Short foot exercise (SFE) can be combined with dynamic functional tasks such as squats; however, it is unclear whether this combination increases intrinsic foot muscle activity. This study aimed to investigate and compare the abductor hallucis muscle (AbdH) activity during SFE in static and dynamic functional tasks. The AbdH electromyography data of 17 healthy participants with and without SFE were analyzed during static tasks (sitting, double-leg standing, and single-leg standing) and dynamic tasks (double-leg squat, single-leg squat, split squat, and heel-raise). The static tasks were performed with SFE for 5 seconds, and the dynamic tasks were performed while performing SFE. AbdH activity with or without SFE during the task was compared using the Friedman and Wilcoxon signed-rank tests. AbdH activity was significantly greater in conditions with SFE than in those without SFE for all tasks (P < 0.01) except for heel-raise (P = 0.163). AbdH activity during SFE in single-leg standing was significantly higher than that in sitting, double-leg standing, and double-leg squats (P < 0.05). AbdH activity during SFE in the single-leg squat was also significantly greater than that in the sitting position (P = 0.024). No significant differences were found in any other between-task comparisons of AbdH activity during SFE. AbdH activity during tasks without SFE revealed significantly lower levels for sitting and double-leg standing compared to single-leg squat, split squat, and heel-raise (P < 0.001). Additionally, the activity in double-leg squat was significantly lower than in both single-leg squat and heel-raise (P < 0.05). Combining dynamic tasks, except for the heel-raise task, with SFE can increase AbdH activity more than dynamic tasks without SFE. Background: Pes planus, commonly known as flatfoot, is a condition in which the medial arch of the foot is abnormally low or absent, leading to the inner part of the foot having less curvature than normal. Symptom recognition and errors in diagnosis are problems encountered in daily practice. Therefore, it is important to improve how a diagnosis is made. With the availability of large datasets, deep neural networks have shown promising capabilities in recognizing foot structures and accurately identifying pes planus. Methods: In this study, we developed a novel fusion model by combining the Vgg16 convolutional neural network (CNN) model with the vision transformer ViT-B/16 to enhance the detection of pes planus. This fusion model leverages the strengths of both the CNN and ViT architectures, resulting in improved performance compared to that in reports in the literature. Additionally, ensemble learning techniques were employed to ensure the robustness of the model. Results: Through a 10-fold cross-validation, the model demonstrated high sensitivity, specificity, and F1 score values of 97.4%, 96.4%, and 96.8%, respectively. These results highlight the effectiveness of the proposed model in quickly and accurately diagnosing pes planus, making it suitable for deployment in clinics or healthcare centers. Conclusions: By facilitating early diagnosis, the model can contribute to the better management of treatment processes, ultimately leading to an improved quality of life for patients. Background: The use of subtalar arthroereisis as an adjunct to the surgical treatment of stage 1 flexible progressive collapsing foot deformity (PCFD) is controversial. The aim was to investigate the clinical outcomes and report the implant removal rate of subtalar arthroereisis as an adjunct for stage 1 PCFD. Methods: A retrospective study of 212 consecutive feet undergoing operative management of stage 1 PCFD with adjunctive subtalar arthroereisis between October 2010 and April 2018. The primary outcome was the Foot and Ankle Outcome Score (FAOS). Secondary outcomes included Foot and Ankle Disability Index (FADI), Euroqol-5D-5L Index and implant removal rate. Results: Post-operative clinical FAOS outcomes were collected for 153 feet (72.2%). At mean 2.5-year follow-up, the mean ± standard deviation FAOS for each domain was as follows; Pain: 81.5 ± 18.5, Symptoms: 79.5 ± 12.9, Activities of Daily Living: 82.5 ± 15.4 and Quality of Life: 64.2 ± 23.7. EQ-5D-5L Index was 0.884 ± 0.152. Pre-operative scores were available for 20 of these feet demonstrating a statistically significant improvement in all FAOS, FADI and EQ-5D-5L domains (p < 0.05). The implant removal rate for persistent sinus tarsi pain was 48.1% (n = 102). Conclusion: Use of a subtalar arthroereisis implant as an adjunct to conventional procedures in stage 1 flexible PCFD can result in significant improvement in pain and function. Purpose: Symptomatic flexible pes planus (SFPP) can cause pain and discomfort when walking or engaging in sportive activities in children and adolescents. SFPP can be treated conservatively with foot orthoses, such as the University of California Berkeley Laboratory (UCBL) foot orthosis, which can improve foot function and reduce pain. Kinesio Tape (KT) has also been used as an adjunct to foot orthoses in the treatment of pes planus. This study aims to compare the effectiveness of the UCBL foot orthosis with and without KT in the treatment of SFPP among amateur juvenile and adolescent athletes. Methods: Fifty patients with SFPP were included in the study. In 27 patients UCBL foot orthosis with KT (group 1) was used whereas in 23 UCBL (group 2) was preferred only. The patients were evaluated with AOFAS and radiological measurements. Results: The mean follow-up period was 28.6 ± 4.3(26) months. At the final follow-up AOFAS of group 1 was significantly higher than group 2. In group 2, 12 patients (%52,17) had pressure sores that caused superficial dermabrasion. Lateral TFMAs and talocalcaneal angle in group 1 was significantly better than group 2. Conclusions: This study attempted to determine if using KT with the UCBL foot orthosis was beneficial to the treatment of SFPP compared to simply wearing the orthosis. Our results suggest that KT is effective in reducing pronation and improving the AOFAS score. The use of UCBL with KT seems to be preferable in children and adolescents with SFPP since it is associated with a lower rate of complication, a higher degree of patient compliance and faster improvement in the radiological and clinical findings, compared to the use of the UCBL orthosis alone. Pes planus occurs due to the loss of the longitudinal arch of the foot, resulting in altered gait mechanics. This may lead to increased complications following total hip arthroplasty (THA). Thus, the aim of this study was to assess the effects that pes planus has on rates of falls, implant complications, fall-related injuries, and times to revision among THA patients. A retrospective review of a private insurance claims database was conducted from 2010 to 2021. Patients who had a diagnosis of congenital or acquired pes planus and cases of THA were identified. Patients undergoing THA with a diagnosis of pes planus were matched to control patients 1:5 based on age, sex, and comorbidity profiles. Logistic regression was utilized to assess for differences in complication rates. A total of 3,622 pes planus patients were matched to 18,094 control patients. The pes planus group had significantly higher rates of falls than the control group (6.93 versus 2.97%, OR [odds ratio]: 2.43; CI [confidence interval]: 2.09 to 2.84; P <.001). Pes planus patients also had significantly greater odds of dislocation (OR: 1.89; CI: 1.58 to 2.27; P <.001), mechanical loosening (OR: 2.43; CI: 2.09 to 2.84; P =.019), and periprosthetic fracture (OR: 2.43; CI: 2.09 to 2.84; P <.001). The pes planus group had significantly greater rates of proximal humerus fractures (P =.008), but no difference was seen in distal radius fractures (P =.102). The time to revision was significantly shorter in the pes planus group (190 versus 554 days, P <.001). Pes planus in patients undergoing THA is associated with increased risk of complications and faster time to revision. BACKGROUND: With lifestyle changes, the prevalence of flatfoot is increasing year by year, with a prevalence of 29%. Flatfoot will lead to an inevitable injury and reduce the quality of life. Short foot exercises can enhance the strength of the intrinsic muscles of the foot and improve the symptoms of flatfoot. However, there is controversy regarding its specific efficacy. OBJECTIVE: This meta-analysis quantitatively evaluates the effect of short foot training on patients with flatfeet and provides evidence to inform the clinical approach to short foot training in patients with flat feet. METHODS: A total of eight databases were searched, including CNKI, WANFANG, VIP, and CBM in Chinese and PubMed, Cochrane, Web of Science, and Embase in English. The timeframe for searching the literature was March 2023 for each database build. English database search terms and search formulas were: (flat foot OR talipes valgus OR talipes calcaneovalgus) AND (short foot exercises OR physical therapy OR neurophysiotherapy). RESULTS: The current pooled results show no significant difference in the improvement of the navicular drop test and foot posture index with short foot exercises compared to controls; only short foot exercises greater than 6 w showed a significant improvement in the navicular drop test, and sensitivity analysis showed a significant improvement in the foot posture index with short foot exercises. CONCLUSION: This systematic review and meta-analysis showed that short foot exercises need a larger sample size to find their effect on improving flat feet; the duration of the intervention is a factor. As most studies are currently unclear whether the participants were patients with flat feet or asymptomatic individuals with flat feet the disease syndrome in patients with flatfoot may also be a factor. Distal extremity misalignment may give rise to pelvic floor dysfunctions (PFDs). This study aimed to compare pelvic floor muscle strength (PFMS) and dysfunctions in women with and without pes planus. Women with (pes planus group, n = 30) and without pes planus (control group, n = 30) were included. The presence of PFDs questioned. Pes planus with the Feiss Line Test, PFMS with the Modified Oxford Scale, and the severity of PFDs with the Pelvic Floor Distress Inventory-20 (PFDI-20), including three subscales (Pelvic Organ Distress Inventory-6 (POPDI-6) for pelvic organ prolapse, Colorectal-Anal Distress Inventory-8 (CRADI-8) for colorecto-anal symptoms, and Urinary Distress Inventory-6 (UDI-6) for urinary symptoms, were assessed. It was seen that no difference was found between groups in terms of PFMS (p > 0.05). However, urinary incontinence and anal incontinence were higher in women with pes planus than in women without pes planus (p < 0.05). Only the PFDI-20, CRADI-8, and UDI-6 scores were higher in women with pes planus compared to controls (p < 0.05). There was no difference was found between groups in terms of POPDI-6 scores (p > 0.05). The PFMS did not change according to the presence of pes planus. However, the prevalence of PFDs and their severity were higher in women with pes planus in comparison to controls. Purpose: We aimed to report early results of performing joint-preserving surgeries for managing spasmodic flatfoot deformity (SFFD) in adolescents. Methods: A prospective case series study including 24 patients (27 feet) diagnosed with idiopathic SFFD not responding to conservative management. After reassessment under anesthesia, surgical procedures included soft tissue releases (Achilles tendon (AT), peroneus brevis (PB), peroneus tertius (PT) (if present), and extensor digitorum longus (EDL)), bony osteotomies (lateral column lengthening (LCL), medial displacement calcaneal osteotomy (MDCO), and double calcaneal osteotomy (DCO)), and medial soft tissue reconstruction or augmentation if needed. Functional evaluation was performed per the American Orthopedic Foot and Ankle Society (AOFAS) score, while radiological parameters included talo-navicular coverage angle (TNCA), talo-first metatarsal angle (AP Meary's angle), calcaneal inclination angle (CIA), talo-calcaneal angle (TCA), talo-first metatarsal angle (Lat. Meary's angle), and tibio-calcaneal angle (TibCA). The preoperative parameters were compared to the last follow-up using the Wilcoxon signed test. Results: The mean age was 15.37 ± 3.4 years, 18 (75%) were boys, and the mean BMI was 28.52 ± 3.5 (kg/m2). Release of AT and fractional lengthening of PL, PT, and EDL were performed in all patients. LCL was needed in eight feet (29.6%), MDCO in 5 (18.5%), and DCO in 14 (51.9%). FDL transfer was required in 12 (44.4%) feet, and repair of the spring ligament in seven (25.9%). The mean operative time was 99.09 ± 15.67 min. All osteotomies were united after a mean of 2.3 ± 0.5 months. After a mean follow-up of 24.12 ± 8.88 months (12 and 36 months), the AOFAS improved from a preoperative mean of 43.89 ± 11.49 to a mean of 87.26 ± 9.92 (P < 0.001). All radiological parameters showed significant improvement, AP Meary's angle from a mean of 20.4 ± 5.3 to a mean of 9.2 ± 2.1, Lat. Meary's angle from − 15.67° ± 6.31 to − 5.63° ± 5.03, TNCA from − 26.48° ± 5.94 to 13.63° ± 4.36, CIA from 12.04° ± 2.63 to 16.11° ± 3.71, TibCA from − 14.04° ± 3.15 to − 9.37° ± 3.34, and TCA Lat. from 42.65° ± 10.68 to 25.60° ± 5.69 (P ≤ 0.001). One developed wound dehiscence (over an MDCO), managed with daily dressings and local antibiotics. Another one developed lateral foot pain after having LCL managed by metal removal. Conclusion: Careful clinical and radiological evaluation for the correct diagnosis of SFFD is paramount. Joint-preserving bony osteotomies combined with selective soft tissue procedures resulted in acceptable functional and radiological outcomes in this young age group. [ABSTRACT FROM AUTHOR]

Objective: To perform a systematic review to examine the diagnostic accuracy of magnetic resonance imaging, ultrasonography, and radiography in the evaluation of posterior tibial tendon dysfunction (PTTD). Materials and methods: Medline, Scopus, Embase, and The Cochrane Central Register of Controlled Trials (CENTRAL) were searched for relevant studies through April 2023. The study quality was assessed using the QUADAS-2 scoring system. Results: Of the initial 634 studies, 12 studies met the quality criteria and were included, with 645 PTTs evaluated with MRI, 133 with US, and 97 with radiography. MRI was found to be more sensitive and specific than ultrasound, radiography, and clinical evaluation for detecting PTTD, with a sensitivity of up to 95%, specificity of up to 100%, and accuracy of 96% for detecting PTT tears. US showed a sensitivity of 80% and specificity of 90% for diagnosing tendinopathy, and a sensitivity of 90% and specificity of 80% for diagnosing peritendinosis when compared to MRI. Weight-bearing radiographs had a sensitivity of 71.4%, specificity of 88.9%, and diagnostic accuracy of 81.3% when diagnosing PTT ruptures. Various radiologic measurements were also found to have a significant relationship with PTT tears but were poorly correlated with PTT tendinosis and isolated tenosynovitis. Conclusion: Magnetic resonance imaging is the preferred imaging tool for evaluating patients with posterior tibial tendon dysfunction, with higher diagnostic accuracy, sensitivity, and specificity compared to ultrasound and radiographic imaging. However, initial imaging with ultrasound and radiographs may be used due to their availability and cost-effectiveness. Clinical relevance: PTTD affects 3% of women ≥ 40 years and 10% of adults ≥ 65 years globally. Using the appropriate imaging study, MRI ensures that patients suffering from PTTD are diagnosed and treated in a timely manner. Background: Flatfeet in children are common, causing concern for parents due to potential symptoms. Technological advances, like 3D foot kinematic analysis, have revolutionized assessment. This review examined 3D assessments in paediatric idiopathic flexible flat feet (FFF). Methods: Searches focused on paediatric idiopathic FFF in PubMed, Web of Science, and SCOPUS. Inclusion criteria required 3D kinematic and/or kinetic analysis during posture or locomotion, excluding non-idiopathic cases, adult feet, and studies solely on pedobarography or radiographs. Results: Twenty-four studies met the criteria. Kinematic and kinetic differences between FFF and typical feet during gait were outlined, with frontal plane deviations like hindfoot eversion and forefoot supination, alongside decreased second peak vertical GRF. Dynamic foot classification surpassed static assessments, revealing varied movement patterns within FFF. Associations between gait characteristics and clinical measures like pain symptoms and quality of life were explored. Interventions varied, with orthoses reducing ankle eversion and knee and hip abductor moments during gait, while arthroereisis normalized calcaneal alignment and hindfoot eversion. Background: Individuals with pes planus tend to overuse the extrinsic foot muscles, such as the tibialis anterior (TA) and peroneus longus (PL), to compensate for the weakened intrinsic foot muscles, such as the abductor hallucis (AbdH). Furthermore, differences in weight-bearing can affect the activity of muscles in both the intrinsic and extrinsic foot muscles. To date, no study has compared the effects of the short foot exercise (SFE) and toe spread-out exercise (TSO) on intrinsic and extrinsic foot muscle activity and the corresponding ratios in different weight-bearing positions. Objects: To compare the effects of the SFE and TSO on AbdH, TA, and PL activity and the AbdH/TA and AbdH/PL activity ratios in the sitting and standing positions in individuals with pes planus. Methods: Twenty participants with pes planus were recruited. Surface electromyography was used to assess the amplitudes of AbdH, TA, and PL activity. Participants performed both exercises while adopting both the sitting and standing positions. Results: No significant interaction between exercise and position was found regarding the activity of any muscle or ratio of the activity, except for PL activity. We observed a significant increase in AbdH activity during the TSO compared to the SFE, and no significant difference in TA and PL activity between the two exercises. AbdH, TA, and PL activity were significantly higher in the standing position than in the sitting position. Furthermore, the AbdH/PL activity ratio significantly increased in the sitting position, although there was a significant increase in AbdH activity in the standing position. Conclusion: In individuals with pes planus, we recommend performing the TSO in the sitting position, which may increase the activity of the AbdH while concurrently decreasing the activity of the TA and PL, thus strengthening the AbdH.

Background. Clinicians employ foot morphology assessment to evaluate the functionality of the method and anticipate possible injuries. This study aims to correlate static foot posture and the dynamic barefoot evaluation in a sample of healthy adult participants. Methods. The foot posture was evaluated using the Foot Posture Index-6 (FPI-6) and the dynamics were evaluated through baropodometric examination. Two operators independently assessed the participants' foot posture through FPI-6, and then a dynamic evaluation was performed by asking them to walk 8 times across a platform. One hundred participants (mean age: 32.15 ± 7.49) were enrolled. Results. The inter-rater agreement between the two assessors was found to be excellent. The majority of the feet belonged to the 0 < FPI < 4 class (32%), followed by the 4 < FPI < 8 (31%) and the FPI > 8 ranges (19.5%). Our "area of contact" analysis showed a significant poor correlation between FPI and total foot, midfoot, and the second metatarsophalangeal joint (MTPJ) (−0.3 < r < 0). Regarding "force" parameters, the analysis showed a poor correlation between the midfoot, hallux, and the second toe (−0.2 < r < 2); finally the "pressure" analysis showed a poor correlation between FPI, the fourth MTPJ, and the second toe (−0.2 < rs < 0.3) and a moderate correlation between the hallux (r = 0.374) and the fifth MTPJ (r = 0.427). The study aimed to determine the efficacy of foot muscle exercises in children with DS having pes planus. Forty-seven subjects randomly assigned to foot muscle exercises (study group) or an arch support insole with one-leg balance exercises (control group), thrice weekly intervention for 12-weeks followed by a home program with residual effect assessed after 24-weeks from baseline. The motor functions were significantly improved in both groups (p = 0.00). A positive residual effect was found in the study group for both parameters. Whilst in the control group it failed to give a positive residual effect for GMFM-88, while PBS yielded positive outcomes. The study group showed significantly better results than the control group in comparison. The novel finding suggests that the foot muscle exercise has the potential to improve motor functions in children with Down syndrome and it can be used as an alternative therapeutic approach to the conventional method. Objective: This study aimed to investigate the effects of individually designed insoles on physical activity level, balance, and functional performance in patients with pes planus. Materials and Methods: 38 participants were divided into 2 groups as 3D printed (n=20) and placebo (n=18). In the 3D printed group, medial longitudinal arch support, medial wedge of the foot, medial heel wedge, and transverse arch support were given to the insoles according to the needs of the participants. The placebo group received placebo insoles. Muscle strength, range of motion, static and dynamic balance, physical and physical activity levels were measured at baseline, and eight weeks later (clinicaltrials.gov ID: NCT05306886). Results: Muscle strengths of the plantar flexion (p=0.030) and eversion increased after the use of insoles in the 3D printed and placebo groups (p=0.020). There was no change in static balance in both groups (p=0.386), but there were significant improvements in dynamic balance in 3D printed groups (p=0.019). Gait speed improved in both 3D printed (p=0.001) and placebo groups (p=0.015). There was no significant difference in the explosive power between the groups (p=0.272). The shuttle run test improved significantly in the placebo group (p=0.015), but no significant change was observed in the 3D printed group (p=0.886). While the physical activity levels in the placebo group showed a significant improvement (p=0.017), the physical activity level decreased in the 3D printed group (p=0.489). Conclusions: In this study, it was found that the use of personalized insoles is effective in improving physical activity, balance, and performance, while placebo insoles were found to be more effective in increasing muscle strength and explosive muscle strength. Introduction: Flatfoot is a condition commonly seen in children; however, there is general disagreement over its incidence, characterization and correction. Painful flatfoot accompanied with musculoskeletal and soft tissue problems requires surgery to avoid arthritis in adulthood, the most common surgical approach being two osteotomies to the calcaneus and medial cuneiform bones of the foot. Objectives: This study focuses on the parametrization of these two bones to understand their bone morphology differences in a population sample among 23 normal subjects. Population differences could help in understanding whether bone shape may be an important factor in aiding surgical planning and outcomes. Methods: A total of 45 sets of CT scans of these subjects were used to generate surface meshes of the two bones and converted to be iso-topological meshes, simplifying the application of Generalized Procrustes Analysis and Principal Component Analysis, allowing the main sources of variation between the subjects to be quantified. Results: For the calcaneus, 16 Principal Components (PCs) and, for the medial cuneiform, 12 PCs were sufficient to describe 90% of the dataset variability. The quantitative and qualitative analyses confirm that for the calcaneus PC1 describes the Achilles attachment location and PC2 largely describes the anterior part of the bone. For the medial cuneiform, PC1 describes the medial part of the bone, while PC2 mainly describes the superior part. Conclusion: Most importantly, the PCs did not seem to describe the osteotomy sites for both bones, suggesting low population variability at the bone cutting points. Further studies are needed to evaluate how shape variability impacts surgical outcomes. Knee and foot deformities refer to structural abnormalities in the knee and foot bones, joints, ligaments, or muscles. Various factors, including genetics, injury, disease, or excessive use, can cause these deformities. These musculoskeletal conditions can significantly impact individuals' quality of life. This study examined foot and knee deformities in 231 young healthy adults (165 men, 66 women) aged 22.6 ± 4.9 years and their association with physical activity and body composition. The postural assessment was performed by two Physiotherapists, with the subject standing in three views: side, anterior, and posterior. Physical activity (Baecke's Habitual Physical Activity Questionnaire) and body composition (InBody 770) were assessed. Results showed that the most common foot deformity was pes planus, while the genu recurvatum was the most common knee deformity among the individuals. Physical activity level was negatively associated with knee and foot deformities. Conversely, body composition differed with the presence of genu recurvatum. These findings present a starting point to understand the occurrence of knee and foot postural alterations according to the individuals' body composition and physical activity profiles, which could support the deployment of tailored interventions among healthy adults. Aims: The aim of this study was to compare the clinical evaluations, disease impact, disability and foot function of symptomatic flexible pes planus (SFPP) deformity patients treated with the University of California at Berkeley Laboratory (UCBL) foot orthosis and Kinesio tape (KT) to those treated only with the UCBL orthosis. Methods: A total of 100 feet in 50 subjects with a mean age of 77.10 months were included in the study. The subjects were divided into two groups: Group 1 (UCBL foot orthosis with KT, n=27) and Group 2 (UCBL-alone, n=23). Group 1 consisted of 27 patients (14 girls, 13 boys) with an average age of 62 months (range: 25 to 165), while Group 2 consisted of 23 patients (10 girls, 13 boys) with an average age of 63 months (range: 30 to 166). Various assessments, including American Orthopaedic Foot and Ankle Society (AOFAS) scores, anteroposterior and lateral talocalcaneal and talo-first metatarsal angles, talonavicular angle, calcaneal pitch angle and clinical examinations, were conducted to foot-specific disease activity, and foot function. Results: Group 1 exhibited mild-to-moderate foot disability and impairments, along with low levels of disease activity. Treatment with UCBL orthosis and Kinesio tape led to significant improvements in all AOFAS scores and foot angles. Substantial improvement in AOFAS scores was observed during the follow-up examination, except for the midfoot score. Discussion: The use of UCBL foot orthosis in conjunction with Kinesio tape appears to be a preferable treatment strategy for children and adolescents with SFPP. This combined approach is associated with a lower rate of complications, higher patient comfort levels, and faster improvement in both radiological and clinical findings when compared to the use of the UCBL orthosis alone. The intrinsic and extrinsic foot softtissue structures that apply force and support the medial longitudinal arch (MLA) have been implicated in the development of flat feet. However, the relationship between the changes in MLA height under increasing load and the morphological and mechanical properties of individual intrinsic and extrinsic foot soft tissue structures is not fully understood. To examine the morphological and mechanical characteristics of the foot soft tissue structures in flat feet when subjected to loading. This study consisted of two studies focusing on the extrinsic foot muscles (10 normal feet/11 flat feet) and intrinsic foot muscles (14 normal feet/13 flat feet). Images of the extrinsic and intrinsic foot muscles and plantar fascia (PF) under 10%, 50%, and 90% body weight conditions were obtained using ultrasound-based shear-wave elastography. The cross-sectional area (CSA) of the peroneus brevis was larger in the flat-foot group than in the normal-foot group under all loading conditions. The CSAs of the intrinsic foot muscles (abductor hallucis, flexor digitorum brevis, and quadratus plantae) and thickness of the PF in the flat-foot group decreased significantly with increasing load. As for mechanical characteristics, the stiffness of the flexor digitorum longus and abductor hallucis was higher in the flat-foot group than in the normal group under high loading conditions. In addition, flat feet with greater flexibility tended to exhibit a greater decrease in PF thickness and smaller increase in stiffness. Excessive stretching of the intrinsic foot muscles and PF occurs in flat feet, and excessive contraction of the flexor digitorum longus may counteract the excessive lowering of the foot arch. Background: Medial longitudinal arch of foot plays a crucial role on maintaining ankle during weight bearing activities. In previous studies arch index derived from foot print shows a negative correlation with the navicular height. There are no other studies have correlated mathematically derived foot print values with standing arch height measurements with an acceptable equation. This study correlates arch index and foot posture index to identify foot arches. Methods: subjects with high arch have taken for the study. The study type is observational study was done on sample size n=60, foot arch index and foot posture index was evaluated with the samples. Findings: a r- value of 0.333, there is a Negative correlation between Arch index and Foot posture index in low arched foot. There is no significant correlation, and the a r- value of 0.173, there is a low positive correlation between arch index and foot posture index in high arched foot. Interpretation: Among high arched foot and low arched foot, Foot Posture Index and Arch Index is correlated, according to Pearson's correlation, only high arched foot shows low positive correlation between Foot Posture Index and Arch Index. Background: Pes planus, commonly known as flat foot, is a deformity that leads to the collapse of the medial longitudinal arch and flattening of the foot against the ground. This study aims to compare the effectiveness of jumping rope exercise and short foot exercise on pain reduction and improvement in quality of life for patients with flatfoot. Purpose: To find out the effect of Jumping rope exercise versus short foot exercise among individuals with flatfoot. Method: A sample of 30 participants who met the inclusion and exclusion criteria were selected for the study. They were divided into two groups: the jumping rope exercise group with ultrasound (n=15) and the short foot exercise group with ultrasound (n=15). The interventions were administered five times a week for three weeks. The entire process was performed from November 2022 to April 2023. Outcome measure: The navicular drop test (NDT) was used to measure the degree of foot pronation, the Numeric Pain Rating Scale (NPRS) was employed to assess pain levels, and the Foot Health Status Questionnaire (FHSQ) was utilized to evaluate participants' quality of life. Results: The mean value of the jumping rope exercise group at the post-test was significantly higher than that of the short foot exercise group, with a p-value of <0.0001 Conclusion: Based on the findings, it can be concluded that jumping rope exercise with ultrasound therapy yields superior outcomes in managing Pes Planus compared to short foot exercise. Background: Pes Planus (flatfoot) only becomes a medical issue when symptoms appear. This disease occurs in both young children and adults. When the medial longitudinal arch (MLA) has raised to the point where it cannot be maintained, the flatfoot changes form. Background: Pes planus, usually called flat foot is a deformity that causes the medial longitudinal arch to collapse and makes the foot flat to the floor level. Flat feet may be caused by aging, pregnancy, heredity, physiological condition or an injury to the foot ligaments. Weight-bearing exercises are performed on an unstable surface such as wobble board and BOSU ball as part of the balance board training technique. Purpose: To find out the effect of BOSU ball exercises versus wobble board exercises among individuals with pes planus. Materials and Methods: A sample of 30 was taken using a navicular drop test on the arch of the foot, NPRS on pain, and Berg balance scale on balance. According to the selection criteria for the study which was divided into two groups, a BOSU ball with ultrasound (n=15) and wobble board with ultrasound (n=15) were given. Interventions were given as 6 sessions a week for 2 weeks. The entire process was performed from November 2022 to April 2023. Result: The mean value of BOSU ball exercises at the post-test was found to be higher than the mean value of wobble board exercises with a p-value of <0.0001. Being male (AOR = 2; 95%CI:1.22-3.30), living in highland altitude (AOR = 8.83; 95% CI: 4.64-16.79), living in midland altitude (AOR = 3.32;95% CI:1.75-6.29), living in an urban area (AOR = 2.42;95% CI:1.15-5.09), insufficient physical activity (AOR = 8.78;95% CI: 4.42-12.3), wearing closed-toe shoes (AOR = 2.33;95%CI:1.27-4.28), obesity (AOR = 6.30;95% CI:3.31-11.9), and foot pain (AOR = 3.52;95%CI:2.08-6.27) had a higher likelihood of flat foot as compared to their counterparts. Conclusion: One in every ten children had a flat foot. Altitude, residence, sex, physical activity, foot pain, body mass index, and type of footwear were found to be factors statistically associated with flat foot. Integrated interventions for children to have sufficient physical activity, wearing sandals, maintaining a healthy body mass index, and flatfoot screening and monitoring are recommended. Background: Flatfeet in children are common, causing concern for parents due to potential symptoms. Technological advances, like 3D foot kinematic analysis, have revolutionized assessment. This review examined 3D assessments in paediatric idiopathic flexible flat feet (FFF). Methods: Searches focused on paediatric idiopathic FFF in PubMed, Web of Science, and SCOPUS. Inclusion criteria required 3D kinematic and/or kinetic analysis during posture or locomotion, excluding non-idiopathic cases, adult feet, and studies solely on pedobarography or radiographs. Results: Twenty-four studies met the criteria. Kinematic and kinetic differences between FFF and typical feet during gait were outlined, with frontal plane deviations like hindfoot eversion and forefoot supination, alongside decreased second peak vertical GRF. Dynamic foot classification surpassed static assessments, revealing varied movement patterns within FFF. Associations between gait characteristics and clinical measures like pain symptoms and quality of life were explored. Interventions varied, with orthoses reducing ankle eversion and knee and hip abductor moments during gait, while arthroereisis normalized calcaneal alignment and hindfoot eversion. Conclusions: This review synthesises research on 3D kinematics and kinetics in paediatric idiopathic FFF, offering insights for intervention strategies and further research.

Introduction: Flexible flatfeet are commonly treated with functional foot orthoses (FFOs) and University of California Berkeley Laboratory (UCBL) designs. This study sought to evaluate the immediate effects of low-profile FFOs and high-profile UCBL orthoses on Foot Posture Index (FPI), spatiotemportalmeasures of gait, and perceived comfort and compliance in adults with flexible flatfeet. Materials and Methods: Fourteen individuals with self-reported asymptomatic flexible flatfeet participated in this randomized cross-sectional study. Custom FFOs and UCBL orthoses were fabricated for each participant, who then walked at a self-selected speed in each condition on a Zeno Mat. Foot Posture Index, perceived comfort and compliance, and spatiotemporal parameters were collected across conditions. Results: There were no significant differences in velocity, cadence, step length, stride width, toe-in/toe-out, or stance center of pressure distance percentage. Stance phase duration significantly increased from baseline to UCBL condition. There was a statistically significant decrease bilaterally in FPI from baseline to FFO and from baseline to UCBL orthosis, but not between FFO and UCBL orthosis. There were no significant differences in comfort and compliance ratings between FFOs and UCBL orthoses. Conclusions:With regard to immediate effects, both the UCBL and FFO designs improve foot posture without significant impact on spatiotemporal gait patterns or perceived comfort and compliance. Additional research is needed to determine the longevity of these effects. Pes planus is one of the most common biomechanical disorders of the lower extremities caused by different intrinsic and extrinsic factors and negatively affects activities of daily living from mild to severe. Balance-based domestic or international studies give limited data on the diagnosis of flatfoot. For this reason, we aimed to give the view of the stati c and dynamic balance states of individuals with pes planus in our clinic. Twenty-two patients diagnosed with pes planus of deformity grade 2 and above according to the Feiss line and 23 volunteers without pes planus were included in the study. All participants were evaluated for static (30TST and single leg stance tests) and dynamic [Berg balance scale (BBS), Tinetti balance & gait test and timed up & go (TUG) tests] balance. The distribution among the groups evaluated with nonparametric tests was significant in terms of both static {[χgroup*tandem(1)=17.107, p=0.000], [χ(1)group*single_leg =13.442, p=0.000]} and dynamic [(Ugroup*berg= 60, p=0.000), (Ugroup*tinetti= 30, p=0.000), (Ugroup*timed_up&go =7.5, p=0.000)] tests. In all static balance and dynamic tests, the time to stay in balance was found to be lower in the pes planus group, as expected. Apart from this, when the relationship between demographic and clinical data, balance tests and groups was evaluated, no statistically significant relationship was found (p>0.05). Introduction: The presence of pes planus may be responsible for the disorder of venous return during ambulation. Our aim in this study is to reveal that the risk of venous insufficiency may be increased in patients with pes planus. Methods: A total of 59 patients with 29 bilateral pes planus and 30 normal arches included in the study. Djian Annonier angle measurement, bilateral lower extremity venous Doppler ultrasonography (USG), foot function index (AFI), Comprehensive Classification System for Chronic Venous Disorders (CEAP) clinical scoring and Short Form-36 (SF-36) was evaluated. Results: There was a statistically significant difference in AFI total, AFI pain, AFI disability and AFI activity limitation scores, bilateral lower extremity doppler USG and CEAP clinical scores in the patient group compared to the control group. Conclusion: These findings suggest that there may be a relationship between pes planus and venous insufficiency. However, large-scale studies with more patient involvement are needed. It was aimed to investigate the effect of custom made insoles on baropodometric analysis parameters in individuals with pes planus, using objective measurement and production methods. 100 individuals with pes planus, aged between 4-18 years in the study. Individuals who met the inclusion criteria and the medial longitudinal arch index were included in the study. Valgus angles of individuals, plantar measurements of the feet before the use of insoles and after 6 months of the use of insoles were evaluated with the Sensor Medica®? device. When the measurement results of the ankle valgus angles of the individuals before and after the insoles were examined, it was observed that there was a statistical decrease in the mean results in both feet (p<0,05). When the plantar pressure results were examined, the load on the medial side of the right foot was reduced significantly (p=0,012). Although there was a decrease in medial longitudinal arch of the right foot, it was not significant. There were no change in the lateral side of the right foot and the medial side of the right rearfoot. The load on the medial left rearfoot and the left medial longitudinal arch were reduced significantly (p=0,004, p=0.021). In study in individuals with pes planus, it was concluded that the foot should be well evaluated before and after technological based applications. Whether the insoles used provide benefits in foot development should be followed up with controls and their suitability should be checked, and changes in their physical capacities should be observed. Purpose : Although flatfoot is a widespread human condition, historical medical texts and ancient illustrations on this deformity are extremely rare. Nowadays, doubts regarding its management remain unsolved. This historical review aims to identify the presence of pes planus since the prehistoric era and examine the treatments proposed over the centuries up to the present. Method: For this propose, we performed an extensive electronic search of the relevant literature, complemented by a manual search of additional sources from archaeological to artistic, literary, historical, and scientific accounts, describing flatfoot and its treatment in different eras. Results: Flatfoot accompanied the evolutionary timeline of human species: from Lucy Australopithecus to Homo Sapiens. It was described among various diseases suffered by Tutankhamun (1343–1324 B.C.), while the first anatomical description dates to Emperor Trajan (53–117 A.D.) and the medical studies of Galen (129–201 A.D.). It was also represented in the anatomical drawings of Leonardo da Vinci (1452–1519) and Girolamo Fabrici d'Acquapendente (1533–1619). Historically, the conservative treatment by insoles was the only one proposed until the nineteenth century. Since then, the most popular surgical procedures performed for correction have been osteotomies, arthrodesis, arthrorisis, and tendon lengthening and transfer. Conclusion: During the centuries, conservative therapeutic strategies have not radically changed in their substance, while operative ones have become the protagonists during the twentieth century up to the present. Estimates of pes planus ("flatfoot") prevalence vary considerably across studies. Moreover, there is uncertainty over which factors are associated with the pes planus prevalence. We aimed to systematically review the prevalence and clinical factors associated with flatfoot among children and adults. We searched Web of Science, PubMed/MEDLINE, and Google Scholar databases reporting population-based flatfoot prevalence. Two reviewers independently extracted the data and assessed the qualities of the studies. Subgroup analysis was conducted to analyze the associated factors on flatfoot prevalence. Frequencies, odds ratios (OR), and 95% confidence intervals (CI) were performed using descriptive analysis and chi-square test accounting for heterogeneity. Any conflict in the data analysis was discussed by all the reviewers. Twelve studies including 2509 flatfoot cases were analyzed (overall prevalence 15.6%, n = 16,000). The subgroup analysis indicated that male gender (OR = 1.26, 95% CI: 1.15-1.37), age groups 3 to 5 years (OR = 2.02, 95% CI: 1.78-2.30) and 11 to 17 years (OR = 1.91, 95% CI: 1.64-2.22), Asian race (OR = 2.34, 95% CI: 2.10-2.60), and obesity (OR = 2.62, 95% CI: 2.06-3.32) were more associated with flatfoot (p <.001). Conversely, female gender (OR = 0.44, 95% CI: 0.40-0.48) and White race (OR = 0.52, 95% CI: 0.47-0.57) were less associated with flatfoot (p <.001). Our findings may be valuable for clinical/surgical settings, particularly, for those modifiable findings and targeted populations. Low back pain is an important cause of pain referrals in the primary care setting. Overall, 85% of initial admissions are considered to be nonspecific low back pain that resolves in a few weeks without the need for additional evaluation. Myofascial pain, lumbar disc herniation, spinal stenosis, facet joint and disc degeneration, and sacroiliac joint pathologies are common causes of mechanical low back pain. Non-mechanical low back pain includes infections of the vertebral corpus and disc, malignant tumors and metastases, and inflammatory diseases. Red flag symptoms suggestive of serious diseases should be evaluated in all patients with low back pain, and the diagnosis process should be accelerated in patients with these symptoms. The so-called "yellow flags" associated with chronic low back pain include psychosocial factors. In the diagnosis process, pain relief is observed in patients with non-specific low back pain without the need for imaging methods. Laboratory tests and imaging methods can be used to distinguish other causes of low back pain.

Introduction: Flatfoot is a condition commonly seen in children; however, there is general disagreement over its incidence, characterization and correction. Painful flatfoot accompanied with musculoskeletal and soft tissue problems requires surgery to avoid arthritis in adulthood, the most common surgical approach being two osteotomies to the calcaneus and medial cuneiform bones of the foot. Objectives: This study focuses on the parametrization of these two bones to understand their bone morphology differences in a population sample among 23 normal subjects. Population differences could help in understanding whether bone shape may be an important factor in aiding surgical planning and outcomes. Methods: A total of 45 sets of CT scans of these subjects were used to generate surface meshes of the two bones and converted to be iso-topological meshes, simplifying the application of Generalized Procrustes Analysis and Principal Component Analysis, allowing the main sources of variation between the subjects to be quantified. Results: For the calcaneus, 16 Principal Components (PCs) and, for the medial cuneiform, 12 PCs were sufficient to describe 90% of the dataset variability. The quantitative and qualitative analyses confirm that for the calcaneus PC1 describes the Achilles attachment location and PC2 largely describes the anterior part of the bone. For the medial cuneiform, PC1 describes the medial part of the bone, while PC2 mainly describes the superior part. Conclusion: Most importantly, the PCs did not seem to describe the osteotomy sites for both bones, suggesting low population variability at the bone cutting points. Further studies are needed to evaluate how shape variability impacts surgical outcomes. Future implications could include better surgical planning and may pave the way for complex robotic surgeries to become a reality.

A cross-sectional analytical study was conducted on 165 healthy young adults. Individuals with pes rectus and pes planus to correlate the medial longitudinal arch height with postural stability, sensory integration and balance and fall risks. Medializing calcaneal osteotomy (MCO) is a flatfoot treatment in stages IIa–IIb. It is true that structural correction is well known, but stress changes in foot tissues have not been sufficiently studied to date. Our objective was to evaluate the stress generated by MCO in both hindfoot and forefoot bones and in some soft tissues that support the arch. A finite element foot model was employed, simulating some situations related to flatfoot development. Results show a higher stress concentration around the osteotomy region when MCO is used in patients with plantar fascia weakness. Pes planus, colloquially known as flatfoot, is a deformity defined as the collapse, flattening or loss of the medial longitudinal arch of the foot. The first standard radiographic examination for diagnosing pes planus involves lateral and dorsoplantar weight-bearing radiographs. Recently, many artificial intelligence-based computer-aided diagnosis (CAD) systems and models have been developed for the detection of various diseases from radiological images. However, to the best of our knowledge, no model and system has been proposed in the literature for automated pes planus diagnosis using X-ray images. This study presents a novel deep learning-based model for automated pes planus diagnosis using X-ray images, a first in the literature. To perform this study, a new pes planus dataset consisting of weight-bearing X-ray images was collected and labeled by specialist radiologists. In the preprocessing stage, the number of X-ray images was augmented and then divided into 4 and 16 patches, respectively in a pyramidal fashion. Thus, a total of 21 images are obtained for each image, including 20 patches and one original image. These 21 images were then fed to the pre-trained MobileNetV2 and 21,000 features were extracted from the Logits layer. Among the extracted deep features, the most important 1312 features were selected using the proposed iterative ReliefF algorithm, and then classified with support vector machine (SVM). The proposed deep learning-based framework achieved 95.14% accuracy using 10-fold cross validation. The results demonstrate that our transfer learning-based model can be used as an auxiliary tool for diagnosing pes planus in clinical practice. [ABSTRACT FROM AUTHOR]

Purpose: This study aims to examine the effect of 6 weeks of combined exercises (CE), short foot exercise (SFE), and SFE with isometric hip abduction (IHA) on dynamic balance (DB), joint position sense (JPS), and strength in women with flexible flatfoot (FF). Methods: A total of 40 women with flexible flatfoot (FF) participated in this semi-experimental study and were randomly assigned to three groups as follows: CE (n=14), SFE (n=13), and IHA (n=13). Six weeks of exercise were split into 3 sessions under supervision and 3 sessions at home were completed by the participants. The Y balance test, goniometer, and dynamometer were used to assess DB, JPS, and strength before and after the exercises, respectively. The mixed-model repeated-measures multivariate analysis of variance statistical test was used to analyze the data with a significance level of 0.05. Results: The results revealed that after performing the exercises, there was a significant difference in the DB in the SFE group in two posteromedial and posterolateral directions; meanwhile, in the IHA group this difference was in three directions, while in the CE group, it was in the posterolateral direction (P0.05). Between the groups, there was no difference in any of the other variables (P>0.05). Conclusion: Considering that the DB changes in the IHA group were more than in the other two groups, these exercises are suggested to improve the DB in women with FF.
