ZusammenfassungKann die primäre Radiochemotherapie (pRCT) möglicherweise als alternative Standardtherapie zur totalen Laryngektomie (TL) angesehen werden? Gemäß der neuen S3-Leitlinie nehme der Patient zwar eine höhere Rückfallrate in Kauf, habe aber die Salvagechirurgie als kurative Option und insgesamt keinen Überlebensnachteil. In mehreren großen Datenbankanalysen und Fallserien findet sich für das T4-Larynxkarzinom ein signifikanter Überlebensnachteil der pRCT gegenüber der primären TL von über 30 %. Die Erfolgsrate der Salvagelaryngektomie beim T4-Karzinom liegt laut Literatur nur bei 25–50 %. Larynxorganerhaltstudien, die zur Empfehlung der pRCT als alternativer Standardtherapie führen könnten, sollten 1.) T4a-Karzinompatienten innerhalb ihrer T‑Kategorie auswerten, 2.) getrennt nach Larynx- und Hypopharynxkarzinomen, 3.) in aussagekräftiger Kollektivgröße, 4.) mit einem Nachbeobachtungszeitraum von mindestens 5 Jahren, 5.) mit onkologischen und 6.) funktionellen Outcome (Dauer von Tracheostoma u./o. PEG, Notwendigkeit und Erfolg einer Salvagelaryngektomie). 7.) Angabe des Kriteriums, das zur T4-Kategorie führte (Knorpeldurchbruch oder Art der extralaryngealen Ausbreitung), 8.) prätherapeutische Larynxfunktionalität (Tracheostoma, PEG). Eine Erfassung der genannten klinischen Daten aller T4-pRCT-Patienten in einer prospektiven beobachtenden Kohortenstudie im deutschsprachigen Raum wird vorgeschlagen. Bei Ablehnung der TL beim T4a-Karzinom sollte in ausführlichen einfühlsamen, aber instruktiven Gesprächen unterschieden werden zwischen einer primären spontanen Abneigung und der differenziert abgewogenen, definitiven Ablehnung. Nicht nur der onkologische, sondern auch der funktionell zu erwartende Outcome sollte in den Entscheidungsprozess einbezogen werden.

Zusammenfassung Hintergrund Derzeit gilt das größere, nicht mehr durch Teilresektionen behandelbare T3-Larynxkarzinom als optimaler Kandidat für einen Larynxorganerhalt (LP) mittels primärer Radiochemotherapie (pRCT). Wann wäre eine primäre Strahlentherapie (pRT) ggf. auch ohne Chemotherapiezusatz vertretbar, und wann selbst beim T3 doch eher die totale Laryngektomie mit risikoadaptierter adjuvanter Therapie (TL±aR[C]T) zu empfehlen? Methodik In der Literatur wurde nach Parametern gesucht, die bei nichtrandomisierten zweiarmigen LP-Studien als Kriterium für den Einschluss in den chirurgischen anstelle des konservativen Arms genannt wurden oder die sich nach konservativer Therapie als signifikante Prognosemarker herausstellen. Hieraus Entwicklung eines Beratungsinstruments für die Therapieentscheidung. Ergebnisse Als signifikante Prognosemarker beschrieben wurden das Tumorvolumen, das Vorhandensein und die Art der Stimmbandfixierung, das Ausmaß der Knorpelinfiltration, der N‑Status und die laryngeale Dysfunktion. Diskussion Beim T3-Larynxkarzinom scheint eine reine pRT vertretbar zu sein, wenn das Volumen 12 ccm, einem Stimmbandfixierungsmuster Succo III/IV, ausgedehnter oder multipler Knorpelinfiltration oder relevanter laryngealer Dysfunktion sollte primär die TL±a[C]RT empfohlen werden.

Chronic rhinosinusitis (CRS) is a heterogeneous disease at the clinical phenotype level (without nasal polyp [CRSsNP] vs with nasal polyp [CRSwNP]) and at the underlying inflammatory endotype level (type 2 vs non-type 2). Whether the endotype is associated with clinical presentation in patients with CRSsNP has yet to be explored in detail.To identify associations between endotypes and their clinical significance in patients with CRSsNP based on tissue interleukin-5 levels.A total of 104 patients with CRSsNP who underwent functional endoscopic sinus surgery between 2013 and 2017 were endotyped. We collected immunologic and clinical parameters and evaluated whether there were associations between the endotype and clinical features using Visual Analog Scale (VAS), Sino-Nasal Outcome Test-22 (SNOT-22), Sniffin' Sticks test, Lund-Mackay CT score, and nasal endoscopy.Mean tissue interleukin-5 levels were used to identify type 2 inflammation (non-type 2: 3.37 vs type 2: 191.98 pg/g tissue; P.001). There were no significant clinical differences measured by patient-reported outcome measures between patients with type 2 CRSsNP and those with non-type 2 CRSsNP preoperatively. Type 2 and non-type 2 CRSsNP did not differentiate in CT score, Sniffin' Sticks test, and nasal endoscopy. Postoperative SNOT-22 and VAS scores correlated well with each other (r = 0.75; P.01). Postoperative VAS scores were in both groups significantly lower than before the operation (type 2: 5.07 vs 2.99; P.01; non-type 2: 5.74 vs 3.22; P.01), but not associated to the inflammatory subtype.The type of inflammation does not affect the symptoms, the computed tomography scan, or the postoperative results in CRSsNP in contrast to former findings in CRSwNP.Belgian registration number (B.U.N.) Background Surgery is standard of care for oral cavity cancer (OCC). We provide a single-institution experience using definitive radiotherapy (RT) with or without concurrent systemic therapy for primary unresectable OCC. Patients and methods We retrospectively examined 49 patients with non-metastatic primary unresectable OCC treated with definitive RT between 2000 and 2019. The majority of patients (63.3%) were treated with definitive chemoradiotherapy while 26.5% were given single-agent cetuximab weekly simultaneous to definitive RT. Five patients were treated with definitive RT alone because of limited disease and no nodal involvement. Results Median follow-up was 73 months (range, 6–236 months), median progression free survival (PFS) was 42 months (range, 2–157 months), median local disease-free survival (LDFS) was 44 months (range, 2–157 months) and median overall survival (OS) from the time of RT initiation was 52 months (range, 5–236 months). There were 65.3% locoregional failures, 84.4% local and 15.6% distant metastasis. The majority of patients with local failure presented with American Joint Committee on Cancer (AJCC) Stage III–IV disease (59.2%). The 5-year Kaplan-Meier estimates for OS (III–IV vs. I–II) was 22.8% vs. 54.2 % (p = 0.03, HR 2.090, 1.1–4.2). Patients who were treated with systemic therapy had a significant better 5-year overall survival compared to those with RT alone (43.9% vs. 23.1%, p = 0.05, 1.0–4.1). RT with doses less than 70 Gy (p = 0.046, HR 2.1 (1.0–4.5) was associated with worse overall survival. Mucositis was the most common ≥ grade 3 acute toxicity and occurred in 19 patients (39%). Incidences of chronic toxicities were loss of taste, trismus, osteoradionecrosis and xerostomia. Conclusions Definitive RT with or without concurrent systemic agents in patients with unresectable OCC resulted in an eloquent rate of locoregional control and good overall survival rates and is currently the best available treatment option in this patient collective.

Zusammenfassung Hintergrund Der Nijmegen Cochlear Implant Questionnaire (NCIQ) ist ein krankheitsspezifischer Fragebogen zur Erhebung der gesundheitsbezogenen Lebensqualität von Patienten vor und nach Cochleaimplantation. Ziel der Arbeit Validierung und Reliabilitätsprüfung der deutschen Übersetzung des NCIQ. Material und Methoden Es wurde eine prospektive Studie an 100 postlingual ertaubten oder hochgradig schwerhörigen Patienten durchgeführt, welche präoperativ sowie 3 und 6 Monate nach einer Cochleaimplantation mittels NCIQ, Abbreviated Profile of Hearing Aid Benefit (APHAB) und Hearing Participation Scale (HPS) untersucht wurden. Als Kontrolle fungierte ein postlingual ertaubtes oder hochgradig schwerhöriges, unbehandeltes Patientenkollektiv (n = 54). Cronbach‑α und Test-Retest-Reliabilität dienten der Reliabilitätsüberprüfung. Es wurde auf Inhalts‑, Übereinstimmungs- und auf diskriminative Validität getestet. Die Konstruktvaliditätsprüfung basiert auf kürzlich veröffentlichen Daten. Als Gütekriterien wurden die Sensitivität und eine ROC(„Receiver Operating Characteristic“)-Analyse, inklusive AUC(„Area Under the ROC Curve“)-Betrachtung, eingesetzt. Ergebnisse Das Test-Retesting ergab nach 3 und 6 Monaten postoperativ stabile NCIQ-Werte. Die Cronbach-α-Werte wiesen auf eine gute interne Konsistenz hin. Der NCIQ diskriminierte valide zwischen behandelten und unbehandelten Patientengruppen. Es ergaben sich statistisch signifikante, wenn auch schwache, Korrelationen zwischen dem NCIQ und dem APHAB (r = −0,22; p = 0,04) und dem HPS (r = 0,30; p = 0,01). Sensitivitäts- und ROC-Analysen zeigten eine gute Messqualität des deutschsprachigen NCIQ. Schlussfolgerung Die deutsche Übersetzung des NCIQ misst zuverlässig und valide die Lebensqualität vor und nach Cochleaimplantation und kann zur klinischen Erfolgskontrolle nach Cochleaimplantationen verwendet werden.

Recent studies highlighted SOX2 and SOX9 as key determinants for cancer-cell plasticity and demonstrated that cisplatin-induced adaptation in oral squamous cell carcinoma (SCC) is acquired by an inverse regulation of both transcription factors. However, the association between SOX2/SOX9-related genetic programs with risk factors and genetic or epigenetic alterations in primary head and neck SCC (HNSCC), and their prognostic value is largely unknown. Here, we identified differentially-expressed genes (DEG) related to SOX2 and SOX9 transcription in The Cancer Genome Atlas (TCGA)-HNSC, which enable clustering of patients into groups with distinct clinical features and survival. A prognostic risk model was established by LASSO Cox regression based on expression patterns of DEGs in TCGA-HNSC (training cohort), and was confirmed in independent HNSCC validation cohorts as well as other cancer cohorts from TCGA. Differences in the mutational landscape among risk groups of TCGA-HNSC demonstrated an enrichment of truncating NSD1 mutations for the low-risk group and elucidated DNA methylation as modulator of SOX2 expression. Gene set variation analysis (GSVA) revealed differences in several oncogenic pathways among risk groups, including upregulation of gene sets related to oncogenic KRAS signaling for the high-risk group. Finally, in silico drug screen analysis revealed numerous compounds targeting EGFR signaling with significantly lower efficacy for cancer cell lines with a higher risk phenotype, but also indicated potential vulnerabilities. Implications: The established risk model identifies patients with primary HNSCC, but also other cancers at a higher risk for treatment failure, who might benefit from a therapy targeting SOX2/SOX9-related gene regulatory and signaling networks.

Zusammenfassung Hintergrund Die Gruppe der Lippen‑, Mundhöhlen- und Pharynxkarzinome (ICD-10: C00–C14) beschreibt eine heterogene Gruppe bösartiger Tumoren, deren Inzidenz, Mortalität und Überleben sich nach Entität stark unterscheiden. Ziel der Arbeit Diese Arbeit gibt eine detaillierte Übersicht epidemiologischer Maßzahlen für diese Tumorentitäten unter Berücksichtigung der Heterogenität bezüglich Alter, Geschlecht, Lokalisation und Stadium. Material und Methoden Inzidenz- und Mortalitätsdaten für Deutschland für die Jahre 1999–2016 wurden aus der interaktiven Datenbank des Zentrums für Krebsregisterdaten (ZfKD) extrahiert. Alters- und Stadienverteilungen und altersstandardisiertes relatives 5‑Jahres-Überleben wurden auf dem gepoolten ZfKD-Datensatz (Diagnosejahre 1999–2017) berechnet. Ergebnisse Im Jahr 2016 lagen die Inzidenz und Mortalität für alle Entitäten insgesamt bei 17,6 bzw. 7,0 pro 100.000 Männern und 6,5 bzw. 1,8 pro 100.000 Frauen. Das relative 5‑Jahres-Überleben für 2015–2017 lag bei 53 % bzw. 63 %. Es zeigten sich deutliche Unterschiede in der Überlebensrate und in der Alters- und Stadienverteilung zwischen den Entitäten. Im zeitlichen Verlauf zeigte sich ein Anstieg des Alters bei Diagnose, insbesondere bei Männern, aber keine Veränderung in der Stadienverteilung. Trotzdem stieg das relative 5‑Jahres-Überleben zwischen 1999–2002 und 2013–2017 von 45 % (Männer) bzw. 59 % (Frauen) auf 52 % bzw. 63 %. Diskussion Die starke Heterogenität der untersuchten Tumoren verdeutlicht die Notwendigkeit einer nach Geschlechtern und Lokalisationen getrennten Betrachtung für eine aussagekräftige Interpretation der epidemiologischen Kennzahlen. Mit Ausbau der klinischen Krebsregistrierung werden in Zukunft zusätzliche Analysen unter Einbezug weiterer wichtiger klinischer Faktoren möglich sein.

OBJECTIVES To evaluate the rate of surgical site infection (SSI) and associated risk factors after parotid gland surgery including the impact of antibiotic prophylaxis. DESIGN Retrospective single-centre clinical study. SETTING Tertiary referral centre for head and neck surgery. PARTICIPANTS Seven hundred and fifty four patients who underwent parotid gland surgery at the University Hospital Heidelberg, Germany, between 2007 and 2014 were enrolled in this study. Data on patient age, American Society of Anesthesiologists (ASA) classification system, smoking status, diabetes mellitus, operation time, and antibiotic prophylaxis were collected. Additionally, the National Healthcare Safety Network (NHSN) risk index was calculated. Association of these factors with SSI was evaluated in univariate analyses and a multivariate logistic regression model. MAIN OUTCOME MEASURES Rate of SSI. RESULTS Twenty four patients (3.2%) had an SSI according to the NHSN definition. In univariate analyses, only smokers (P = .048) and male patients (P = .01) had a significantly higher rate of SSI. Since the majority of smokers were men (62.3%), the effect of male gender, smoking, together with the NHSN risk index was further investigated as predictors of SSI within a logistic regression model. All three predictors showed a significant effect on SSI. CONCLUSIONS Parotid gland surgery has a low rate of SSI. In our cohort, male gender, smoking and high NHSN risk index scores were significantly associated with SSI, whereas antibiotic prophylaxis had no protective effect.

Background: Carcinomas of the lips are a relatively common malignancy of the head and neck region, accounting for roughly one quarter of all oral cavity cancers. Compared to other oral cancer sites, this location has a favorable prognosis, with 5-year survival rates between 85% and 95%. This study summarizes our institutional experience in utilizing postoperative radiation for patients with squamous cell carcinoma of the upper and/or lower lip following incomplete surgical resection or positive lymph node involvement with extracapsular extension. Methods: We retrospectively reviewed the medical records of all patients at the University Hospital of Heidelberg between 2005 and 2018 treated with postoperative radiotherapy of the upper and lower lip. Nineteen patients were identified with a median age at diagnosis of 67 years (range, 41–95 years), with 58% male and 42% female patients. Fourteen patients (73.7%) underwent neck dissection, with 5 (35.7%) found to have extracapsular extension (ECE) and positive resection margin (R1/2), 2 (14.3%) only ECE, and 7 (50.0%) with only R1/2. All patients received a median cumulative dose of 66.0 Gy (range, 60.0–70.0 Gy) in a median of 2.0 Gy per fraction (range, 1.8–2.2 Gy). Results: Median follow-up was 5.2 years. The median progression-free survival (PFS) was 3.9 years (range, 0.2–12.4 years), local disease-free survival (LDFS) was 4 years (range, 1–12 years) and overall survival (OS) was 5.2 years (range, 0.2–12.4 years). The 5-year Kaplan-Meier estimates for OS, PFS, and LDFS were 61.4%, 85.7%, and 100.0%, respectively. At last follow-up, 13 patients (68.4%) were still alive. Although no patient developed locoregional relapse, two patients developed distant relapse at a median of 15 months after radiotherapy. There was a statistically significant improvement in OS in patients treated with higher radiotherapy doses (>60.0 Gy, p = 0.044) compared to lower radiotherapy doses. PFS was significantly improved among patients who had N0 disease, with a negative resection margin, without ECE, and who were treated with intensity-modulated radiotherapy to doses >60.0 Gy. No grade 3/4 toxicity was detected; the most common grade 1/2 toxicities included dermatitis (n = 11, 57.9%), oral mucositis (n = 8, 42.1%), and dysphagia (n = 8, 42.1%). Conclusion: Our results demonstrate excellent local control and OS with acceptable toxicity when utilizing postoperative radiotherapy in patients with squamous cell carcinoma of the upper and lower lip, despite unfavorable characteristics (advanced T or N stage and/or ECE).

Background: The induction and regulation of immune responses depend on human leucocyte antigen (HLA) molecules that present peptides derived from mutated neoantigens or tumor-associated antigens to cytotoxic T cells. The natural variation of HLA molecules might differ between tumor patients and the normal population. Thus, there might be associations between the frequencies of HLA alleles and the survival of tumor patients. Methods: This issue was studied in a cohort of 84 patients with head and neck squamous cell carcinomas (HNSCCs) of different localizations. The cohort was followed up for more than 10 years. HLA-A/B/C CTS-PCR-SSP typing at 1 field level from blood samples was performed, and the results were correlated with survival. Results: HLA-A*02 was the most prevalent allele in our cohort and was present in 51.1% of patients. The HLA-A*25 and HLA-C*06 alleles exhibited a significantly higher frequency in cancer patients than in the normal population of 174 blood and kidney donors (p = 0.02 and p = 0.01, respectively, Fisher’s exact test). For HLA-C*04, a negative impact on overall survival in univariate analysis (p = 0.045) and a negative, but statistically insignificant effect on survival toward poorer survival in multivariate analysis (HR: 1.82; 95% CI: 0.99–3.34, p = 0.053) were observed. In addition, HLA-A*02 was also beneficial for overall survival and progression-free survival in multivariate analysis (HR 0.54; 95% CI: 0.31–0.92; p = 0.023). Conclusion: HLA-A*02 allele expression might not only predict better survival but might also indicate superior tumor antigen presentation and, thus, help to select patients who could benefit from T-cell-dependent immunotherapies.

Purpose Assessing cochlear implant (CI)-associated patient outcomes is a focus of implant research. Most studies have analyzed outcomes retrospectively with low patient numbers and few measurement time points. In addition, standardized CI-specific health-related quality of life (HRQoL) instruments have not been used. To address this, we prospectively assessed HRQoL in patients before and after implantation. Methods We assessed HRQoL using the Nijmegen Cochlear Implant Questionnaire (NCIQ), Abbreviated Profile of Hearing Aid Benefit (APHAB), Hearing Participation Scale (HPS), and the Visual Analogue Scale (VAS) in 100 deaf or severely hearing-impaired patients (57 unilaterally deaf and 43 bilaterally deaf) before and 3, 6, and 12 months after cochlear implantation. We compared the results of unilaterally and bilaterally hearing-impaired patients and patients with or without a hearing aid. Principal component (PCA) and exploratory factor analyses (EFA) were also conducted. Results The NCIQ measured improvements in all 6 domains after CI and correlated well with other QoL instruments. The PCA revealed that the NCIQ can be better explained by physical, physical advanced, and socio-psychological components. The APHAB score ameliorated over time, except for the background noise domain. The overall HPS score improved over time, but the hearing handicap subscore significantly decreased. Sociodemographic influences on the questionnaire scores were relatively weak. Conclusion Assessing HRQoL is essential for quantifying the patient outcome after CI. NCIQ scores in our patient cohort showed improved HRQoL in all domains and we recommend that the NCIQ be used as a first-line questionnaire for assessing QoL in hearing-impaired patients after CI.

Purpose The study aimed to determine normative values for the Tonsillectomy Outcome Inventory 14 (TOI-14) in a healthy middle-European cohort. We also compared these generated values with TOI-14 scores from a patient population with recurrent tonsillitis (RT) and explored the factorial structure of the TOI-14. Methods We systematically studied the responses of healthy individuals (reference cohort) and patients with RT (clinical cohort) to the TOI-14 survey. The reference cohort contained 1000 participants, who were recruited using the Respondi panel for market and social science research. This subsample was quoted to the population distribution of the German Microcensus and selected from a non-probability panel. Tonsillitis patients were assessed before and 6 and 12 months after tonsillectomy. Data were analysed using principal component and exploratory factor analyses. Results The PCA revealed three TOI-14 domains (physiological, psychological and socio-economic), which explained 73% of the total variance. The reference cohort perceived a good quality of life (QOL) with a TOI-14 total score of 11.8 (physiological: 8.0, psychological: 5.8, and socio-economic subscale score: 13.9). TOI-14 scores were higher in the patient cohort, indicating that the TOI-14 discriminates between patients with RT and healthy individuals with no RT. Age and female gender significantly influenced the total TOI-14 score, especially in the psychological (age) and socio-economic (gender) subscales. Conclusion We have developed a set of normative values that, together with the TOI-14, can determine the disease burden indicating tonsillectomy.

Surgery for early-stage squamous cell carcinoma of the nasal vestibule (SCCNV) may lead to facial disfigurement. We wanted to investigate if skin-preserving excision of SCCNV with reconstruction is oncologically, aesthetically, and functionally justifiable in cases with proximity to the skin. From 2010 to 2016, 16 patients underwent skin-preserving excision of T1-2 N0 SCCNV by a lateral rhinotomy approach at a tertiary referral center. The inner nose was reconstructed using a mucoperichondrial septal flap for the inner lining and a septal pivot flap and/or auricular cartilage grafting for the framework. Nasal appearance was measured on pre- and postoperative photographs. Median follow-up was 5.4 years. Three (19%) patients received adjuvant radiotherapy. Two (12.5%) recurrences occurred locally, but not at the site of preserved skin. The Kaplan-Meier estimate of local control rate after 5 years was 83%. All patients could be salvaged, giving an ultimate control rate of 100%. Nasal tip projection decreased by 6.7% (p 0.001), and it was retained normal or near normal in 87.5% of patients. Nasal axis changed by 1.7° (p = 0.5). Nasal deviation occurred in 6.25% (1/16) of patients, and minor alar retraction in 6.25% of patients (1/16). Nasal breathing was normal or close to normal in 75% (12/16) of patients. Skin-preserving excision of SCCNV is oncologically justifiable in selected cases even in proximity to nasal skin. Early inner nasal reconstruction preserves nasal form and function to a high degree. This technique is a suitable alternative to rhinectomy and to avoid the sequelae of radiotherapy in selected cases.

OBJECTIVE Generation of pilot data for planning of prospective BET-studies for treatment of dilatory Eustachian tube (ET) dysfunction in children. STUDY DESIGN Retrospective multicenter analysis. SETTING Nine ENT departments at tertiary care teaching hospitals. PATIENTS 4-12-year-old children with chronic otitis media with effusion (COME) for more than 3 months or more than 3 episodes of acute otitis media during the last year, having failed standard surgical therapy at least once. INTERVENTION BET with or without paracentesis, ventilation tube insertion, or tympanoplasty. MAIN OUTCOME MEASURES Tympanic membrane appearance, tympanometry, and hearing threshold. RESULTS Two hundred ninety-nine ETs of 167 children were treated. Mean age was 9.1 years (95% confidence interval [95% CI]: 8.7-9.4 yr). In 249 ears (83.3%), COME and/or retraction of the tympanic membrane were the indication for BET. Median hearing threshold was 20 dB HL (95% CI: 0-46 dB). One hundred fifty-five ears (51.8%, 95% CI: 46.1-57.4%) showed a tympanogram type B. Treatment consisted of BET without other interventions ("BET-only") in 70 children, 128 ears. Median length of follow-up for 158 (94.6%) children was 2.6 months (95% CI: 0.3-16.1 mo). After treatment, the tympanic membrane appeared normal in 196 ears (65.6%, 95% CI: 60.0-70.8%, p

Since December 2019, the SARS-CoV-2 virus has been rapidly spreading worldwide. In Germany, an exponential increase in the number of infections was registered at the beginning of March 2020 and led to a call of the Ministry of Health to create more capacity for intensive medical treatment in hospitals. The aim of the present study was to determine the effects of the SARS-CoV-2 pandemic on Oto-Rhino-Laryngology (ORL) university hospitals regarding patient care.An online survey was sent out to all chairmen of the 39 ORL university hospitals in Germany. The answers to the questions referred to the defined period from March 15All chairmen of the 39 university hospitals in Germany participated in the survey. The collected data prove the considerable impact on organizational, structural and medical aspects of patient care. For example, the surveyed clinics reported a decrease in outpatient cases by 73.8 % to 26.2 ± 14.2 % and in surgical treatments by 65.9 % to 34.1 ± 13.9 %. In contrast, emergency treatment remained unchanged or even increased in 80 % of the facilities and surgical treatment of emergency patients remained unchanged or even increased in more than 90 %. Emergency outpatient and surgical treatment of patients was provided throughout the pandemic in all facilities. In total, about 35 000 outpatients and about 12 000 surgical cases were postponed. As a result of the acute structural changes, the potential danger of falling below current treatment standards was seen in individual areas of patient care.The assessment of the impact of the SARS-CoV-2 pandemic is heterogeneous. The majority of the chairmen are critically aware of the risk of falling below current medical treatment standards or guidelines. In the phase of an exponential increase in the number of infections, significant changes in treatment processes had to be accepted for understandable reasons. However, with the currently significantly reduced number of infections, falling below treatment standards and guidelines should not be allowed to remain constant and tolerated.This study shows a differentiated picture with regard to the effects of the SARS-CoV-2 pandemic on outpatient, inpatient and operative patient care at the ORL university hospitals in Germany and illustrates the importance of these institutions for ensuring patient care during this critical phase.Seit März 2020 wurden infolge des exponentiellen Anstiegs der Infektionen mit SARS-CoV-2 in Deutschland/Europa die Krankenhäuser durch das Gesundheitsministerium aufgefordert, verstärkt Kapazitäten zur intensivmedizinischen Behandlung zu schaffen. An vielen Kliniken wurden deshalb kurzfristig massive Umstrukturierungen und Ressourcenverschiebungen vorgenommen. Ziel der vorliegenden Arbeit war die Erfassung der Auswirkungen der SARS-CoV-2-Pandemie auf die universitäre Hals-Nasen-Ohren(HNO)-Heilkunde in Bezug auf die Krankenversorgung.Eine Online-Befragung wurde an alle Direktorinnen und Direktoren der 39 Universitäts-HNO-Kliniken in Deutschland versendet. Die Fragen bezogen sich auf den definierten Zeitraum vom 15.03. bis 15.04.2020 und die Beantwortung wurde mithilfe des Online-Befragungstools „SurveyMonkey“ durchgeführt. 87 Fragen bezogen sich auf allgemeine Angaben, die Krankenversorgung und die strukturellen Auswirkungen in der jeweiligen Einrichtung.An der Befragung beteiligten sich alle Direktorinnen und Direktoren der 39 Universitäts-HNO-Kliniken in Deutschland. Die erhobenen Daten belegen die erheblichen Auswirkungen auf organisatorische, strukturelle und medizinische Aspekte der Krankenversorgung. So gaben die befragten Kliniken einen Rückgang der ambulant behandelten Fälle um 73,8 % auf 26,2 ± 14,2 % und der operativen Behandlungen um 65,9 % auf 34,1 ± 13,9 % an. Im Gegensatz hierzu erfolgte in 80 % der Kliniken eine unveränderte oder sogar vermehrte Notfallbehandlung und in mehr als 90 % eine unveränderte oder sogar vermehrte operative Behandlung von Notfällen. In allen Kliniken war die ambulante und operative Notfallbehandlung während der Pandemie durchgehend gewährleistet. Insgesamt wurden an allen HNO-Universitätskliniken circa 35 000 ambulante und circa 12 000 operative Fälle verschoben. Infolge der akuten strukturellen Veränderungen wurde in einzelnen Bereichen der Krankenversorgung die potenzielle Gefahr einer Unterschreitung geltender Behandlungsstandards gesehen.Die Bewertung der Auswirkungen der SARS-CoV-2-Pandemie fällt heterogen aus. Kritisch wird mehrheitlich die Gefahr der Unterschreitung geltender Behandlungsstandards oder Leitlinien bewertet. In der Phase der exponentiellen Zunahme der Infektionszahlen mussten teilweise erhebliche Veränderungen von Behandlungsabläufen aus nachvollziehbaren Gründen akzeptiert werden. Mit den aktuell deutlich reduzierten Infektionszahlen darf aber eine Unterschreitung von Behandlungsstandards und Leitlinien nicht verstetigt und toleriert werden.Die hier dargestellte Untersuchung zeigt ein differenziertes Bild hinsichtlich der Auswirkungen der SARS-CoV-2-Pandemie auf die ambulante, stationäre und operative Krankenversorgung an den Kliniken der universitären HNO-Heilkunde in Deutschland und verdeutlicht die Bedeutung der Universitätskliniken für die Krankenversorgung während dieser kritischen Phase.

Background Head and neck squamous cell carcinoma (HNSCC) patients frequently develop treatment resistance to cetuximab, a monoclonal antibody against EGFR, as well as radiotherapy. Here we addressed extracellular signal-regulated kinase 1/2 (ERK1/2) regulation by cetuximab or fractionated irradiation (IR) and conducted in silico prognostic evaluation of the EGFR-MAPK axis in HNSCC. Methods Expression of ERK1/2 phosphorylation (pERK1/2) was determined in HNSCC cell lines, which were treated with cetuximab or fractionated-IR. Furthermore, the effect of fractionated IR on pERK1/2 was confirmed in an ex vivo HNSCC tissue culture model. Expression and prognostic significance of EGFR-ERK axis was evaluated in a cohort of radiotherapy plus cetuximab-treated HNSCC. Correlations among EGFR-MAPK signalling components and association between transcript and protein expression profiles and patient survival in HNSCC were analysed using publicly available databases. Results ERK1/2 phosphorylation was rebounded by prolonged cetuximab administration and was induced by fractionated IR, which could be suppressed by a MEK inhibitor as a radiosensitiser. In silico assessments suggested that EGFR-MAPK cascade genes and proteins could predict HNSCC patients’ survival as a prognostic signature. Conclusions Activation of ERK1/2 signalling contributes to the cellular defence of HNSCC against cetuximab and fractionated IR treatment. EGFR-MAPK axis has a prognostic significance in HNSCC.

Purpose Cancer-associated fibroblasts (CAFs) expressing fibroblast activation protein (FAP) have been associated with the aggressive nature of head and neck cancers (HNCs). These tumours grow diffusely, leading to extremely challenging differentiation between tumour and healthy tissue. This analysis aims to introduce a novel approach of tumour detection, contouring and targeted radiotherapy of HNCs using visualisation of CAFs: PET-CT with 68Ga-radiolabeled inhibitors of FAP (FAPI). Methods FAPI PET-CT was performed without complications prior to radiotherapy in addition to contrast enhanced CT (CE-CT) and MRI on 14 patients with HNC. First, for tissue biodistribution analysis, volumes of interest were defined to quantify SUVmean and SUVmax in tumour and healthy parenchyma. Secondly, using four thresholds of three-, five-, seven- and tenfold increase of FAPI enhancement in the tumour as compared with normal tissue, four different gross tumour volumes (FAPI-GTV) were created automatically. These were compared with GTVs created conventionally with CE-CT and MRI (CT-GTV). Results The biodistribution analysis revealed high FAPI avidity within tumorous lesions (e.g. primary tumours, SUVmax 14.62 ± 4.44; SUVmean 7.41 ± 2.39). In contrast, low background uptake was measured in healthy tissues of the head and neck region (e.g. salivary glands: SUVmax 1.76 ± 0.31; SUVmean 1.23 ± 0.28). Considering radiation planning, CT-GTV was of 27.3 ml, whereas contouring with FAPI resulted in significantly different GTVs of 67.7 ml (FAPI × 3, p = 0.0134), 22.1 ml (FAPI × 5, p = 0.0419), 7.6 ml (FAPI × 7, p = 0.0001) and 2.3 ml (FAPI × 10, p = 0.0001). Taking these significant disparities between the GTVs into consideration, we merged FAPI-GTVs with CT-GTVs. This resulted in median volumes, that were, as compared to CT-GTVs, significantly larger with FAPI × 3 (54.7 ml, + 200.5% relative increase, p = 0.0005) and FAPI × 5 (15.0 ml, + 54.9%, p = 0.0122). Furthermore, FAPI-GTVs were not covered by CE-CT-based planning target volumes (CT-PTVs) in several cases. Conclusion We present first evidence of diagnostic and therapeutic potential of FAPI ligands in head and neck cancer. Larger studies with histopathological correlation are required to validate our findings.

Could primary chemoradiotherapy (pCRT) possibly be viewed as an alternative standard therapy to upfront total laryngectomy (TL)? According to the new German S3 guideline, despite higher rates of local recurrence, there would be no survival disadvantage and salvage surgery would be a curative option. In several large database studies and case series, statistically significant survival disadvantages of more than 30% between pCRT and TL have been reported for T4 laryngeal cancer. According to the literature, the success rate of salvage TL for T4 laryngeal cancer is only about 25-50%. Larynx preservation (LP) studies which could qualify the recommendation of pCRT as an alternative standard therapy to TL in T4 carcinomas should 1) evaluate T4a cancers within the T4 category; 2) perform subgroup analysis of laryngeal and hypopharyngeal cancers; 3) be sufficiently highly powered; 4) provide long-term outcomes of at least 5 years; 5) with oncological and 6) functional outcomes (duration of the need for tracheostomy and/or feeding tube dependency; necessity and success of salvage laryngectomies). 7) Specification of the criteria of the respective T4 classification (invasion through the outer cortex of the cartilage, or infiltration of which extralaryngeal structures) and 8) evaluation of pretreatment laryngeal function (at least: tracheostomy, feeding tube dependency). Collection of all the aforementioned data of T4 patients treated with pCRT in a large prospective observational cohort study in German-speaking countries is suggested. In case of rejection of TL by T4 laryngeal cancer patients, differentiation between primary spontaneous reluctance and a definitive, carefully considered decision is important. This distinction should be achieved by sensitive discussions. Not only oncological but also functional outcome probabilities should be included in the overall decision-making process.Kann die primäre Radiochemotherapie (pRCT) möglicherweise als alternative Standardtherapie zur totalen Laryngektomie (TL) angesehen werden? Gemäß der neuen S3-Leitlinie nehme der Patient zwar eine höhere Rückfallrate in Kauf, habe aber die Salvagechirurgie als kurative Option und insgesamt keinen Überlebensnachteil. In mehreren großen Datenbankanalysen und Fallserien findet sich für das T4-Larynxkarzinom ein signifikanter Überlebensnachteil der pRCT gegenüber der primären TL von über 30 %. Die Erfolgsrate der Salvagelaryngektomie beim T4-Karzinom liegt laut Literatur nur bei 25–50 %. Larynxorganerhaltstudien, die zur Empfehlung der pRCT als alternativer Standardtherapie führen könnten, sollten 1.) T4a-Karzinompatienten innerhalb ihrer T‑Kategorie auswerten, 2.) getrennt nach Larynx- und Hypopharynxkarzinomen, 3.) in aussagekräftiger Kollektivgröße, 4.) mit einem Nachbeobachtungszeitraum von mindestens 5 Jahren, 5.) mit onkologischen und 6.) funktionellen Outcome (Dauer von Tracheostoma u./o. PEG, Notwendigkeit und Erfolg einer Salvagelaryngektomie). 7.) Angabe des Kriteriums, das zur T4-Kategorie führte (Knorpeldurchbruch oder Art der extralaryngealen Ausbreitung), 8.) prätherapeutische Larynxfunktionalität (Tracheostoma, PEG). Eine Erfassung der genannten klinischen Daten aller T4-pRCT-Patienten in einer prospektiven beobachtenden Kohortenstudie im deutschsprachigen Raum wird vorgeschlagen. Bei Ablehnung der TL beim T4a-Karzinom sollte in ausführlichen einfühlsamen, aber instruktiven Gesprächen unterschieden werden zwischen einer primären spontanen Abneigung und der differenziert abgewogenen, definitiven Ablehnung. Nicht nur der onkologische, sondern auch der funktionell zu erwartende Outcome sollte in den Entscheidungsprozess einbezogen werden.

By today's standard, the optimal treatment of every individual tumor patient is discussed and determined in an interdisciplinary tumor board. According to the new S3 guidelines, larger volume T3 laryngeal cancers which are no longer safely resectable with larynx-sparing surgery are ideal candidates for a larynx preservation approach using primary chemoradiation (pCRT). So far, no clear criteria have been defined under what circumstances primary radiotherapy alone (pRT) might be acceptable in case chemotherapy (CT) is prohibited or in what cases, even in T3, upfront total laryngectomy with risk-adapted adjuvant treatment (TL±a[C]RT) should be recommended.The literature was searched for parameters chosen as criteria for an inclusion in the surgical rather than the conservative arm in non-randomized LP studies or which proved to be significant prognostic markers after conservative treatment. Hieraus Entwicklung eines Beratungsinstruments für die Therapieentscheidung.Als signifikante Prognosemarker beschrieben wurden das Tumorvolumen, das Vorhandensein und die Art der Stimmbandfixierung, das Ausmaß der Knorpelinfiltration, der N‑Status und die laryngeale Dysfunktion.Beim T3-Larynxkarzinom scheint eine reine pRT vertretbar zu sein, wenn das Volumen 3,5 ccm für glottische bzw. 6 ccm für supraglottische Tumoren beträgt und keine weiteren Risikofaktoren vorliegen. Die pCRT kann als Standard des konservativen LP bei Tumoren mit einem Volumen zwischen 6 ccm und 12 ccm gelten, bei einer Stimmbandfixierung Succo I/II, einer allenfalls minimalen Knorpelinfiltration und einer hohen nodulären Tumorlast. Bei Tumoren mit einem Volumen 12 ccm, einem Stimmbandfixierungsmuster Succo III/IV, ausgedehnter oder multipler Knorpelinfiltration oder relevanter laryngealer Dysfunktion sollte primär die TL±a[C]RT empfohlen werden.

The Nijmegen Cochlear Implant Questionnaire (NCIQ) is a disease-specific questionnaire to determine the health-related quality of life (HRQoL) of patients before and after cochlear implantation.This study aimed to assess the validity and reliability of the German translation of the NCIQ.A prospective study was performed in 100 postlingually deaf or severely hearing-impaired patients. HRQoL was assessed using the NCIQ, the Abbreviated Profile of Hearing Aid Benefit (APHAB), and the Hearing Participation Scale (HPS) before as well as 3 and 6 months after cochlear implantation. An untreated group of postlingually deaf or severely hearing-impaired patients (n = 54) served as a control. Cronbach's α and test-retest reliability were measured. The content, discrimination, and agreement validity were tested. The evaluation of construct validity was based on recently published data. Sensitivity and receiver operating curve (ROC) analysis, including consideration of the area under the curve (AUC), were used as quality criteria.The test-retest analysis showed stable NCIQ values 3 and 6 months postoperatively. The Cronbach's α values indicated good internal consistency. The NCIQ validly discriminated between treated and untreated patient groups. There were statistically significant albeit weak correlations between the NCIQ and the APHAB (r = -0.22; p = 0.04) and the HPS (r = 0.30; p = 0.01). Es ergaben sich statistisch signifikante, wenn auch schwache, Korrelationen zwischen dem NCIQ und dem APHAB (r = −0,22; p = 0,04) und dem HPS (r = 0,30; p = 0,01). Background and aim The primary aim of our study was to evaluate percutaneous endoscopic gastrostomy (PEG) tube placement depending on body weight and body mass index in patients undergoing radiotherapy (RT) for head and neck cancer (HNC). A secondary aim was to evaluate the course of weight change following PEG placement. Methods We retrospectively reviewed the medical records of 186 patients with HNC undergoing radiotherapy (RT) or chemoradiotherapy (CRT) at our institution between January 2010 and August 2017. Initial weight and nutritional intake were analyzed prior to RT initiation and then followed throughout treatment until completion. Based on these data, the indication of PEG placement was determined. Medical records were also reviewed to analyze PEG-related acute toxicities. Results A total of 186 patients met inclusion criteria. Patients were most commonly male (n=123, 66.1%) with squamous cell carcinoma (n=164, 88.2%). Purpose This study aimed to assess the feasibility of carbon ion reirradiation (CIR) for recurrent head and neck cancer (HNC). Methods and Materials This retrospective study included 229 patients with recurrent HNC who were treated with CIR between 2010 and 2017. We assessed progression-free survival, overall survival, pattern of failure, and toxicity. Of the primary tumors, 54.1% were adenoid cystic carcinomas, 26.2% were squamous cell carcinomas, 8.3% were adenocarcinomas, and 11.4% were other tumor entities. Results The median radiation therapy interval was 3.9 years (range, 0.3-46.5 years), and patients received a median dose of 51 Gy (relative biological effectiveness [RBE]; range, 30-66 Gy [RBE]) in 3 Gy (RBE) fractions. The median cumulative lifetime dose after CIR was 132.8 Gy (range, 88.8-155.0 Gy). The median local progression-free survival after CIR was 24.2 months (95% confidence interval, 19.4-29.0 months), and the median overall survival was 26.1 months (95% confidence interval, 21.9-30.3 months). Serious acute toxicity (grade ≥3) after CIR included laryngeal edema, grade 4 (n = 2; 0.9%); dysphagia, grade 3 (n = 3; 1.3%); fistula, grade 3 (n = 1; 0.4%); and impaired hearing, grade 3 (n = 1; 0.4%). Late toxicities of grades 3 or higher (n = 18; 14.5%) included central nervous system necrosis, grades 4/3 (n = 1; 0.8%/n = 5; 4.0%); optic nerve disorder, grades 4/3 (n = 2; 1.6%/n = 2; 1.6%); impaired hearing, grade 3 (n = 5; 4.0%), osteonecrosis, grade 3 (n = 1; 0.8%); and carotid blowout, grade 4 (n = 1; 0.8%). Conclusions In patients with locally recurrent HNC, CIR was a feasible, effective treatment with acceptable toxicity and good local control. Thus, CIR represented a valuable alternative to surgical salvage and palliative chemotherapy in selected patients.

Background The objective of this investigation is to evaluate the outcomes and toxicity of carbon-ion re-irradiation (CIR) in patients with rare head and neck cancers (HNC). There is a paucity of data regarding treatment approaches in this patient cohort, which we aim to address in this work. Methods Thirty-two (n = 32) consecutive patients with uncommon HNC treated between 2010 and 2017 were retrospectively analyzed in terms of clinical outcomes, patterns of failure, and toxicity. Results Mucoepidermoid carcinoma (MEC) was the most common histology (22%). Patients received a median cumulative dose equivalent in 2 Gy fractions (EQD2) after CIR of 128.6 Gy (range, 105.8–146.5 Gy). The local and distant control rates 1 year after CIR were 66 and 72%. No serious acute or late toxicity (≥ grade 3) after CIR was observed. Conclusions CIR may represent an effective and safe treatment alternative to palliative systemic therapies in these rare indications.

OBJECTIVES Though xerostomia is a frequent oral symptom, there is no validated disease-specific questionnaire in German. The purpose of this study was to translate and validate versions of the Xerostomia Inventory and the Summated Xerostomia Inventory in a German-speaking population. PARTICIPANTS AND METHODS Thirty-nine patients including 18 patients suffering from radiation-induced xerostomia enrolled in this study. Both questionnaires were translated into German language according to international accepted guidelines. For validation, we evaluated reliability, validity, and responsiveness using the COSMIN manual for cross-cultural adaptation. RESULTS Cronbach's α was 0.92 for XI and 0.91 for SXI, showing both high internal consistency. Patients suffering from xerostomia showed significantly higher average scores demonstrating its discriminant validity. Confirmatory factor analysis showed excellent "goodness-of-fit" values for SXI and good to moderate values for XI, confirming the assumed factor structures. The Xerostomia Inventory and its summated version both showed excellent test-retest reliability in the non-xerostomia group (ICC = 0.85 and 0.84). CONCLUSIONS The XI and SXI in their cross-cultural adapted versions are the first validated self-report assessments for xerostomia in German language. They are characterized by practical design and can be easily interpreted by the treating physician.

Parotidectomy worsens quality of life (QoL) in the short-term, but the long-term impact is unknown. In this study, we analysed the long-term effects of parotidectomy on QoL.In this prospective long-term follow-up study, participants were divided into three groups: short-term (ST) follow-up of six weeks, long-term (LT) follow-up of 13 years and short- and long-term (SLT) follow-up. QoL was assessed using the Parotidectomy Outcome Inventory (POI-8). Parotidectomies were classified based on whether the great auricular nerve (GAN) had been preserved or sacrificed.In total, 164 observations were analysed, 74 in the LT group, 57 in the ST group and 33 in the SLT group. Hypoaesthesia was a major problem and facial palsy was a minor problem. Pain (p0.01) and hypoaesthesia (p0.001) were significantly lower after 13 years compared with after six weeks, and QoL was higher after 13 years compared with after six weeks (p = 0.04). The disease-specific impairment rate decreased from 70% at short-term follow-up to 30% at long-term follow-up. Removal of the GAN was associated with hypoaesthesia in the ST group (p = 0.028).Hypoaesthesia has a long-term impact on the QoL, and this should be emphasised during preoperative discussions.Risultati a lungo termine e qualità della vita dopo parotidectomia per patologie benigne.La parotidectomia peggiora la qualità della vita (QoL) a breve termine; l’impatto a lungo termine non è noto e abbiamo voluto analizzarlo in questo studio.Questo è uno studio prospettico di follow-up a lungo termine. I partecipanti sono stati divisi in tre gruppi: follow-up a breve termine (ST) di sei settimane, follow-up a lungo termine (LT) di 13 anni e follow-up a breve e lungo termine (SLT). La QoL è stata valutata utilizzando il Parotidectomy Outcome Inventory (POI-8). Le parotidectomie sono state distinte in base alla conservazione o al sacrificio del nervo grande auricolare (GAN).Sono stati arruolati 164 pazienti, 74 nel gruppo LT, 57 nel gruppo ST e 33 nel gruppo SLT. L’ipoestesia è stato il problema principale, la paralisi facciale un problema minore. Il dolore (p0,01) e l’ipoestesia (p0,001) erano significativamente ridotti dopo 13 anni rispetto a dopo sei settimane e la QoL era maggiore dopo 13 anni rispetto a sei settimane (p = 0,04). Il tasso di compromissione della malattia è diminuito dal 70% a breve termine al 30% a lungo termine. La rimozione del GAN è stata associata a ipoestesia nel gruppo ST (p = 0,028).L’ipoestesia ha un impatto a lungo termine sulla QoL e dovrebbe essere discussa durante i colloqui preoperatori.

For advanced laryngeal cancers, after randomized prospective larynx preservation studies, nonsurgical therapy has been applied on a large scale as an alternative to laryngectomy. For T4 laryngeal cancer, poorer survival has been reported after nonsurgical treatment. Is there a need to fear worse survival also in T3 tumors? The outcomes of 121 T3 cancers treated with pCRT, pRT alone, or surgery were evaluated in an observational cohort study in Germany. In a multivariate Cox regression of the T3 subgroup, no survival difference was noted between pCRT and total laryngectomy with risk-adopted adjuvant (chemo)radiotherapy (TL ± a(C)RT) (HR 1.20, 95%-CI: 0.57–2.53, p = 0.63). However, survival was significantly worse after pRT alone than after TL ± a(C)RT (HR 4.40, 95%-CI: 1.72–11.28, p = 0.002). A literature search shows that in cases of unfavorable prognostic markers (bulky tumors of 6–12 ccm, vocal cord fixation, minimal cartilage infiltration, or N2–3), pCRT instead of pRT is indicated. In cases of pretreatment dysphagia or aspiration requiring a feeding tube or tracheostomy, gross or multiple cartilage infiltration, or tumor volume &gt, 12 ccm, outcomes after pCRT were significantly worse than those after TL. In these cases, and in cases where pCRT is indicated but the patient is not suitable for the addition of chemotherapy, upfront total laryngectomy with stage-appropriate aRT is recommended even in T3 laryngeal cancers.

T1 glottic cancer is a highly treatable disease with local control (LC) rates over 90% by either primary radiotherapy (pRT) or transoral laser microsurgery (TLM). LC of T2 glottic cancers is 15 percent points poorer on average. However, salvage after pRT entails more than 50% total laryngectomy. Therefore, there is a need for enhanced LC. Altered fractionation regimens improved LC in T1 but not in T2. For this reason, for T2, alternative strategies must be considered. In a large observational cohort study including 531 early-stage laryngeal cancers, a small number of patients were treated with primary chemoradiotherapy (pCRT). In multivariable analysis, factors associated with significantly poorer outcomes included age, comorbidities, supraglottic localization, and T category. While there was a significant difference between pRT and surgery (HR 1.79, 95%-CI: 1.15–2.79), there was none between pCRT and surgery (HR 0.70, 95%-CI: 0.33–1.51). There is evidence from the literature that pCRT in early glottic cancers could yield results that surpass the limits so far experienced in radiotherapy alone with acceptable toxicity. Thus, prospective randomized studies with larger numbers of patients are warranted.

Lip, oral cavity, and pharynx cancers (ICD-10: C00-C14) describe a heterogeneous group of tumors with strong variations in incidence, mortality, and survival by entity.This work provides a detailed overview of epidemiologic measures for these tumor entities, taking into account heterogeneity in age, sex, location, and stage.Incidence and mortality data for Germany for the years 1999-2016 were extracted from the interactive database of the Center for Cancer Registry Data (ZfKD). Age and stage distributions and five-year relative survival were calculated on the pooled ZfKD data set (diagnosis years 1999-2017).In 2016, overall incidence and mortality for all entities were 17.6 and 7.0 per 100,000 men and 6.5 and 1.8 per 100,000 women, respectively. The five-year relative survival in 2015-2017 was 53 and 63%, respectively. There were marked differences in survival as well as age and stage distributions between entities. Trend analyses showed an increase in age at diagnosis, particularly in male patients, and no change in stage distributions. However, five-year relative survival increased from 45% (men) and 59% (women) in 1999-2002 to 52% and 63% in 2013-2017.The marked heterogeneity of the studied tumors highlights the need to differentiate the analysis by sex and entity for meaningful interpretation of epidemiologic metrics. With the expansion of clinical cancer registration in Germany, additional analyses including other important clinical factors will be possible in the future.HINTERGRUND: Die Gruppe der Lippen‑, Mundhöhlen- und Pharynxkarzinome (ICD-10: C00–C14) beschreibt eine heterogene Gruppe bösartiger Tumoren, deren Inzidenz, Mortalität und Überleben sich nach Entität stark unterscheiden.Diese Arbeit gibt eine detaillierte Übersicht epidemiologischer Maßzahlen für diese Tumorentitäten unter Berücksichtigung der Heterogenität bezüglich Alter, Geschlecht, Lokalisation und Stadium.Inzidenz- und Mortalitätsdaten für Deutschland für die Jahre 1999–2016 wurden aus der interaktiven Datenbank des Zentrums für Krebsregisterdaten (ZfKD) extrahiert. Alters- und Stadienverteilungen und altersstandardisiertes relatives 5‑Jahres-Überleben wurden auf dem gepoolten ZfKD-Datensatz (Diagnosejahre 1999–2017) berechnet.Im Jahr 2016 lagen die Inzidenz und Mortalität für alle Entitäten insgesamt bei 17,6 bzw. 7,0 pro 100.000 Männern und 6,5 bzw. 1,8 pro 100.000 Frauen. Das relative 5‑Jahres-Überleben für 2015–2017 lag bei 53 % bzw. 63 %. Es zeigten sich deutliche Unterschiede in der Überlebensrate und in der Alters- und Stadienverteilung zwischen den Entitäten. Im zeitlichen Verlauf zeigte sich ein Anstieg des Alters bei Diagnose, insbesondere bei Männern, aber keine Veränderung in der Stadienverteilung. Trotzdem stieg das relative 5‑Jahres-Überleben zwischen 1999–2002 und 2013–2017 von 45 % (Männer) bzw. 59 % (Frauen) auf 52 % bzw. 63 %.Die starke Heterogenität der untersuchten Tumoren verdeutlicht die Notwendigkeit einer nach Geschlechtern und Lokalisationen getrennten Betrachtung für eine aussagekräftige Interpretation der epidemiologischen Kennzahlen. Mit Ausbau der klinischen Krebsregistrierung werden in Zukunft zusätzliche Analysen unter Einbezug weiterer wichtiger klinischer Faktoren möglich sein.
