2. Clinical Outcome Predictive Value of Procalcitonin in Patients Suspected with Infection in the Emergency Department

Procalcitonin (PCT) may be useful for early risk stratification in the emergency department (ED), but the transposition of published data to routine emergency practice is sometimes limited. An observational retrospective study was conducted in the adult ED of the Reims University Hospital (France). Over one year, 852 patients suspected of infection were included, of mean age 61.7 years (SD: 22.6), and 624 (73.2%) were hospitalized following ED visit. Overall, 82 (9.6%) patients died during their hospitalization with an odds ratio (OR) of 5.10 (95% CI: 2.19–11.87) for PCT ≥ 0.5, in multivariate logistic regression analyses. Moreover, 78 (9.2%) patients were admitted to an ICU, 74 (8.7%) had attributable bacteremia, and 56 (6.6%) evolved toward septic shock with an OR of 4.37 (2.08–9.16), 6.38 (2.67–15.24), and 6.38 (2.41–16.86), respectively, for PCT ≥ 0.5. Tourism is one of the fastest growing industries in the world. Its rapid expansion has been considered as an interesting possibility for sustainable development including poverty reduction in many countries. Its importance as a growth driver derives from its potential towards exploitation of economies of scale, relieving the foreign exchange constraint, raising efficiency through increased competition, promoting the diffusion of technical knowledge, as an important source of income, employment and wealth. The Zimbabwean tourism sector which has been identified as one of pillars of economic growth in the Medium term Plan (2012-2015) has undergone tremendous changes over the year. These changes reveal that the tourism sector in the country has some form of resilience to challenges that may affect other economic sectors. The study evaluates the major factors underpinning the resilience of the tourism sector and its contribution to the broader macroeconomic aggregates in comparison to the other economic indicators. A descriptive approach is used in the study to describe the data and characteristics of the tourism sector. This methodology is employed when researchers wants to gain a better understanding of a topic, and involve documentary review and descriptive data analysis to explain the phenomena. A comparative analysis was done with other regional countries to determine the position of the sector in the region and also to draw lessons from those countries with a view of proffering solutions on how the industry can spur growth and economic development in the country.

5. The Hairdresser of Harare, Questioning Gender and Sexuality in a Zimbabwean Novel

As gay characters have been rather rare in Zimbabwean literature, Tendai Huchu’s first novel, The Hairdresser of Harare attracted a lot of attention when it was published in 2010. The story of Vimbai, a single mother who dreams of owning her own salon, and Dumisani, her friend who tries to hide his sexual preferences, thus works as a catalyst to explore different aspects of a society in crisis. For those reasons, the author has been both praised and criticized. In this article, I argue that, in the novel, the depiction of homosexuality is closely related to the exploration of popular literary genres such as the romance novel. The subversion of “chick lit,” for instance, allows Tendai Huchu to question fixed gender categories and sexuality. The author reveals the violence Dumisani undergoes by using several conflicting narratives that try to depict and analyze homosexuality.

King Shaka, the founding father of the Zulu empire, has been an inspiration for African authors, whether they write in French or in English. Among all those rewritings of History, the Congolese poet Tchicaya U Tam’si stands out as he bases his work on a South African novel by Thomas Mofolo to write first a radio series (1957), then an actual play, Le Zulu, performed in Avignon twenty years later (1976). Although Tchicaya’s work for the radio has not got much attention yet, it seems to be an important part of the creative process. This serialization of the novel gives us a unique insight into the appropriation of the character by a major francophone writer.

La revendication d’un domaine de la « communication publique » constitue une forme d’aboutissement d’un long processus de valorisation d’un domaine de compétence et d’exercice distinct d’autres professions proches et de reconnaissance d’une spécificité et d’un savoir-faire qui justifierait la revendication d’une labellisation en tant que profession. C’est en effet sous les termes de « communication publique » et de « communicants publics » que l’on désigne aujourd’hui en France un secteur d’activités pour l’essentiel constitué de professionnels rattachés aux institutions politiques et administratives. En combinant démarche compréhensive et objectivation, nous reviendrons, dans ce travail, sur les conditions sociopolitiques qui ont permis de poser progressivement, en France, les bases de reconnaissance de l’existence d’une « nouvelle » profession, pour nous intéresser ensuite aux enjeux de l’institutionnalisation des métiers de la communication publique à travers la contribution de la principale association de « professionnels de la communication publique » (Cap’Com), en mettant l’accent sur la portée et les limites de cette action ainsi que les modèles dont elle s’est inspirée pour son travail de légitimation professionnelle. The identification of a specific field of “Public Communication” marks the end of a long process in which the existence of a set of skills quite distinct from those of other related and/or competing professions has finally been recognised. It also acknowledges the specific area of expertise that justifies its claim to be classed as a “profession”. Indeed, in France today the terms “public communication” and “public communicator” are used to denote a sector of the economy that is composed mainly of professional people working for political and administrative institutions. Approaching the subject comprehensively and objectively, this study examines the socio-political conditions that laid the foundations in France for the gradual recognition of a “new” profession. We then consider the issues surrounding the institutionalisation of public communication professions by looking at the contribution made by the principal association of “public communications professionals” (Cap’Com), while emphasising the extent and limits of its activities and the models that provided the inspiration for its work in placing the profession on a legitimate footing.

8. A Class III Histidine Kinase Acts as a Novel Virulence Factor in Botrytis cinerea

Filamentous ascomycetes contain large numbers of histidine kinases (HK) that belong to eleven classes. Members of class III from different species were previously shown to be involved in osmoregulation and resistance to dicarboximide and phenylpyrrole fungicides. We have inactivated the gene encoding the single group III HK, BOS1, in the economically important plant pathogen Botrytis cinerea. BOS1 inactivation had pleiotropic effects on the fungus. Besides the expected osmosensitivity and resistance to fungicides, null mutants presented additional characteristics indicating that BOS1 is necessary for normal macroconidiation and full virulence. On standard culture media, null mutants very rarely formed conidiophores and those few conidiophores failed to produce conidia. This defect could be partially restored with 1 M sorbitol, suggesting that another BOS1-independent signal cascade may be involved in macroconidiation. The mutants were not found to be hypersensitive to various oxidative stresses but were more resistant to menadione. Finally, pathogenicity tests showed that bos1-null mutants were significantly reduced in the ability to infect host plants. Appressorium morphogenesis was not altered; however, in planta growth was severely reduced. To our knowledge, this is the first class III HK characterized as a pathogenicity factor in a plant-pathogenic ascomycete.

9. A functional bikaverin biosynthesis gene cluster in rare strains of Botrytis cinerea is positively controlled by VELVET.

The gene cluster responsible for the biosynthesis of the red polyketidic pigment bikaverin has only been characterized in Fusarium ssp. so far. Recently, a highly homologous but incomplete and nonfunctional bikaverin cluster has been found in the genome of the unrelated phytopathogenic fungus Botrytis cinerea. In this study, we provided evidence that rare B. cinerea strains such as 1750 have a complete and functional cluster comprising the six genes orthologous to Fusarium fujikuroi ffbik1-ffbik6 and do produce bikaverin. Phylogenetic analysis confirmed that the whole cluster was acquired from Fusarium through a horizontal gene transfer (HGT). In the bikaverin-nonproducing strain B05.10, the genes encoding bikaverin biosynthesis enzymes are nonfunctional due to deleterious mutations (bcbik2-3) or missing (bcbik1) but interestingly, the genes encoding the regulatory proteins BcBIK4 and BcBIK5 do not harbor deleterious mutations which suggests that they may still be functional. Heterologous complementation of the F. fujikuroi Δffbik4 mutant confirmed that bcbik4 of strain B05.10 is indeed fully functional. Deletion of bcvel1 in the pink strain 1750 resulted in loss of bikaverin and overproduction of melanin indicating that the VELVET protein BcVEL1 regulates the biosynthesis of the two pigments in an opposite manner. Although strain 1750 itself expresses a truncated BcVEL1 protein (100 instead of 575 aa) that is nonfunctional with regard to sclerotia formation, virulence and oxalic acid formation, it is sufficient to regulate pigment biosynthesis (bikaverin and melanin) and fenhexamid HydR2 type of resistance. Finally, a genetic cross between strain 1750 and a bikaverin-nonproducing strain sensitive to fenhexamid revealed that the functional bikaverin cluster is genetically linked to the HydR2 locus.

10. Functional and structural comparison of pyrrolnitrin- and iprodione-induced modifications in the class III histidine-kinase Bos1 of Botrytis cinerea.

Dicarboximides and phenylpyrroles are commonly used fungicides against plant pathogenic ascomycetes. Although their effect on fungal osmosensing systems has been shown in many studies, their modes-of-action still remain unclear. Laboratory- or field-mutants of fungi resistant to either or both fungicide categories generally harbour point mutations in the sensor histidine kinase of the osmotic signal transduction cascade.In the present study we compared the mechanisms of resistance to the dicarboximide iprodione and to pyrrolnitrin, a structural analogue of phenylpyrrole fungicides, in Botrytis cinerea. Pyrrolnitrin-induced mutants and iprodione-induced mutants of B. cinerea were produced in vitro. For the pyrrolnitrin-induced mutants, a high level of resistance to pyrrolnitrin was associated with a high level of resistance to iprodione. For the iprodione-induced mutants, the high level of resistance to iprodione generated variable levels of resistance to pyrrolnitrin and phenylpyrroles. All selected mutants showed hypersensitivity to high osmolarity and regardless of their resistance levels to phenylpyrroles, they showed strongly reduced fitness parameters (sporulation, mycelial growth, aggressiveness on plants) compared to the parental phenotypes. Most of the mutants presented modifications in the osmosensing class III histidine kinase affecting the HAMP domains. Site directed mutagenesis of the bos1 gene was applied to validate eight of the identified mutations. Structure modelling of the HAMP domains revealed that the replacements of hydrophobic residues within the HAMP domains generally affected their helical structure, probably abolishing signal transduction. Comparing mutant phenotypes to the HAMP structures, our study suggests that mutations perturbing helical structures of HAMP2-4 abolish signal-transduction leading to loss-of-function phenotype. The mutation of residues E529, M427, and T581, without consequences on HAMP structure, highlighted their involvement in signal transduction. E529 and M427 seem to be principally involved in osmotic signal transduction.

Forty three Chilean Botrytis cinerea isolates of different fenhexamid sensitivities, obtained from table grapes, were phenotypically analyzed and sequenced for the erg27 gene that encodes the 3-ketoreductaseenzyme. Fifteen isolates were highly resistant to fenhexamid (HydR3+) with conidial germination EC50values >5 μg·mL-1 and colony growth EC50 values >2 μg·mL-1. Five isolates had slight to moderate resistance levels (HydR3-) with conidial germination EC50 values between 0.7 and 2.6 μg·mL-1 and colony growth EC50 values between 0.4 and 3 μg·mL-1. Botrytis bunch rot of grapes is mainly controlled by applying fungicides at three crop stages: the end of flowering (BBCH 68), bunch closure (BBCH 77) and the beginning of veraison (BBCH 81). The phenylpyrroles derivative fudioxonil is among the most effective fungicides registered to control Botrytis cinerea. Its effectiveness was investigated in relation to spray timing, fungicide resistance and defence responses of grapevine. Frequencies of B. cinerea strains which were resistant to fungicides were evaluated at harvest. The frequencies of resistant phenotypes were similar in all treatments except for a class of multidrug resistant strains (MDR 1) whose frequency increased after fudioxonil applications. None of the treatments tested induced defence responses in flowers/berries after fungicide application, suggesting that fudioxonil effectiveness was not related to a stimulation of plant defence processes. The standard program of three fungicide applications provided the best control of B. cinerea in the Champagne region in comparison with a single treatment of fudioxonil at any of the crop stages tested.

The grey mould fungus Botrytis cinerea causes losses of commercially important fruits, vegetables and ornamentals worldwide. Fungicide treatments are effective for disease control, but bear the risk of resistance development. The major resistance mechanism in fungi is target protein modification resulting in reduced drug binding. Multiple drug resistance (MDR) caused by increased efflux activity is common in human pathogenic microbes, but rarely described for plant pathogens. Annual monitoring for fungicide resistance in field isolates from fungicide-treated vineyards in France and Germany revealed a rapidly increasing appearance of B. cinerea field populations with three distinct MDR phenotypes. All MDR strains showed increased fungicide efflux activity and overexpression of efflux transporter genes. Similar to clinical MDR isolates of Candida yeasts that are due to transcription factor mutations, all MDR1 strains were shown to harbor activating mutations in a transcription factor (Mrr1) that controls the gene encoding ABC transporter AtrB. MDR2 strains had undergone a unique rearrangement in the promoter region of the major facilitator superfamily transporter gene mfsM2, induced by insertion of a retrotransposon-derived sequence. MDR2 strains carrying the same rearranged mfsM2 allele have probably migrated from French to German wine-growing regions. The roles of atrB, mrr1 and mfsM2 were proven by the phenotypes of knock-out and overexpression mutants. As confirmed by sexual crosses, combinations of mrr1 and mfsM2 mutations lead to MDR3 strains with higher broad-spectrum resistance. An MDR3 strain was shown in field experiments to be selected against sensitive strains by fungicide treatments. Our data document for the first time the rising prevalence, spread and molecular basis of MDR populations in a major plant pathogen in agricultural environments. These populations will increase the risk of grey mould rot and hamper the effectiveness of current strategies for fungicide resistance management.

We study graph weights (i.e., graph invariants) which arise naturally in Mayer's theory and Ree-Hoover's theory of virial expansions in the context of a non-ideal gas. We give special attention to the Second Mayer weight $w_M(c)$ and the Ree-Hoover weight $w_{RH}(c)$ of a $2$-connected graph $c$ which arise from the hard-core continuum gas in one dimension. These weights are computed using signed volumes of convex polytopes naturally associated with the graph $c$. Among our results are the values of Mayer's weight and Ree-Hoover's weight for all $2$-connected graphs $b$ of size at most $8$, and explicit formulas for certain infinite families.

The Black Insider was published in 1992 as the first of three volumes dedicated to the posthumous works of Dambudzo Marechera. This text actually appears to be one of the versions rejected by editors prior to the acceptance of Black Sunlight (1980). Through the editing work and close analysis of genetic material consisting of several manuscripts including a poetic sequence due to conclude Black Sunlight, this article aims at finding a new angle to assess Marechera’s prose and his creative process. In order to get a better grasp on the elements at stake, we have decided to focus on the evolution of one character, Owen, who changes his name and status to become Nick in Black Sunlight.

As a playwright, the congolese writer Tchicaya U Tam'si (1931-1988) has only authored three plays : Le Zulu, Le Destin glorieux du Maréchal Nnikon-Nniku prince qu'on sort and Le Bal de Ndinga. Nonetheless, many theatrical adaptations of his novels, short stories and poems reveal both Congolese and French artists wished to re-appropriate the legacy left by U Tam'si they used to call "the father of our dream". The analysis of three performances based upon U Tam'si's writings and put on in Brazzaville and Pointe-Noire shows how the stage transforms the texts and how the real author becomes a fiction character.

In older adults, prognostic performances of admission biomarkers have been poorly investigated. This study aims to compare the prognostic abilities of usual admission biomarkers, especially PCT and CRP, for major clinical outcomes, comparing older to younger adults diagnosed with an infection in the ED, and to investigate the prognostic abilities of PCT and CRP depending on the glomerular filtration rate (GFR).It was an observational, single-center, retrospective study, conducted in the Reims University Hospital, France. Endpoints were bacteremia, septic shock, and in-hospital mortality, related to the same ED visit.Over 1 year, 852 patients were included with 291 (34.2%) ≥75 years, and 127 (15.3%) patients had a GFR30 mL.minCaution must be taken when interpreting admission biomarkers, as their prognostic abilities are lower in older adults or in patients with renal insufficiency diagnosed with an infection.

International audience; In modern cropping systems, the near-universal use of plant protection products selects for resistance in pest populations. The emergence and evolution of this adaptive trait threaten treatment efficacy. We identified determinants of fungicide resistance evolution and quantified their effects at a large spatiotemporal scale. We focused on Zymoseptoria tritici, which causes leaf blotch in wheat. Phenotypes of qualitative or quantitative resistance to various fungicides were monitored annually, from 2004 to 2017, at about 70 sites throughout 22 regions of France (territorial units of 25000km2 on average). We modelled changes in resistance frequency with regional anti-Septoria fungicide use, yield losses due to the disease and the regional area under organic wheat. The major driver of resistance dynamics was fungicide use at the regional scale. We estimated its effect on the increase in resistance and relative apparent fitness of each resistance phenotype. The predictions of the model replicated the spatiotemporal patterns of resistance observed in field populations (R2 from 0.56 to 0.82). The evolution of fungicide resistance is mainly determined at the regional scale. Background In adults, the most common cause of out-of-hospital cardiac arrests (OHCA) is acute coronary artery occlusion. If an immediate coronary angiogram (CAG) is recommended for survivors presenting a ST segment elevation on the electrocardiogram (ECG) performed after resuscitation, there is still a debate regarding the best strategy in patients without ST segment elevation. Hypothesis Performing an immediate CAG after an OHCA without ST segment elevation on the post-resuscitation ECG and no obvious non-cardiac cause of arrest could lead to a better 180-day survival rate with no or minimal neurological sequel as compared with a delayed CAG performed 48 to 96 hours after the arrest. Design The EMERGE trial is a prospective national, randomized, open and parallel group trial, in which 970 survivors of OHCA will be randomized (1:1) to either immediate (as soon as possible after return of spontaneous circulation) or delayed (48 to 96 h) CAG. Participants will be OHCA patients with no ST segment elevation on the post resuscitation ECG and no obvious non-cardiac cause of arrest. The primary endpoint of the study is the 180-day survival rate with no or minimal neurological sequel corresponding to Cerebral Performance Category (CPC) 1 or 2. The secondary endpoints are: occurrence of shock during the first 48 hours, ventricular tachycardia and/or fibrillation during the first 48 hours, change in left ventricular ejection fraction between baseline and 180 days assessed by echocardiogram, neurological status evaluated by the CPC score at intensive care unit (ICU) discharge and day 90 neurological status assessed by the Glasgow Outcome Scale Extended score (GOSE) at 90 and 180 days, overall survival rate, and hospital length of stay. Summary The EMERGE trial is a prospective, multicenter, randomized, controlled trial that will assess the 180-day survival rate with no or minimal neurologic sequel in patients resuscitated from an OHCA without ST segment elevation and who will be managed with either immediate or delayed CAG.

La présente étude vise à dresser un survol des principaux constats tirés des revues systématiques et des méta-analyses portant sur l’effet du leadership sur la santé mentale et le mieux-être au travail, et de proposer une analyse critique de leur application au contexte des travailleurs expatriés. Les bases de données Medline, EMBASE, EBM et Web of Knowledge ont été consultées avec des mots-clés spécifiques au leadership, à la santé mentale et au mieux-être. Les résultats montrent que malgré l’augmentation des études examinant les effets du leadership sur la santé mentale en contexte organisationnel, la problématique demeure sous-étudiée en gestion internationale des ressources humaines. À cet effet, des recommandations sont formulées afin que les superviseurs soient mieux outillés pour répondre aux situations dans un contexte où leurs subordonnés sont déployés à l’étranger.

Introduction: In adults, the most common cause of out-of-hospital cardiac arrests (OHCA) is coronary artery disease. If an immediate coronary angiogram (CAG) is recommended for survivors presenting a ST segment elevation (ST +) on the electrocardiogram (ECG) performed after resuscitation, there is still a debate regarding the best strategy in patients without ST +. Hypothesis: Performing an immediate CAG after an OHCA without ST + on the post-resuscitation ECG and no obvious non-cardiac cause of arrest leads to a better outcome. Methods: The EMERGE trial is a multicenter, randomized, controlled trial that assessed the 180-day survival rate with no or minimal neurologic sequel in patients resuscitated from an OHCA without ST + randomized (1:1) to either immediate or delayed (48 to 96h) CAG. The primary endpoint of the study is the 180-day survival rate with no or minimal neurological sequel (Cerebral Performance Category (CPC) 1 or 2). The secondary endpoints are: occurrence of shock during the first 48 hours, ventricular tachycardia and/or fibrillation during the first 48 hours, change in left ventricular ejection fraction between baseline and 180 days assessed by echocardiogram, neurological status evaluated by the CPC scale at intensive care unit (ICU) discharge and day 90 neurological status assessed by the Glasgow Outcome Scale Extended score (GOSE) at 90 and 180 days, overall survival rate, and hospital length of stay. Results: 279 patients were enrolled, 141 in the immediate and 138 in the delayed CAG group. Mean age was 65 and 195 were males. The mean delays between randomization and CAG were 0.6 ± 3.7 hours and 55.1 ± 37.2 hours in the immediate and delayed CAG group respectively. The 180-day survival rates among patients with CPC 1 or 2 were 34.1% and 30.7% in the immediate and delayed CAG group, respectively (hazard ratio: 0.87 [95% CI, 0.65; 1.15], P=0.324). The 180-day survival rates in the whole population were 36.2% and 33.3% in the immediate and delayed CAG group, respectively (HR: 0.86 [95% CI, 0.64-1.15], P=0.308). Conclusions: In patients successfully resuscitated after an OHCA without ST +, a strategy of immediate CAG was not found to be better than a strategy of delayed CAG with respect to 180-day survival rate with no or minimal neurological sequel.

Introduction Initial procalcitonin (PCT) levels may fail in mortality and septic shock prediction and raise cost-effectiveness issues. Since measurement of lactate, C-reactive protein (CRP), white blood cells and neutrophils is common in the emergency department (ED), we compared prediction abilities of these biomarkers to PCT. Methods From January 1st to December 31st, 2018, an observational, single center, retrospective study was conducted in the adult ED of the Reims University Hospital (France). Endpoints were bacteremia, septic shock, and in-hospital mortality, related to the same ED visit. Results Over one year, 459 patients suspected with infection were included, of mean age 60.4 years (SD: 22.0), with 50.8% male, and 364 (79.3%) were hospitalized following ED visit. Overall, 45 (9.8%) patients had a bacteremia, 39 (8.5%) a septic shock and 54 (11.8%) died during their hospitalization. PCT and CRP showed the best discrimination for bacteremia, with an area under curve (AUC) of 0.68 for PCT and 0.65 for CRP. PCT and lactate showed similar good discriminative power for septic shock, with an AUC of 0.78 for both, and poor discrimination for in-hospital mortality, with an AUC of 0.62 for PCT and 0.69 for lactate. Systolic blood pressure and pulse oximetry showed similar discrimination for septic shock as PCT or lactate, while they showed higher discrimination for in-hospital mortality than PCT. Conclusion Usual admission biomarkers lack clinical utility in predicting septic shock or in-hospital mortality. CRP and PCT are poorly efficient in predicting bacteremia.

International audience; Procalcitonin (PCT) may be useful for early risk stratification in the emergency department (ED), but the transposition of published data to routine emergency practice is sometimes limited. An observational retrospective study was conducted in the adult ED of the Reims University Hospital (France). Over one year, 852 patients suspected of infection were included, of mean age 61.7 years (SD: 22.6), and 624 (73.2%) were hospitalized following ED visit. Overall, 82 (9.6%) patients died during their hospitalization with an odds ratio (OR) of 5.10 (95% CI: 2.19–11.87) for PCT ≥ 0.5, in multivariate logistic regression analyses. Moreover, 78 (9.2%) patients were admitted to an ICU, 74 (8.7%) had attributable bacteremia, and 56 (6.6%) evolved toward septic shock with an OR of 4.37 (2.08–9.16), 6.38 (2.67–15.24), and 6.38 (2.41–16.86), respectively, for PCT ≥ 0.5. BACKGROUND Management of pesticide resistance is a major issue in modern agricultural systems, particularly in the context of the broader challenge of reducing pesticide use. However, such management must be adapted to resistance dynamics, which remains challenging to predict due to its dependence on many biological traits of pests, interactions with the environment and pesticide use. We retrospectively studied the evolution of reported resistances to four modes of action (benzimidazoles, quinone outside inhibitors, sterol demethylation inhibitors and succinate dehydrogenase inhibitors), in French populations of the wheat pathogen Zymoseptoria tritici. RESULTS We used statistical models to analyse the Performance trial network data set (2004-2017; ∼ 70 locations in France yearly). They highlighted contrasting behaviours between phenotypes, for example: (i) stable spatial distributions and colonization front structures over time, and (ii) different frequency growth rates at the national scale and between regions. CONCLUSION We provide a quantitative description of the spatiotemporal patterns of resistance evolution for fungicides with several modes of action. Moreover, we highlight some unexpected resistance dynamics in France, with major differences between the north and south. This complex pattern of resistance evolution in French populations is consistent with previous descriptions of dynamics at the European scale. These results should make it easier to anticipate evolution locally and to improve the management of resistance. © 2019 Society of Chemical Industry.

Background and importance Early identification of the cause of shock is associated with better prognosis. Objective The aim of this study was to explore the performances of an ultrasound protocol (echoSHOCK) to diagnose the cause of shock in the emergency department (ED). Design, settings and participants This was a prospective study performed in two EDs. Included patients were older than 18 years admitted with shock. After routine workup strategy, the suspected cause of shock and the planned treatment were reported. The echoSHOCK protocol, using only B mode, was then performed. After performing echoSHOCK, the investigator reported the same two items. Intervention echoSHOCK protocol that assessed: compressive pericardial effusion (tamponade), right ventricle dilatation and flattening of the septum, left ventricle dimension and systolic function, indices of hypovolemia. Outcomes measure and analysis We defined four different causes for shock (tamponade, acute cor pulmonale, cardiogenic and hypovolemia). The primary endpoint was the degree of agreement of the routine workup and echoSHOCK with an expert panel. Main results 85 patients [mean age of 73 (14) years] were included. Kappa coefficients between routine strategy and echoSHOCK for the cause of shock, with the expert panel were 0.33 (95% CI, 0.26-0.4) and 0.88 (95% CI, 0.83-0.93), respectively. Likewise, for the planned treatment, kappa were 0.21 (95% CI, 0.14-0.28) and 0.9 (95% CI, 0.85-0.94), respectively. The physician's confidence increased from 3.9 (2.1) before echoSHOCK to 9.3 (1.1) after, (P Conclusion This study suggested that echoSHOCK significantly increased the ability to determine the cause of undifferentiated shock in the ED.

International audience; L’objectif de ce dossier est de mettre en évidence la frontière poreuse entre univers en ligne et univers hors ligne que révèle le discours de haine dans les réseaux socio-numériques. Cette porosité se comprend à travers cinq problématiques que ce dossier cherche à étayer : (1) la question des représentations préalables qui nourrissent le discours de haine ; (2) son orientation actionnelle et les actes de langage opérés ; (3) les rapports de force qu’il cristallise ; (4) la question des cibles concernées ; (5) la tension sous-jacente entre discours de haine et liberté d’expression. Dans cette perspective, « en ligne » et « hors ligne » ne sont pas des zones distinctes ou des environnements discursifs différents, mais plutôt des espaces qui s'interpénètrent, compte-tenu des spécificités propres aux différents supports. Les luttes définitionnelles de ce qui est socialement et juridiquement acceptable se jouent hors ligne et, de manière plus brutale peut-être, en ligne, parce qu’exacerbées et polarisées, incitées en cela par les formats courts, l’anonymat et la spontanéité. De ce fait, le discours de haine met à l’épreuve les principes démocratiques de liberté d’expression et d’opinion, en ce qu’il place au centre du jeu la lutte pour le dicible légitime. Quand des groupes s’opposent, c’est autour du pouvoir des mots et de ses formes énonciatives que l’affrontement se déroule ; c’est aussi parce que ce pouvoir de nommer touche à l’identité de soi et d’autrui qu’il est empreint de tant d’affect.

L’objectif de ce dossier est de mettre en evidence la frontiere poreuse entre univers en ligne et univers hors ligne que revele le discours de haine dans les reseaux socio-numeriques. Cette porosite se comprend a travers cinq problematiques que ce dossier cherche a etayer : (1) la question des representations prealables qui nourrissent le discours de haine ; (2) son orientation actionnelle et les actes de langage operes ; (3) les rapports de force qu’il cristallise ; (4) la question des cibles concernees ; (5) la tension sous-jacente entre discours de haine et liberte d’expression. Dans cette perspective, « en ligne » et « hors ligne » ne sont pas des zones distinctes ou des environnements discursifs differents, mais plutot des espaces qui s'interpenetrent, compte-tenu des specificites propres aux differents supports. Les luttes definitionnelles de ce qui est socialement et juridiquement acceptable se jouent hors ligne et, de maniere plus brutale peut-etre, en ligne, parce qu’exacerbees et polarisees, incitees en cela par les formats courts, l’anonymat et la spontaneite. De ce fait, le discours de haine met a l’epreuve les principes democratiques de liberte d’expression et d’opinion, en ce qu’il place au centre du jeu la lutte pour le dicible legitime. Quand des groupes s’opposent, c’est autour du pouvoir des mots et de ses formes enonciatives que l’affrontement se deroule ; c’est aussi parce que ce pouvoir de nommer touche a l’identite de soi et d’autrui qu’il est empreint de tant d’affect.

SummaryResearch rationale: In modern cropping systems, the near-universal use of plant protection products selects for resistance in pest populations. The emergence and evolution of this adaptive trait threaten treatment efficacy. We identified determinants of fungicide resistance evolution and quantified their effects at a large spatiotemporal scale.Methods: We focused onZymoseptoria tritici, which causes leaf blotch in wheat. Phenotypes of qualitative or quantitative resistance to various fungicides were monitored annually, from 2004 to 2017, at about 70 sites throughout 22 regions of France (territorial units of 25 000km2on average). We modelled changes in resistance frequency with regional anti-Septoriafungicide use, yield losses due to the disease and the regional area under organic wheat.Key results: The major driver of resistance dynamics was fungicide use at the regional scale. We estimated its effect on the increase in resistance and relative apparent fitness of each resistance phenotype. The predictions of the model replicated the spatiotemporal patterns of resistance observed in field populations (R2from 0.56 to 0.82).Main conclusion: The evolution of fungicide resistance is mainly determined at the regional scale. This study therefore showed that collective management at the regional scale could effectively complete local actions.

Technology has enabled banks to introduce new products that integrate markets, simplify operations and enable expansion of business at low cost, expand to new markets, take new risks and deepen their markets. Zimbabwe registered significant growth in adoption and diffusion of financial innovations over the past two decades, which coincided with a shift in the structure of credit portfolios of banks, and growth in credit as well as risk appetite. This study empirically evaluates the impact of financial innovations in influencing bank behaviour, specifically, portfolio structure risk appetite and delivery channels of banks in Zimbabwe. The study applied co-relational analysis, Fully Modified OLS and the Dynamic OLS estimation models as well as Autoregressive Granger causality approaches. Empirical results show that technology has the capacity to influence activities of banks in risk management, credit and delivery of banking service in lowincome countries. Precisely, financial innovation influences increase in credit towards previously high-risk areas, compositions of credit portfolios in banks and support growth in number of bank accounts. Causality was found to run from financial innovation to bank behaviour, and only in the long run.

The mainly limitations and problems in test carried out with conventional dead load tribometers are the limits on the loading to a constant load applied by a mass weight and intense uncontrolled vibrations at high loads or high speeds. Nanovea has developed a groundbreaking high load tribometer with an active applied load up to 2000 N and a dual-load control system. In this paper we describe our system and we present the results of the first test carried out and a comparison with the dead load systems giving a clear idea of the improvements granted by the use of a controlled pneumatic load also over the traditional load-controlled techniques.
