Abstract

Chitosan is a marine-origin polysaccharide obtained from the deacetylation of chitin, the main component of crustaceans’ exoskeleton, and the second most abundant in nature. Although this biopolymer has received limited attention for several decades right after its discovery, since the new millennium chitosan has emerged owing to its physicochemical, structural and biological properties, multifunctionalities and applications in several sectors. This review aims at providing an overview of chitosan properties, chemical functionalization, and the innovative biomaterials obtained thereof. Firstly, the chemical functionalization of chitosan backbone in the amino and hydroxyl groups will be addressed. Then, the review will focus on the bottom-up strategies to process a wide array of chitosan-based biomaterials. In particular, the preparation of chitosan-based hydrogels, organic–inorganic hybrids, layer-by-layer assemblies, (bio)inks and their use in the biomedical field will be covered aiming to elucidate and inspire the community to keep on exploring the unique features and properties imparted by chitosan to develop advanced biomedical devices. Given the wide body of literature that has appeared in past years, this review is far from being exhaustive. Selected works in the last 10 years will be considered.

Abstract

We make use of a previous well tested Galactic model, but describing the observational behavior of the various stellar components in terms of suitable assumptions on their evolutionary status. In this way we are able to predict the expected distribution of Galactic White Dwarfs (WDs), with results which appear in {\em rather good} agreement with recent estimates of the local WD luminosity function. The predicted occurrence of WDs in deep observations of selected galactic fields is presented, discussing the role played by WDs in star counts. The effects on the theoretical predictions of different White Dwarfs evolutionary models, ages, initial mass functions and relations between progenitor mass and WD mass are also discussed., Comment: 9 pages with 11 eps figs

Abstract

Taking advantage of recent HST data for field stars in the region of the Galactic globular cluster NGC 6397, we tested the predictions of several Galactic models with star counts reaching a largely unexplored range of magnitudes, down to $V$$\sim$ 26.5. After updating the input stellar ($V-I$) colors, we found that the two-component Bahcall-Soneira (B&S) model can be put in satisfactory agreement with observations for suitable choices of disk/spheroid luminosity functions. However if one assumes the Gould, Bahcall and Flynn (1996, 1997) disk luminosity function (LF) together with the Gould, Flynn and Bahcall (1998) spheroid LF, there is no way to reconcile the predicted and observed $V $-magnitude distribution. We also analysed the agreement between observed and predicted magnitude and color distributions for two selected models with a thick disk component. Even in this case there are suitable combinations of model parameters and faint magnitude LFs which can give a reasonable agreement with observational star counts both in magnitude and in color, the above quoted combination of Gould et al. (1997, 1998) LFs giving again predictions in clear disagreement with observations., Comment: 11 pages with 17 eps figs

Abstract

On the basis of theoretical evolutionary expectations, we develop a Galactic model reproducing star counts and synthetic color-magnitude diagrams of field stars, which include the white dwarf (WD) population. In this way we are able to evaluate the expected occurrence of WDs in deep observations at the various photometric bands and Galactic coordinates, discussing the contribution of the WDs of the various Galactic components. The effects on the theoretical predictions of different WD evolutionary models, ages, initial mass functions and relations between progenitor mass and WD mass are discussed., Comment: 4 pages, 3 eps figures. Abstract

Starch in its native structure, is the largest energy source in human and animal diet; moreover it is capturing growing attention as a renewable natural resource in material science. Indeed by chemical, physical or enzymatic modification, it can change its physio-chemical properties and becomes suitable in the food industry as densifier or in packaging as biodegradable plastic (1). This work aims at studying different strategies for the modification of starch in order to introduce a photoresponsive behavior by a cinnamyl group. The photoreversible functionality may allow the cross-linking of starch with other (Bio)(macro)molecules, including inorganic matrices for the design of hybrid materials (see Orsini abstract). Modification of starch was accomplished by different chemistries (i.e. etherification, esterification) in order to optimize the degree of substitution, which is fundamental in determining the subsequent photoreactive process. The experimental determination of the degree of substitution (DS), defined as the average number of functional groups introduced in the anhydro glucose unit (AGU), is a key parameter. In order to properly quantify the degree of substitution DS the 1H-NMR technique is preferred to titration, since titration suffers from lack of accuracy and reproducibility (2). In the 1H-NMR analysis the dissolution of the sample is a key issue for the quantitative estimation of DS. However, dissolution of starch and its derivatives is not trivial (3).

Abstract

Polysaccharides natural polymers are very abundant in nature and available in most part of heart and seas. Among polysaccharides starch is capturing attention because of its large availability in nature. Starch is an example of renewable resource suitable as raw material for plastic consumer market. Compared to plastics derived by fossil resources it has the advantages of biodegradability, biocompatibility and possible conversion to energy source such as biogas. Nowadays modified starch has not yet mechanical robustness and chemical inertness to be competitive with plastics present on the market, however the research is intense and with interesting sparks. Some research activities concern the modification of starch with natural molecules. Cinnamic acid, in particular, is characterized by anti-inflammatory properties. The studies are devoted to the synthesis of an augmented polymer by merging the biocompatibility with the therapeutic properties of the functional group. Present work study different strategies for the modification of starch (Scheme 1) in order to introduce the cinnamyl group. Moreover this group allows the photoinduced cross-linking of starch improving mechanical properties. Modification of starch is accomplished by different chemistries in order to optimize the degree of substitution (DS), defined as the average number of functional groups introduced in the anhydro glucose unit (AGU). DS is the key parameter to quantify the modification of the polymer and 1H-NMR is the chosen for this determination, preferred to titration that suffers from lack of accuracy and reproducibility.1 In the 1H-NMR analysis the dissolution of the sample and the presence of hydroxyl groups make hard the accurate DS determination.

Abstract

The present study reports on the synthesis of a new alkoxysilane-bearing light-responsive cinnamyl group and its application as a surface functionalization agent for the development of SiO2 nanoparticles (NPs) with photoreversible tails. In detail, cinnamic acid (CINN) was activated with N-hydroxysuccinimide (NHS) to obtain the corresponding NHS-ester (CINN-NHS). Subsequently, the amine group of 3-aminopropyltriethoxysilane (APTES) was acylated with CINN-NHS leading to the generation of a novel organosilane, CINN-APTES, which was then exploited for decorating SiO2 NPs. The covalent bond to the silica surface was confirmed by solid state NMR, whereas thermogravimetric analysis unveiled a functionalization degree much higher compared to that achieved by a conventional double-step post-grafting procedure. In light of these intriguing results, the strategy was successfully extended to naturally occurring sepiolite fibers, widely employed as fillers in technological applications. Finally, a preliminary proof of concept of the photoreversibility of the obtained SiO2@CINN-APTES system has been carried out through UV diffuse reflectance. The overall outcomes prove the consistency and the versatility of the methodological protocol adopted, which appears promising for the design of hybrid NPs to be employed as building blocks for photoresponsive materials with the ability to change their molecular structure and subsequent properties when exposed to different light stimuli.

Abstract

Background: Risdiplam is an oral small molecule approved for the treatment of patients with spinal muscular atrophy, with approval for use in patients with type 2 and type 3 spinal muscular atrophy granted on the basis of unpublished data. The drug modifies pre-mRNA splicing of the SMN2 gene to increase production of functional SMN. We aimed to investigate the safety and efficacy of risdiplam in patients with type 2 or non-ambulant type 3 spinal muscular atrophy. Methods: In this phase 3, randomised, double-blind, placebo-controlled study, patients aged 2–25 years with confirmed 5q autosomal recessive type 2 or type 3 spinal muscular atrophy were recruited from 42 hospitals in 14 countries across Europe, North America, South America, and Asia. Participants were eligible if they were non-ambulant, could sit independently, and had a score of at least 2 in entry item A of the Revised Upper Limb Module. Patients were stratified by age and randomly assigned (2:1) to receive either daily oral risdiplam, at a dose of 5·00 mg (for individuals weighing ≥20 kg) or 0·25 mg/kg (for individuals weighing <20 kg), or daily oral placebo (matched to risdiplam in colour and taste). Randomisation was conducted by permutated block randomisation with a computerised system run by an external party. Patients, investigators, and all individuals in direct contact with patients were masked to treatment assignment. The primary endpoint was the change from baseline in the 32-item Motor Function Measure total score at month 12. All individuals who were randomly assigned to risdiplam or placebo, and who did not meet the prespecified missing item criteria for exclusion, were included in the primary efficacy analysis. Individuals who received at least one dose of risdiplam or placebo were included in the safety analysis. SUNFISH is registered with ClinicalTrials.gov, NCT02908685. 34. Positive Youth Development Programs in Low- and Middle-Income Countries: A Conceptual Framework and Systematic Review of Efficacy

Abstract

PURPOSE: Positive youth development (PYD) has served as a framework for youth programs in high-income countries since the 1990s and has demonstrated broad behavioral health and developmental benefits. PYD programs build skills, assets, and competencies; foster youth agency; build healthy relationships; strengthen the environment; and transform systems to prepare youth for successful adulthood. The goal of this article was to systematically review the impact of PYD programs in low- and middle-income countries (LMICs). METHODS: Targeted searches of knowledge repository Web sites and keyword searches of Scopus and PubMed identified over 21,500 articles and over 3,700 evaluation reports published between 1990 and mid-2016. Ninety-four PYD programs with evaluations in LMICs were identified, of which 35 had at least one experimental or rigorous quasi-experimental evaluation. RESULTS: Sixty percent of the 35 programs with rigorous evaluations demonstrated positive effects on behaviors, including substance use and risky sexual activity, and/or more distal developmental outcomes, such as employment and health indicators. CONCLUSIONS: There is promising evidence that PYD programs can be effective in LMICs; however, more rigorous examination with long-term follow-up is required to establish if these programs offer benefits similar to those seen in higher income countries.

35. Iontophoretic Transepithelial Collagen Cross-Linking Versus Epithelium-Off Collagen Cross-Linking in Pediatric Patients: 3-Year Follow-Up

Abstract

Purpose: To compare 3-year iontophoretic transepithelial corneal cross-linking (I-ON CXL) outcomes with epithelium-off collagen cross-linking (epi-off CXL) in pediatric patients.Methods: Forty eyes of 28 consecutive pediatric patients [mean age 14.3+/-2.5 (SD) years; range, 9-18 years] with keratoconus were evaluated. Twenty eyes of 15 patients underwent I-ON and 20 eyes of 13 patients epi-off CXL. Mean corrected distance visual acuity, spherical equivalent, maximum keratometry (Kmax), posterior elevation of the thinnest point, and thickness of the thinnest point were evaluated. The Student t test was used to compare baseline and postoperative data. Keratoconus progression as a function of preoperative Kmax and cone location was evaluated.Results: At 36 months, corrected distance visual acuity statistically improved (from 0.18+/-0.1 to 0.10+/-0.1 logarithm of the minimum angle of resolution) in epioff CXL, whereas spherical equivalent and posterior elevation of the thinnest point did not significantly change. Mean Kmax increased from the baseline +0.8 diopters (D) in epi-off and +2.9D in I-ON. In both groups, the thinnest point decreased. Keratoconus progression, defined by an increase of Kmax reading of the anterior corneal surface of at least 1.00D, occurred in 25% of epi-off and 50% of I-ON CXL over the 3-year period. Kmax value in I-ON, and cone location in both groups, seemed to be factors influencing the disease progression.Conclusions: In pediatric patients, 3 years after treatment, epi-off CXL halted keratoconus progression in 75% of eyes, whereas I-ON CXL seemed to slow down keratoconus progression in 50% of eyes, mainly in those with highest Kmax and paracentral cone.

Abstract

Background: To evaluate refractive outcome 24 months after Deep Anterior Lamellar Keratoplasty (DALK) in pediatric patients by comparing results achieved using mechanical trephine and femtosecond laser.Methods: Twenty eyes of 20 patients affected by keratoconus were evaluated. To perform big-bubble DALK, 10 eyes (Group 1; mean age 11.2 +/- 22 years) were subjected to the Hessburg-Barron mechanical trephine and the remaining 10 eyes (Group 2; mean age 11.3 +/- 3.1 years) to a 150 kHz femtosecond laser that performed mushroom incisions. Preoperative thinnest point in the corneal thickness map and K readings were measured by the Sirius Scheimpflug camera. We also evaluated corrected distance visual acuity (CDVA) as logMAR value using spectacles, spherical equivalent and refractive astigmatism.Results: Mean preoperative thinnest point and pre- and post-operative K readings did not show significant difference (P> 0.05) between the two groups. CDVA, spherical equivalent and refractive astigmatism were respectively, 0.14 +/- 0.08 logMAR and 0.13 +/- 0.10 logMAR (P= 0.8), - 4.2 +/- 1.1 D and - 2.8 +/- 1.2 D (P= 0.03), 4.4 +/- 2.0 D and 3.6 +/- 1.2 D (P= 0.4) in Groups 1 and 2. All DALK procedures were uneventful.Conclusion: Our findings suggest that femtosecond laser compared to mechanical trephine could significantly reduce the spherical equivalent amount in pediatric big-bubble DALK.

Abstract

Child marriage and subsequent early first birth is a considerable social, economic and health concern, and a pervasive practice in sub-Saharan Africa and South Asia. This study explores barriers and facilitators to family planning among women and girls, and their marital decision-makers subsequent to receipt of child marriage prevention programmes in Ethiopia and India. In-depth interviews with 128 women and girls who were married as minors or who cancelled or postponed marriage as minors and their marital decision-makers were analysed using content analysis. Respondents identified social norms, including child marriage and pressure to have children, and lack of information as barriers to family planning. Benefits included delayed first birth and increased birth spacing, improved maternal and child health and girls' educational attainment. Respondents associated family planning use with delayed pregnancy and increased educational attainment, particularly in Ethiopia. Child marriage prevention programmes were identified as important sources of family planning information. Ethiopia's school-based programme strengthened access to health workers and contraception more so than India's community-based programme. Findings highlight young wives' vulnerability with regard to reproductive control, and support the need for multi-sector approaches across communities, schools and community health workers to improve family planning among young wives.

Abstract

Adolescence is a critical stage of life characterised by rapid biological, emotional, and social development. It is during this time that every person develops the capabilities required for a productive, healthy, and satisfying life. In order to make a healthy transition into adulthood, adolescents need to have access to health education, including education on sexuality1; quality health services, including sexual and reproductive; and a supportive environment both at home and in communities and countries.The global community increasingly recognises these vital needs of adolescents, and there is an emerging consensus that investing intensively in adolescents’ health and development is not only key to improving their survival and wellbeing but critical for the success of the post-2015 development agenda.2 The suggested inclusion of adolescent health in the United Nations secretary general’s Global Strategy for Women’s and Children’s Health is an expression of this growing awareness and represents an unprecedented opportunity to place adolescents on the political map beyond 2015. Ensuring that every adolescent has the knowledge, skills, and opportunities for a healthy, productive life and enjoyment of all human rights3 is essential for achieving improved health, social justice, gender equality, and other development goals.We argue that the priority in the revised Every Women Every Child Global Strategy needs to be giving adolescents a voice, expanding their choices and control over their bodies, and enabling them to develop the capabilities required for a productive, healthy, and satisfying life. We call for a global, participatory movement to improve the health of the world’s adolescents as part of a broader agenda to improve their wellbeing and uphold their rights.

Abstract

An increasing number of foreigners was observed in Italy even among chronic dialysis patients. We compared demographic, clinical, treatment characteristics and survival probability between groups of Italian and foreign patients with chronic dialysis in Lazio.We analysed data from the Lazio Dialysis and Transplantation Registry from 2004 to 2012 (4,076 prevalent chronic dialysis patients in 31-12-2012). Among 7,970 incident patients, we evaluated, by country of birth, survival probability by using Kaplan-Meier method and mortality risk, through multiple Cox regression.We observed an increase of foreigners among chronic dialysis patients in Lazio from 2004 (4.4%) up to 2012 (7.6%, test for trend: p0,001). Compared with Italians, foreign patients were younger (53.816.3 vs. 68.713.6) and more frequently: women (42.7% vs. 37.7%), HbsAg-positive (18.1% vs. 13.9%), not vaccinated if HBV susceptible (26.8% vs. 20.9%), late referral (21.6% vs. 12.9%) and suitable for kidney transplantation (21.7% vs. 9.9%). Foreigners compared with Italians had higher survival probability at 1 year (91.9% vs. 84.7%) and 5 years (74,6% vs. 51,5%) after chronic dialysis has been started. We observed a lower mortality risk (HR=0.71;IC95%:0.58-0.87) among foreign patients even after adjustment for many potential confounding factors. Our findings suggest that health status and treatment of end stage renal disease, including access to kidney transplantation, were similar between groups of Italian and foreign patients in Lazio. Observed differences in patients' characteristics were probably attributable to younger age of foreigners. However, further analyses are needed to fully explain higher survival probability in chronic dialysis among foreigners compared with Italians.

Abstract

Contemporary societies are greatly challenged by paradoxes in all facets of life. Digital communication encodes and produces meaning by making use of these contradictory relations. In this contribution, three digital paradoxes will be presented. The first paradox is grounded in the process of remediation in digital settings which mirrors a contradictory double logic. Digital culture, in fact, wants both to multiply its media and to eliminate all traces of mediation. The second antinomy has its origins in the new concept of linguistic entropy: a structured information disorder that is regulated by usability and multimodality.
