Clinicopathological correlates of prostate cancer associated with a pseudocapsule at T2-weighted magnetic resonance (MR) imaging are presented in a retrospective series of 15 patients. Of 15 tumors, 14 involved the peripheral zone. Extracapsular extension was seen in 14 cases. Tumor Gleason score was 8 or above in 12 of 15 cases, and ductal type adenocarcinoma was identified in 4 cases. Step section histopathological correlation (n=5) demonstrated that the pseudocapsule corresponded with dense compressive or reactive peritumoral fibrosis. A pseudocapsule around prostate cancer at T2-weighted MR imaging is a rare finding that appears to be associated with high grade and stage disease.

15. The utility of orbital imaging in the evaluation of orbital disease.

Purpose: This study investigates the accuracy of either computerized tomography (CT) or magnetic resonance imaging (MRI) for the evaluation of various orbital diseases. Methods: We collected 126 CT scans and 65 MRI scans from 144 subjects and asked two radiologists to interpret the images without clinical information. Images included 14 with a clinical diagnosis of orbital infection, 144 with orbital inflammation, and 33 with orbital neoplasm. The inflammatory diseases included thyroid eye disease (TED, n = 69), non-specific orbital inflammation (NSOI, n = 44), IgG4-related disease (IgG4-RD, n = 15), sarcoidosis (Sarcoid, n = 9), granulomatosis with polyangiitis (GPA, n = 5), and Erdheim-Chester disease (ECD, n = 2). Results: The balanced accuracy (BA) for the two radiologists ranged from 0.87 to 0.90 for cellulitis, 0.81 to 0.86 for inflammation, and 0.82 to 0.85 for neoplasm. Radiologists were excellent at recognizing GPA (BA = 0.98 to 0.99) and very good for TED (BA = 0.80 to 0.86). They also did well identifying IgG4-RD (BA = 0.75 to 0.77), but slightly less well for NSOI (BA = 0.69 to 0.75) and poorly for Sarcoid (BA = 0.48 to 0.50). Conclusions: CT or MRI scanning contributes to the evaluation of patients with orbital disease, but accuracy does varies based depending on the diagnosis. We could not evaluate issues such as determination of disease activity, variability based on the unit used for imaging or the skills beyond those of our two specialized neuroradiologists. Future studies should directly compare the two imaging modalities and assess the utility of imaging to determine disease activity. [ABSTRACT FROM AUTHOR]

- Abstract
A man in his late 40s with no known past medical history was unresponsive for an unknown period of time. Crushed pills and white residue were found on a nearby table. On presentation he was obtunded and unresponsive to verbal commands but withdrawing to painful stimuli. The initial urine drug screen was negative, but a urine fentanyl screen was subsequently positive with a level of 137.3 ng/mL. MRI of the brain showed reduced diffusivity and fluid attenuated inversion recovery (FLAIR) hyperintensity symmetrically in the bilateral supratentorial white matter, cerebellum and globus pallidus. Alternative diagnoses such as infection were considered, but ultimately the history and workup led to a diagnosis of fentanyl-induced leukoencephalopathy. Three days after admission the patient became able to track, respond to voice and follow basic one-step commands. The patient does not recall the mechanism of inhalation. While there are case reports of heroin-induced leukoencephalopathy following inhaled heroin use and many routes of fentanyl, this is the first reported case of a similar phenomenon due to fentanyl inhalation. - Abstract
Background In a population-based setting, we investigated the risks of testing positive for SARS-CoV-2 and developing severe COVID-19 outcomes among cancer patients compared with the general population.Methods In nationwide cohorts, we identified all individuals in Norway, Denmark and Iceland who tested positive for SARS-CoV-2 or had a severe COVID-19 outcome (hospitalisation, intensive care, and death) from March until December 2020, using data from national health registries. We estimated standardised incidence ratios (SIRs) with 95% confidence intervals (CIs) comparing cancer patients with the general population.Findings During the first wave of the pandemic, cancer patients in Norway and Denmark had higher risks of testing SARS-CoV-2 positive compared to the general population. Throughout 2020, recently treated cancer patients were more likely to test SARS-CoV-2 positive. In Iceland, cancer patients experienced no increased risk of testing positive. The risk of COVID-19-related hospitalisation was higher among cancer patients diagnosed within one year of hospitalisation (Norway: SIR = 2.43, 95% CI 1.89-3.09; Denmark: 2.23, 1.96-2.54) and within five years (Norway: 1.58, 1.35-1.83; Denmark: 1.54, 1.42-1.66). Risks were higher in recently treated cancer patients and in those diagnosed with haematologic malignancies, colorectal or lung cancer. Risks of COVID-19-related intensive care and death were higher among cancer patients. Interpretation Cancer patients were at increased risk of testing positive for SARS-CoV-2 during the first pandemic wave when testing availability was limited, while relative risks of severe COVID-19 outcomes remained increased in cancer patients throughout 2020. Recent cancer treatment and haematologic malignancy were the strongest risk factors.

- Abstract
Background and purpose: Stage at cancer diagnosis is an important predictor of cancer survival. TNM stage is constructed for anatomic solid cancer diagnoses from tumor size (T), nodal spread (N) and distant metastasis (M) and categorized in groups 0-I, II, II and IV. TNM stage is imperative in cancer diagnosis, management and control, and of high value in cancer surveillance, for example, monitoring of stage distributions. This study yields an overview of TNM availability and trends in stage distribution in the Nordic countries for future use in monitoring and epidemiologic studies. Material and methods: TNM information was acquired from the cancer registries in Denmark, Norway, Sweden, and Iceland during 2004-2016 for 26 cancer sites in the three former countries and four in Iceland. We studied availability, comparability, and distribution of TNM stage in three periods: 2004-2008, 2009-2013, and 2014-2016, applying a previously validated algorithm of 'N0M0 for NXMX'. For cancers of colon, rectum, lung, breast, and kidney, we examined TNM stage-specific 1-year relative survival to evaluate the quality in registration of TNM between countries. Results: Denmark, Sweden, and Iceland exhibited available TNM stage proportions of 75-95% while proportions were lower in Norway. Proportions increased in Sweden over time but decreased in Denmark. One-year relative survival differed substantially more between TNM stages than between countries emphasizing that TNM stage is an important predictor for survival and that stage recording is performed similarly in the Nordic countries. Interpretation: Assessment and registration of TNM stage is an imperative tool in evaluations of trends in cancer survival between the Nordic countries. [ABSTRACT FROM AUTHOR]

Mental health disorders are prevalent among individuals with intellectual disabilities (ID). However, there is a lack of research on the impact of concomitant autism spectrum disorders (ASD) or attention deficit hyperactivity disorder (ADHD) on the mental health within this population. We aimed to investigate the prevalence of mental health disorders and registered healthcare visits due to self-harm among individuals with ID. We used administrative data for all healthcare with at least one recorded diagnosis of mental health disorder or self-harm during 2007–2017 among people with a diagnosis of Down syndrome (DS; n = 1298) and with ID without DS (IDnonDS; n = 10,671) using the rest of the population in Stockholm Region (n = 2,048,488) for comparison. The highest odds ratios for a mental health disorder were present in females with IDnonDS (9.01) followed by males with IDnonDS (8.50), compared to the general population. The ORs for self-harm among individuals with IDnonDS were high (8.00 for females and 6.60 for males). There were no registered cases of self-harm among individuals with DS. The prevalence of an anxiety or affective disorder was higher among individuals with ID including DS with concomitant ASD or ADHD. Neighbourhood socio-economic status was associated with a lower occurrence of mental health disorders and self-harm in wealthier areas for all outcomes and for all groups. Self-harm and psychiatric comorbidities were common among individuals with ID without DS with an attenuated difference among those with concomitant ASD or ADHD, which calls for attention. Introduction: Communication is said to be one of the most important yet most challenging aspects of leadership. Today's globalized world creates new dimensions, requirements and higher demands are placed on managers' internal communication in order to be able to handle different cultural aspects and employees. Managers may face a myriad of challenges such as language barriers, cultural diversity through different values, behavioral patterns of team members and way of thinking. Previous research on communication shows that the focus has been on external communication, there has been limited connection between cultural impact and internal communication, and the concept of communication channels within internal communication, from a managerial perspective, is still rather unexamined. Purpose: By examining managers' understanding of cultural aspects in internal communication in multicultural settings, we aim to increase the understanding of what is required of managers, when communicating internally, so that the message is interpreted and understood by the various employees in a multicultural team. By adding the perspective of communication styles and communication channels into internal communication in multicultural settings, a broader understanding of the important tools for effective communication can be understood. Research methodology: The study is a case study based on a qualitative research method with an inductive approach. Twelve recorded semi-structured interviews with six main questions, and several sub-questions, were conducted with twelve various managers who lead multicultural teams in six different global companies. På BillerudKorsnäs pappersbruk i Karlsborg finns det arbetsmoment vid en bestrykningsmaskin som riskerar att utsätta operatörerna för förslitningsskador. Bestrykningsmaskinen är en maskin där pappret bestryks med en smet för öka glansen och få en jämnare yta. Arbetsmomentet innebär att sprätta bort de yttersta varven papper från de färdiga rullarna med bestruket papper, eftersom det inte uppfyller kvalitetskraven med avseende på glans och ytjämnhet. Vissa papperskvaliteter är väldigt töjbara vilket innebär en stor belastning på axlar och händer när man sprättar pappret. Syftet med examensarbetet var att tillsammans med operatörer och processingenjörer, designa och konstruera en ny modell på sprättare, som har förbättrad funktion och ergonomi för att minimera risken med förslitningsskador. Avgränsningar gjordes till att konstruera en sprättare för sprättning av färdigt bestruken kundrulle vid rullmaskinen.Olika konceptförslag togs fram med hjälp av Computer-Aided-Design (CAD) och prototyper tillverkades därefter med hjälp av 3D-skrivare för testning och utvärdering. Riskbedömning utfördes för att sammanställa vilka risker den nya sprättaren medförde. Arbetet resulterade i en färdigtillverkad fungerande prototyp med tillhörande ritningsunderlag tillräckligt detaljerat för en framtida serietillverkning.Slutsatsen är ett nytt skärverktyg som nu har designats och tillverkats anpassat efter operatörernas krav på ergonomi och funktion, och därmed minskar riskerna med att utsätta operatörerna för förslitningsskador., At BillerudKorsnäs' paper mill in Karlsborg, there is working tasks on a coating machine that risks exposing the operators to wear and tear injuries. The coating machine is a machine where the paper is coated with a glaze to increase the gloss and get a smoother surface. For this machine, one of the working tasks is, by using a hand tool, cut the outer layers of paper from the finished rolls of coated paper, which is necessary to avoid quality loss regarding to gloss and surface smoothness. Some of the paper qualities is very tough and stretchable, which means a great strain on the shoulders and hands when cutting the paper. The purpose of the project was to, together with operators and process engineers, design and construct a new model of the cutting tool with improved function and ergonomics to minimize the risk for wear and tear injuries. The work is limited to the construction of a cutting tool for a completely coated paper roll at the rolling machine.Different concepts proposals were developed by using Computer-Aided-Design (CAD) and prototypes were then manufactured for testing and evaluation by using 3D printers. A risk assessment was performed to compile which risks the new cutting tool entailed. The work resulted in a manufactured well-working prototype with drawings and 3D-models suitable to initiate mass production of the tool.The conclusion is a new cutting that has been designed and built to suit the operators’ requirements of ergonomics and function, which reduces the risks of exposing the operators to wear and tear damage.

- Abstract
Background: A recent overview of cancer survival trends 1990–2016 in the Nordic countries reported continued improvements in age-standardized breast cancer survival among women. The aim was to estimate age-specific survival trends over calendar time, including life-years lost, to evaluate if improvements have benefited patients across all ages in the Nordic countries. Methods: Data on breast cancers diagnosed 1990–2016 in Denmark, Finland, Iceland, Norway, and Sweden were obtained from the NORDCAN database. Age-standardized and age-specific relative survival (RS) was estimated using flexible parametric models, as was reference-adjusted crude probabilities of death and life-years lost. Results: Age-standardized period estimates of 5-year RS in women diagnosed with breast cancer ranged from 87% to 90% and 10-year RS from 74% to 85%. Ten-year RS increased with 15–18 percentage points from 1990 to 2016, except in Sweden (+9 percentage points) which had the highest survival in 1990. The largest improvements were observed in Denmark, where a previous survival disadvantage diminished. Most recent 5-year crude probabilities of cancer death ranged from 9% (Finland, Sweden) to 12% (Denmark, Iceland), and life-years lost from 3.3 years (Finland) to 4.6 years (Denmark). Although survival improvements were consistent across different ages, women aged ≥70 years had the lowest RS in all countries. Period estimates of 5-year RS were 94–95% in age 55 years and 84–89% in age 75 years, while 10-year RS were 88–91% in age 55 years and 69–84% in age 75 years. Women aged 40 years lost on average 11.0–13.8 years, while women lost 3.8–6.0 years if aged 55 and 1.9–3.5 years if aged 75 years. Conclusions: Survival for Nordic women with breast cancer improved from 1990 to 2016 in all age groups, albeit with larger country variation among older women where survival was also lower. Women over 70 years of age have not had the same survival improvement as women of younger age.

På BillerudKorsnäs pappersbruk i Karlsborg finns det arbetsmoment vid en bestrykningsmaskin som riskerar att utsätta operatörerna för förslitningsskador. Bestrykningsmaskinen är en maskin där pappret bestryks med en smet för öka glansen och få en jämnare yta. Arbetsmomentet innebär att sprätta bort de yttersta varven papper från de färdiga rullarna med bestruket papper, eftersom det inte uppfyller kvalitetskraven med avseende på glans och ytjämnhet. Vissa papperskvaliteter är väldigt töjbara vilket innebär en stor belastning på axlar och händer när man sprättar pappret. Syftet med examensarbetet var att tillsammans med operatörer och processingenjörer, designa och konstruera en ny modell på sprättare, som har förbättrad funktion och ergonomi för att minimera risken med förslitningsskador. Avgränsningar gjordes till att konstruera en sprättare för sprättning av färdigt bestruken kundrulle vid rullmaskinen.Olika konceptförslag togs fram med hjälp av Computer-Aided-Design (CAD) och prototyper tillverkades därefter med hjälp av 3D-skrivare för testning och utvärdering. Riskbedömning utfördes för att sammanställa vilka risker den nya sprättaren medförde. Arbetet resulterade i en färdigtillverkad fungerande prototyp med tillhörande ritningsunderlag tillräckligt detaljerat för en framtida serietillverkning.Slutsatsen är ett nytt skärverktyg som nu har designats och tillverkats anpassat efter operatörernas krav på ergonomi och funktion, och därmed minskar riskerna med att utsätta operatörerna för förslitningsskador. At BillerudKorsnäs' paper mill in Karlsborg, there is working tasks on a coating machine that risks exposing the operators to wear and tear injuries. The coating machine is a machine where the paper is coated with a glaze to increase the gloss and get a smoother surface. For this machine, one of the working tasks is, by using a hand tool, cut the outer layers of paper from the finished rolls of coated paper, which is necessary to avoid quality loss regarding to gloss and surface smoothness. Some of the paper qualities is very tough and stretchable, which means a great strain on the shoulders and hands when cutting the paper. The purpose of the project was to, together with operators and process engineers, design and construct a new model of the cutting tool with improved function and ergonomics to minimize the risk for wear and tear injuries. The work is limited to the construction of a cutting tool for a completely coated paper roll at the rolling machine.Different concepts proposals were developed by using Computer-Aided-Design (CAD) and prototypes were then manufactured for testing and evaluation by using 3D printers. A risk assessment was performed to compile which risks the new cutting tool entailed. The work resulted in a manufactured well-working prototype with drawings and 3D-models suitable to initiate mass production of the tool.The conclusion is a new cutting that has been designed and built to suit the operators’ requirements of ergonomics and function, which reduces the risks of exposing the operators to wear and tear damage.
