Chronic kidney disease (CKD) is a leading cause of mortality and morbidity around the world. The prevalence of CKD increases steadily over the past decade in parallel to the rapid expansion of diabetic population. Apart from increased mortality, CKD also has significant impact on quality of life and the economy. The approach to deal with the global CKD epidemic is multifaceted. Early detection by screening high-risk individuals such as those with hypertension and diabetes is important and cost-effective. However, low CKD awareness in many countries may impose barriers to early intervention. Hence raising CKD awareness among public and policy makers should be encouraged. In addition, the use of peritoneal dialysis, a less costly and home-based dialysis modality compared with in-center haemodialysis, should be promoted to maximize access to dialysis with limited resources. Finally, ongoing research and clinical trials through international collaborations could provide further insight into the pathophysiology of CKD progression, and establish the foundation for development of specific therapeutic agents to retard progression to end stage renal failure.

To investigate the effect of antibiotic lock solutions for preventing catheter-related bacteraemia in patients receiving haemodialysis.Retrospective study.University teaching hospital, Hong Kong.Consecutive patients from March 2006 to April 2007 who had central venous catheter insertion for haemodialysis in our centre were included in this historically controlled study. In all, 75 patients had catheters with heparin solution alone and 74 had catheters with a gentamicin antibiotic lock. The majority of catheters were non-tunnelled (95%). Cumulative catheter survival free of catheter-related bacteraemia in the two groups was compared.Baseline characteristics in the two groups were similar apart from a slightly lower serum albumin level in those with gentamicin locks. There were 18 and five catheter-related bacteraemia episodes before and after recourse to gentamicin antibiotic locks, respectively. Staphylococcus aureus contributed to over half (65%) of the total bacteraemia episodes. Use of gentamicin antibiotic locks significantly reduced catheter-related bacteraemia episodes per 1000 catheter days from 4.6 to 1.5 (P=0.002). Kaplan-Meier survival analysis using the log rank test showed significantly better bloodstream infection-free survival associated with using gentamicin antibiotic locks (P=0.032). A similar survival advantage was associated with gentamicin antibiotic locks when the analysis was restricted to non-tunnelled catheters. There was no significant association of catheter-related bacteraemia with patient age, obesity, gender, baseline serum albumin level, or diabetes mellitus. No serious adverse events were attributable to the use of gentamicin antibiotic locks.Use of gentamicin lock solutions effectively reduced catheter-related bacteraemia in haemodialysis patients, including those with non-tunnelled catheters.

Sufficient data are available to support the contention that renal and peritoneal clearances are not equivalent, and that loss of residual renal function (RRF) cannot be completely compensated by an increase in the exchange volume or frequency of peritoneal dialysis. When RRF is minimal (for example, renal Kt/V is 0.1 - 0.3), increasing the peritoneal Kt/V beyond the "conventional" value recommended by the Dialysis Outcomes Quality Initiative yields little additional clinical benefit. The cut-off peritoneal (not total) Kt/V is possibly 1.6 - 1.7. However, delivery of peritoneal small-solute clearance below that cut-off level has a major detrimental effect on clinical outcome in CAPD patients with little RRF. Measures to preserve RRF therefore become an important goal in the treatment of CAPD patients. In short, with regard to RRF (renal Kt/V), higher is always better, and we should always try to preserve it. For peritoneal Kt/V, higher is better only up to a certain limit. The importance of aspects of adequate dialysis other than small-solute removal--especially fluid removal, blood pressure control, nutrition, acid-base balance, mineral metabolism, and anemia and lipid control--cannot be sufficiently emphasized.

The proportion of end-stage renal disease patients on continuous ambulatory peritoneal dialysis (CAPD) has increased by twofold in Hong Kong over last two decades, accounting for 80% of the entire dialysis population. Our encouraging outcome results--a 2-year actuarial patient survival of 83%, and a 2-year technique survival of 72.8%--further testify to the success of CAPD in our territory. A relatively constant successful outcome has been achieved despite an increasing prevalence of diabetes mellitus and an aging population. The present review postulates the reasons behind our success. Apart from reimbursement policy, technique-related factors, and center effects, inherent patient factors--including baseline survival advantage of Chinese patients, genetic difference, cardiovascular risk factors, and possibly lower dialysis volume requirement and compliance--are thought to be contributory. As for the future, more efforts are needed to further improve the technique survival rate and the nutritional status, psychosocial well-being, and rehabilitation of CAPD patients. Judicious assessment of peritoneal dialysis adequacy and preservation of residual renal function should be constantly exercised to tailor treatment to the needs of Chinese CAPD patients.

Sleep disorders, such as daytime sleepiness, insomnia, restless legs syndrome (RLS) and obstructive sleep apnea syndrome (OSAS) have been reported as prevalent among patients with end stage renal failure(ESRF).As there is little published data from Southeast Asia, a sleep questionnaire was administered to all patients (N=43, 27 males) on chronic hemodialysis (HD) at the Prince of Wales Hospital, Hong Kong, to assess their sleep complaints.The mean age was 49.5 +/- 11.3 years (mean I SD) with mean body mass index (BMI) of 22.6 +/- 3.6 kg/m2. Frequent awakenings and sleep onset insomnia were the most frequent complaints (79% each), while daytime sleepiness occurred in 74% of patients. Sleep maintenance insomnia and pruritus occurred in 64% and 60% of patients respectively. Symptoms of RLS were reported by 70% of patients. The prevalence of OSAS was estimated by the frequency of observed choking (4.7% of cases), witnessed apnea (14%), snoring and witnessed apnea (9.3%), disruptive snoring (14%), disruptive snoring and witnessed apnea (2.3%), extremely loud snoring (4.7%).Our questionnaire survey revealed a high prevalence of sleep complaints such as frequent awakenings, daytime sleepiness, insomnia and RLS in patients with ESRF on maintenance HD, but a relatively low prevalence of OSAS, which may be related to the low BMI of our ESRF patients.
