Abstract

Introduzione. Ad oggi restano ancora da scoprire le implicazioni a lungo termine per i pazienti che sopravvivono, dopo ospedalizzaizone, all'infezione da SARS-CoV-2, soprattutto per coloro che hanno contratto una forma severa di COVID-19. Obbiettivo. Misurare il carico di malattia residua ancora rilevabile a circa tre mesi e un anno dalla dimissione in pazienti sottoposti a procedure invasive di ventilazione durante la degenza indice. Materiali e metodi. È stato condotto uno studio osservazionale monocentrico includendo pazienti dimessi vivi dal ricovero ospedaliero con diagnosi principale di COVID-19 e sottoposti a procedure ventilatorie durante la degenza ospedaliera. I sintomi e un'ampia gamma di indagini strumentali ed esami TC sono stati valutati dopo circa tre mesi e un anno dalla dimissione indice da un team multidisciplinare utilizzando procedure standardizzate. Risultati. Tra marzo e giugno 2020 sono stati arruolati 203 pazienti che hanno accettato di sottoporsi a una visita ambulatoriale dopo tre mesi dalla dimissione. Circa il 60% dei pazienti aveva almeno un sintomo, il 6,5% aveva un basso livello di saturazione durante la lettura dell'ossimetria e l'85,0% aveva almeno un'anomalia dell'analisi del sangue. Più del 20% dei pazienti presentava almeno il 50% del parenchima polmonare affetto da anomalie. Durante la visita è stata richiesta una consulenza specialistica per il 21,2% dei soggetti. A livello globale, poco più del 20% dei pazienti presentava ancora una grave malattia residua da COVID-19. La durata della degenza ospedaliera superiore a 30 giorni era significativamente associata al rischio di avere una malattia residua grave. Conclusione. Un'ampia percentuale di pazienti presentava ancora segni clinici e strumentali di malattia residua circa due mesi dopo la dimissione e ad un anno, seppur in una percentuale più ridotta si protraevano anomalie. Fornire una valutazione coordinata e multidisciplinare a lungo termine, attraverso la costruzione di protocolli standardizzati dovrebbe essere presa in considerazione per tali pazienti, anche alla luce della programmazione sanitaria futura. To measure the burden of residual disease still detectable approximately three months and one year after discharge in patients underwent to mechanical ventilation during the index hospital stay. Methods. A single-center observational study was performed by including patients discharged alive from hospital admission with main diagnosis of COVID-19, and who underwent mechanical ventilation during hospital stay. Symptoms and a wide range of instrumental signs and CT exams were assessed after approximately three months and one years from index discharge by a multidisciplinary team using standardized procedures. Results. Between March and June 2020, 203 patients were enrolled and agreed to undergo an outpatient visit after two months since discharge. About 60% patients had at least one symptom, 6.5% had low level of saturation during oximetry reading and 85.0% had at least a blood test abnormality. More than 20% patients had at least 50% of the lung parenchyma affected by abnormalities. During the visit, a specialist consultation was requested for 21.2% of the subjects. Globally, just above 20% of the patients still presented severe COVID-19 Residual Illness. Duration of hospital stay longer than 30 days was significantly associated with the risk of having severe residual illness. Conclusion. A large proportion of patients still had clinical and instrumental signs of residual illness approximately two months after discharge. Provision of a coordinated, multidisciplinary long-term assessment should be considered for such patients experiencing critical COVID-19 disease, also in the light of future health planning

Abstract

Timely identification of COVID-19 patients at high risk of mortality can significantly improve patient management and resource allocation within hospitals. This study seeks to develop and validate a data-driven personalized mortality risk calculator for hospitalized COVID-19 patients. De-identified data was obtained for 3,927 COVID-19 positive patients from six independent centers, comprising 33 different hospitals. Demographic, clinical, and laboratory variables were collected at hospital admission. The COVID-19 Mortality Risk (CMR) tool was developed using the XGBoost algorithm to predict mortality. Its discrimination performance was subsequently evaluated on three validation cohorts. The derivation cohort of 3,062 patients has an observed mortality rate of 26.84%. Increased age, decreased oxygen saturation (≤ 93%), elevated levels of C-reactive protein (≥ 130 mg/L), blood urea nitrogen (≥ 18 mg/dL), and blood creatinine (≥ 1.2 mg/dL) were identified as primary risk factors, validating clinical findings. The model obtains out-of-sample AUCs of 0.90 (95% CI, 0.87–0.94) on the derivation cohort. In the validation cohorts, the model obtains AUCs of 0.92 (95% CI, 0.88–0.95) on Seville patients, 0.87 (95% CI, 0.84–0.91) on Hellenic COVID-19 Study Group patients, and 0.81 (95% CI, 0.76–0.85) on Hartford Hospital patients. The CMR tool is available as an online application at covidanalytics.io/mortality_calculator and is currently in clinical use. The CMR model leverages machine learning to generate accurate mortality predictions using commonly available clinical features. This is the first risk score trained and validated on a cohort of COVID-19 patients from Europe and the United States.

6. Phenytoin and gingival mucosa: A molecular investigation

Abstract

Several distinct classes of drugs, such as anticonvulsants, immunosuppressants, and calcium channel blockers, caused gingival overgrowth. One of the main drugs associated with the gingival overgrowth is the anti-epileptic such as phenytoin, which affects gingival tissues by altering extracellular matrix metabolism. In our study, we evaluate the effect of phenytoin, a drug whose active substance is phenytoin, on gingival fibroblasts of healthy volunteers. Gene expression of 29 genes was investigated in gingival fibroblasts' cell culture treated with phenytoin compared with untreated cells. Among the studied genes, only 13 genes (CXCL5, CXCL10, CCR1, CCR3, CCR5, CCR6, IL-1A, IL-1B, IL-5, IL-7, IL-6R, BMP-2, and TNFSF-10) were statistically significant. All but one gene resulted downregulated after 24 h of treatment with phenytoin. BPM2 was the only, although weakly, up-expressed gene. Probably, we have not highlighted overexpression of the other inflammatory molecules because the study was performed on healthy people. Many studies show that phenytoin induces the overexpression of these cytokines but, probably, in our study, the drug does not have the same effect because we used gingival fibroblasts of healthy people.

Abstract

Health authorities, medical directors and managers are called to face increasing and inter-related challenges regarding the ongoing financial crisis, aging of the population and the need to acquire innovative medical technologies. These challenges need to be tackled in order to ensure sustainability of the healthcare systems and appropriateness and quality of healthcare services. In Italy, the role of medical directors has progressively evolved over the years and now requires strengthened managerial skills and additional professional expertise and skills to plan, implement and evaluate the new systems of government. We performed an extensive literature review and a qualitative study involving six semi-structured interviews with key actors (health managers) in order to explore and clarify the possible future role of medical directors in Italy and what training should be provided to residents in Hygiene and Preventive Medicine.

Abstract

OBJECTIVE:To assess General Pratictioner's (GPs) knowledge and attitudes about HPV infection and prevention.DESIGN AND PARTICIPANTS:A semi-structured survey was conducted from November to December 2013 among Italian GPs. Descriptive and univariate analyses were carried out.MAIN OUTCOME MEASURES AND RESULTS:938 GPs were included in the study. 15% participated in continuing medical education courses focusing on HPV. GPs identified as HPV transmission routes: sexual (100%), cutaneous (15%), transplacental (13%), haematic (9%) and by air (2%); they considered HPV-related diseases: cervical (98%), vulvar and vaginal (42%), anal (39%), penile (38%) and oral (38%) cancer, genital warts (79%) and respiratory papillomatosis (12%). They knew HPV vaccination is to prevent HPV-associated cancer (60%), in particular cervical cancer (35%), genital warts (3%) and sexually transmitted diseases (2%). A total of 73% were aware of the existence of both available vaccines, 69% believed that immunization target population should be females before initiation of sexual activity, 87% knew the age of vaccine administration. No significant difference in knowledge was retrieved by age, gender, level of education or region of origin.CONCLUSION:We report a lack of knowledge on HPV infection and vaccination in GPs. GPs have a key role in the Italian health system. Although 12-year old patients are a small percentage of their patients, it is of fundamental importance to promote medical education and training among GPs in order to meet HPV coverage targets and control HPV-associated diseases.

Abstract

INTRODUZIONE: Il nuovo Decreto Interministeriale sul Riordino delle scuole di specializzazione di area sanitaria pubblicato nel Febbraio 2015 prevede tra le attività professionalizzanti obbligatorie per la Scuola di Specializzazione in Igiene e Medicina Preventiva la partecipazione alla stesura di revisioni sistematiche e metanalisi, considerati strumenti fondamentali della medicina basata sulle evidenze. La scuola di Parma offre agli specializzandi un percorso formativo in metodologia della ricerca inserito nell’ambito delle collaborazioni internazionali instaurate. METODI: Presentiamo due progetti di revisioni sistematiche condotte dalla scuola di Parma in collaborazione, rispettivamente con il Centers for Disease Control and Prevention (CDC) nel 2014 e con l’Organizzazione Mondiale della Sanità (OMS) nel 2015. La prima revisione ha raccolto le evidenze disponibili sull’ efficacia di interventi che utilizzano new media per promuovere le vaccinazioni. Il secondo progetto ha stimato l’impatto sulla ricerca di un documento di indirizzo pubblicato dall’ OMS sulla prevenzione ed il controllo della co-infezione TB-HIV. RISULTATI: I medici in formazione specialistica, coordinati dai docenti della scuola e coadiuvati da un dottorando di ricerca, hanno condotto le due revisioni sistematiche seguendo le linee guida “Prepared Items for Systematic Reviews and Meta- Analysis” (PRISMA) e sviluppando strategie di ricerca sui database Medline e Embase. I criteri di inclusione ed esclusione sono stati definiti a priori in un protocollo, l’ eleggibilità degli studi è stata valutata in doppio cieco mediante un processo a due fasi: un primo screening è stato eseguito su titolo ed abstract, un secondo sul full text. Nel progetto su new media e vaccinazioni sono stati selezionati 37.634 articoli, di cui 156 (0,4%) trattenuti al primo screening e 19 (12%) al secondo. Nel progetto su TB-HIV sono stati selezionati un 9.543 articoli di cui 2237 (23,44%) trattenuti al primo screening e 1473 (65,8%) al secondo. In entrambi i casi per gli studi inclusi nelle revisioni, i dati rilevanti sono stati estratti utilizzando fogli di calcolo predefiniti, organizzati in database e analizzati. I risultati sono stati riassunti in report, pubblicati su rivista peer reviewed e presentati a convegni internazionali. CONCLUSIONI: Le revisioni sistematiche e le metanalisi sono il livello più alto della gerarchia delle evidenze scientifiche e come tali la base fondamentale per guidare le scelte in ambito clinico, di prevenzione e di politica sanitaria. Padroneggiarne la metodologia deve essere obiettivo prioritario dei medici in formazione specialistica in Igiene e Medicina preventiva. Gli esempi descritti dimostrano come questo sia stato raggiunto nella realtà della nostra scuola.

Abstract

Objectives: In 2010, the WHO issued 77 priority research questions (PRQs) to address HIV-associated TB. Objective of the this study was to assess the impact of defining the research agenda in stimulating and directing research around priority research questions. Methods: We used number and type of scientific publications as a proxy to quantitatively assess the impact of research agenda setting. We conducted 77 single systematic reviews – one for every PRQ – building 77 different search strategies using PRQs’ keywords. Multivariate logistic regression models were applied to assess the quantity and quality of research produced over time and accounting for selected covariates. Results: In 2009–2015, PRQs were addressed by 1631 publications (median: 11 studies published per PRQ, range 1–96). The most published area was ‘Intensified TB case finding’ (median: 23 studies/PRQ, range: 2–74). Abstract

Objective: To assess General Pratictioner's (GPs) knowledge and attitudes about HPV infection and prevention., Design and Participants: A semi-structured survey was conducted from November to December 2013 among Italian GPs. Descriptive and univariate analyses were carried out., Main Outcome Measures and Results: 938 GPs were included in the study. 15% participated in continuing medical education courses focusing on HPV. GPs identified as HPV transmission routes: sexual (100%), cutaneous (15%), transplacental (13%), haematic (9%) and by air (2%); they considered HPV-related diseases: cervical (98%), vulvar and vaginal (42%), anal (39%), penile (38%) and oral (38%) cancer, genital warts (79%) and respiratory papillomatosis (12%). They knew HPV vaccination is to prevent HPV-associated cancer (60%), in particular cervical cancer (35%), genital warts (3%) and sexually transmitted diseases (2%). A total of 73% were aware of the existence of both available vaccines, 69% believed that immunization target population should be females before initiation of sexual activity, 87% knew the age of vaccine administration. No significant difference in knowledge was retrieved by age, gender, level of education or region of origin., Conclusion: We report a lack of knowledge on HPV infection and vaccination in GPs. GPs have a key role in the Italian health system. Although 12-year old patients are a small percentage of their patients, it is of fundamental importance to promote medical education and training among GPs in order to meet HPV coverage targets and control HPV-associated diseases.
