The authors report a case of a 74-year-old woman found to have an extremely rare case highlighted by multidetector computed tomography (MDCT) angiography, with the presence of a replaced right hepatic artery (RRHA) arising from the splenic artery (SA). In this case, the SA arose from a gastrosplenic trunk (GST). The GST had an endoluminal diameter of 9.2 mm at its origin and a length of 9.3 mm. It arose directly from the anterior abdominal aortic wall, at the level of the T12–L1 intervertebral disc. The SA branched off from the GST and travelled in front of the abdominal aorta (AA) for 18.2 mm up to the level of the L1–L2 intervertebral disc. The SA then continued along an upward and tortuous path towards the splenic hilum. The inflection point of the SA trunk was located above the origin of superior mesenteric artery (SMA). The RRHA arose from the right of this inflection point. The RRHA had an endoluminal diameter of 3.0 mm at its origin and a length of 96.0 mm; it had a downward trajectory towards the hepatic hilum. The common hepatic artery (CHA) had an endoluminal diameter of 6.2 mm at origin and arose directly from the anterior wall immediately to the right of the mediosagittal plane of the AA. Knowledge of this rare anatomical variation is important for interventional radiologists, oncologists, hepatic and abdominal surgeons.

Aims: Long noncoding RNAs (lncRNAs) play key roles in the pathophysiology of DKD involving actions of microRNAs (miRNAs). The aims of the study were to establish the involvement of selected lncRNAs in the epigenetic mechanisms of podocyte damage and tubular injury in DKD of type 2 diabetes mellitus (DM) patients in relation to a particular miRNAs profile. Methods: A total of 136 patients with type 2 DM and 25 healthy subjects were assessed in a cross-sectional study concerning urinary albumin: creatinine ratio (UACR), eGFR, biomarkers of podocyte damage (synaptopodin, podocalyxin) and of proximal tubule (PT) dysfunction (Kidney injury molecule-1-KIM-1, N-acetyl-D-glucosaminidase-NAG), urinary lncRNA metastasis-associated lung adenocarcinoma transcript 1 (MALAT1), nuclear-enriched abundant transcript 1 (NEAT1), myocardial infarction-associated transcript (MIAT), taurine-upregulated gene 1 (TUG1), urinary miRNA21, 124, 93, 29a. Aim: The involvement of proinflammatory interleukins (IL) in diabetic kidney disease of Type 2 diabetes mellitus (DM) patients was studied in relation to a particular miRNA profile. Materials & methods: A total of 117 patients with Type 2 DM and 11 controls were enrolled in a case series study. Serum and urinary ILs and miRNAs were assessed. Results: IL-1α correlated with miRNA21, 124, estimated glomerular filtration rate (eGFR) and negatively with miRNA125a and 192; IL-8 with miRNA21, 124, eGFR and negatively with miRNA125a, 126 and 146a; IL-18 with miRNA21, 124 and negatively with miRNA146a, 192, eGFR. Conclusion: There is an association between specific serum and urinary ILs and serum and urinary miRNAs profiles in the inflammatory response in Type 2 DM patients with diabetic kidney disease.

Background and Aims Glomerular injury and proximal tubule (PT) dysfunction have intricate mechanisms in diabetic kidney disease (DKD). Pro-inflammatory cytokines are involved in the initiation and progression of DKD through mediating the inflammatory response, both at glomerular and proximal tubular level. miRNAs are able to modulate cellular and biochemical functions, thus intervening in the pathogenesis of DKD. The aim of the study, performed on patients with type 2 diabetes mellitus (DM), was to evaluate selective pro-inflammatory cytokines in relation to biomarkers of podocyte lesion and of PT dysfunction. Particular molecular pathways, such as specific miRNA profiles operating in this relation, have also been studied. Method A number of 126 patients with type 2 DM, staged by albuminuria [39 normoalbuminuric – urinary albumin/creatinine ratio UACR)300mg/g], and 23 healthy control subjects were included in a cross-sectional study. All patients were evaluated concerning biomarkers of podocyte injury (nephrin, podocalyxin, synaptopodin) and of PT dysfunction [Kidney injury molecule-1(KIM-1), N-acetyl-beta-D-glucuronidase (NAG), alpha 1- microglobulin]. Also, serum and urinary levels of specific interleukins (IL-6, IL-17), serum cystatin C, and eGFR were determined. Serum and urinary miRNAs (miRNA-21, miRNA-124, miRNA-146a, miRNA-192) were assessed by RT-PCR. Results The biomarkers of podocyte lesion and of PT dysfunction were increased, even in normoalbuminuric type 2 DM patients. Serum and urinary IL-6 and IL-17 showed increased levels in type 2 DM patients, across all groups studied. The model provided by univariable regression analysis showed that IL-6 and IL-17 correlated directly with biomarkers of podocyte injury (nephrin, podocalyxin, synaptopodin), of PT dysfunction (KIM-1, NAG, alpha 1-microglobulin), as well as with UACR. Negative correlations have been identified regarding eGFR. The authors report the case of a 53-year-old male found to have an extremely rare case of a triple anatomical variation highlighted by multidetector computed tomography (MDCT) angiography, with the presence of a hepato-spleno-mesenteric trunk (HSMT) in association with an accessory left hepatic artery (ALHA) and a common trunk origin of right (RIPA) and left (LIPA) inferior phrenic arteries from left gastric artery (LGA) arising independently from the abdominal part of aorta (AA). The HSMT with an endoluminal diameter of 10.9 mm at its origin, and a length of 4 mm arose from the anterior wall of the AA at the level of 1∕2 upper part of the L1 vertebral body. From the distal portion of HSMT, give birth to the hepato-splenic trunk (HST) and to the superior mesenteric artery (SMA). HST, with a diameter at origin of 9.2 mm and 22.3 mm long, has an upward trajectory and done with the anterior face of AA an open angle to the top of 69°. From the distal part of the HST, arise common hepatic artery (CHA) and splenic artery (SA). The LGA, with an endoluminal diameter of 4.2 mm at origin, arose directly from the anterior wall of the AA at the level of the lower 1∕3 of T12 vertebral body, 8.2 mm above the origin of the HSMT. It ran upwards in front of the AA and after 59.5 mm gave rise to an ALHA. At 18.6 mm from its aortic origin, LGA gives birth to an inferior phrenic artery trunk (IPAT), which has at origin an endoluminal diameter of 2.6 mm and a length of 2.4 mm. The RIPA and LIPA have to origin a diameter of 2.3 mm and 1.7 mm, respectively. Knowledge of this anatomical variation is important for anatomists, interventional radiologists, vascular medicine experts, oncologists, vascular, and hepatic surgeons.

We report a very rare case of a 67-year-old male with the presence of a common hepatic artery (CHA) arising from the left gastric artery (LGA) in association with a presence of a gastro-splenic trunk (GST), found incidentally on multidetector computed tomography (MDCT) angiography, used to investigate peripheral vascular disease. The GST arises from the anterior aspect of the abdominal aorta (AA), at the level of lower 1∕3 of L1 vertebral body. The GST has a slightly concave trajectory to the right, and ends dividing into splenic artery (SA) and LGA. In the initial part of its trajectory, the SA it is wedged at 180°, pointing to the left, to the splenic hilum. The LGA has two different portions: the first dilated, initially oriented towards the higher, and then aligns to the infero-lateral left and gives birth to the second portion; the narrow portion, oriented initially horizontally, and then lower to the right. Dilated portion of LGA is continued with CHA. The CHA trunk is cuddling in a horizontal plane, at 180°, and is then oriented towards the fissure of the ligamentum venosum for entering in the liver parenchyma. At 51.7 mm from the origin, the CHA gives rise to the left hepatic artery (LHA), and after another 58 mm to the right hepatic artery (RHA), and finally continues with the gastroduodenal artery (GDA). Knowledge of this anatomical variation should be considered in planning and performing vascular surgery in the supramesocolic floor of the abdominal cavity.

6-ethyl chenodeoxycholic acid (6ECDA) and deoxycholic acid (DCA) are two similar-structured cholanoic acids, the first being used in the treatment of primary biliary cholangitis, while the second is one of the natural secondary bile acids, currently being tested as a treatment component in the destruction of gallstones. This study was realised as no literature reference was found over thermal behaviour and the evaluation of the decomposition kinetics of the compounds from this class. Both acids were characterised by UATR-FTIR spectroscopy, thermal analysis and the kinetics of decomposition in dynamic oxidative atmosphere, by employing three isoconversional methods, namely Friedman, Kissinger–Akahira–Sunose and Flynn–Wall–Ozawa, respectively, the modified NPK method. Isoconversional methods suggested that no modification of the decomposition mechanism occurs with the modification of thermal treatment of the samples. Later, the modified NPK method was employed in order to estimate the kinetic triplet that describes the decomposition process of both compounds. All kinetic methods indicate a very good thermal stability of the bile acids, with apparent activation energies between 190.6 and 208.7 kJ mol−1 for the decomposition of 6ECDA and 205.5 and 221.7 kJ mol−1 for the decomposition of CDA, respectively. The modified NPK method suggested that both bile acids are degraded by the contribution of two distinctive chemical processes without any physical process associated.

In this paper, we present the spectroscopic description (by the means of UATR-FTIR spectroscopy) for four naturally occurring bile acids, namely lithocholic acid (LC), taurocholic acid sodium salt hydrate (TC), taurodeoxycholic acid sodium salt hydrate (TDC) and chenodeoxycholic acid (CDC), as well as their thermal behaviour in dynamic oxidative atmosphere, by TG/DTG/HF means. It was shown that all samples have a good thermal stability, favourized by the presence of cholan-24-oic structural moiety in all samples, as well as the solid-state structure (intermolecular H-bondings for the carboxylic acids CDC and LC and saline structures for TDC and TC, respectively). These data are simple, but of great interest in identifying the presence of these compounds by fast, reproducible and precise instrumental techniques in complex mixtures, such as biliary stones.

Aim To assess the onset of early left ventricular (LV) systolic and diastolic function impairment and the subclinical atherosclerosis following chemotherapy in patients diagnosed with acute myeloid leukemia (AML). A longitudinal, randomized, five year follow-up clinical and experimental study was conducted during May 1, 2011 - June 30, 2016. A number of 125 eligible postmenopausal women (BMD T-score spine (-2.8 � 0.5)); BMD at the femoral hip (-2.6 � 0.8), treated only with biphosphonates, were enrolled in the study. The patient�s age ranged between 60 and 65 years. All the women were stone formers, with more than five episodes of crystalluria in the last five years. Exclusion criteria: kidney chronic diseases, use of drugs known to affect bone metabolism and renal function, patients with multi-morbidities. The aim of our study was to assess the connection between postmenopausal osteoporosis and the risk of worsening calcium balance in urolithiasis, based on the complex thermogravimetric findings and porosity studies of renal stones eliminated by these patients. The study of renal-urinary concretions offers the possibility to amplify the specific effects of the thermal decomposition processes of substances, reflecting the sample�s global composition. The research of calculi porosity gives information about the dissolution possibilities of the renal-urinary concretions. In conclusion, in postmenopausal women, proper administration of bisphosphonate therapy must accompany the supplementary medication with calcium and vitamin D, always correlated to the metabolic status of the osteoporotic patient.

Nortriptyline (NRT), usually conditioned as hydrochloride, is a second-generation tricyclic antidepressant that is primarily used as a treatment option for major depression and chronic pain. In this study, a compatibility evaluation between an active pharmaceutical ingredient (API) and various excipients that form binary mixtures is presented. As investigational methods, ATR-FTIR analysis and PXRD data were recorded for the binary mixtures under ambient conditions, and were later investigated by thermal analysis (TG—thermogravimetry/DTG—derivative thermogravimetry/HF—heat flow) in dynamic air atmosphere. Nortriptyline hydrochloride (NRT) was used as an active pharmaceutical ingredient, and binary mixtures with 13 excipients from different categories were formed (mass ratio 1:1), namely: microcrystalline cellulose (MC), calcium lactate (CaLact), stearic acid (StA), lactose monohydrate (Lact), polyvinylpyrrolidone K30 (PVP), magnesium stearate (MgSt), sodium carboxymethylcellulose (CMCNa), sorbitol (Sorb), mannitol (Man), gelatine (Gel), silica (SiO2), starch (Starch), and talc (T). The PXDR patterns determined for the binary mixtures showed no incompatibility between the components since the diffraction profile consists in the superposing of the components’ patterns, while the ATR-FTIR study indicated interactions in the discussed binary mixtures, suggesting that NRT is incompatible with PVP, Sorb, Man, StA, and Lact even under ambient conditions. Under thermal stress, interactions between NRT and excipients were observed at temperatures below the melting point of the API in the binary mixtures with PVP, Sorb, Man, StA, and Lact, while at high temperatures, all binary mixtures are thermally degraded due to thermolysis.

We report a very rare case of a 57-year-old male who presented four left renal arteries (RAs) [one main RA and three additional renal arteries (AdRAs)] highlighted incidentally on multidetector computed tomography (MDCT) angiography, which was used to investigate the vascular system of the lower limbs. The distance between the extreme points of RAs origin (upper and lower points of origin) from abdominal aorta (AA) was in the left part of 9.83 cm. The distance between the extreme points of penetration (upper and lower points of penetration) into the left renal parenchyma was 5.23 cm. At the level of origin, the main left RA has an endoluminal diameter of 0.63 cm, much larger in comparison to the other additional left RAs (0.43 cm, 0.33 cm and 0.28 cm, respectively). The length of the main left RA was 2.16 cm, significantly shorter in comparison with the other additional left RAs (2.21 cm, 4.26 cm and 4.73 cm, respectively). The second left RA was the main RA; the first left RA was AdRA (polar superior RA); the third left RA was AdRA (hilar RA); the fourth RA was AdRA (polar inferior RA). Knowledge of this anatomical variation should be considered in planning and performing renal vessel surgery, and kidney transplantation.

The authors illustrate a case of a 61-year-old male who presented an extremely rare association of anatomical variations highlighted by multi-detector computed tomography (MDCT) angiography, with a replaced right hepatic artery (RRHA) arising from inferior pancreaticoduodenal artery (IPDA), in association with left multiple renal arteries (RAs). The celiac trunk (CT) arises from the abdominal aorta (AA), at the level of middle 1∕3 of L1 vertebral body. The superior mesenteric artery (SMA) origin was located at the anterior aspect of AA, at 2.5 mm below the origin of CT, at the level of L1∕L2 intervertebral discs. The SMA has at origin an endoluminal diameter of 11.3 mm. At 22.7 mm from its aortic origin, from the right aspect of the SMA trunk, arises IPDA. At 10.4 mm from its origin in IPDA, arises RRHA with a 78.5 mm artery length and the endoluminal diameter at origin of 2.9 mm. From the arising point, the RRHA is oriented ascending to the right, passing initially posterior to the hepatic portal vein and the head of the pancreas, then lateral to the head of the pancreas and posterior to the hepatic portal vein, after entering the hepatic parenchyma to bifurcate into the anterior and posterior branches. From left aspect of AA arise three RAs: one main, one additional (from AA), and an accessory renal (from left common iliac artery). Knowledge of this hepatic and renal anatomical variation is important for interventional radiologists, vascular experts, oncologists, vascular, hepatic and urologic surgeons.

Background: The association of vascular remodeling in the kidney and the brain with a particular microRNAs (miRNA) profile is not well studied.Methods: Seventy-six patients with Type 2 diabetes and 11 healthy subjects were assessed concerning urine albumin: creatinine ratio (UACR), biomarkers of podocyte injury and of proximal tubule (PT) dysfunction. MiRNA were quantified in blood and urine by a real-time PCR System. Cerebrovascular ultrasound measurements were performed in the carotid and middle cerebral arteries.Results: MiRNA21 and miRNA124 correlated positively with nephrin, podocalyxin, synaptopodin, urinary N-acetyl-D-glucosaminidase (NAG), urinary kidney-injury molecule-1 (KIM-1), UACR, and negatively with eGFR; miRNA125a, 126, 146a, 192 correlated negatively with nephrin, podocalyxin, synaptopodin, urinary NAG, urinary KIM-1, UACR, and directly with eGFR. Plasma miRNA-21 and miRNA192 correlated directly with cerebral hemodynamics parameters of atherosclerosis and arteriosclerosis. MiRNA-124, 125a, 126, 146a showed negative correlations with the same parameters.Conclusions: In Type 2 diabetes patients there is an association of vascular remodeling in the brain and the kidney with a specific miRNAs pattern. Cerebrovascular changes occur even in normoalbuminuric patients, with 'high-to-normal' levels of podocyte injury and PT dysfunction biomarkers. These phenomena may be explained by the variability of miRNA expression within the two organs in early DKD.

The influence of excipients on the stability of sodium levothyroxine pentahydrate (LTSS) under ambient conditions and thermal stress was evaluated. Since LTSS is a synthetic hormone with a narrow therapeutic index, the interactions of LTSS with excipients can lead to a drastic diminution of therapeutic activity. Ten commonly used pharmaceutical excipients with different roles in solid formulations were chosen as components for binary mixtures containing LTSS, namely, starch, anhydrous lactose, D-mannitol, D-sorbitol, gelatin, calcium lactate pentahydrate, magnesium stearate, methyl 2-hydroxyethyl cellulose (Tylose), colloidal SiO2 (Aerosil) and talc. As investigational tools, universal attenuated total reflectance- Fourier transform infrared spectroscopy UATR-FTIR spectroscopy and thermal analysis were chosen and used as follows: UATR-FTIR spectra were drawn up for samples kept under ambient conditions, while thermoanalytical tools (TG/DTG/HF data) were chosen to evaluate the inducing of interactions during thermal stress. The corroboration of instrumental results led to the conclusion that LTSS is incompatible with lactose, mannitol and sorbitol, and these excipients should not be considered in the development of new generic solid formulations.

BACKGROUND/AIM Chemotherapy-induced cardiotoxicity may be observed during treatment or may cause severe cardiac failure as the main cause of death, even several years after therapy implementation. Background: Diabetic nephropathy is a severe complication of Type 2 diabetes. Tubular lesions may play an important role in its early stages. The aim of our study was to determine if atorvastatin protects the podocytes and the proximal tubule in patients with Type 2 diabetes. Methods: A total of 63 patients with Type 2 diabetes completed this 6-months prospective pilot study. They were randomized to continue rosuvastatin therapy (control group) or to be administered an equipotent dose of atorvastatin (intervention group), and were assessed regarding urinary podocytes, podocyte-associated molecules, and biomarkers of proximal tubule dysfunction. The authors describe a case of a 61-year-old female patient, which presented on multidetector computed tomographic (MDCT) angiography a gastrosplenic trunk (GST) and common hepatic artery (CHA) arose independently from abdominal aorta (AA). The GST arose from the anterior wall of the AA, at the level of upper edge of the L1 vertebral body. The left gastric artery (LGA) arose from the superior wall of the GST. The splenic artery (SA) continuous the path of GST. The CHA arose from the anterior wall of the AA, at the level of upper one third of the L1 vertebral body, at 15.3 mm above the origin of superior mesenteric artery (SMA). The incidence and developmental and clinical significance of this vascular variation is discussed with a detailed review of the literature.

The active form of vitamin D, 1,25-dihydroxycholecalciferol (1,25(OH)2D3), was reported to improve vascular function in patients with diabetes, yet the underlying mechanisms remain to be fully elucidated. Monoamine oxidase (MAO), a mitochondrial enzyme, with two isoforms (A and B) that generates hydrogen peroxide (H2O2) as by-product, has been recently reported to contribute to the pathogenesis of endothelial dysfunction in diabetes. The present study assessed the interaction between vitamin D and MAO in the vascular wall in the setting of type 1 experimental diabetes. To this aim, diabetes was induced in male Wistar rats via a single injection of streptozotocin (STZ, 50 mg/kg, IP) and 1 month later thoracic aortas were harvested and used for organ bath studies and H2O2 measurements. MAO expression was assessed by immunohistochemistry and RT-PCR. Endothelial function was evaluated in isolated aortic rings in the absence vs. presence of 1,25(OH)2D3 (100 nM, 24 h incubation). In diabetic animals, we found a significant reduction in the endothelial-dependent relaxation to acetylcholine and an increased expression of the MAO-A isoform, respectively. Vitamin D significantly improved vascular function, mitigated oxidative stress and decreased MAO-A expression in diabetic vascular preparations. In conclusion, MAO-A is induced in diabetic aortas and vitamin D can improve diabetes-induced endothelial dysfunction by modulating the MAO-A expression.

While laparoscopic cholecystectomy can be a routine procedure when biliary anatomy is normally located, cystic artery variations can easily disorientate the inexperienced surgeon to the anatomy of the hepatobiliary triangle. This study presents the clinically important anatomical variations of the cystic artery. PubMed, Medline, Cochrane Database of Systematic Reviews, and Google Scholar databases were searched to conduct a review of the existing English literature on the clinically important cystic artery variations. An aberrant vessel was defined as a vessel that originated from an atypical source and/or one that was present in a specimen in addition to the normal vessel. The cystic artery originated typically from the right hepatic artery (79.02 %) and was found in the hepatobiliary triangle in only 5427 of 6661 (81.5 %) cases. Clinically important cystic artery variations are (1) the cystic artery located anterior to the common hepatic duct or common bile duct found in 485 of 2704 (17.9 %) and 228 of 4202 (5.4 %) of cases, respectively, (2) the cystic artery located inferior to the cystic duct found in 38 of 770 (4.9 %) of cases, (3) short cystic arteries found in 98 of 1037 (9.5 %) cases and (4) multiple cystic arteries found in (8.9 %) of cases. These variations are common in the general population and can lead to inadvertent ligation of biliary ducts or aberrant vessels. Therefore, it is important for the hepatobiliary surgeon to be aware of these vascular anomalies to avoid operative complications.

Introduction Multiple surgical approaches and combinations thereof have been described to gain access to the jugular foramen. In an area laden with important neurovascular structures, care must be taken in choosing the best surgical approach for treatment of rare pathologies involving this region. Methods This manuscript provides a comprehensive review of the relevant anatomy along with an overview of the various approaches to the jugular foramen. In an attempt to simplify the various concepts, we propose a basic distinction into anterolateral and posterolateral approaches based on the main trajectory targeting the jugular foramen. Conclusion The anatomy surrounding the jugular foramen is exceedingly complex and requires in-depth understanding of skull base and head and neck relationships.

External to internal carotid artery anastomoses are normally found in man. However, such connections at the skull base between the cavernous internal carotid artery and middle meningeal artery are rarely discussed in the literature and when present, are difficult, if not impossible, to see with imaging studies unless pathologically enlarged. Therefore, the aim of the present anatomic study was to investigate such connections via cadaveric dissection. In 12 latex-injected adult cadaveric heads (24 sides), microdissection was performed to investigate for connections at the skull base between the cavernous part of the internal carotid and middle meningeal arteries. Additionally, neck dissections were performed to observe for atherosclerosis of the internal carotid artery. Statistical analyses were performed for differences between sides and gender. On 14 sides (58.3 %) (nine left and five right sides), arterial anastomoses were identified between the middle meningeal and internal carotid arteries. All connections were small with a mean length of 8 mm and a mean diameter of 0.9 mm. These connections were found between the lateral aspect of the cavernous internal carotid artery and the medial side of the main trunk of the middle meningeal artery on 64.3 % of sides, the anterior branch of the middle meningeal artery on 7 % of sides, and the posterior branch of the middle meningeal artery on 28.6 % of sides. The size of the middle meningeal and internal carotid arteries was within normal limits in all specimens. Although not statistically significant, connections tended to be more frequently found on the left sides. The majority of cadaveric sides in our study were found to have anastomoses at the skull base between the cavernous segment of the internal carotid and middle meningeal arteries. These very small connections would most likely not be visualized on imaging studies. These connections most likely function in times of carotid artery ischemia that is chronic in nature (e.g., Moya Moya disease) and may, in the past, have been misidentified as a human rete mirabile.

Knowledge of the lymphatic system plays critical importance in surgical oncology. The study of the lymphatic system and its role in tumor metastasis continues to advance with new anatomical and surgical studies, and with a new study, we can gain a better understanding on how aggressive surgeons need to be with nodal dissection while balancing the complications with overly aggressive approaches. The lymphatics of the inguinal region represent a network of lymph nodes and vessels, which act as the bridge among the lower extremities, pelvis, perineum, and rest of the body. These lymph nodes are of particular importance in the metastatic spread of genitourinary and lower gastrointestinal epithelial cancer to the inguinal nodes. The aim of this article is to reveal the literature with regard to the inguinal lymph nodes and their relation in various carcinomas.

MicroRNAs (miRNAs) are short non-coding RNA species that are important post-transcriptional regulators of gene expression. The aim of the study was to establish a potential explanation of podocyte damage and proximal tubule (PT) dysfunction induced by deregulated miRNAs expression in the course of type 2 diabetes mellitus (DM). A total of 68 patients with type 2 DM and 11 healthy subjects were enrolled in a cross-sectional study and assessed concerning urinary albumin:creatinine ratio (UACR), urinary N-acetyl-β-D-glucosamininidase (NAG), urinary kidney injury molecule-1, urinary nephrin, podocalyxin, synaptopodin, estimated glomerular filtration rate (eGFR), urinary miRNA21, miRNA124, and miRNA192. In univariable regression analysis, miRNA21, miRNA124, and miRNA192 correlated with urinary nephrin, synaptopodin, podocalyxin, NAG, KIM-1, UACR, and eGFR. Aims To evaluate if there is a link between inflammation (expressed by inflammatory cytokines) and the early stage of diabetic kidney disease (DKD), as shown by markers of podocyte damage and proximal tubular (PT) dysfunction. Methods In this study were enrolled 117 type 2 DM patients (36-normoalbuminuria, 42-microalbuminuria, 39- macroalbuminuria), and 11 healthy subjects. Serum and urinary IL-1 alpha, IL-8, IL-18, urinary albumin:creatinine ratio (UACR), eGFR, biomarkers of podocyte damage (podocalyxin, synaptopodin, nephrin) and of PT dysfunction (KIM-1, NAG) were assessed. Results In multivariable regression urinary Il-1 alpha correlated positively with podocalyxin and NAG (p Conclusions Pro-inflammatory interleukins are associated with podocyte injury and PT dysfunction in early DKD. We present a case of a 73-year-old male with peripheral vascular disease of the lower limbs, who on shown MDCT angiography, to have a hepatomesenteric trunk (HMT), and left gastric artery (LGA) and splenic artery (SpA) arising independently from the abdominal aorta (AA), and with additional right renal arteries. The HMT with a length of 5 mm (with an aspect of a common stem origin of common hepatic artery (CHA) and superior mesenteric artery (SMA) arose from the anterior wall of the AA at the level of upper one-third of the L2 vertebral body. The CHA originating from the superior part of the right edge of the HMT, have an ascendent path in front of AA, and finally fork in gastroduodenal artery and hepatic artery proper. With a descending path the SMA across the left renal vein, uncinate process, and inferior part of the duodenum (D3) made an aortomesenteric angle of 61°. The aortomesenteric distance at the level of the L3 vertebral body was 51 mm. The present case is only the 13th reported HMT in association with an independently arising LGA and SpA from the AA, the first case report with this condition using MDCT angiography. Knowledge of the variations in origin and distribution of the HMT is important for planning and performing procedures such as duodenopancreatectomy, liver transplantation, and chemoembolization of the pancreas and hepatic tumors.

Bilateral tracheal bronchus is a rare variation of the tracheobronchial tree. We present a 1-year 7-month-old male patient who presented with sepsis following endotracheal intubation. Upon review of multidetector computed tomography images, the patient was diagnosed with displaced bilateral tracheal bronchus. Imaging showed a right-sided anomalous bronchus arising 0.9 cm proximal to the carina. The left-sided anomalous bronchus arose 0.7 cm proximal to the carina, mimicking a tracheal trifurcation. When viewed together, the close proximity of both the right and left tracheal bronchi to the carina created a distinct tracheal quadrifurcation. This rare anatomic variation was additionally associated with an anorectal malformation (anal atresia). Unrecognized tracheal bronchus in patients undergoing endotracheal intubation can lead to serious complications. While bilateral tracheal bronchus is described in the literature, we are unaware of any case similar to this patient presentation. We present and analyze this unusual case of bilateral tracheal bronchus. The anatomy and clinical significance of this variation is then discussed.

Occipital neuralgia is a debilitating disorder first described in 1821 as recurrent headaches localized in the occipital region. Other symptoms that have been associated with this condition include paroxysmal burning and aching pain in the distribution of the greater, lesser, or third occipital nerves. Several etiologies have been identified in the cause of occipital neuralgia and include, but are not limited to, trauma, fibrositis, myositis, fracture of the atlas, and compression of the C-2 nerve root, C1-2 arthrosis syndrome, atlantoaxial lateral mass osteoarthritis, hypertrophic cervical pachymeningitis, cervical cord tumor, Chiari malformation, and neurosyphilis. The management of occipital neuralgia can include conservative approaches and/or surgical interventions. Occipital neuralgia is a multifactorial problem where multiple anatomic areas/structures may be involved with this pathology. A review of these etiologies may provide guidance in better understanding occipital neuralgia.

Aortoiliac occlusive disease is a subset of peripheral arterial disease involving an atheromatous occlusion of the infrarenal aorta, common iliac arteries, or both. The disease, as it is known today, was described by the French surgeon Rene Leriche as a thrombotic occlusion of the end of the aorta. Leriche successfully linked the anatomic location of the occlusion with a unique triad of symptoms, including claudication, impotence, and decreased peripheral pulses. The anatomical location of the atheromatous lesions also has a direct influence on classification of the disease, as well as choice of treatment modality. Considering its impact on diagnosis and treatment, we aimed to provide a detailed understanding of the anatomical structures involved in aortoiliac occlusive disease. Familiarity with these structures will aid the physician in interpretation of radiologic images and surgical planning.

Ectopia cordis (EC) is a rare congenital anomaly associated with the heart positioned outside of the thoracic cavity either partially or completely. The ectopic heart can be found along a spectrum of anatomical locations, including the cervical, thoracic and abdominal regions and in most cases, it protrudes outside the chest through a split sternum. Although the first case of EC was identified during the early 1600s only 91 cases have been reported since then in the literature. This review will discuss the history and prevalence of EC, its etiology, morphology, presentation and symptoms, complications, diagnosis, treatment and management and prognosis.

Tracheobronchial variations can be found during routine bronchoscopy or computed tomography. Previous sources estimate an incidence of 1-12%; however, these variations are often asymptomatic. Symptomatic patients present typically with cough and lower respiratory tract infection. Knowledge and understanding of tracheobronchial variations have important implications for diagnosis of symptomatic patients and performing certain procedures, including bronchoscopy and endotracheal intubation. In this review, we describe the most commonly encountered variations, tracheal bronchus and accessory cardiac bronchus, along with three minor abnormalities of this region. We also review the various imaging modalities in the diagnosis and treatment of these conditions.

Background Recent advances in Bell’s palsy (BP) were reviewed to assess the current trends in its management and prognosis. Material/Methods We retrieved the literature on BP using the Cochrane Database of Systematic Reviews, PubMed, and Google Scholar. Key words and phrases used during the search included ‘Bell’s palsy’, ‘Bell’s phenomenon’, ‘facial palsy’, and ‘idiopathic facial paralysis’. Emphasis was placed on articles and randomized controlled trails (RCTs) published within the last 5 years. Results BP is currently considered the leading disorder affecting the facial nerve. The literature is replete with theories of its etiology, but the reactivation of herpes simplex virus isoform 1 (HSV-1) and/or herpes zoster virus (HZV) from the geniculate ganglia is now the most strongly suspected cause. Despite the advancements in neuroimaging techniques, the diagnosis of BP remains one of exclusion. In addition, most patients with BP recover spontaneously within 3 weeks. Conclusions Corticosteroids are currently the drug of choice when medical therapy is needed. Antivirals, in contrast, are not superior to placebo according to most reliable studies. At the time of publication, there is no consensus as to the benefit of acupuncture or surgical decompression of the facial nerve. Long-term therapeutic agents and adjuvant medications for BP are necessary due to recurrence and intractable cases. In the future, large RCTs will be required to determine whether BP is associated with an increased risk of stroke.

The authors report a case of a 44-year-old male found to have unusual origins of the celiac trunk (CT) and superior mesernteric artrery (SMA) as revealed by routine multidetector computed tomograph (MDCT) angiography. The CT and SMA originate from the thoracic aorta (TA) 21 mm and 9 mm above the aortic hiatus, respectively. The median arcuate ligament (MAL) is located at the level of the L1–L2 intervertebral disc. The course of the CT descends in the thoracic cavity making a 14° acute downward angle in front of the TA; below the level of the MAL, the CT descends, making an angle of 47°. The course of the SMA descends at both the thoracic and abdominal level making an angle of 17°, and having an aortomesenteric distance of 9 mm at the level of the third part of the duodenum. In the present case, the supradiaphragmatic origin of the CT and the SMA was determined by their incomplete caudal descent, associated with a pronounced apparent descent of the diaphragm. A thoracic origin of the CT and SMA and the acute downward aortomesenteric angle (17°) associated with a reduced aortomesenteric distance at the level of the third part of the duodenum (9 mm), although no clinical signs are present, may predispose the patient to develop simultaneously a triple syndrome: the compression of CT by MAL (celiac axis compression syndrome), the compression of SMA by MAL (superior mesenteric artery compression syndrome), and the compression of the duodenum by the SMA (superior mesenteric artery syndrome). Aims Detection of podocytes in the urine of patients with type 2 diabetes may indicate severe injury to the podocytes. In the course of type 2 diabetes the proximal tubule is involved in urinary albumin processing. We studied the significance of podocyturia in relation with proximal tubule dysfunction in type 2 diabetes. Methods A total of 86 patients with type 2 diabetes (34-normoalbuminuria; 30-microalbuminuria; 22-macroalbuminuria) and 28 healthy subjects were enrolled in the study and assessed concerning urinary podocytes, podocyte-associated molecules, and biomarkers of proximal tubule dysfunction. Urinary podocytes were examined in cell cultures by utilizing monoclonal antibodies against podocalyxin and synaptopodin. Results Podocytes were detected in the urine of 10% of the healthy controls, 24% of the normoalbuminuric, 40% of the microalbuminuric, and 82% of the macroalbuminuric patients. In multivariate logistic regression analysis, urinary podocytes correlated with urinary albumin:creatinine ratio (p = 0.006), urinary nephrin/creat (p = 0.001), urinary vascular endothelial growth factor/creat (p = 0.001), urinary kidney injury molecule-1/creat (p = 0.003), cystatin C (p = 0.001), urinary advanced glycation end-products (p = 0.002), eGFR (p = 0.001). Conclusions In patients with type 2 diabetes podocyturia parallels proximal tubule dysfunction independently of albuminuria and renal function decline. Advanced glycation end-products may impact the podocytes and the proximal tubule.

We describe in this paper a rare case of a 45-year-old male with a common stem origin of the left gastric artery (LGA), right inferior phrenic artery (RIPA), and left inferior phrenic artery (LIPA), in association with the presence of a hepatosplenomesenteric trunk (HSMT) arising from the abdominal aorta (AA), as revealed by routine multidetector computed tomography (MDCT) angiography. The common stem origin of the LGA, RIPA, and LIPA had an endoluminal diameter of 3.3 mm, the LGA of 2.8 mm. The endoluminal diameter of the RIPA and LIPA was at the origin of approximately 1 mm, complicating selective chemoembolization of the liver parenchyma. Clin. We describe in this article a rare case of a 39-year-old male with an inferior phrenic arteries trunk (IPAaT) originating from a common stem with a superior additional left renal artery (SAdLRA) from the abdominal aorta as revealed by routine multidetector computed tomography angiography. The IPAaT with an endoluminal diameter at the origin of 2.8 mm had an upward path with a total length of 18.4 mm, forking to the right inferior phrenic artery and left inferior phrenic artery. These two arteries had an endoluminal diameter at the origin of 1.7 mm and 2.0 mm, respectively. The presence of the common stem of the IPAaT with a SAdLRA and the length of the IPAaT complicate selective chemoembolization of the liver parenchyma.

The authors describe a case of a 57-year-old male patient with peripheral vascular disease which presented on MDCT angiography the absence of the celiac trunk (CT). The left gastric artery (LGA), common hepatic artery (CHA), and splenic artery (SA) (with an internal diameter at origin of 3.3, 6.0, and 6.2 mm, respectively) originated directly and independently from the abdominal aorta (AA). The LGA arose from the anterior wall of the AA [27 mm above the origin of the superior mesenteric artery (SMA)]. The SA and CHA arose from the left side of the anterolateral wall of the AA at 19 and 14 mm (respectively) above the origin of the SMA. The incidence and developmental and clinical significance of this vascular variation are discussed with a detailed review of the literature.

The proatlas is derived from the fourth occipital sclerotome in human embryos. It usually fuses with the three upper occipital sclerotomes to form the occipital bone. However, this does not always occur. Manifestations of a partial proatlas structure may persist due to failure of fusion. These embryological remnants can induce several symptoms in humans, ranging from mild to severe. On occasion, this structure can go unnoticed until a precipitating traumatic event results in symptoms. Proatlas segmentation abnormalities form bony masses at C1 and the foramen magnum. A number of surgical procedures have been devised to rectify the resulting neural compression and vascular compromise. This paper will discuss the development of the proatlas and the resultant anomalies associated with its failure to merge with the occipital sclerotomes to form the occipital bone. In addition, some consideration of comparative anatomy and surgical techniques will be presented.
