Abstract

Purpose: to evaluate the use, effectiveness, and adverse events of intravenous brivaracetam (BRV) in status epilepticus (SE). Methods: a retrospective multicentric study involving 24 Italian neurology units was performed from March 2018 to June 2020. A shared case report form was used across participating centres to limit biases of retrospective data collection. Diagnosis and classification of SE followed the 2015 ILAE proposal. We considered a trial with BRV a success when it was the last administered drug prior the clinical and/or EEG resolution of seizures, and the SE did not recur during hospital observation. In addition, we considered cases with early response, defined as SE resolved within 6 h after BRV administration. Results: 56 patients were included (mean age 62 years; 57 % male). A previous diagnosis of epilepsy was present in 21 (38 %). Regarding SE etiology classification 46 % were acute symptomatic, 18 % remote and 16 % progressive symptomatic. SE episodes with prominent motor features were the majority (80 %). BRV was administered as first drug after benzodiazepine failure in 21 % episodes, while it was used as the second or the third (or more) drug in the 38 % and 38 % of episodes respectively. The median loading dose was 100 mg (range 50−300 mg). BRV was effective in 32 cases (57 %). An early response was documented in 22 patients (39 % of the whole sample). The use of the BRV within 6 h from SE onset was independently associated to an early SE resolution (OR 32; 95 % CI 3.39–202; p = 0.002). No severe treatment emergent adverse events were observed. Conclusion: BRV proved to be useful and safe for the treatment of SE. Time to seizures resolution appears shorter when it is administered in the early phases of SE.

Abstract

Introduction: In randomized controlled trials, add-on brivaracetam (BRV) reduced seizure frequency in patients with drug-resistant focal epilepsy. Most real-world research on BRV has focused on refractory epilepsy. The aim of this analysis was to assess the 12-month effectiveness and tolerability of adjunctive BRV when used as early or late adjunctive treatment in patients included in the BRIVAracetam add-on First Italian netwoRk Study (BRIVAFIRST). Methods: BRIVAFIRST was a 12-month retrospective, multicenter study including adult patients prescribed adjunctive BRV. Effectiveness outcomes included the rates of sustained seizure response, sustained seizure freedom, and treatment discontinuation. Safety and tolerability outcomes included the rate of treatment discontinuation due to adverse events (AEs) and the incidence of AEs. Data were compared for patients treated with add-on BRV after 1-2 (early add-on) and ≥ 3 (late add-on) prior antiseizure medications. Results: A total of 1029 patients with focal epilepsy were included in the study, of whom 176 (17.1%) received BRV as early add-on treatment. Abstract

Iron is a fundamental element for virtually all forms of life. Despite its abundance, its bioavailability is limited, and thus, microbes developed siderophores, small molecules, which are synthesized inside the cell and then released outside for iron scavenging. Once inside the cell, iron removal does not occur spontaneously, instead this process is mediated by siderophore-interacting proteins (SIP) and/or by ferric-siderophore reductases (FSR). In the past two decades, representatives of the SIP subfamily have been structurally and biochemically characterized; however, the same was not achieved for the FSR subfamily. Here, we initiate the structural and functional characterization of FhuF, the first and only FSR ever isolated. FhuF is a globular monomeric protein mainly composed by α-helices sheltering internal cavities in a fold resembling the "palm" domain found in siderophore biosynthetic enzymes. Paramagnetic NMR spectroscopy revealed that the core of the cluster has electronic properties in line with those of previously characterized 2Fe-2S ferredoxins and differences appear to be confined to the coordination of Fe(III) in the reduced protein. In particular, the two cysteines coordinating this iron appear to have substantially different bond strengths. In similarity with the proteins from the SIP subfamily, FhuF binds both the iron-loaded and the apo forms of ferrichrome in the micromolar range and cyclic voltammetry reveals the presence of redox-Bohr effect, which broadens the range of ferric-siderophore substrates that can be thermodynamically accessible for reduction. Abstract

Background: In randomized controlled trials, add-on brivaracetam (BRV) reduced seizure frequency in patients with drug-resistant focal epilepsy. Studies performed in a naturalistic setting are a useful complement to characterize the drug profile. Objective: This multicentre study assessed the effectiveness and tolerability of adjunctive BRV in a large population of patients with focal epilepsy in the context of real-world clinical practice. Methods: The BRIVAFIRST (BRIVAracetam add-on First Italian netwoRk STudy) was a retrospective, multicentre study including adult patients prescribed adjunctive BRV. Patients with focal epilepsy and 12-month follow-up were considered. Main outcomes included the rates of seizure‐freedom, seizure response (≥50% reduction in baseline seizure frequency), and treatment discontinuation. The incidence of adverse events (AEs) was also considered. Analyses by levetiracetam (LEV) status and concomitant use of strong enzyme-inducing antiseizure medications (EiASMs) and sodium channel blockers (SCBs) were performed. Results: A total of 1029 patients with a median age of 45years (33–56) was included. At 12 months, 169 (16.4%) patients were seizure-free and 383 (37.2%) were seizure responders. Abstract

Aims: The aim of this study was to search for a molecule selectively expressed by marginal zone (MZ) lymphomas (MZLs), whose diagnosis is currently based on morphological criteria and negativity for markers detectable in other B-cell lymphomas. Methods and results: Two thousand one hundred and four peripheral lymphomas of various types were immunostained with a monoclonal antibody against immunoglobulin superfamily receptor translocation-associated 1 (IRTA1), which recognizes the equivalents of MZ in human lymphoid tissues other than spleen. IRTA1 expression was restricted to extranodal (93%) and nodal MZLs (73%) and to lymphomas with MZ differentiation. Extranodal MZL cells with the strongest IRTA1 expression were usually located adjacent to epithelia, mimicking the IRTA1 expression pattern of normal and acquired mucosa-associated lymphoid tissue (MALT). The cytological features, growth pattern and IRTA1 positivity in nodal MZLs suggest they may derive from IRTA1(+) perifollicular B cells or monocytoid B cells detectable in reactive lymph nodes. Double immunostaining for IRTA1/bcl-6 tracked the colonization of B-cell follicles by MZL cells, and showed modulation of their phenotype (e.g. acquisition of bcl-6) during recirculation through germinal centres. MZL cells differentiating into plasma cells usually lost IRTA1. Conclusions: These results further expand our knowledge of the biology of MZLs, and highlight IRTA1 as the first positive marker for MZLs, enabling more accurate diagnosis of these neoplasms

Abstract

La riflessione pedagogica, nell’indagare le possibilità educative lungo tutto l’arco della vita, richiama alla necessità di analizzare in modo multidisciplinare le questioni attuali per addivenire a progettualità formative di sviluppo efficaci e orientate alla “ecologia integrale”. La rilevanza etico-educativa della nozione di sostenibilità si pone come luogo ermeneutico per provocare la consapevolezza critica della pedagogia. La progettazione pedagogica si confronta con diversi saperi in uno scenario, mutevole come quello attuale, in cui le città rappresentano una prospettiva emblematica di formazione e di confronto nel rispetto dei diritti umani e della custodia del creato. Tra i modelli talora contrapposti di riforma dell’istruzione e divergenti politiche per l’ambiente e per il lavoro, la riflessione sull’educazione è impegnata a prefigurare la trasformazione della società nel tempo futuro e a favorire nelle coscienze individuali quella sensibilità ecologica capace di muovere comportamenti sostenibili e responsabili. Aziende, imprese sociali e nuove professionalità rappresentano ambiti emblematici per dare vita a Human Grids alleanze sul piano della progettazione educativa e di sviluppo. Promuovere un’economia civile implica educare in modo intenzionale a comportamenti non-strumentali ma espressivi di virtù civili, per un futuro contrassegnato da una “crescita umana” durevole, connotata da “cura, attenzione, riguardo e rispetto per l’ambiente.

Abstract

Biogenesis of iron–sulfur cluster proteins is a highly regulated process that requires complex protein machineries. In the cytosolic iron–sulfur protein assembly machinery, two human key proteins—NADPH-dependent diflavin oxidoreductase 1 (Ndor1) and anamorsin—form a stable complex in vivo that was proposed to provide electrons for assembling cytosolic iron–sulfur cluster proteins. The Ndor1–anamorsin interaction was also suggested to be implicated in the regulation of cell survival/death mechanisms. In the present work we unravel the molecular basis of recognition between Ndor1 and anamorsin and of the electron transfer process. This is based on the structural characterization of the two partner proteins, the investigation of the electron transfer process, and the identification of those protein regions involved in complex formation and those involved in electron transfer. We found that an unstructured region of anamorsin is essential for the formation of a specific and stable protein complex with Ndor1, whereas the C-terminal region of anamorsin, containing the [2Fe-2S] redox center, transiently interacts through complementary charged residues with the FMN-binding site region of Ndor1 to perform electron transfer. Our results propose a molecular model of the electron transfer process that is crucial for understanding the functional role of this interaction in human cells.

Abstract

Hodgkin’s lymphoma (HL) is a heterogeneous lymphoid malignancy curable in more than 80% of cases. Its morbility and mortality are mostly contingent upon chemo- and radio-therapy side effects including cardiovascular or pulmonary diseases, infections and second malignancies. Indeed, the high incidence of secondary cancers and leukemias in potentially cured LH patients supports that genomic instability and defective DNA repair system have a crucial role in secondary cancerogenesis/leukemogenesis processes as they let the emergence of genomic aberrations and clonal cell evolution towards a transformed phenotype. The aim of our study was to define the genomic profile associated with HL evolution towards secondary leukemia. To the purpose, formalin-fixed/paraffin-embedded biopsies from 4 LH patients who developed a secondary acute myeloid leukemia (AML) were analyzed by mean of array-based comparative genomic hybridization (aCGH). This method allows the identification of changes in DNA sequence copy number (amplifications or deletions) at high resolution. The microarrays used in our study contain 287 genomic targets involved in most human cancers, including oncogenes, tumor-suppressor genes and DNA sequences localized within chromosomal regions most frequently rearranged. DNAs from lymphonode biopsies of LH patient who developed AML were compared with pooled DNAs from reactive and LH lymphonodes (comparable for histotype and follow-up) from cured patients. CGH profiles of single LH patients were extensively altered. However, they shared a common pattern of structural genomic alterations. Bioinformatic analysis of CGH results performed with dedicated softwares let distinguish statistatically significant (ratio >1,2) amplifications relative to 3 DNA sequences (AFM137XA11, FGFR1, PPABP) and statistatically significant (ratio< 0,83) deletions in 4 (AFM217YD10, FGR(SRC2), GATA3, TOP1, WT1). CGH data relative to the above mentioned sequences were further validated by FISH and immunohistochemistry. In spite of the low number of LH patients our results suggest that the evolution to AML in LH patients may be genetically determined. The role of single genes in leukemic transformation requires further investigation.

Abstract

Photosensitivity is a heritable abnormal cortical response to flickering light, manifesting as particular electroencephalographic changes, with or without seizures. Photosensitivity is prominent in a very rare epileptic encephalopathy due to de novo CHD2 mutations, but is also seen in epileptic encephalopathies due to other gene mutations. We determined whether CHD2 variation underlies photosensitivity in common epilepsies, specific photosensitive epilepsies and individuals with photosensitivity without seizures. We studied 580 individuals with epilepsy and either photosensitive seizures or abnormal photoparoxysmal response on electroencephalography, or both, and 55 individuals with photoparoxysmal response but no seizures. We compared CHD2 sequence data to publicly available data from 34 427 individuals, not enriched for epilepsy. We investigated the role of unique variants seen only once in the entire data set. We sought CHD2 variants in 238 exomes from familial genetic generalized epilepsies, and in other public exome data sets. We identified 11 unique variants in the 580 individuals with photosensitive epilepsies and 128 unique variants in the 34 427 controls: unique CHD2 variation is over-represented in cases overall (P = 2.17 × 10(-5)). Among epilepsy syndromes, there was over-representation of unique CHD2 variants (3/36 cases) in the archetypal photosensitive epilepsy syndrome, eyelid myoclonia with absences (P = 3.50 × 10(-4)). CHD2 variation was not over-represented in photoparoxysmal response without seizures. Zebrafish larvae with chd2 knockdown were tested for photosensitivity. Chd2 knockdown markedly enhanced mild innate zebrafish larval photosensitivity. CHD2 mutation is the first identified cause of the archetypal generalized photosensitive epilepsy syndrome, eyelid myoclonia with absences. Abstract

“Il ruolo dell’insegnante tra metodo e qualità dell’esperienza scolastica” intitola un percorso di ricerca-formazione realizzato tra aprile e ottobre 2011 con gli insegnanti delle scuole dell’infanzia, primarie e secondarie di primo grado del Comune di Cinisello Balsamo (MI). L’obiettivo della ricerca ha riguardato la possibilità di ricostruire, a partire da situazioni scolastiche concrete, in cosa consistesse il “modo di fare scuola” agito quotidianamente da ogni insegnante; come esso incidesse sulla qualità dell’esperienza che allievi e insegnanti vissuta a scuola e quindi come, a partire dalla pratica, si potesse ridefinire ciò che qualifica la professionalità dell’insegnante oggi, rispetto a situazioni cosiddette “di disagio”. Dal punto di vista pedagogico, la ricerca presuppone la specificità dell’esperienza educativa, sottolineandone da un lato la complessità intrinseca, dall’altro la problematicità delle connessioni con l’educazione informale. Inserita nell’ambito delle ricerche pedagogiche empiriche di tipo qualitativo, idiografico e intensivo, la ricerca ha sviluppato un percorso strutturato in gruppo. Riferendosi all’approccio di Clinica della Formazione, è stata animata dalla preoccupazione di consentire ai partecipanti di pervenire a una propria consapevolezza didattico-metodologica e di individuare i propri riferimenti deontologici a partire da una riflessione sull’esperienza agita. Tra i risultati, è stata evidenziata la presenza di una pratica didattico-educativa esperta e in divenire, che, se riesce a fronteggiare situazioni difficili, tuttavia non viene legittimata come “modalità di fare scuola” nel confronto con modelli di insegnamento tradizionali, a cui, pur riconoscendo l’anacronismo rispetto alla situazione attuale, insegnanti e dispositivi scolastici sembrano comunque riferirsi.
