Abstract: HERMES Pathfinder is an in-orbit demonstration consisting of a constellation of six 3U cubesats hosting simple but innovative X-ray/gamma-ray detectors for the monitoring of cosmic high-energy transients. HERMES-PF, funded by ASI and by the EC Horizon 2020 grant, is scheduled for launch in Q1 2025. An identical X-ray/gamma-ray detector is hosted by the Australian 6U cubesat SpIRIT, launched on December 1st 2023. The main objective of HERMES-PF/SpIRIT is to demonstrate that high energy cosmic transients can be detected efficiently by miniatured hardware and localized using triangulation techniques. The HERMES-PF X-ray/gamma-ray detector is made by 60 GAGG:Ce scintillator crystals and 12 2x5 silicon drift detector (SDD) mosaics, used to detect both the cosmic X-rays directly and the optical photons produced by gamma-ray interactions with the scintillator crystals. This design provides a unique broad band spectral coverage from a few keV to a few MeV. Furthermore, the use of fast GAGG:Ce crystals and small SDD cells allows us to reach an exquisite time resolution better than a microsecond. We present a progress report on the missions focusing the discussion on the scientific innovation of the project and on the main lessons learned during the project development including: the importance and the challenges of using distributed architectures to achieve ambitious scientific objectives; the importance of developing critical technologies under science agreements for the realization of high-performing but low-cost payloads; best use of COTS technologies in scientific missions. We finally discuss the prospects of applying these concepts for the creation of an all-sky, all-time monitor to search for the high-energy counterparts of gravitational wave events that Advanced LIGO/Virgo/Kagra will find at the end of this decade and the Einstein Telescope during the 2030s., Comment: proceedings of the 75th International Astronautical Congress (IAC), Milan, Italy, 14-18 October 2024

Abstract: HERMES (High Energy Rapid Modular Ensemble of Satellites) Pathfinder is a space-borne mission based on a constellation of six nano-satellites flying in a low-Earth orbit (LEO). The 3U CubeSats, to be launched in early 2025, host miniaturized instruments with a hybrid Silicon Drift Detector/GAGG:Ce scintillator photodetector system, sensitive to X-rays and gamma-rays in a large energy band. HERMES will operate in conjunction with Australian Space Industry Responsive Intelligent Thermal (SpIRIT) 6U CubeSat, launched in December 2023. HERMES will probe the temporal emission of bright high-energy transients such as Gamma-Ray Bursts (GRBs), ensuring a fast transient localization in a field of view of several steradians exploiting the triangulation technique. HERMES intrinsically modular transient monitoring experiment represents a keystone capability to complement the next generation of gravitational wave experiments. Abstract: Due to be launched in late 2021, the Imaging X-Ray Polarimetry Explorer (IXPE) is a NASA Small Explorer mission designed to perform polarization measurements in the 2-8 keV band, complemented with imaging, spectroscopy and timing capabilities. At the heart of the focal plane is a set of three polarization-sensitive Gas Pixel Detectors (GPD), each based on a custom ASIC acting as a charge-collecting anode. In this paper we shall review the design, manufacturing, and test of the IXPE focal-plane detectors, with particular emphasis on the connection between the science drivers, the performance metrics and the operational aspects. We shall present a thorough characterization of the GPDs in terms of effective noise, trigger efficiency, dead time, uniformity of response, and spectral and polarimetric performance. In addition, we shall discuss in detail a number of instrumental effects that are relevant for high-level science analysis -- particularly as far as the response to unpolarized radiation and the stability in time are concerned., Comment: To be published in Astroparticle Physics

Abstract: The High Energy Rapid Modular Ensemble of Satellites (HERMES) Technological and Scientific pathfinder is a space borne mission based on a constellation of LEO nanosatellites. The payloads of these CubeSats consist of miniaturized detectors designed for bright high-energy transients such as Gamma-Ray Bursts (GRBs). This platform aims to impact Gamma Ray Burst (GRB) science and enhance the detection of Gravitational Wave (GW) electromagnetic counterparts. This goal will be achieved with a field of view of several steradians, arcmin precision and state of the art timing accuracy. The localization performance for the whole constellation is proportional to the number of components and inversely proportional to the average baseline between them, and therefore is expected to increase as more. In this paper we describe the Payload Data Handling Unit (PDHU) for the HERMES-TP and HERMES SP mission. The PDHU is the main interface between the payload and the satellite bus. The PDHU is also in charge of the on-board control and monitoring of the scintillating crystal detectors. We will explain the TM/TC design and the distinct modes of operation. Abstract: The HERMES-TP/SP mission, based on a nanosatellite constellation, has very stringent constraints of sensitivity and compactness, and requires an innovative wide energy range instrument. The instrument technology is based on the "siswich" concept, in which custom-designed, low-noise Silicon Drift Detectors are used to simultaneously detect soft X-rays and to readout the optical light produced by the interaction of higher energy photons in GAGG:Ce scintillators. To preserve the inherent excellent spectroscopic performances of SDDs, advanced readout electronics is necessary. In this paper, the HERMES detector architecture concept will be described in detail, as well as the specifically developed front-end ASICs (LYRA-FE and LYRA-BE) and integration solutions. The experimental performance of the integrated system composed by scintillator+SDD+LYRA ASIC will be discussed, demonstrating that the requirements of a wide energy range sensitivity, from 2 keV up to 2 MeV, are met in a compact instrument., Comment: 12 pages, 10 figures. Abstract: The High Energy Modular Ensemble of Satellites (HERMES) project is aimed to realize a modular X/gamma-ray monitor for transient events, to be placed on-board of a CubeSat bus. This expandable platform will achieve a significant impact on Gamma Ray Burst (GRB) science and on the detection of Gravitational Wave (GW) electromagnetic counterparts: the recent LIGO/VIRGO discoveries demonstrated that the high-energy transient sky is still a field of extreme interest. The very complex temporal variability of GRBs (up to the millisecond scale) combined with the spatial and temporal coincidence between GWs and their electromagnetic counterparts suggest that upcoming instruments require sub-ms time resolution combined with a transient localization accuracy lower than a degree. The current phase of the ongoing HERMES project is focused on the realization of a technological pathfinder with a small network (3 units) of nano-satellites to be launched in mid 2020. We will show the potential and prospects for short and medium-term development of the project, demonstrating the disrupting possibilities for scientific investigations provided by the innovative concept of a new "modular astronomy" with nano-satellites (e.g. low developing costs, very short realization time). Finally, we will illustrate the characteristics of the HERMES Technological Pathfinder project, demonstrating how the scientific goals discussed are actually already reachable with the first nano-satellites of this constellation. The detector architecture will be described in detail, showing that the new generation of scintillators (e.g. GAGG:Ce) coupled with very performing Silicon Drift Detectors (SDD) and low noise Front-End-Electronics (FEE) are able to extend down to few keV the sensitivity band of the detector. Abstract: The rapid advancement of next-generation sequencing (NGS) technology and the decrease in costs for whole-exome sequencing (WES) and whole-genome sequening (WGS), has prompted its clinical application in several fields of medicine. Currently, there are no specific guidelines for the use of NGS in the field of neonatal medicine and in the diagnosis of genetic diseases in critically ill newborn infants. As a consequence, NGS may be underused with reduced diagnostic success rate, or overused, with increased costs for the healthcare system. Most genetic diseases may be already expressed during the neonatal age, but their identification may be complicated by nonspecific presentation, especially in the setting of critical clinical conditions. The differential diagnosis process in the neonatal intensive care unit (NICU) may be time-consuming, uncomfortable for the patient due to repeated sampling, and ineffective in reaching a molecular diagnosis during NICU stay. Serial gene sequencing (Sanger sequencing) may be successful only for conditions for which the clinical phenotype strongly suggests a diagnostic hypothesis and for genetically homogeneous diseases. Newborn screenings with Guthrie cards, which vary from country to country, are designed to only test for a few dozen genetic diseases out of the more than 6000 diseases for which a genetic characterization is available. The use of WES in selected cases in the NICU may overcome these issues. We present an intersociety document that aims to define the best indications for the use of WES in different clinical scenarios in the NICU. We propose that WES is used in the NICU for critically ill newborn infants when an early diagnosis is desirable to guide the clinical management during NICU stay, when a strong hypothesis cannot be formulated based on the clinical phenotype or the disease is genetically heterogeneous, and when specific non-genetic laboratory tests are not available. The use of WES may reduce the time for diagnosis in infants during NICU stay and may eventually result in cost-effectiveness.

Abstract: Launched on 2021 December 9, the Imaging X-ray Polarimetry Explorer (IXPE) is a NASA Small Explorer Mission in collaboration with the Italian Space Agency (ASI). The mission will open a new window of investigation—imaging x-ray polarimetry. The observatory features three identical telescopes, each consisting of a mirror module assembly with a polarization-sensitive imaging x-ray detector at the focus. A coilable boom, deployed on orbit, provides the necessary 4-m focal length. The observatory utilizes a three-axis-stabilized spacecraft, which provides services such as power, attitude determination and control, commanding, and telemetry to the ground. During its 2-year baseline mission, IXPE will conduct precise polarimetry for samples of multiple categories of x-ray sources, with follow-on observations of selected targets.

Abstract: While X-ray Spectroscopy, Timing and Imaging have improved verymuch since 1962, when the first astronomical non-solar source was discovered, especially with the launch of Newton/X-ray Multi-Mirror Mission, Rossi/X-ray Timing Explorer and Chandra/Advanced X-ray Astrophysics Facility, the progress of X-ray polarimetry has been meager. This is in part due to the lack of sensitive polarization detectors, in part due to the fate of approved missions and in part because the celestial X-ray sources appeared less polarized than expected. Only one positive measurement has been available until now. Indeed the eight Orbiting Solar Observatory measured the polarization of the Crab nebula in the 70s. The advent of techniques of microelectronics allowed for designing a detector based on the photoelectric effect in gas in an energy range where the optics are efficient in focusing X-rays. Herewe describe the Instrument, which is the major contribution of the Italian collaboration to the SmallExplorer mission called IXPE, the Imaging X-ray Polarimetry Explorer, which will be flown in late 2021. The instrument, is composed of three Detector Units, based on this technique, and a Detector Service Unit. Three Mirror Modules provided by Marshall Space Flight Center focus X-rays onto the detectors. In the following we will show the technological choices, their scientific motivation and the results from the calibration of the Instrument. IXPE will perform imaging, timing and energy resolved polarimetry in the 2-8 keV energy band opening this window of X-ray astronomy to tens of celestial sources of almost all classes., 25 pages, 12 figures, 5 tables (accepted for publication in Astronomical Journal)

Abstract: HERMES-TP/SP (High Energy Rapid Modular Ensemble of Satellites Technologic and Scientific Pathfinder) is a constellation of six 3U nano-satellites hosting simple but innovative X-ray detectors, characterized by a large energy band and excellent temporal resolution, and thus optimized for the monitoring of Cosmic High Energy transients such as Gamma Ray Bursts and the electromagnetic counterparts of Gravitational Wave Events, and for the determination of their positions. The projects are funded by the Italian Ministry of University and Research and by the Italian Space Agency, and by the European Union Horizon 2020 Research and Innovation Program under Grant Agreement No. 821896. HERMES-TP/SP is an in-orbit demonstration, that should be tested starting from 2022. Abstract: Due to be launched in late 2021, the Imaging X-Ray Polarimetry Explorer (IXPE) is a NASA Small Explorer mission designed to perform polarization measurements in the 2-8 keV band, complemented with imaging, spectroscopy and timing capabilities. At the heart of the focal plane is a set of three polarization-sensitive Gas Pixel Detectors (GPD), each based on a custom ASIC acting as a charge-collecting anode. In this paper we shall review the design, manufacturing, and test of the IXPE focal-plane detectors, with particular emphasis on the connection between the science drivers, the performance metrics and the operational aspects. We shall present a thorough characterization of the GPDs in terms of effective noise, trigger efficiency, dead time, uniformity of response, and spectral and polarimetric performance. In addition, we shall discuss in detail a number of instrumental effects that are relevant for high-level science analysis -- particularly as far as the response to unpolarized radiation and the stability in time are concerned., Comment: To be published in Astroparticle Physics

Conceived as a mini-constellation of six 3U nano-satellites hosting a new miniaturized detector, HERMES-TP/SP aims at the detection and accurate localisation of bright high-energy transients such as Gamma-Ray Bursts. The large energy band, the excellent temporal resolution and the wide field of view that characterize the detectors of the constellation represent the key features for the next generation high-energy all-sky monitor with good localisation capabilities that will play a pivotal role in the future of Multi-messenger Astronomy. In this work, we will describe in detail the temporal techniques that allow the localisation of bright transient events taking advantage of their almost simultaneous observation by spatially spaced detectors. THESEUS is designed to carry on-board two wide and deep sky monitoring instruments for X/gamma-ray transients detection: a wide-field soft X-ray monitor with imaging capability (Soft X-ray Imager, SXI, 0.3 - 5 keV), a hard X-ray, partially-imaging spectroscopic instrument (X and Gamma Imaging Spectrometer, XGIS, 2 keV - 10 MeV), and an optical/near-IR telescope with both imaging and spectroscopic capability (InfraRed Telescope, IRT, 0.7 - 1.8 µm). The spacecraft will be capable of performing fast repointing of the IRT to the error region provided by the monitors, thus allowing it to detect and localize the transient sources down to a few arcsec accuracy, for immediate identification and redshift determination. Abstract: Promoting a healthy lifestyle during the first years of life is a key strategy for controlling obesity risk in later life, having good-quality epidemiological data on eating habits of infants and toddlers can improve awareness and possibly the education given by pediatricians to parents and children. With this aim, we performed a survey about the dietary pattern of Italian children in early childhood. We described the intake of energy, macronutrients and fiber, minerals, and vitamins of 443 Italian children (range 6.4&ndash, 131 months), through a three-day food record filled out by their parents and assessed by family pediatricians. The results were compared with the Italian Dietary Reference Values. The median protein intake, in g/kg per body weight, exceeded the average requirement in all age groups, and in the 12&ndash, 36 month period, the intake as % of energy was outside the reference range (&gt, 15%). The majority of the children consumed quantities of simple carbohydrates (consisting of both natural sugars and free or added sugars, 82.3% of the children in the study) and saturated fats (69% of the children in the study) above the limits of the Italian Dietary Reference Values, with low intake of fiber and polyunsaturated fats. Median mineral intake, in our study, was different depending on age, while vitamin D intake was very low in all age groups. This is one of the few studies reporting on the nutrient intake of Italian children with reference to nutrition recommendations in order to identify the principal nutritional errors. The present results underline the need for healthcare policies starting from the first years of life in order to ameliorate nutrient intake during childhood, possibly impacting long-term health outcomes.

Abstract: GrailQuest (Gamma-ray Astronomy International Laboratory for Quantum Exploration of Space-Time) is an ambitious astrophysical mission concept that uses a fleet of small satellites whose main objective is to search for a dispersion law for light propagation in vacuo. Within Quantum Gravity theories, different models for space-time quantization predict relative discrepancies of the speed of photons w.r.t. the speed of light that depend on the ratio of the photon energy to the Planck energy. This ratio is as small as 10-23 for photons in the γ- ray band (100 keV). Therefore, to detect this effect, light must propagate over enormous distances and the experiment must have extraordinary sensitivity. Gamma-Ray Bursts, occurring at cosmological distances, could be used to detect this tiny signature of space-time granularity. This can be obtained by coherently combine a huge number of small instruments distributed in space to act as a single detector of unprecedented effective area. This is the first example of high-energy distributed astronomy: a new concept of modular observatory of huge overall collecting area consisting in a fleet of small satellites in low orbits, with sub-microsecond time resolution and wide energy band (keV-MeV). The enormous number of collected photons will allow to effectively search these energy dependent delays. Moreover, GrailQuest will allow to perform temporal triangulation of impulsive events with arc-second positional accuracies: an extraordinary sensitive X-ray/Gamma all-sky monitor crucial for hunting the elusive electromagnetic counterparts of Gravitational Waves, that will play a paramount role in the future of Multi-messenger Astronomy. A pathfinder of GrailQuest is already under development through the HERMES (High Energy Rapid Modular Ensemble of Satellites) project: a fleet of six 3U cube-sats to be launched by the end of 2022.

Abstract: Background: Consumers' conviction of the benefits of probiotics is influenced by their existing beliefs and by the information they receive from healthcare professionals. The attitude of healthcare professionals towards commercially available probiotics will, therefore, determine how trustworthy and beneficial these products are perceived by consumers. Furthermore, due to European Union legislation, companies are prohibited from displaying information on product packaging; therefore, consumers are dependent primarily on healthcare professionals for correct information and guidance on the use of these products. The aim of this pilot study was to explore the understanding and use of probiotics in clinical practice by professionals who are involved in child healthcare in different European countries and to assess how much they value the scientific evidence behind these products., Methods: The study was performed using a cross-sectional, descriptive, 30-question online questionnaire circulated among healthcare professionals belonging to three professional categories that are typically involved in childhood probiotic prescription: paediatricians, dieticians and general practitioners. The questionnaire was developed using web-based standard guidelines, and the questions were modelled on those used in previously published probiotics studies., Results: Overall, 27,287 healthcare professionals belonging to three major European scientific societies were contacted by the organizations participating in the study. In total, 1360 valid questionnaires were recorded, and the results were statistically analysed., Conclusions: The results emphasize the importance for healthcare professionals to be properly educated and updated on probiotics. An improved knowledge about probiotics led to increased prescriptive confidence. To disseminate accurate information on probiotics, healthcare professionals look for appropriate and scientifically validated educational platforms to acquire information, explore concerns and barriers and look for positive approaches towards recommending probiotics.

Abstract: This paper describes the Paediatric Ambulatory Consulting Service (PACS) project, developed by ASL-BT (Azienda Sanitaria Locale, Barletta-Andria-Trani), an Italian regional Public Health Centers network, in response to the current global situation of economic distress.PACS consist in integrating existing public health care services that are independently provided by hospitals and the Primary Care Paediatrics network. It has been developed with the aim to establish innovative yet efficient managerial solutions able to rationalize the resources not weakening the quality of services provided to the population.

Abstract: Background: Celiac disease (CD) is a chronic autoimmune illness of the small intestine triggered by gluten consumption in genetically predisposed individuals. CD presentation is not limited to the gastrointestinal tract and it is still under-diagnosed. Complete resolution of clinical manifestations follows if a gluten-free diet is adopted. In western countries, CD prevalence is approximately 1%. Age of onset is often between 6 months and 7 years.We assessed the approach to diagnosis and management of celiac patients by the paediatricians in Puglia Region, Italy., Methods: We conducted a cross-sectional survey among the 589 Apulian Family Paediatricians (FPs) during January 2011-January 2012 using a self-administered web-based standardized questionnaire including self-assessment of their knowledge, diagnostic path and type of management they would follow for CD, clinical information on their celiac patients. We assessed associations among the explored variables by defining double-entry contingency tables and calculating Odds Ratio (OR) with 95% Confidence Intervals (CIs)., Results: The 218 (37%) FPs participating in the study reported 1,020 CD patients (representing approximately 1% of the child population covered by the enrolled FPs). Of them, 55% were female; 45% were aged 5-10 years. Weight loss and stunting were the main reported symptoms at diagnosis (41%). The majority (98%) of FPs requested anti-transglutaminase antibody (tTG-Ab) titres for CD diagnosis. Approximately 78% of FPs recommended gluten introduction in the diet of infants at the age of 6 months; 12% and 8% recommended introduction of gluten before and after 6 months of age respectively.The degree of knowledge for either CD diagnosis making process or CD related diseases was medium/high in 97% and 82% of the participating FPs respectively. FPs (83%) who had a medium or high degree of knowledge of CD patients' diet were more likely to experience low or no difficulty in providing their patients with dietary advices (OR:5.5; 95%CI:1.7-17.5)., Conclusions: Apulian FPs report a good degree of knowledge of CD, its diagnosis and its management. We will diffuse results and recommendations to all paediatricians in the Region. Actions aiming to continued education on CD in medical under and postgraduate trainings are crucial to prevent under-diagnosis.
