Objectives: To investigate coping strategies used by patients submitted to heart transplantation and whether they are related to the perception of the disease and transplantation., Methods: Cross-sectional study with quantitative and qualitative analysis. The sample of 32 patients was assessed by the Ways of Coping Scale and socio-demographic questionnaire, and five of them were selected for interviews. The internal consistency of the scale was assessed, the variables and strategies involved were crossed and content analysis of interviews, investigating the existence of a relationship with the speech of the participants., Results: The individuals have used all coping styles, with a predominance of the problem-focused strategy. Psychologically prepared patients showed a statistically significant increase in the use of problem-focused coping and seek for social support. However, a significant increase in the use of emotion-focused coping was observed in patients who were not prepared. Analysis through the method of Bardin showed as categories: disease; reaction to call; transplantation; fantasies; postoperative; team and coping., Conclusion: Patients with a transplanted heart make use of all coping strategies, with a predominance of the problem-focused strategy. Psychologically prepared individuals used more active coping strategies, which highlights the importance of psychological support during the process.

Risk of viral and/or prion disease transmission associated with the use of fetal bovine serum in clinical cell culture has led to the increasing use of autologous human serum in tissue engineering. A relatively large volume of blood is needed and so, to decrease patient discomfort, we have investigated the feasibility of taking blood when the patient is anesthetized. Two serum samples were prepared from each of 22 patients: (1). from the awake patient (PRE) and (2). from the patient 5 min after induction of general anesthesia (PER). The sera were compared for their ability to support the in vitro proliferation of primary human chondrocytes, determined by cell counting. The effects of anesthetic agents on the PER/PRE cell number ratio were established by analysis of variance and stepwise multilinear regression analysis. The PER sample supported higher growth in 2 of 22 patients, equivalent growth in another 11, and significantly lower growth in the remaining 8. Only the opiate analgesics (fentanyl [Sublimaze], alfentanyl [Rapifen], and diamorphine) had a significant and inhibitory effect on chondrocyte proliferation. It is suggested that opiate analgesics be avoided when blood is taken to support the in vitro growth of human cells.

An intra-arterial continuous display oxygen electrode for radial artery cannulation is now available in the UK. Nine catheters were used in patients during and after hypothermic cardiac surgery. Results obtained were compared with those from conventional intermittent blood gas samples. Slow temperature response times negate its use during hypothermic surgery. A good correlation was found during the postoperative period. Indications for its use are discussed.

A case is reported of acute rhabdomyolytic renal failure following surgery for minor superficial skin burns. Rhabdomyolysis apparently started peri-operatively. Possible initiating causes are discussed, notably tourniquet ischaemia. The pathophysiology, diagnosis and treatment are reviewed.
