Abstract

Coronavirus disease-19 (COVID-19)-related severe acute respiratory distress syndrome can lead to acute cor pulmonale. We report a case of acute cor pulmonale secondary to severe COVID-19 acute respiratory distress syndrome diagnosed with transesophageal echocardiography. Almitrine infusion allowed rapid enhancement of right ventricular function as well as improvement in oxygenation. (Level of Difficulty: Intermediate.)

Abstract

We report the case of an 80-year-old woman who presented one episode of cardiopulmonary arrest and two episodes of acute airway obstruction. We found in this patient the presence of tracheomalacia caused by megaesophagus compression secondary to achalasia probably responsible for episodes of acute airway obstruction and cardiopulmonary arrest.

Abstract

Summary Background The antiviral efficacy of remdesivir against SARS-CoV-2 is still controversial. We aimed to evaluate the clinical efficacy of remdesivir plus standard of care compared with standard of care alone in patients admitted to hospital with COVID-19, with indication of oxygen or ventilator support. Methods DisCoVeRy was a phase 3, open-label, adaptive, multicentre, randomised, controlled trial conducted in 48 sites in Europe (France, Belgium, Austria, Portugal, Luxembourg). Adult patients (aged ≥18 years) admitted to hospital with laboratory-confirmed SARS-CoV-2 infection and illness of any duration were eligible if they had clinical evidence of hypoxaemic pneumonia, or required oxygen supplementation. Exclusion criteria included elevated liver enzymes, severe chronic kidney disease, any contraindication to one of the studied treatments or their use in the 29 days before random assignment, or use of ribavirin, as well as pregnancy or breastfeeding. Participants were randomly assigned (1:1:1:1:1) to receive standard of care alone or in combination with remdesivir, lopinavir–ritonavir, lopinavir–ritonavir and interferon beta-1a, or hydroxychloroquine. Randomisation used computer-generated blocks of various sizes; it was stratified on severity of disease at inclusion and on European administrative region. Remdesivir was administered as 200 mg intravenous infusion on day 1, followed by once daily, 1-h infusions of 100 mg up to 9 days, for a total duration of 10 days. It could be stopped after 5 days if the participant was discharged. The primary outcome was the clinical status at day 15 measured by the WHO seven-point ordinal scale, assessed in the intention-to-treat population. Safety was assessed in the modified intention-to-treat population and was one of the secondary outcomes. This trial is registered with the European Clinical Trials Database, EudraCT2020-000936-23, and ClinicalTrials.gov, NCT04315948. Findings Between March 22, 2020, and Jan 21, 2021, 857 participants were enrolled and randomly assigned to remdesivir plus standard of care (n=429) or standard of care only (n=428). 15 participants were excluded from analysis in the remdesivir group, and ten in the control group. At day 15, the distribution of the WHO ordinal scale was: (1) not hospitalised, no limitations on activities (61 [15%] of 414 in the remdesivir group vs 73 [17%] of 418 in the control group); (2) not hospitalised, limitation on activities (129 [31%] vs 132 [32%]); (3) hospitalised, not requiring supplemental oxygen (50 [12%] vs 29 [7%]); (4) hospitalised, requiring supplemental oxygen (76 [18%] vs 67 [16%]); (5) hospitalised, on non-invasive ventilation or high flow oxygen devices (15 [4%] vs 14 [3%]); (6) hospitalised, on invasive mechanical ventilation or extracorporeal membrane oxygenation (62 [15%] vs 79 [19%]); (7) death (21 [5%] vs 24 [6%]). The difference between treatment groups was not significant (odds ratio 0·98 [95% CI 0·77–1·25]; p=0·85). There was no significant difference in the occurrence of serious adverse events between treatment groups (remdesivir, 135 [33%] of 406 vs control, 130 [31%] of 418; p=0·48). Three deaths (acute respiratory distress syndrome, bacterial infection, and hepatorenal syndrome) were considered related to remdesivir by the investigators, but only one by the sponsor's safety team (hepatorenal syndrome). Interpretation No clinical benefit was observed from the use of remdesivir in patients who were admitted to hospital for COVID-19, were symptomatic for more than 7 days, and required oxygen support. 6. Les complications de la transfusion sanguine

Abstract

Resume Depuis l’affaire francaise tres mediatisee du « sang contamine » un ensemble de reglementations specifiques et de procedures ont ete etablies pour ameliorer la securite transfusionnelle. L’amelioration des techniques de preparation des produits sanguins labiles y a contribue egalement. La mise en place d’une organisation de l’hemovigilance (creee en 1993) a permis d’identifier et de definir l’incidence des differents effets indesirables de la transfusion sanguine. Les risques encourus lors d’une transfusion sanguine, meme s’ils ne sont pas tres frequents, sont suffisamment graves pour justifier une evaluation prealable du rapport benefice-risque par les medecins. Les complications de la transfusion sont directement en relation avec le terrain, le capital genetique du patient transfuse, la qualite des produits sanguins labiles administres mais egalement en lien avec le defaut de pratique. Elles ont change en France au fil des annees. Actuellement, le risque viral a fortement recule et les reactions febriles non hemolytiques et les allergies representent aujourd’hui les principales complications de la transfusion. Le risque immunologique apparait maintenant nettement superieur au risque infectieux. La surcharge vasculaire represente encore la premiere cause de mortalite post-transfusionnelle en France. Ce travail a ete redige pour fournir aux acteurs de la transfusion une synthese sur les complications de la transfusion sanguine. Elles ne sont pas comparables suivant les pays en raison de l’heterogeneite des preparations des produits sanguins labiles, des differentes politiques de prevention dans la selection des donneurs et de la diversite de fonctionnement des systemes d’hemovigilance (systematique ou selectif). Dans cette revue generale, nous etudierons les complications de la transfusion selon le mecanisme physiopathologique. Nous insisterons sur les complications graves menacant le pronostic vital. La premiere partie etant consacree aux complications immunologiques, la deuxieme aux complications infectieuses, la troisieme aux accidents de surcharge. La derniere partie s’interessera tout particulierement a la transfusion massive.

Abstract

Objective: To report the hemodynamic effect of to the molecular adsorbent recirculating system (MARS™) therapy for patients in refractory vasoplegic shock due to calcium channel blocker (CCB) poisoning Methods: We report a retrospective cohort of patients who were hospitalized for CCB poisoning with refractory vasoplegic shock and treated by MARS therapy, at Amiens Hospital University, from January 2010 to December 2019. Improvement in hemodynamic was assessed by dynamic changes in mean arterial pressure (MAP) and norepinephrine levels over a 24-h period after MARS therapy. Cardiac function was assessed by transthoracic echocardiography. Results: MARS therapy was performed on seven patients for CCB poisoning. CCB poisoning included nicardipine ( n = 3, 43%) amlodipine ( n = 3, 43%), and verapamil ( n = 1, 14%). The median time to start MARS therapy was 24 [14–27] h after drug ingestion and 6 [2–9] h after ICU admission. Cardiac output was preserved for all patients. MAP values improved from 56 [43–58] to 65 [61–78] 16 mmHg ( p = 0.005). Norepinephrine dose significantly decreased from 3.2 [0.8–10] µg/kg/min to 1.2 [0.1–1.9] µg/kg/min ( p = 0.008) and lactate level decreased from 3.2 [2.4–3.4] mmol/l−1 to 1.6 [0.9–2.2] mmol/l−1 ( p = 0.008). The median length of ICU stay was 4 (2–7) days and hospital stay was 4 (4–16) days. No complication related to the MARS therapy were reported. No patient died and all were discharged from the hospital. Conclusion: We reported the largest case-series of MARS therapy for refractory vasoplegic shock due to CCB poisoning. We observed that MARS therapy was associated with an improvement of hemodynamic parameters.

Abstract

Background Data on microcirculatory pattern of COVID-19 critically ill patients are scarce. The objective was to compare sublingual microcirculation parameters of critically ill patients according to the severity of the disease. Methods The study is a single-center prospective study with critically ill COVID-19 patients admitted in ICU. Sublingual microcirculation was assessed by IDF microscopy within 48 hours of ICU admission. Microcirculatory flow index (MFI), proportion of perfused vessel (PPV), total vessel density (TVD), De Backer score (DBS), perfused vessel density (PVD) and heterogeneity index (HI) were assessed. Patients were divided in 2 groups (severe and critical) according to the World health organization definition. Findings From 19th of March to 7th of April 2020, 43 patients were included. Fourteen patients (33%) were in the severe group and twenty-nine patients (67%) in the critical group. Patients in the critical group were all mechanically ventilated. The critical group had significantly higher values of MFI, DBS and PVD in comparison to severe group (respectively, PaCO2: 49 [44–45] vs 36 [33–37] mmHg; p-2; p = 0.033, PVD: 12.5 ± 3.0 vs 10.1 ± 2.4 mm.mm-2; p = 0.020). PPV, HI and TVD were similar between groups Correlation was found between microcirculatory parameters and PaCO2 levels. Conclusion Critical COVID-19 patients under mechanical ventilation seem to have higher red blood cell velocity than severe non-ventilated patients.

Abstract

International audience; Rationale: Invasive tracheobronchial aspergillosis (ITBA) is an uncommon but severe clinical form of invasive pulmonary aspergillosis in which the fungal infection is entirely or predominantly confined to the tracheobronchial tree.Objectives: To analyze the diagnostic and prognostic differences between tracheobronchial aspergillosis and pulmonary aspergillosis without tracheobronchial lesions among patients admitted to the ICU with severe influenza.Methods: This retrospective, observational study included critically ill patients with influenza associated with pulmonary aspergillosis from three hospital ICUs between 2010 and 2019. Patient characteristics and clinical and mycologic data at admission and during ICU stay were collected in a database to evaluate variables in the two groups.Measurements and Main Results: Thirty-five patients admitted to the ICU with severe influenza and pulmonary aspergillosis were included. Ten patients were included in the group with ITBA (n = 10 of 35; 28.6%), and 25 patients were included in the group without ITBA. The group with ITBA comprised more patients with active smoking, diabetes mellitus, and higher severity scores (Simplified Acute Physiology Score II). Ninety-day mortality rates in the groups with and without ITBA were 90% and 44%, respectively (P = 0.02). Abstract

Coronavirus disease-19 (COVID-19)-related severe acute respiratory distress syndrome can lead to acute cor pulmonale. We report a case of acute cor pulmonale secondary to severe COVID-19 acute respiratory distress syndrome diagnosed with transesophageal echocardiography. Almitrine infusion allowed rapid enhancement of right ventricular function as well as improvement in oxygenation. (Level of Difficulty: Intermediate.).

17. Facteurs pronostiques de la survie des patients trachéotomisés dans le service de pneumologie et réanimation respiratoire du CHU Amiens-Picardie

Abstract

Introduction La tracheotomie est une technique connue depuis 3500 ans mais dont les indications changent du fait des progres de la reanimation et de l’essor de la ventilation non invasive dans la prise en charge des insuffisants respiratoires. Peu d’etudes recentes ont evalue le pronostic a long terme de la tracheotomie chez les patients souffrant d’une pathologie respiratoire obstructive ou restrictive. Methodes Nous avons realise une etude retrospective sur une periode de 20 ans (1997–2017) concernant 165 patients tracheotomises au cours de leur sejour dans le service de pneumologie et reanimation respiratoire du CHU Amiens-Picardie. L’objectif principal etait l’evaluation de la survie globale des patients. Les objectifs secondaires etaient : l’evaluation de la mortalite en reanimation et intra-hospitaliere, l’analyse des complications liees a la tracheotomie et l’evaluation de la qualite de vie et de la dependance respiratoire des patients. Resultats Parmi les 165 patients, 73 (44,2 %) presentaient une insuffisance respiratoire chronique (35 obstructifs, 38 restrictifs dont 10 pathologies neuromusculaires). Cent trente-huit (83,6 %) patients sont decedes dont 61 (37 %) en intra-hospitalier. La mediane de survie etait de 566 jours. En analyse multivariee selon le modele de Cox, 4 facteurs pronostiques etaient mis en evidence : l’existence d’une comorbidite cardiaque (OR = 1,866 [1,255–2,783] ; p = 0,002), l’âge superieur a 70 ans (OR = 1,831 [1,128–2,974] ; p = 0,014) et l’existence d’une insuffisance respiratoire chronique obstructive (OR = 1,552 [1,014–2,377] ; p = 0,043) etaient significativement associes au risque de deces. L’existence d’une myopathie etait significativement associee a un meilleur pronostic (OR = 0,421 [0,184–0,963] ; p = 0,041). Des complications respiratoires ou hemorragiques etaient retrouvees respectivement chez 31 (18,8 %) et 13 (7,9 %) patients. Quatre deces (2,4 %) etaient lies a ces complications. Parmi les 104 patients sortis vivants d’hospitalisation, le sevrage ventilatoire avait ete obtenu chez 59 patients (56,7 %). Conclusion Nous avons mis en evidence 3 facteurs pronostiques pejoratifs (comorbidite cardiaque, âge superieur a 70 ans et insuffisance respiratoire chronique obstructive) et 1 facteur protecteur (existence d’une myopathie) significativement associes a la survie a long terme des patients tracheotomises. Le pronostic de ces patients reste sombre et l’amelioration de leur qualite de vie necessite une prise en charge pluridisciplinaire.

Abstract

BackgroundData on microcirculatory pattern of COVID-19 critically ill patients are scarce. The objective was to compare sublingual microcirculation parameters of critically ill patients according to the severity of the disease.MethodsThe study is a single-center prospective study with critically ill COVID-19 patients admitted in ICU. Sublingual microcirculation was assessed by IDF microscopy within 48 hours of ICU admission. Microcirculatory flow index (MFI), proportion of perfused vessel (PPV), total vessel density (TVD), De Backer score (DBS), perfused vessel density (PVD) and heterogeneity index (HI) were assessed. Patients were divided in 2 groups (severe and critical) according to the World health organization definition.FindingsFrom 19th of March to 7th of April 2020, 43 patients were included. Fourteen patients (33%) were in the severe group and twenty-nine patients (67%) in the critical group. Patients in the critical group were all mechanically ventilated. The critical group had significantly higher values of MFI, DBS and PVD in comparison to severe group (respectively, PaCO2: 49 [44-45] vs 36 [33-37] mmHg; pConclusionCritical COVID-19 patients under mechanical ventilation seem to have higher red blood cell velocity than severe non-ventilated patients.

Abstract

International audience; Abstract Background The COVID-19 pandemic is a heavy burden in terms of health care resources. Future decision-making policies require consistent data on the management and prognosis of the older patients (> 70 years old) with COVID-19 admitted in the intensive care unit (ICU). Methods Characteristics, management, and prognosis of critically ill old patients (> 70 years) were extracted from the international prospective COVID-ICU database. A propensity score weighted-comparison evaluated the impact of intubation upon admission on Day-90 mortality. Results The analysis included 1199 (28% of the COVID-ICU cohort) patients (median [interquartile] age 74 [72–78] years). Fifty-three percent, 31%, and 16% were 70–74, 75–79, and over 80 years old, respectively. The most frequent comorbidities were chronic hypertension (62%), diabetes (30%), and chronic respiratory disease (25%). Median Clinical Frailty Scale was 3 (2–3). Upon admission, the PaO 2 /FiO 2 ratio was 154 (105–222). 740 (62%) patients were intubated on Day-1 and eventually 938 (78%) during their ICU stay. Overall Day-90 mortality was 46% and reached 67% among the 193 patients over 80 years old. Mortality was higher in older patients, diabetics, and those with a lower PaO 2 /FiO 2 ratio upon admission, cardiovascular dysfunction, and a shorter time between first symptoms and ICU admission. In propensity analysis, early intubation at ICU admission was associated with a significantly higher Day-90 mortality (42% vs 28%; hazard ratio 1.68; 95% CI 1.24–2.27; p
