Abstract During the winter 2023–2024, an upsurge of Mycoplasma pneumoniae (M.pneumoniae) was noted in the Netherlands. To investigate the distribution of M.pneumoniae sequence types from different patient populations and to explore genotypic macrolide resistance which is common in East Asia but not (yet) in Europe. M.pneumoniae positive throat/nasal samples from participatory respiratory surveillance, patients visiting general practitioners with an acute respiratory infection including community acquired pneumonia (CAP) and hospitalised patients with CAP were included, representing different disease severity. The M.pneumoniae were typed with multilocus sequence typing and the 23 S rRNA region was sequenced to determine macrolide resistance markers. In total, 153 M.pneumoniae were sequenced, six sequence types (STs) and only one bacterium with macrolide resistance marker were detected. No link between STs or bacterial load (PCR cycle threshold) and source population of M.pneumoniae was detected. In the Netherlands, the M.pneumoniae upsurge in 2023–2024 existed of multiple commonly found STs. No link between ST and severity of illness was detected. Macrolide resistance remained sporadic.

The requirements for standardized ileal digestible (SID) lysine due to lean tissue growth rate (LTGR) in growing-finishing pigs were quantified on the basis of data from the literature. The same was done for those requirements due to LTGR; protein deposition in mammary tissue, uterine tissue, fetal tissue and placental tissue; and colostrum and milk protein synthesis in gestating and lactating sows. These processes were expressed in terms of LTGR, litter size, piglet birth weight, preweaning survival rate, litter weaning weight and teat number. The 2013--2023 genetic trends of those traits in the PIC-L02, PIC-L03, PIC-L65 and PIC-L800 populations were estimated and converted to genetic trends in lysine requirements. On this basis, the daily SID lysine requirement of Camborough (i.e., PIC-L02 × PIC-L03) sows increases by 0.47 g/d (i.e., by 2% of the current mean level) per year: 0.10 g/d per year and 0.37 g/d per year for gestational and lactational processes, respectively. The daily SID lysine requirement of growing-finishing progeny of Camborough sows and either PIC-L65 or PIC-L800 boars increases by 0.09 g/d (i.e., by 0.6% of the current mean level) per year.

This study aimed to investigate genetic parameters for sow pelvic organ prolapse in purebred and crossbred herds. Pelvic organ prolapse was recorded as normal or prolapsed on the individual sow level across 32 purebred and 8 crossbred farms. In total, 75,162 purebred Landrace sows from a single maternal line were recorded between 2018 and 2023, while 18,988 commercial two-way crossbred (Landrace x Large White) sows were available between 2020 and 2023. There were 5,122,005 animals included in the pedigree. The prolapse in purebreds and crossbreds was considered two different traits in the model. Pedigrees of the crossbred sows were determined based on genotypes through parentage assignment. The average incidence rates were 1.81% and 3.93% for purebreds and crossbreds, respectively. The bivariate model incorporated fixed effects of parity group and region with random effects of contemporary group (farm and mating year and month at the first parity), additive genetic, and residual. Genetic parameter estimates were obtained using BLUPF90+ with the AIREML option. The estimated additive variance was larger in crossbreds than in purebreds. Estimates of heritability in the observed scale were 0.09 (0.006) for purebreds and 0.11 (0.014) for crossbreds, with a genetic correlation of 0.83 using a linear model. Results suggested that including data from crossbreds with higher incidence rate is beneficial and selection to reduce the prolapse incidence in purebred sow herds would also benefit commercial crossbred sow herds.

Abstract This article delves into vulnerability in the context of Dutch education law. It discusses the challenges faced by parents seeking ‘appropriate education’ (rather than schools offering special education) for children that require additional assistance. The article highlights how the Dutch Law on Appropriate Education, implemented over a decade ago, has strengthened the legal protection of parents. Prior to this law, navigating the educational system for children with special needs was complex and burdensome. The Law on Appropriate Education established the Disputes Committee for Appropriate Education (DCAE), which offers recommendations to school boards in case of disputes between parents and schools. The article examines the ongoing debate about making DCAE rulings binding and investigates whether this change would enhance legal protection of parents and children seeking appropriate education, aligning with a more responsive approach to administrative law. We study temporal entanglement in dual-unitary Clifford circuits with probabilistic measurements preserving spatial unitarity. We exactly characterize the temporal entanglement barrier in the measurement-free regime, exhibiting ballistic growth and decay and a volume-law peak. In the presence of measurements, we relate the temporal entanglement to the scrambling properties of the circuit. For “good scramblers” measurements do not qualitatively change the temporal entanglement profile but only result in a reduced entanglement velocity, whereas for “poor scramblers” the initial ballistic growth of temporal entanglement with bath size is modified to diffusive. This difference is understood through a mapping of the underlying operator dynamics to a biased and an unbiased persistent random walk, respectively. In the latter case measurements additionally modify the ballistic decay to the perfect dephaser limit, with vanishing temporal entanglement, to an exponential decay, which we describe through a spatial transfer matrix method. This spatial dynamics is shown to be described by a non-Hermitian hopping model, exhibiting a PT-breaking transition at a critical measurement rate p=1/2. In all cases the peak value of the temporal entanglement barrier exhibits volume-law scaling for all measurement rates.

Entanglement membrane theory is an effective coarse-grained description of entanglement dynamics and operator growth in chaotic quantum many-body systems. The fundamental quantity characterizing the membrane is the entanglement line tension. However, determining the entanglement line tension for microscopic models is in general exponentially difficult. We compute the entanglement line tension in a recently introduced class of exactly solvable yet chaotic unitary circuits, so-called generalized dual-unitary circuits, obtaining a nontrivial form that gives rise to a hierarchy of velocity scales with v_{E}

The Hayden-Preskill protocol probes the capability of information recovery from local subsystems after unitary dynamics. As such it resolves the capability of quantum many-body systems to dynamically implement a quantum error-correcting code. The transition to coding behavior has been mostly discussed using effective approaches, such as entanglement membrane theory. Here, we present exact results on the use of Hayden-Preskill recovery as a dynamical probe of scrambling in local quantum many-body systems. We investigate certain classes of unitary circuit models, both structured Floquet (dual-unitary) and Haar-random circuits. We discuss different dynamical signatures corresponding to information transport or scrambling, respectively, that go beyond effective approaches. Surprisingly, certain chaotic circuits transport information with perfect fidelity. In integrable dual-unitary circuits, we relate the information transmission to the propagation and scattering of quasiparticles. Using numerical and analytical insights, we argue that the qualitative features of information recovery extend away from these solvable points. Our results suggest that information recovery protocols can serve to distinguish chaotic and integrable behavior, and that they are sensitive to characteristic dynamical features, such as long-lived quasiparticles or dual-unitarity.

Lifecycle assessment (LCA) quantified changes in environmental impact categories (global warming, eutrophication, etc.) from 2021 to 2030 due to genetic trends in (re)production traits in pig lines of the breeding company Genus-PIC. The 2030 levels were projected with selection index theory based on weightings of traits in the breeding goals and genetic covariances among them. The projected improvement was 0.9% annually for most impact categories. Another LCA compared the impacts of 2021 North American pig production based on PIC genetics versus the industry average. Software openLCA converted material and energy flows to impact categories of frameworks ReCiPe-2016, PEF-3.1, and IPCC-2021. Flows came from data recorded by customers (1.1/4.7 million sows/finishing pigs) and by subscribers to a third-party data aggregator (1.3/9.1 million). PIC genetics have a 7–8% better impact than industry average for 13/18 categories of ReCiPe-2016, 19/25 of PEF-3.1, and all categories of IPCC-2001. Pig breeding delivers positive environmental outcomes as correlated responses to selection for profitability-oriented breeding goals. This trend is additive; technology development will increase it. Different investment levels in breeding population structure and technology and different operational efficiencies of breeding companies cause substantial differences in the environmental impact of pig production.

Background Current guidelines recommend either low-molecular weight heparin (LMWH) or direct oral anticoagulants (DOACs) as first-line treatment in cancer-associated venous thromboembolism (VTE).

Dolutegravir is a highly potent HIV integrase strand transfer inhibitor that is recommended for first-line anti-retroviral treatment in all major treatment guidelines. A recent study has shown that people taking this class of anti-retroviral treatment have a substantially higher risk of early-onset cardiovascular disease, a condition shown previously to be associated with increased platelet reactivity. To date, few studies have explored the effects of dolutegravir on platelet activation. Accordingly, the current study was undertaken with the primary objective of investigating the effects of dolutegravir on the reactivity of human platelets in vitro. Platelet-rich plasma, isolated platelets, or buffy coat cell suspensions prepared from the blood of healthy adults were treated with dolutegravir (2.5–10 µg/ml), followed by activation with adenosine 5’-diphosphate (ADP), thrombin, or a thromboxane A2 receptor agonist U46619. Expression of platelet CD62P (P-selectin), formation of heterotypic neutrophil:platelet aggregates, and calcium (Ca2+) fluxes were measured using flow cytometry and fluorescence spectrometry, respectively. Dolutegravir caused dose-related potentiation of ADP-, thrombin- and U46619-activated expression of CD62P by platelets, as well as a significant increases in formation of neutrophil:platelet aggregates. These effects were paralleled by a spontaneous, receptor-independent elevation in cytosolic Ca2+ that appears to underpin the mechanism by which the antiretroviral agent augments the responsiveness of these cells to ADP, thrombin and U46619. The most likely mechanism of dolutegravir-mediated increases in platelet cytosolic Ca2+ relates to a combination of lipophilicity and divalent/trivalent metal-binding and/or chelating properties of the anti-retroviral agent. These properties are likely to confer ionophore-type activities on dolutegravir that would promote movement of Ca2+ across the plasma membrane, delivering the cation to the cytosol where it would augment Ca2+-dependent intracellular signaling mechanisms. These effects of dolutegravir may lead to hyper-activation of platelets which, if operative in vivo, may contribute to an increased risk for cardiometabolic co-morbidities.

IntroductionLitter size in pigs has increased steadily since 1990. Because of unfavorable genetic correlations with piglet mortality, breeding goals should include survival traits next to litter size. Unbalanced breeding programs that neglect this requirement have produced increased mortality levels, attracting negative public attention. Balanced breeding does not have this disadvantage, but the general public is largely unaware of this.MethodsWe present long-term time trends as realized in commercial breeding. The data includes (i) phenotypes of litter size, piglet birth weight, and piglet mortality, as used in routine breeding value estimation; and (ii) the genomic Best Linear Unbiased Prediction (gBLUP) estimated breeding values thus obtained. Piglet mortality (2001–2022) and birth weight (2009–2022) phenotypes were related to litter size by recording year. Estimated breeding values (EBVs) for the mortality traits were regressed on those for litter size by birth year (2012–2022).ResultsAverage litter size is very weakly correlated to the mortality (R2 ≤ 0.06) and birth weight (0.07 ≤ R2 ≤ 0.26) traits, and those correlations are unfavorable (antagonistic) within each year. However, all traits analyzed here show favorable simultaneous phenotypic and genetic trends over time: the antagonisms are neutralized by balanced breeding. Above the annual mean litter size level, farrowing and lactation mortality rates increased with increasing litter size in every year (unfavorable), but the annual intercepts and the slopes decreased from 2001 to 2022 (favorable). Average litter birth weight decreased with litter size in every year (unfavorable), but the annual intercepts increased and the slopes decreased from 2009 to 2022 (favorable). Imiquimod (IMQ) is a topical agent that induces local inflammation via the Toll-like receptor 7 pathway. Recently, an IMQ-driven skin inflammation model was developed in healthy volunteers for proof-of-pharmacology trials. The aim of this study was to profile the cellular, biochemical, and clinical effects of the marketed anti-inflammatory compound prednisolone in an IMQ model. This randomized, double-blind, placebo-controlled study was conducted in 24 healthy volunteers. Oral prednisolone (0.25 mg/kg/dose) or placebo (1:1) was administered twice daily for 6 consecutive days. Two days after treatment initiation with prednisolone or placebo, 5 mg imiquimod (IMQ) once daily for two following days was applied under occlusion on the tape-stripped skin of the back for 48 h in healthy volunteers. Non-invasive (imaging and biophysical) and invasive (skin punch biopsies and blister induction) assessments were performed, as well as IMQ ex vivo stimulation of whole blood. Prednisolone reduced blood perfusion and skin erythema following 48 h of IMQ application (95% CI [−26.4%, −4.3%], p = 0.0111 and 95% CI [−7.96, −2.13], p = 0.0016). Oral prednisolone suppressed the IMQ-elevated total cell count (95% CI [−79.7%, −16.3%], p = 0.0165), NK and dendritic cells (95% CI [−68.7%, −5.2%], p = 0.0333, 95% CI [−76.9%, −13.9%], p = 0.0184), and classical monocytes (95% CI [−76.7%, −26.6%], p = 0.0043) in blister fluid. Notably, TNF, IL-6, IL-8, and Mx-A responses in blister exudate were also reduced by prednisolone compared to placebo. Oral prednisolone suppresses IMQ-induced skin inflammation, which underlines the value of this cutaneous challenge model in clinical pharmacology studies of novel anti-inflammatory compounds. In these studies, prednisolone can be used as a benchmark.

Soil properties are important drivers of species distribution and community structure in grassland. This study was undertaken to assess the influence of soil properties on woody plant distribution around six selected communally managed rangelands in the District. At each communal rangeland, a total of 25 plots of 20 × 20 m were surveyed to record the density, frequency, and composition of woody species. Soil samples were collected for thirteen soil variables. A Tukey HSD (Tukey’s honestly significant difference) post hoc test was used to compare soil properties and canonical correspondence analysis (CCA) to relate the soil properties to the woody species distribution. The study recorded a total of 17 woody species in 9 families. Fabaceae was the most dominant family, and Senegalia mellifera was the most abundant and frequent encroaching species. Most of the species were native, whereas Prosopis velutina was the only invasive alien species recorded. Senegalia mellifera, P. velutina, and Terminalia sericea were considered the most encroaching in the study sites, with densities exceeding 2000 TE ha-1 (i.e., tree equivalent). CCA results exhibited the strong effect of soil variables on the distribution of woody plant species. CCA ordination analyses showed that K was the most influential soil variable on woody species distributions, followed by Mg, CEC, Na, pH, sand, clay and silt. In terms of woody distribution, the CCA diagram showed similarities between Disaneng, Logageng and Tshidilamolomo. This study provides baseline information on woody species diversity for future management of this ecosystem.

Parthenium hysterophorus L. (Asteraceae), commonly known as famine weed in South Africa, is one of the most invasive weeds worldwide, accountable for losses to biodiversity, agriculture, the economy, and the health of livestock and human beings. The spread of this weed is not easy to manage or reverse. With globalization, trade, and human movement, it will continue to spread, allowing its range to expand into new areas. This weed has serious adverse effects on rangeland and agricultural crop production. Because of its unique characteristics, its invasiveness and competitive success rates are attributed to its high reproductive ability, fast growth rate, tolerance to environmental stresses, climate change adaptability, allelopathic potential, and unpalatability to animals. Parthenium hysterophorus dominates diverse habitats by spreading quickly as a contaminant of grain and other crop products and through farm machinery. It is widely distributed and has become problematic in various countries, including South Africa. It has successfully invaded several provinces in the country since its introduction in 1880. Since plant invasions are not universal, this study aimed to review research that has been conducted on the aggressive weed P. hysterophorus to understand the ecological characteristics that enable its successful establishment, the economic costs associated with its detrimental impacts, and to briefly assess knowledge gaps to improve its management strategies.

Abstract The selection and introduction of disease resistance genes in livestock not only provide health benefits to animals but opportunities for breeders and farmers to meet the growing demand for high-quality meat and milk while reducing agriculture’s footprint on the environment. As traditional methods of classical breeding and selection for trait improvement are slow, recent progress in several areas of biology including (a) understanding host–pathogen interactions, (b) inexpensive and rapid DNA sequencing, and (c) robust gene editing like CRISPR-Cas provide geneticists tools to accelerate discovery and deployment of disease resistance alleles in livestock. Using these advances, the introduction of resistance genes into commercially relevant germplasm requires access to genetically superior livestock, an infrastructure for scalable allele deployment, freedom to operate, global regulatory approvals, and acceptance of gene edited livestock by producers and consumers. Importantly, academic researchers have recently discovered that modification of the CD163 gene in pigs can confer resistance to the virus that causes porcine reproductive and respiratory syndrome (PRRS). While this achievement represents a major step towards solving an important disease in livestock, to realize the positive impact on animal health while benefiting the pork industry and consumers, it is necessary to introduce this recessive disease resistance allele into commercial breeding populations. Rather than backcrossing the resistance gene from a few non-commercial founders, as a global supplier of high genetic merit livestock genetics, Genus plc and its pig division PIC (Pig Improvement Company) with Genus R&D have mobilized advances in reproductive biology, gene editing, DNA sequencing, and bioinformatics to simultaneously generate and introduce a single modified CD163 allele across four genetically diverse porcine lines of commercial importance that prevents PRRS virus (PRRSV) infection. This report focuses on technical aspects for a scaled gene editing program to consider for rapid and efficient generation and advancement of a small population of non-transgenic founder pigs for commercial breeding. This high genetic merit herd containing a PRRS disease resistance allele will provide important benefits to animal health and food chain value once approved for commercial sale and export.

Abstract Background Postnatal piglet survival is important both in economic and animal welfare terms. It is influenced by the piglet’s own direct genetic effects and by maternal genetic effects of the dam, associated with milk production and mothering abilities. These genetic effects might be correlated, affected by other non-genetic factors and unfavourably associated with other reproduction traits such as litter size, which makes the development of optimal breeding strategies a challenge. To identify the optimum selection strategy for piglet survival, a selection experiment was carried out to compare responses in survival and reproduction traits to selection on only direct, only maternal, or both genetic effects of postnatal survival. The data of the experiment were recorded from outdoor reared pigs, with first- and second-generation sires selected based on their estimated breeding values for maternal and direct effects of postnatal survival of indoor reared offspring, respectively, with the opportunity to identify potential genotype-by-environment interaction. Results A Bayesian multivariate threshold-linear model that was fitted to data on 22,483 piglets resulted in significant (Pr(h2 > 0) = 1.00) estimates of maternal and direct heritabilities between 0.12 and 0.18 for survival traits and between 0.29 and 0.36 for birth weight, respectively. Selection for direct genetic effects resulted in direct and maternal responses in postnatal survival of 1.11% ± 0.17 and − 0.49% ± 0.10, respectively, while selection for maternal genetic effects led to greater direct and maternal responses, of 5.20% ± 0.34 and 1.29% ± 0.20, respectively, in part due to unintentional within-litter selection. Selection for both direct and maternal effects revealed a significant lower direct response (− 1.04% ± 0.12) in comparison to its expected response from single-effect selection, caused by interactions between direct and maternal effects. Conclusions Selection successfully improved post- and perinatal survival and birth weight, which indicates that they are genetically determined and that genotype-by-environment interactions between outdoor (experimental data) and indoor (selection data) housed pigs were not important for these traits. A substantially increased overall (direct plus maternal) response was obtained using selection for maternal versus direct or both direct and maternal effects, suggesting that the maternal genetic effects are the main limiting factor for improving piglet survival on which selection pressure should be emphasized.

Primary open-angle glaucoma (POAG) is highly heritable, yet not well understood from a genetic perspective. Here, the authors perform a meta-analysis of genome-wide association studies in 34,179 POAG cases, identifying 44 previously unreported risk loci and mapping effects across multiple ethnicities.

Abstract Background In premenopausal women, treatment with direct oral factor Xa inhibitors is associated with an increased risk of heavy menstrual bleeding (HMB) compared with vitamin K antagonists (VKA). Treatment with the direct oral thrombin inhibitor dabigatran appears to be associated with a reduced risk of HMB compared with VKA. These findings come from small observational studies or post hoc analyses of trials in which HMB was not a primary outcome. Use of tranexamic acid during the menstrual period may be effective in patients with HMB, but prospective data regarding efficacy and safety in patients on anticoagulant treatment are lacking. Rationale and Design A direct comparison of a factor Xa inhibitor and a thrombin inhibitor with HMB as primary outcome, as well as an evaluation of the effects of adding tranexamic acid in women with anticoagulant‐associated HMB is highly relevant for clinical practice. The MEDEA study is a randomized, open‐label, pragmatic clinical trial to evaluate management strategies in premenopausal women with HMB associated with factor Xa inhibitor therapy. Outcomes Women using factor Xa inhibitors with proven HMB, as assessed by a pictorial blood loss assessment chart (PBAC) score of >150, will be randomized to one of three study arms: (i) switch to dabigatran; (ii) continue factor Xa inhibitor with addition of tranexamic acid during the menstrual period; or (iii) continue factor Xa inhibitor without intervention. The primary outcome is the difference in PBAC score before and after randomization. Here, we present the rationale and highlight several unique features in the design of the study.

Here, the authors report a multi-ethnic genome wide association meta-analysis of 12 studies from the International Cataract Genetics Consortium. They find six new loci associated with age-related nuclear cataract, in addition to replicating the association at CRYAA, and suggest a strong genetic link between age-related nuclear and congenital cataracts.

Abstract Background Fighting and controlling epidemic and endemic diseases represents a considerable cost to livestock production. Much research is dedicated to breeding disease resilient livestock, but this is not yet a common objective in practical breeding programs. In this paper, we investigate how future breeding programs may benefit from recent research on disease resilience. Main body We define disease resilience in terms of its component traits resistance (R: the ability of a host animal to limit within-host pathogen load (PL)) and tolerance (T: the ability of an infected host to limit the damage caused by a given PL), and model the host's production performance as a reaction norm on PL, depending on R and T. Based on this, we derive equations for the economic values of resilience and its component traits. A case study on porcine respiratory and reproductive syndrome (PRRS) in pigs illustrates that the economic value of increasing production in infectious conditions through selection for R and T can be more than three times higher than by selection for production in disease-free conditions. Although this reaction norm model of resilience is helpful for quantifying its relationship to its component traits, its parameters are difficult and expensive to quantify. We consider the consequences of ignoring R and T in breeding programs that measure resilience as production in infectious conditions with unknown PL—particularly, the risk that the genetic correlation between R and T is unfavourable (antagonistic) and that a trade-off between them neutralizes the resilience improvement. We describe four approaches to avoid such antagonisms: (1) by producing sufficient PL records to estimate this correlation and check for antagonisms—if found, continue routine PL recording, and if not found, shift to cheaper proxies for PL; (2) by selection on quantitative trait loci (QTL) known to influence both R and T in favourable ways; (3) by rapidly modifying towards near-complete resistance or tolerance, (4) by re-defining resilience as the animal's capacity to resist (or recover from) the perturbation caused by an infection, measured as temporal deviations of production traits in within-host longitudinal data series. Conclusions All four alternatives offer promising options for genetic improvement of disease resilience, and most rely on technological and methodological developments and innovation in automated data generation.

Breast cancer cells exploit the up-regulation or down-regulation of immune checkpoint proteins to evade anti-tumor immune responses. To explore the possible involvement of this mechanism in promoting systemic immunosuppression, the pre-treatment levels of soluble co-inhibitory and co-stimulatory immune checkpoint molecules, as well as those of cytokines, chemokines, and growth factors were measured in 98 newly diagnosed breast cancer patients and compared with those of 45 healthy controls using multiplex bead array and ELISA technologies. Plasma concentrations of the co-stimulatory immune checkpoints, GITR, GITRL, CD27, CD28, CD40, CD80, CD86 and ICOS, as well as the co-inhibitory molecules, PD-L1, CTLA-4 and TIM-3, were all significantly lower in early breast cancer patients compared to healthy controls, as were those of HVEM and sTLR-2, whereas the plasma concentrations of CX3CL1 (fractalkine), CCL5 (RANTES) and those of the growth factors, M-CSF, FGF-21 and GDF-15 were significantly increased. However, when analyzed according to the patients’ breast cancer characteristics, these being triple negative breast cancer (TNBC) vs. non-TNBC, tumor size, stage, nodal status and age, no significant differences were detected between the plasma levels of the various immune checkpoint molecules, cytokines, chemokines and growth factors. Additionally, none of these biomarkers correlated with pathological complete response. This study has identified low plasma levels of soluble co-stimulatory and co-inhibitory immune checkpoint molecules in newly diagnosed, non-metastatic breast cancer patients compared to healthy controls, which is a novel finding seemingly consistent with a state of systemic immune dysregulation. Plausible mechanisms include an association with elevated levels of M-CSF and CCL5, implicating the involvement of immune suppressor cells of the M2-macrophage/monocyte phenotype as possible drivers of this state of systemic immune quiescence/dysregulation.

Acute hand injuries are routinely managed by family medicine and primary care physicians. An appropriate initial assessment and treatment, early referral to a hand surgeon when indicated, and timeous referral to a hand therapist are imperative. A patient case report is presented reporting on the initial and subsequent assessment, treatment and outcomes at 3, 6, 7 and 9 months for a patient who sustained an acute finger injury. Finger range of motion (ROM), sensation, pain, time of wound closure, hand function measured with the standardised disability of the shoulder, arm and hand (DASH) questionnaire were the outcomes used. Pain, crepitus, decreased sensation, decreased ROM right index finger proximal interphalangeal joint (PIPJ) and dense scarring was measured at 9 months. Missed injuries or lack of recognition of injury severity leads to delayed referral to specialist hand surgeons and therapists, which lengthens recovery time and leads to sub-optimal outcomes. This article aims to provide the primary care practitioner with the initial management of a patient who sustained a traumatic hand injury whilst using a power tool.

Invasive alien species represent one of the main threats to biodiversity and species extinction. This is the case for the genus Prosopis, among which Prosopis velutina is the most invasive and common tree species along the Molopo River in the North-West Province, impacting native plant communities. However, its impact on the composition and diversity of native woody species remains poorly investigated in the area. Thus, this study aimed to assess the impact of P. velutina on native woody plant composition and diversity across three sites along the Molopo River. At each site, five quadrats of 20 × 20 m2 were randomly established in invaded and adjacent uninvaded stands. A comparative methodological approach was adopted, and the woody plants in invaded and uninvaded stands with similar site conditions were sampled. The results showed that native woody species density differed significantly (p < 0.05) between invaded and uninvaded stands, except for Bray sites, where there was a marginal difference (p = 0.6). The overall native woody species density decreased by 79.7% in the invaded stand. However, non-metric multidimensional scaling (nMDS) and analysis of similarity (ANOSIM) indicated significant differences in native tree composition between invaded and uninvaded stands at all sites. In all three sites, all ecological indices had significantly lower values in invaded stands compared to uninvaded stands. The decrease in all ecological indices in invaded over uninvaded stands indicated that P. velutina invasion reduced the diversity of native woody plant species. Due to the incessant spread of P. velutina, it may become a long-term dominant species with an increasing impact on the native vegetation. Therefore, the findings of this study call for urgent management and appropriate control measures against the ongoing spread of this invader within the riparian zones of the Molopo River in North-West Province.

We present a rare case of an infected stent graft in a venous bypass. A 79-year-old male patient who underwent previous venous bypass surgery was treated with a Viabahn stent graft for aneurysmal dilatation of this bypass. Three years later the patient was admitted in our clinic with infection of the stent graft. He was treated with resection of the infected stent graft and restoration of blood flow using a 100% biological prosthesis as an interposition conduit.Infected stent grafts are rare. In this case report we provide an overview of the available literature. Like in most cases, our patient was treated with excision of the stent graft and arterial reconstruction.

Xeroderma Pigmentosum (XP), an autosomal recessive disorder characterized by ultraviolet radiation-induced abnormalities of DNA excision and repair pathways is associated with early development of cutaneous cancers. Intracellular oxidative stress has also been proposed as a contributor to the occurrence of skin cancers. However, little is known about the possible augmentative contributions of chronic inflammation, immune suppression and oxidative stress to the pathogenesis of malignancies associated with other subtypes of XP. This has been addressed in the current study, focused on the measurement of systemic biomarkers of inflammation, immune dysfunction and oxidative damage in XP patients, consisting of XP-C, XP-D and XP-E cases, including those XP-C cases who had already developed multiple skin malignancies. The inflammatory biomarker profile measured in XP patients and healthy control subjects included the cytokines, interleukins (ILs)-2, -4, -6, -10, interferon-γ (IFN- γ) and tumor-necrosis factor-α (TNF-α), the acute phase reactant, C-reactive protein (CRP), and cotinine (as an objective indicator of smoking status). Immune suppression was detected according to the levels of five soluble inhibitory immune checkpoint proteins (CTLA-4, PD-1, PD-L1, LAG-3 and TIM-3), as well as those of vitamin D, while oxidative stress was determined according to the circulating levels of the DNA adduct, 8-hydroxy-2-deoxyguanosine (8-OH-dG). The contributions to energy and protein consumption of animal-source food (ASF) and its commodities bovine, pig, sheep and goat and poultry meat, fish and seafood, dairy products, and eggs were studied by multiple log-log-inverse regression of 1961–2017 consumption (MJ of energy, grams of protein) on income and year within country. The “year” variable implicitly captures time-dependent non-income factors such as prices, climate, agricultural area, urbanization, globalization, gender equality, religion. Fitting the latter six factors explicitly produced unrealistic results, likely due to insufficient within-country variation over time. All consumption patterns differed between countries, and changed over time; these differences and changes were related to income, but considerably more related to time-dependent non-income factors. Within-country estimates of the income elasticity (β) of total energy and protein consumption ranged from −1 to +1: when income increased by 1%, consumption changed by −1 to +1%. The corresponding estimates of the non-income time elasticity (γ) ranged from −0.05 to +0.05% per year: every year, adjusted for income, consumption changed by −0.05 to +0.05%. The β and γ estimates for the contribution of ASF to energy and protein consumption ranged twice as wide as these; those for the contributions of the individual commodities ranged at least three times as wide. The β and γ estimates for those commodities change considerably over time in many countries; their association to each other is very variable too, both between and within countries. Much of this variation takes place at the lower consumption levels. Considering all this, any attempt to forecast the consumption of animal source food (and particularly of its individual commodities) on a more detailed level than globally and on a longer term than a decade should include an income-independent time factor and be very careful with regard to the elasticity coefficients used.

There is increasing awareness of an association between the uptake of the HIV integrase inhibitor, dolutegravir, in first-line antiretroviral regimens with unusual weight gain and development of the metabolic syndrome, particularly in African women. Although seemingly unexplored, the development of systemic inflammation linked to the putative pro-inflammatory activity of dolutegravir represents a plausible pathophysiological mechanism of this unusual weight gain. This possibility was explored in the current study undertaken to investigate the effects of dolutegravir (2.5–20 μg/mL) on several pro-inflammatory activities of neutrophils isolated from the blood of healthy, adult humans. These activities included the generation of reactive oxygen species (ROS), degranulation (elastase release) and alterations in the concentrations of cytosolic Ca2+ using chemiluminescence, spectrophotometric and fluorimetric procedures, respectively. Exposure of neutrophils to dolutegravir alone resulted in the abrupt, dose-related, and significant (p < 0.0039–p < 0.0022) generation of ROS that was attenuated by the inclusion of the Ca2+-chelating agent, EGTA, or inhibitors of NADPH oxidase (diphenyleneiodonium chloride, DPI), phospholipase C (U733122), myeloperoxidase (sodium azide) and phosphoinositol-3-kinase (wortmannin). In addition, exposure to dolutegravir augmented the release of elastase by stimulus-activated neutrophils. These pro-inflammatory effects of dolutegravir on neutrophils were associated with significant, rapid, and sustained increases in the concentrations of cytosolic Ca2+ that appeared to originate from the extracellular compartment, seemingly consistent with an ionophore-like property of dolutegravir. These findings are preliminary and necessitate verification in the clinical setting of HIV infection. Nevertheless, given the complex link between inflammation and obesity, these pro-inflammatory interactions of dolutegravir with neutrophils may contribute to unexplained weight gain, possibly via the development of insulin resistance.

Dual-unitary circuits have emerged as a minimal model for chaotic quantum many-body dynamics in which the dynamics of correlations and entanglement remains tractable. Simultaneously, there has been intense interest in the effect of measurements on the dynamics of quantum information in many-body systems. In this work we introduce a class of models combining dual-unitary circuits with particular projective measurements that allow the exact computation of dynamical correlations of local observables, entanglement growth, and steady-state entanglement. We identify a symmetry preventing a measurement-induced phase transition and present exact results for the intermediate critical purification phase.

Optical Coherence Tomography (OCT) enables non-invasive imaging of the retina and is used to diagnose and manage ophthalmic diseases including glaucoma. We present the first large-scale genome-wide association study of inner retinal morphology using phenotypes derived from OCT images of 31,434 UK Biobank participants. We identify 46 loci associated with thickness of the retinal nerve fibre layer or ganglion cell inner plexiform layer. Only one of these loci has been associated with glaucoma, and despite its clear role as a biomarker for the disease, Mendelian randomisation does not support inner retinal thickness being on the same genetic causal pathway as glaucoma. We extracted overall retinal thickness at the fovea, representative of foveal hypoplasia, with which three of the 46 SNPs were associated. We additionally associate these three loci with visual acuity. In contrast to the Mendelian causes of severe foveal hypoplasia, our results suggest a spectrum of foveal hypoplasia, in part genetically determined, with consequences on visual function.

Occult cancer is detected in about 5% of patients with unprovoked venous thromboembolism (VTE) in the 12 months following VTE diagnosis. Current guidance suggests conducting a ‘limited’ cancer screening in these patients, consisting of medical history taking, physical examination, routine blood tests, chest X-ray, and age- and gender-specific testing, over full-body imaging. However, almost half of underlying cancers remain undetected with this approach. Blood-based liquid biopsies may provide an attractive addition or alternative to current cancer screening strategies, with a potentially higher detection rate while avoiding radiation or invasive testing.The PLATO-VTE study is an ongoing, investigator-initiated, multinational, prospective, observational cohort study comparing the sensitivity of novel biomarkers for detecting cancer with that of limited cancer screening in the setting of unprovoked VTE. Patients older than 40 years with a first episode of unprovoked VTE are eligible, while those with major and minor transient provoking risk factors for VTE are excluded. Patients undergo standard-of-care ‘limited’ cancer screening and are followed for 12 months for the occurrence of cancer. A blood sample for biomarker analysis is drawn within 10 days; a second sample is taken at 3 months to assess test result consistency over time. Three biomarkers are assessed: platelet mRNA, circulating tumor DNA, and plasma proteomics analysis. The sensitivity and predictive value of the biomarkers at baseline will be compared with those of limited screening.The results from the PLATO-VTE study may lead to reconsider current approaches for cancer screening in patients with unprovoked VTE.

Recent works have investigated the emergence of a new kind of random matrix behaviour in unitary dynamics following a quantum quench. Starting from a time-evolved state, an ensemble of pure states supported on a small subsystem can be generated by performing projective measurements on the remainder of the system, leading to a $\textit{projected ensemble}$. In chaotic quantum systems it was conjectured that such projected ensembles become indistinguishable from the uniform Haar-random ensemble and lead to a $\textit{quantum state design}$. Exact results were recently presented by Ho and Choi [Phys. Rev. Lett. 128, 060601 (2022)] for the kicked Ising model at the self-dual point. We provide an alternative construction that can be extended to general chaotic dual-unitary circuits with solvable initial states and measurements, highlighting the role of the underlying dual-unitarity and further showing how dual-unitary circuit models exhibit both exact solvability and random matrix behaviour. Building on results from biunitary connections, we show how complex Hadamard matrices and unitary error bases both lead to solvable measurement schemes.

Abstract Background Optimal contributions selection (OCS) provides animal breeders with a framework for maximising genetic gain for a predefined rate of inbreeding. Simulation studies have indicated that the source of the selective advantage of OCS is derived from breeding decisions being more closely aligned with estimates of Mendelian sampling terms ($$\hat{a}$$ a^ ) of selection candidates, rather than estimated breeding values (EBV). This study represents the first attempt to assess the source of the selective advantage provided by OCS using a commercial pig population and by testing three hypotheses: (1) OCS places more emphasis on $$\hat{a}$$ a^ compared to EBV for determining which animals were selected as parents, (2) OCS places more emphasis on $$\hat{a}$$ a^ compared to EBV for determining which of those parents were selected to make a long-term genetic contribution (r), and (3) OCS places more emphasis on $$\hat{a}$$ a^ compared to EBV for determining the magnitude of r. The population studied also provided an opportunity to investigate the convergence of r over time. Results Selection intensity limited the number of males available for analysis, but females provided some evidence that the selective advantage derived from applying an OCS algorithm resulted from greater weighting being placed on $$\hat{a}$$ a^ during the process of decision-making. Male r were found to converge initially at a faster rate than female r, with approximately 90% convergence achieved within seven generations across both sexes. Conclusions This study of commercial data provides some support to results from theoretical and simulation studies that the source of selective advantage from OCS comes from $$\hat{a}$$ a^ . The implication that genomic selection (GS) improves estimation of $$\hat{a}$$ a^ should allow for even greater genetic gains for a predefined rate of inbreeding, once the synergistic benefits of combining OCS and GS are realised.

We consider a unitary circuit where the underlying gates are chosen to be R-matrices satisfying the Yang-Baxter equation and correlation functions can be expressed through a transfer matrix formalism. These transfer matrices are no longer Hermitian and differ from the ones guaranteeing local conservation laws, but remain mutually commuting at different values of the spectral parameter defining the circuit. Exact eigenstates can still be constructed as a Bethe ansatz, but while these transfer matrices are diagonalizable in the inhomogeneous case, the homogeneous limit corresponds to an exceptional point where multiple eigenstates coalesce and Jordan blocks appear. Remarkably, the complete set of (generalized) eigenstates is only obtained when taking into account a combinatorial number of nontrivial vacuum states. In all cases, the Bethe equations reduce to those of the integrable spin-1 chain and exhibit a global SU(2) symmetry, significantly reducing the total number of eigenstates required in the calculation of correlation functions. A similar construction is shown to hold for the calculation of out-of-time-order correlations.

In specific open systems with collective dissipation the Liouvillian can be mapped to a non-Hermitian Hamiltonian. We here consider such a system where the Liouvillian is mapped to an XXZ Richardson-Gaudin integrable model and detail its exact Bethe ansatz solution. While no longer Hermitian, the Hamiltonian is pseudo-Hermitian/PT symmetric, and as the strength of the coupling to the environment is increased, the spectrum in a fixed symmetry sector changes from a broken pseudo-Hermitian phase with complex conjugate eigenvalues to a pseudo-Hermitian phase with real eigenvalues, passing through a series of exceptional points and associated dissipative quantum phase transitions. The homogeneous limit supports a nontrivial steady state, and away from this limit this state gives rise to a slow logarithmic growth of the decay rate (spectral gap) with system size. Using the exact solution, it is furthermore shown how at large coupling strengths the ratio of the imaginary to the real part of the eigenvalues becomes approximately quantized in the remaining symmetry sectors.

Abstract Background We investigated the epidemiology and prevalence of potential risk factors of tuberculosis (TB) recurrence in a population-based registry cohort of 8084 TB cases between 1995 and 2013. Methods An episode of recurrent TB was defined as a case re-registered in the National Infectious Disease Register at least 360 days from the date of the initial registration. A regression model was used to estimate risk factors for recurrence in the national cohort. To describe the presence of known risk factors for recurrence, patient records of the recurrent cases were reviewed for TB diagnosis confirmation, potential factors affecting the risk of recurrence, the treatment regimens given and the outcomes of the TB episodes preceding the recurrence. Results TB registry data included 84 patients, for whom more than 1 TB episode had been registered. After a careful clinical review, 50 recurrent TB cases (0.6%) were identified. The overall incidence of recurrence was 113 cases per 100,000 person-years over a median follow up of 6.1 years. For the first 2 years, the incidence of recurrence was over 200/100000. In multivariate analysis of the national cohort, younger age remained an independent risk factor at all time points, and male gender and pulmonary TB at 18 years of follow-up. Among the 50 recurrent cases, 35 patients (70%) had received adequate treatment for the first episode; in 12 cases (24%) the treating physician and in two cases (4%) the patient had discontinued treatment prematurely. In one case (2%) the treatment outcome could not be assessed. Conclusions In Finland, the rate of recurrent TB was low despite no systematic directly observed therapy. The first 2 years after a TB episode had the highest risk for recurrence. Among the recurrent cases, the observed premature discontinuation of treatment in the first episode in nearly one fourth of the recurrent cases calls for improved training of the physicians.

Abstract Background Lethal recessive genetic variants are maintained at relatively low frequencies in a population in the heterozygous state, but by definition are fatal and therefore unobserved in the homozygous state. Since haplotypes allow the tagging of rare and untyped genetic variants, they have potential for studying lethal recessive variants. In this study, we used a large commercial population to identify putative lethal recessive haplotypes that impact either the total number born (TNB) or the number born alive (NBA) as a proportion of the total number born (NBA/TNB). We also compared the use of haplotypes with a single nucleotide polymorphism (SNP)-by-SNP approach and examined the benefits of using additional haplotypes imputed from low-density genotype data for the detection of lethal recessive variants. Candidate haplotypes were identified using population-wide haplotype frequencies and within-family analyses. These candidate haplotypes were subsequently assessed for putative lethal recessive effects on TNB and NBA/TNB by comparing carrier-to-carrier matings with carrier-to-non-carrier matings. Results Using both medium-density and imputed low-density genotype data six regions were identified as containing putative lethal recessive haplotypes that had an effect on TNB. It is likely that these regions were related to at least four putative lethal recessive variants, each located on a different chromosome. Evidence for putative lethal recessive effects on TNB was found on chromosomes 1, 6, 10 and 14 using haplotypes. Using haplotypes from individuals genotyped only at medium-density or a SNP-by-SNP approach did not detect any lethal recessive effects. No lethal recessive haplotypes or SNPs were detected that had an effect on NBA/TNB. Conclusions We show that the use of haplotypes from combining medium-density and imputed low-density genotype data is superior for the identification of lethal recessive variants compared to both a SNP-by-SNP approach and to the use of only medium-density data. We developed a formal statistical framework that provided sufficient power to detect lethal recessive variants in species, which produce large full-sib families, while reducing false positive or type I errors. Applying this framework results in improvements in reproductive performance by purging lethal recessive alleles from a population in a timely and cost-effective manner.

Access to bank financing is regularly rated as one of the biggest obstacles to SME sustainability. With the introduction of the independent review as an alternative to the statutory audit, banks may have inevitably lost their risk assessment reassurance that audits provided. Previous research found that banks have adjusted to this situation by no longer insisting on audited financial statements. The research undertaken in this study aims to, firstly, investigate SME owner/managers’ perceptions about what banks require when assessing bank loan applications; and, secondly, to gauge SME owner/manager’s attitudes towards the value they perceive accounting may contribute to their sustainability. It was found from an SME perspective, that although an independent review is the current way to go, many SME owner/managers consider their own accounting skills, as the language of business, to be lacking. This could potentially have repercussions in inadvertently promoting further asymmetric financial information, and thereby limiting successes in obtaining finance.

It is accepted that SMEs are major contributors to global employment and GDP. Similarly, SMEs’ reliance on bank finance to maintain financial and operational sustainability is also globally accepted. In 2008, the Company’s Act of South Africa was amended to scrap the statutory audit requirement for qualifying entities, with the aim of alleviating the administrative burden of SMEs and increase their sustainability potential. As sound as this strategy may have been, a grey area arose in that banks may still insist on audited financial statements. This study investigates the question as to whether South African banks still consider audited financial statements as key in evaluating SME bank finance applications. This was done by analyzing the major banks’ requirements per their policies and follow-up discussions with loan officers. Contrary to expectations, the historic focus per audited financial statements was considered of much less importance than progressive future-oriented management statements and reports.

We aimed to evaluate the performance of the Khorana score in predicting venous thromboembolic events in ambulatory cancer patients. Embase and MEDLINE were searched from January 2008 to June 2018 for studies which evaluated the Khorana score. Two authors independently screened studies for eligibility, extracted data, and assessed risk of bias. Additional data on the 6-month incidence of venous thromboembolism were sought by contacting corresponding authors. The incidence in each Khorana score risk group was estimated with random effects meta-analysis. A total of 45 articles and eight abstracts were included, comprising 55 cohorts enrolling 34,555 ambulatory cancer patients. For 27,849 patients (81%), 6-month follow-up data were obtained. Overall, 19% of patients had a Khorana score of 0 points, 64% a score of 1 or 2 points, and 17% a score of 3 or more points. The incidence of venous thromboembolism in the first six months was 5.0% (95%CI: 3.9-6.5) in patients with a low-risk Khorana score (0 points), 6.6% (95%CI: 5.6-7.7) in those with an intermediate-risk Khorana score (1 or 2 points), and 11.0% (95%CI: 8.8-13.8) in those with a high-risk Khorana score (3 points or higher). Of the patients with venous thromboembolism in the first six months, 23.4% (95%CI: 18.4-29.4) had been classified as high risk according to the Khorana score. In conclusion, the Khorana score can be used to select ambulatory cancer patients at high risk of venous thromboembolism for thromboprophylaxis; however, most events occur outside this high-risk group.

The detection of herpes simplex viruses and Treponemal pallidum from genital lesions requires efficient sampling of genetic material for a reliable molecular diagnosis. From 460 patients attending the Public Health clinic, two swabs (dry cotton swabs and Eswabs) per patient were collected in alternating order from the same lesion. Additionally, three storage conditions of Eswabs up to 28 days were evaluated to assess the stability of DNA over time. Out of the 830 PCRs performed, 20 (2.4%) PCRs were discordant between the two swabs. No significant differences were observed between the two sample types. HSV1 and HSV2 could be reliably detected from Eswabs up to 28 days when kept at room temperature. A single swab from a genital lesion is sufficient for reliable diagnosis of α-herpes viruses and Treponemal pallidum, for which both a dry cotton swab or Eswab could be used.

This study aims to assess the potential risks of setting up a hydrogen infrastructure in the Netherlands. An integrated risk assessment framework, capable of analyzing projects, identifying risks and comparing projects, is used to identify and analyze the main risks in the upcoming Dutch hydrogen infrastructure project. A time multiplier is added to the framework to develop parameters. The impact of the different risk categories provided by the integrated framework is calculated using the discounted cash flow (DCF) model. Despite resource risks having the highest impact, scope risks are shown to be the most prominent in the hydrogen infrastructure project. To present the DCF model results, a risk assessment matrix is constructed. Compared to the conventional Risk Assessment Matrix (RAM) used to present project risks, this matrix presents additional information in terms of the internal rate of return and risk specifics.

The objective of this study is to investigate the extent to which strategic management process is utilized within the mining industry. Strategic planning is an organizational management activity that is used to set priorities, focus energy and resources, strengthen operations, ascertain that employees and other stakeholders are working toward common goals, establish agreement around intended outcomes, and assess and adjust the organization’s direction in response to a changing environment. A typical strategy management process has the following steps: initial assessment, situation analysis, strategy formulation, strategy implementation, monitoring and evaluation. The other objective is to determine which analytical tools are commonly used for situational, internal and external assessment as input to the strategic management process. A structured questionnaire was used for the study. A total of 300 mines were randomly selected from a research population of mining organizations in South Africa, Africa and globally. The respondents were all part of senior management. A response rate of 64% was achieved. The results indicated that about 20% of the organizations did not institutionalize their strategic planning functions and did not have a good strategic foundation. The results also showed that 60% were not satisfied with their productivity and 30% indicated that their cash flows were not stable at all. There was a significant number of organizations who do not use strategic analytical tools. A statistically and practically significant positive relationship was found between strategic management dimensions and business performance implying that the use of strategic management process can lead to improved business performance. The main objective of this research study is to investigate the extent to which business intelligence, competitive intelligence and marketing intelligence are used within the mining industry. Business intelligence, competitive intelligence and marketing intelligence are the management tools used to mine information to produce up-to-date intelligence and knowledge for operative and strategic decision making. A structured questionnaire is used for the study. A total of 300 mines are randomly selected from a research population of mining organizations in South Africa, Africa and globally. The respondents are all part of senior management. A response rate of 64% is achieved. The results indicat that more than half of the respondents do not have real-time intelligence and proper data mining tools to identify patterns and relationships within a data warehouse. Although a large proportion agrees that their organizations have systematic ways of gathering these different types of intelligence and use them for strategic decision making, there is a significant proportion that did not have any systems. Statistically and practically significant positive relationships with a large effect are found among the dimensions of business intelligence, marketing intelligence, competitive intelligence and perceived business performance
