Consequentemente, cresce também o encaminhamento desses pacientes para os diversos centros de transplante renal credenciados no país. Ao realizar o transplante estes pacientes tornam-se imunossuprimidos, e portanto um grupo de risco para várias infecções imunopreveníveis por vacinas. Há grandes desafios na avaliação dos riscos infecciosos prévio ao transplante, sendo a avaliação vacinal uma importante etapa na preparação destes pacientes para o transplante. O presente estudo tem como objetivo avaliar a situação vacinal de pacientes renais crônicos pré-transplante e sugerir a implementação de atenção sistematizada através da elaboração e análise da base de dados. A pesquisa incluiu pacientes com doença renal crônica, inscritos no Cadastro Técnico de Transplante Renal, atendidos pelo Ambulatório de Pré-Transplante Renal da Universidade Estadual de Campinas (Unicamp), no período de Fevereiro de 2014 a junho de 2016. Verificamos prevalência de vacinação completa em apenas 73% dos pacientes para vacina de hepatite B (HB) e 67% para vacina contra influenza. Nas demais vacinas propostas pelo calendário vacinal não houve prevalência que atingisse 50% dos pacientes. Não houve impacto direto da orientação vacinal entre os pacientes na cobertura vacinal para nenhum tipo de vacina pesquisada. O local predominante de vacinação mostrou impacto estatístico com maior prevalência das vacinas de difteria, tétano (dT), sarampo, caxumba, rubéola (SCR) e hepatite B, quando realizadas em Unidade Básica de Saúde (UBS), contra maior prevalência da vacina contra pneumococo (pneumo23) quando realizada em Clínicas de Terapia Renal Substitutiva (TRS). As entrevistas com as equipes cuidadoras revelaram a pouca importância dada ao tema imunização, sugerindo correlação entre as taxas vacinais dos pacientes com o grau de prioridade dada a cada vacina. Desta maneira sugerimos a integração de centro transplantador com as equipe cuidadoras, através de palestras, verificação periódica de cartão vacinal, e orientações sobre o calendário de imunização. Esta integração sensibilizaria as equipes sobre a importância da vacinação, aumentando a proteção vacinal e o cuidado do paciente portador de doença renal crônica na fase pre-transplante Abstract: The number of patients with chronic kidney disease is increasing every year. Consequently, the referral of these patients to the various renal transplant centers also increases. After transplantation these patients become immunosuppressed, and therefore a risk group for several vaccine immunopreventable infections. There are major challenges in evaluation of infectious risks prior to transplantation, and vaccination status is an important step in this preparation. The aim of the present study was to evaluate the vaccine situation in pre-transplant chronic renal disease and to suggest a systematized care through the elaboration and analysis of the database. The study included patients with chronic kidney disease enrolled in the renal transplant technical register, attended by the Pre-Transplant Renal Clinic from University of Campinas (Unicamp), from February 2014 to June 2016. We found prevalence of complete vaccination in only 73% of patients for hepatitis B vaccine (HB) and 67% for influenza vaccine. The prevalence was less than 50% of the patients with others vaccines. There was no direct impact of vaccine orientation among patients on vaccine coverage for any type of vaccine investigated. The prevalent site of vaccination showed a higher prevalence of diphtheria, tetanus (dT), measles, mumps, rubella (SCR) and hepatitis B vaccines, when performed in a Primary Health Unit (UBS), against a higher prevalence of pneumococcus (Pneumo23) when performed in Renal Replacement Therapy (TRS) Clinics. Interviews with caregivers revealed little importance given to the topic of immunization, suggesting a correlation between the vaccine rates with the degree of priority given to each vaccine. In conclusion, we suggest the integration of transplantation centers with the caregivers clinics, through lectures, periodic check of vaccination cards, and guidelines on the immunization schedule to sensitize these teams for the importance of vaccination, increasing vaccine protection and care of the patient with chronic kidney disease in the pre-transplantation phase Mestrado Clínica Médica Mestre em Ciências

Orientadores: Paulo Eduardo Neves Ferreira Velho, Alessandra Sussulini Dissertação (mestrado) - Universidade Estadual de Campinas, Faculdade de Ciências Médicas Resumo: As bartoneloses são doenças negligenciadas, emergentes, mundialmente distribuídas, que apresentam um amplo espectro de manifestações clínicas, incluindo as dermatológicas, e muitos aspectos ainda não desvendados. São causadas pelas Bartonella spp., bactérias gram-negativas, de crescimento lento, majoritariamente zoonóticas e transmitidas por artrópodes hematófagos. A Bartonella henselae se destaca como a espécie mais comum em infecções humanas, responsável pela doença da arranhadura do gato. Pacientes imunodeficientes são suscetíveis a condições mais graves e potencialmente fatais. O diagnóstico das infecções por Bartonella spp. é desafiador, principalmente por se tratarem de bactérias pouco conhecidas e de difícil cultivo. Não existe um método padrão ouro e a combinação de cultura e técnicas moleculares tem sido apontada como estratégia para aumento da sensibilidade, mas trata-se uma abordagem cara, morosa e trabalhosa, de modo que alternativas melhores são necessárias. Nesse contexto, a MALDI-TOF MS tem revolucionado a rotina de identificação de bactérias como um método rápido, fácil e eficiente, mas seu potencial de aplicação na investigação das Bartonella spp. ainda é pouco explorado, especialmente, em se tratando de microrganismos fastidiosos, no que diz respeito à influência do tempo de crescimento dessas bactérias em sua identificação. A presente dissertação tem seu conteúdo apresentado em formato alternativo, com os resultados descritos através de dois artigos: "Manifestações cutâneas das bartoneloses" e "A influência do tempo de crescimento na identificação de cepas de Bartonella henselae por espectrometria de massas MALDI-TOF". O primeiro artigo teve como objetivo realizar uma revisão da literatura a respeito das principais manifestações clínicas das bartoneloses humanas, sobretudo cutâneas, bem como aspectos relacionados à emergência, negligência, prevenção, diagnóstico e tratamento dessas doenças. Foi possível observar que a diversidade e complexidade das condições associadas às infecções por Bartonella spp., bem como suas limitações diagnósticas, representam um desafio para a comunidade médica e científica, de modo que mais estudos são necessários para aumentar a compreensão dessas doenças. A publicação do referido artigo é uma contribuição para difundir o conhecimento sobre as bartoneloses humanas, possibilitando que sejam mais consideradas para diagnóstico, identificadas e tratadas adequadamente. O segundo artigo teve como objetivo avaliar a influência do tempo de crescimento de cepas de B. henselae em sua identificação através da MALDI-TOF MS. Para tanto, foram obtidos espectros de referência de três cepas de B. henselae (IAL 3714, IAL 3715 e IAL 3716) com diferentes tempos de crescimento (14 e 28 dias) para criação de entradas no banco de dados. As cepas foram novamente cultivadas para obtenção de novos espectros e comparação com os primeiros pelo programa Biotyper®. Observou-se que cepas com tempos de crescimento diferentes apresentaram diferenças quantitativas e qualitativas em seus espectros de massas e, consequentemente, baixa similaridade quando comparadas pelo programa, comprometendo sua identificação. Como o tempo de crescimento não é uma varável controlável no cultivo dessas bactérias, sugeriu-se a inclusão de entradas em bancos dados com espectros de referência de colônias com tempos de crescimento diferentes para aumentar as chances de identificação. Esse estudo contribuiu para o possível estabelecimento da MALDI-TOF MS como uma alternativa promissora na identificação de cepas de Bartonella spp Abstract: Bartonellosis are neglected, emerging and globally distributed diseases that presents a wide spectrum of clinical manifestations, including dermatological, and many aspects that are still covered. They are caused by Bartonella spp., Gram-negative, slow-growing bacteria, mostly zoonotic and transmitted by blood-sucking arthropods. Bartonella henselae stands out as the most common species in human infections, responsible for cat scratch disease. Immunocompromised patients are susceptible to more serious and potentially fatal conditions. The diagnosis of Bartonella spp. infection is challenging, mainly because they are poorly known and difficult to culture bacteria. There is no a gold standard method and combination of culture and molecular techniques has been suggested to improve sensitivity, but it is an expensive, time-consuming and labor-intensive approach, so that better alternatives are required. In this context, MALDI-TOF MS has revolutionized the routine of bacteria identification as a fast, easy and efficient method, but its potential of application in the investigation of Bartonella spp. it is still unexplored, especially, considering the fastidious nature of this microorganism, with regard to the influence of the growth time of these bacteria in their identification. This dissertation has its content presented in an alternative format, with the results described through two articles: "Cutaneous manifestations of bartonellosis" and "The influence of growth time on identification of Bartonella henselae strains by MALDI-TOF mass spectrometry". The first article aimed to conduct a review of the literature regarding the main clinical manifestations of human bartonellosis, especially cutaneous, as well as aspects related to the emergence, negligence, prevention, diagnosis and treatment of these diseases. It was possible to observe that the diversity and complexity of the conditions associated with Bartonella spp. infection, as well as their diagnostic limitations, represent a challenge for the medical and scientific community, so that more studies are necessary to increase the understanding of these diseases. The publication of this article is a contribution to spread knowledge about human bartonellosis, allowing them to be further examined for diagnosis, identified and treated properly. The second article aimed to evaluate the influence of the growth time of B. henselae strains on their identification by MALDI-TOF MS. For this purpose, reference spectra of three B. henselae strains (IAL 3714, IAL 3715 and IAL 3716) with different growth times (14 and 28 days) were obtained to create database entries. The strains were then newly cultivated for obtention of new spectra and comparison with database entries through the software Biotyper®. It was observed that strains with different growth times have quantitative and qualitative differences in their mass spectra and, consequently, low similarity when compared by the software, compromising the identification. As the growth time is not a controllable variable in the culture of these bacteria, it was suggested to include database entries with reference spectra of colonies with different growth times to increase the chances of identification.
