Household air pollution (HAP) from cooking with solid fuels used during pregnancy has been associated with adverse pregnancy outcomes. Abstract

Household air pollution from solid cooking fuel use during gestation has been associated with adverse pregnancy and birth outcomes. The Household Air Pollution Intervention Network (HAPIN) trial was a randomized controlled trial of free liquefied petroleum gas (LPG) stoves and fuel in Guatemala, Peru, India, and Rwanda. A primary outcome of the main trial was to report the effects of the intervention on infant birth weight. Here we evaluate the effects of a LPG stove and fuel intervention during pregnancy on spontaneous abortion, postpartum hemorrhage, hypertensive disorders of pregnancy, and maternal mortality compared to women who continued to use solid cooking fuels. Pregnant women (18-34 years of age; gestation confirmed by ultrasound at 9-19 weeks) were randomly assigned to an intervention (n = 1593) or control (n = 1607) arm. Intention-to-treat analyses compared outcomes between the two arms using log-binomial models. Among the 3195 pregnant women in the study, there were 10 spontaneous abortions (7 intervention, 3 control), 93 hypertensive disorders of pregnancy (47 intervention, 46 control), 11 post postpartum hemorrhage (5 intervention, 6 control) and 4 maternal deaths (3 intervention, 1 control). Compared to the control arm, the relative risk of spontaneous abortion among women randomized to the intervention was 2.32 (95% confidence interval (CI): 0.60, 8.96), hypertensive disorders of pregnancy 1.02 (95% CI: 0.68, 1.52), postpartum hemorrhage 0.83 (95% CI: 0.25, 2.71) and 2.98 (95% CI: 0.31, 28.66) for maternal mortality. In this study, we found that adverse maternal outcomes did not differ based on randomized stove type across four country research sites.

Abstract

Traditional gravimetric instruments used for personal air pollution exposure measurements are often cumbersome and noisy and do not offer real‐time assessment capability. The Enhanced Children's MicroPEM (ECM) is a comparatively lightweight and quiet instrument designed to capture both integrated filter‐based gravimetric samples and real‐time continuous (nephelometric) particulate matter with an aerodynamic diameter of 2.5 μm or less (PM2.5) concentrations. We assessed the performance and reliability of collocated ECMs in a subset of pregnant women participating in a randomized controlled trial in Guatemala to test a liquefied petroleum gas (LPG) cookstove (intervention) versus biomass (control) for cooking. We compared average daily (24 h) PM2.5 concentrations for the paired gravimetric (n = 219) and paired adjusted nephelometric (n = 221) samples using Spearman correlation and Bland–Altman agreement; we also assessed reliability based on the intraclass correlation coefficient (ICC) and root mean square error (RMSE). Median PM2.5 gravimetric concentrations were 93.5 μg/m3 (interquartile range (IQR) = 52.6–160.5) and 21.4 μg/m3 (IQR = 12.0–32.0) in the control and LPG intervention groups, respectively; the median adjusted nephelometric concentrations were 83.7 μg/m3 (IQR = 47.6–148.9) and 22.6 μg/m3 (IQR = 17.5–29.7) in the control and LPG groups, respectively. Spearman correlations were higher in the control arm (0.91) than in the LPG intervention arm (0.67) in the gravimetric comparisons. The same trend was observed for adjusted nephelometric measurements in the control (0.92) and intervention arms (0.75). ICC values were high in both gravimetric (0.93) and nephelometric (0.95) collocations. Small differences in RMSE were observed for the gravimetric (26.69 μg/m3) and nephelometric collocations (31.76 μg/m3). Our findings demonstrate strong reliability between collocated ECMs for both gravimetric and adjusted nephelometric PM2.5 personal exposure samples in rural Guatemala. Abstract

Exposure to household air pollution has been linked to adverse health outcomes among women aged 40–79. Little is known about how shifting from biomass cooking to a cleaner fuel like liquefied petroleum gas (LPG) could impact exposures for this population. We report 24-h exposures to particulate matter (PM2.5), black carbon (BC), and carbon monoxide (CO) among women aged 40 to <80 years participating in the Household Air Pollution Intervention Network trial. 209 participants were randomized to the intervention and received an LPG stove and continuous fuel supply; controls used biomass (n= 209). Exposures were measured up to six times; we used mixed-effects models to estimate differences between intervention and control groups. Preintervention exposures between groups were comparable; median postintervention exposures were 62% (76.3 vs 29.3 μg/m3), 73% (10.4 vs 2.8 μg/m3), and 57% (1.4 vs 0.6 ppm) lower for PM2.5, BC, and CO among LPG users than for controls. Reductions were similar across countries; 70% of PM2.5exposures after intervention were below the annual WHO interim target I (IT-1) value of 35 μg/m3. We provide evidence that implementing an LPG intervention can reduce air pollution exposure over an 18-month period to at or below the annual WHO IT-1 guideline.

Introduction Globally, nearly 3 billion people burn solid fuels (e.g., wood, dung, charcoal) in inefficient and poorly vented combustion devices (i.e., open fires, traditional stoves) to meet daily cooking needs [...], Background: High quality personal exposure data is fundamental to understanding the health implications of household energy interventions, interpreting analyses across assigned study arms, and characterizing exposure-response relationships for household air pollution. This paper describes the exposure data collection for the Household Air Pollution Intervention Network (HAPIN), a multicountry randomized controlled trial of liquefied petroleum gas stoves and fuel among 3,200 households in India, Rwanda, Guatemala, and Peru. Objectives: The primary objectives of the exposure assessment are to estimate the exposure contrast achieved following a clean fuel intervention and to provide data for analyses of exposure-response relationships across a range of personal exposures. Methods: Exposure measurements are being conducted over the 3-y time frame of the field study. We are measuring fine particulate matter [PM Conclusions: The tools and approaches being used for HAPIN to estimate personal exposures build on previous efforts and take advantage of new technologies. Abstract

There is limited data on personal exposure to fine particulate matter (PM2.5) for mothers in informal settlements in Africa. Identifying and characterizing the sources of exposure to PM2.5for this population is key to finding potential solutions for mitigating air pollution exposures. As part of the USAID-funded Kuboresha Afya Mitaani Urban Maternal, Newborn and Child Health project in Nairobi, Kenya, this study aimed to (1) characterize and map environments that contribute to fine particulate matter (PM2.5) exposure for mothers and infants and (2) determine which factors drive exposure to PM2.5. Mothers were enrolled and measured for 24 hours with PM2.5monitors and GPS loggers in two informal settlements in Nairobi (Dagoretti and Starehe), with complete data received from 73 participants. Two ambient PM2.5monitors were also installed in each of the respective communities. Time-activity surveys were administered following the monitoring period. Mean daily exposures were 39.9 and 45.5 µg m−3in Dagoretti and Starehe, respectively, with 60% of samples exceeding the annual WHO Annual Interim-1 Target of 35 µg m−3. Normal daily activities such as sleeping, resting, running errands, and visiting with friends constituted over three-quarters of exposure sampling time. These activities were not associated with elevated PM2.5exposures, and generally tracked diurnal ambient patterns (morning and evening peaks). Personal exposures, however, did peak higher than ambient in the evenings when cooking was most common, and cooking with wood or charcoal was associated with higher PM2.5personal exposures (daily mean of 54.2 µg m−3[n= 16], compared to 40.3 µg m−3for those who used gas, liquid fuels or electricity [n= 57]). These results suggest that transitioning households to cleaner fuels, such as electricity, LPG, or ethanol, would be the most promising intervention that could rapidly reduce exposures of mothers in informal settlements where biomass use is common. At the same time, larger-scale sectoral efforts are needed to bring ambient PM2.5concentrations, and thus the overall population exposures, closer to WHO guidelines.

Abstract

BACKGROUND: Exposure to PM2.5 arising from solid fuel combustion is estimated to result in ~2.3 million premature deaths and 91 million lost disability-adjusted life years annually. Interventions attempting to mitigate this burden have had limited success in reducing exposures to levels thought to provide substantive health benefits. OBJECTIVES: This paper reports exposure reductions achieved by a liquified petroleum gas (LPG) stove and fuel intervention for pregnant mothers in the Household Air Pollution Intervention Network (HAPIN) randomized controlled trial. METHODS: The HAPIN trial included 3,195 households primarily using biomass for cooking in Guatemala, India, Peru, and Rwanda. Twenty-four-hour exposures to PM2.5, carbon monoxide (CO), and black carbon (BC) were measured for pregnant women once before randomization into control (푛=1,605) and LPG (푛=1,590) arms and twice thereafter (aligned with trimester). Changes in exposure were estimated by directly comparing exposures between intervention and control arms and by using linear mixed-effect models to estimate the impact of the intervention on exposure levels. RESULTS: Median postrandomization exposures of particulate matter (PM) with aerodynamic diameter =2.5 μm (PM2.5) in the intervention arm were lower by 66% at the first (71.5 vs. 24.1 μg/m³), and second follow-up visits (69.5 vs. 23.7 μg/m³) compared to controls. BC exposures were lower in the intervention arm by 72% (9.7 vs. 2.7 μg/m³) and 70% (9.6 vs. 2.8 μg/m³) at the first and second follow-up visits, respectively, and carbon monoxide exposure was 82% lower at both visits (1.1 vs. 0.2 ppm) in comparison with controls. Exposure reductions were consistent over time and were similar across research locations. DISCUSSION: Postintervention PM2.5 exposures in the intervention arm were at the lower end of what has been reported for LPG and other clean fuel interventions, with 69% of PM2.5 samples falling below the World Health Organization Annual Interim Target 1 of 35 μg/m³. Abstract

The Household Air Pollution Intervention Network (HAPIN) trial is a randomised controlled trial in Guatemala, India, Peru and Rwanda to assess the health impact of a clean cooking intervention in households using solid biomass for cooking. The HAPIN intervention—a liquefied petroleum gas (LPG) stove and 18-month supply of LPG—has significant value in these communities, irrespective of potential health benefits. For control households, it was necessary to develop a compensation strategy that would be comparable across four settings and would address concerns about differential loss to follow-up, fairness and potential effects on household economics. Each site developed slightly different, contextually appropriate compensation packages by combining a set of uniform principles with local community input. In Guatemala, control compensation consists of coupons equivalent to the LPG stove’s value that can be redeemed for the participant’s choice of household items, which could include an LPG stove. In Peru, control households receive several small items during the trial, plus the intervention stove and 1 month of fuel at the trial’s conclusion. Rwandan participants are given small items during the trial and a choice of a solar kit, LPG stove and four fuel refills, or cash equivalent at the end. India is the only setting in which control participants receive the intervention (LPG stove and 18 months of fuel) at the trial’s end while also being compensated for their time during the trial, in accordance with local ethics committee requirements. The approaches presented here could inform compensation strategy development in future multi-country trials.

Abstract

Because people spend a majority of their time indoors, indoor air quality (IAQ) can have a significant impact on human health, safety, productivity, and comfort. Because of the diversity and dynamics of people's indoor activities, it is important to monitor IAQ for each individual. Most existing air-quality sensing systems are stationary or focus on outdoor air quality. In contrast, we propose MAQS, a user-centered mobile sensing system for IAQ monitoring. MAQS users carry portable, indoor location tracking and IAQ sensing devices that provide personalized IAQ information in real time. To improve accuracy and energy efficiency, MAQS incorporates three novel techniques: (1) an accurate temporal n-gram augmented Bayesian room localization method that requires few Wi-Fi fingerprints; (2) an air-exchange-rate-based IAQ sensing method, which measures general IAQ using only C[O.sub.2] sensors; and (3) a zone-based proximity detection method for collaborative sensing, which saves energy and enables data sharing among users. MAQS has been deployed and evaluated through a real-world user study. This evaluation demonstrates that MAQS supports accuratepersonalized IAQ monitoring and quantitative analysis with high energy efficiency. Abstract

Assessment of personal exposure to PM2.5 is critical for understanding intervention effectiveness and exposure-response relationships in household air pollution studies. In this pilot study, we compared PM2.5 concentrations obtained from two next-generation personal exposure monitors (the Enhanced Children MicroPEM or ECM; and the Ultrasonic Personal Air Sampler or UPAS) to those obtained with a traditional Triplex Cyclone and SKC Air Pump (a gravimetric cyclone/pump sampler). We co-located cyclone/pumps with an ECM and UPAS to obtain 24-hour kitchen concentrations and personal exposure measurements. We measured Spearmen correlations and evaluated agreement using the Bland-Altman method. We obtained 215 filters from 72 ECM and 71 UPAS co-locations. Overall, the ECM and the UPAS had similar correlation (ECM ρ = 0.91 vs UPAS ρ = 0.88) and agreement (ECM mean difference of 121.7 µg/m3 vs UPAS mean difference of 93.9 µg/m3 ) with overlapping confidence intervals when compared against the cyclone/pump. When adjusted for the limit of detection, agreement between the devices and the cyclone/pump was also similar for all samples (ECM mean difference of 68.8 µg/m3 vs UPAS mean difference of 65.4 µg/m3 ) and personal exposure samples (ECM mean difference of -3.8 µg/m3 vs UPAS mean difference of -12.9 µg/m3 ). Both the ECM and UPAS produced comparable measurements when compared against a cyclone/pump setup.

Abstract

La resolución 0001 de 2015 establece el registro de sustancias y productos químicos controlados por la Dirección Nacional de Antinarcóticos, en dicha resolución se proponen todos los requerimientos que deben cumplir las empresas que utilizan este tipo de sustancias, sin embargo, no se presentan claramente medidas de prevención de accidentes por su uso, almacenamiento y transporte. Teniendo en cuenta esta necesidad surge la idea de realizar una guía que facilite y reúna la información en cuanto al almacenamiento, manejo y transporte de sustancias y productos químicos controlados, puesto que cada día se deben adoptar métodos que mejoren las condiciones de trabajo de las personas y mitigue la exposición de riesgo que se puedan presentar. En el contenido de esta guía se plantean inicialmente los peligros relacionados con seguridad industrial que pueden afectar al trabajador, a continuación se propone una metodología para el adecuado almacenamiento, posteriormente se plantean recomendaciones para la manipulación de las sustancias, para finalmente proponer diversas actividades lúdicas para el fortalecimiento del aprendizaje temático.
