9. Designing a resilience-based intervention program for children with cancer and their families: a study protocol.

Abstract

Background: Advances in pediatric oncology have significantly increased survival rates, yet have introduced challenges in managing long-term treatment side effects. This study process introduces an interdisciplinary clinical intervention program rooted in the family resilience framework, aimed at improving wellbeing across the cancer trajectory for children and their families, especially those in Canadian communities far from specialized oncology centers with limited access to resources. Methods: Employing an intervention mapping approach, this program collaboratively involves patients, families, professionals, and researchers. It aims to identify vulnerability factors, establish a logic model of change, and devise comprehensive strategies that include professional interventions alongside selfmanagement tools. These strategies, tailored to address biopsychosocial and spiritual challenges, are adapted to the unique contexts of communities distant from specialized cancer treatment centers. A mixed-methods approach will evaluate program effectiveness. Expected results: Anticipated outcomes include the empowerment of families with self-management tools and professional support, designed to mitigate biopsychosocial and spiritual complications. By addressing the specific needs and limitations of these communities, the program strives to improve the overall health and well-being of both undergoing treatment and survivorship phases. Discussion: By focusing on comprehensive care that includes both professional interventions and self-management, this initiative marks a significant shift toward a holistic, family-centered approach in pediatric oncology care for remote communities. It underlines the necessity of accessible interventions that confront immediate and long-term challenges, aiming to elevate the standard of care by emphasizing resilience, professional support, and family empowerment in underserved areas. Abstract

In Zeami’s treatises, there is no place for what the modern Japanese would call a character (人物 jinbutsu). Instead, Zeami describes human forms (人体 jintai) produced by the actor on stage. Their shapes (姿 sugata/shi) are associated with the metaphors of the flower (花 hana/ka) which represents the beauty of the role on the stage, and of the the seed (種 tane/shu) which refers to its potentiality. Paradoxically, this lively metaphor is based on the artificial performance of a masked actor compared to a puppet (操り ayatsuri). Instead of a transcendental subject, we have an artificial form that will take the freshness and beauty of life through the actor’s mastery., Les traités de Zeami sur le nō n’accordent aucune place à ce que le japonais moderne désignerait comme « personnage » (人物 jinbutsu). À la place de cette notion, nous trouvons des formes humaines en performance (人体 jintai). Ces formes (姿 sugata/shi) sont associées à la double métaphore de la fleur (花 hana/ka) qui en figure la beauté sur scène, et de la semence (種 tane/shu) qui désigne la performance dans sa potentialité. Paradoxalement, cette métaphore pleine de vie de la fleur repose pourtant sur le jeu artificiel d’un acteur masqué comparé à une marionnette (操り ayatsuri). En lieu et place d’un sujet transcendant, nous avons affaire à l’immanence d’une forme dénuée de réalisme, à laquelle le savoir-faire de l’acteur donne la fraîcheur et la beauté de la vie.

Abstract

Neuroimaging studies have described the brain alterations in primary progressive aphasia (PPA) variants (semantic, logopenic, nonfluent/agrammatic). However, few studies combined T1, FDG-PET, and diffusion MRI techniques to study atrophy, hypometabolism, and tract alterations across the three PPA main variants. We therefore explored a large early-stage cohort of semantic, logopenic and nonfluent/agrammatic variants (N = 86) and of 23 matched healthy controls with anatomical MRI (cortical thickness), FDG PET (metabolism) and diffusion MRI (white matter tracts analyses), aiming at identifying cortical and sub-cortical brain alterations, and confronting these alterations across imaging modalities and aphasia variants. In the semantic variant, there was cortical thinning and hypometabolism in anterior temporal cortices, with left-hemisphere predominance, extending toward posterior temporal regions, and affecting tracts projecting to the anterior temporal lobes (inferior longitudinal fasciculus, uncinate fasciculus) and tracts projecting to or running nearby posterior temporal cortices: (superior longitudinal fasciculus, inferior frontal-occipital fasciculus). In the logopenic variant metabolic alterations were more extensive than atrophy affecting mainly the left temporal-parietal junction and extending toward more anterior temporal cortices. Metabolic and tract data were coherent given the alterations of the left superior and inferior longitudinal fasciculus and the left inferior frontal-occipital fasciculus. In the nonfluent/agrammatic variant cortical thinning and hypometabolism were located in the left frontal cortex but Broca's area was only affected on metabolic measures. Metabolic and tract alterations were coherent as reflected by damage to the left uncinate fasciculus connecting with Broca's area. Our findings provide a full-blown statistically robust picture of brain alterations in early-stage variants of primary progressive aphasia which has implications for diagnosis, classification and future therapeutic strategies. They demonstrate that in logopenic and semantic variants patterns of brain damage display a non-negligible overlap in temporal regions whereas they are substantially distinct in the nonfluent/agrammatic variant (frontal regions). These results also indicate that frontal networks (combinatorial syntax/phonology) and temporal networks (lexical/semantic representations) constitute distinct anatomo-functional entities with differential vulnerability to degenerative processes in aphasia variants. Finally, the identification of the specific damage patterns could open an avenue for trans-cranial stimulation approaches by indicating the appropriate target-entry into the damaged language system.

27. Exercise and bariatric surgery: A systematic review and meta-analysis of the feasibility and acceptability of exercise and controlled trial methods

Abstract

This systematic review and meta-analysis assessed the feasibility and acceptability of exercise and controlled trial methods in adults awaiting or having undergone bariatric surgery (BS). Search methods used to identify relevant articles were inclusion of articles identified in a systematic review, new database search of articles published 2019–2021, and hand searching reference lists. Titles/abstracts and full-texts were screened by two reviewers independently against inclusion criteria: adults awaiting or having undergone BS, controlled trial, exercise group compared with a comparison group without exercise. Twenty-eight articles were reviewed; most interventions were supervised, performed after BS, and lasted ≤13 weeks. Pooled data for exercise intervention attendance and dropout rates were 84% (k = 10) and 5% (k = 19), respectively, though possibly misestimated due to poor/selective reporting. Median study and recruitment duration were 18 weeks and 24 months, respectively, with a pooled enrollment rate of 2.5 participants/month. Pooled data for refusal to participate, enrollment, and retention rates were 23% (k = 16), 43% (k = 18), and 87% (k = 26), respectively. Despite the lack of data available in studies included, exercise and controlled trial methods seem feasible and acceptable for adults awaiting or having undergone BS. To better identify methodological or practical challenges, and assess bias, better reporting of feasibility and acceptability indicators is needed in future studies.

Abstract

Background: Regular physical activity (PA) is recommended to optimize weight and health outcomes in patients who have undergone metabolic and bariatric surgery (MBS). However, >70% of patients have low PA levels before MBS that persist after MBS. Although behavioral interventions delivered face-to-face have shown promise for increasing PA among patients who have undergone MBS, many may experience barriers, preventing enrollment into and adherence to such interventions. Delivering PA behavior change interventions via telehealth to patients who have undergone MBS may be an effective strategy to increase accessibility and reach, as well as adherence. Objective: This paper reports the protocol for a study that aims to assess the feasibility and acceptability of the protocol or methods and the Telehealth Bariatric Behavioral Intervention (TELE-BariACTIV). The intervention is designed to increase moderate-to-vigorous intensity PA (MVPA) in patients awaiting bariatric surgery and is guided by a multitheory approach and a patient perspective. Another objective is to estimate the effect of the TELE-BariACTIV intervention on presurgical MVPA to determine the appropriate sample size for a multicenter trial. Methods: This study is a multicenter trial using a repeated (ABAB’A) single-case experimental design. The A phases are observational phases without intervention (A1=pre-MBS phase; A2=length personalized according to the MBS date; A3=7 months post-MBS phase). The B phases are interventional phases with PA counseling (B1=6 weekly pre-MBS sessions; B2=3 monthly sessions starting 3 months after MBS). The target sample size is set to 12. Participants are inactive adults awaiting sleeve gastrectomy who have access to a computer with internet and an interface with a camera. Purpose. To detect changes in optic nerve head (ONH) vascularization in glaucoma patients using spectral-domain OCT angiography (OCT-A). Material and Method. Fifty glaucoma patients and 30 normal subjects were evaluated with OCT-A (AngioVue®, Optovue). The total ONH vessel density and temporal disc vessel density were measured. Clinical data, visual field (VF) parameters, and spectral-domain OCT evaluation (RNFL: retinal nerve fiber layer thickness, GCC: ganglion cell complex thickness, and rim area) were recorded for glaucoma patients. International audience; Dans cette communication, nous souhaitons présenter notre projet de jouer la Médée de Sénèque sous forme de nō. Nous expliquerons d’abord les raisons de ce projet : le nō nous permet de proposer un spectacle total utilisant masque, musique, chant, danse et chœur. En d’autres termes, le nō permet d’aborder la tragédie ancienne non pas comme un simple drame, mais comme une opsis, un spectacle. Nous montrerons que la notion de shodan – en tant qu’unité spectaculaire mobilisant l’ouïe et la vue – peut être utilisée pour jouer une tragédie ancienne. Dans un second temps, nous montrerons comment la Médée de Sénèque peut être jouée sous forme de nō moyennant quelques adaptations : réécriture des scènes, découpage en shodan, musique, masques, distribution des rôles du shite, du waki et du chœur. Enfin nous souhaiterions aborder, en dialogue avec les participants, la faisabilité du projet : public, langue, troupe, production de la pièce…

International audience; According to many scholars, Seneca’s tragedies lack a dramatic structure. Thus, they wouldn’t fit to the stage. However, if we compare Seneca to another tradition, the Japanese noh, plot doesn’t appear necessary for the stage. When comparing two similar plays – Senecas’Medea and the noh Kanawa – we discover that the show doesn’t rely on a drama but rather on a theatrical codification. On the contrary to a realistic drama, the protagonist doesn’t perform a story but rather elements of show based on music and dance. Instead of the scenes, we have what we can call modules: elements of the show that have their own specificity and that work together as visual and aural components.; On a souvent reproché aux tragédies de Sénèque de ne pas être suffisamment dramatiques et, à cause de cela, d’être inadaptées à la scène. La comparaison avec le nō permet de changer cette approche et d’envisager la tragédie sans intrigue. En confrontant deux pièces apparentées par leur histoire – la Médée de Sénèque et le nō Kanawa – nous constatons qu’il s’agit dans les deux cas de spectacles fondés sur la codification de l’espace et du jeu, à l’opposé des théâtres de la représentation. Le spectacle repose non sur un drame abstrait ou sur une psychologie réaliste mais sur des structures attendues. Le nō nous permet de substituer aux scènes, la notion de modules : des formes spectaculaires autonomes et typiques, caractérisées par une forte autonomie et suivant d’une pièce à l’autre un enchaînement codifié.

Abstract

International audience; Entrance monologues are one of the most striking features of the characters in Seneca’s tragedy that we don’t find in his Attic models. Often seen as a psychological parenthesis, this specificity is supposed to create a break in the dramatic action. However, if we go back to the text of those scenes, we can understand them as a theatrical device: they allow the entering character to give life to his mask and prepare his meeting with other characters. Entrance monologues are linked with physical descriptions and create a dramatic tension at every new entrance of a character in the play.; L’usage d’un monologue d’entrée des personnages est l’une des caractéristiques principales de Sénèque par rapport aux auteurs de tragédies attiques. Interprétée comme une parenthèse psychologique, cette spécificité suspendrait le temps dramatique. Cependant, l’examen attentif de ces passages permet de relever un rôle spectaculaire : moments d’animation du masque, ils préparent la rencontre entre un personnage qui entre et un personnage déjà présent. Associé à des descriptions réciproques, le monologue d’entrée crée une tension spectaculaire à chaque nouvelle entrée de rôle.
