for measuring transdiagnostic eating behaviors. Its applicability in the Italian context, supported by robust psychometric properties, suggests its utility for both research and clinical purposes. The findings affirm its potential to inform interventions aimed at enhancing psychological health. Level of evidence: Level V, descriptive study.

- Abstract
Background: The ever-increasing spread of Internet-based systems for common mental disorders has generated the need for brief online screening methods. This study aims to test the psychometric properties of the Web Screening Questionnaire (WSQ) to examine its suitability for screening for common mental health problems among a community sample of Italian adults. Methods: A total of 1282 subjects (F = 819; mean age = 42.05) answered the WSQ. Its discriminant characteristics were examined with other validated selected scales for measuring mental health widely used in the Italian population using sensitivity, specificity, and area under the curve (AUC), as well as positive (PPV) and negative predictive values (NPV). Results: Most of the WSQ subscales exhibited moderate to high specificity values. Specifically, the scales of ‘agoraphobia’ (0.947; 95%CI [0.934, 0.960]), ‘anxiety’ (0.959; 95%CI [0.946, 0.970]), and ‘panic disorder’ (0.973; 95%CI [0.964, 0.981]) showed the highest values whilst the ‘obsessive-compulsive’ dimension had the lowest value at 0.838, 95%CI [0.815, 0.861]. With exceptions observed for ‘depression’ (0.716; 95%CI [642, 798]) and ‘alcohol abuse’ (0.760; 95%CI [560, 920]), instead, the WSQ demonstrated critical sensitivity values (<0.6) in all dimensions. Conclusions: The WSQ was appropriate for discriminating between people with and without a psychiatric condition, as it helps to confirm the absence of disorders. However, further diagnostic procedures are required, in case of a positive WSQ screening result.
