Abstract

Seed features are a useful and relevant source of taxonomic information, especially when evaluated based on morphometric and micromorphological approaches. Here, we studied 21 populations from four out of the five species of the Mediterranean Linaria purpurea group (Plantaginaceae). Linaria capraria (endemic to Tuscan Archipelago) and L. cossonii (endemic to Sardinia and Tunisia) are easily recognizable based on overall morphometry and SEM analysis, respectively. On the contrary, L. arcusangeli (endemic to southern Sardinia) and L. purpurea (endemic to peninsular Italy and Sicily) show very similar seeds, whose features do not guarantee a reliable identification, although these two species are otherwise well distinct for their habit and flower features. Accordingly, by integrating all the available information, all four species are well supported on morphological grounds. [ABSTRACT FROM AUTHOR]

Abstract

BACKGROUND: Nephritis is a common manifestation of IgA vasculitis and is morphologically indistinguishable from IgA nephropathy. While MEST-C scores are predictive of kidney outcomes in IgA nephropathy, their value in IgA vasculitis nephritis has not been investigated in large multiethnic cohorts. METHODS: Biopsies from 262 children and 99 adults with IgA vasculitis nephritis ( N =361) from 23 centers in North America, Europe, and Asia were independently scored by three pathologists. MEST-C scores were assessed for correlation with eGFR/proteinuria at biopsy. Because most patients ( N =309, 86%) received immunosuppression, risk factors for outcomes were evaluated in this group using latent class mixed models to identify classes of eGFR trajectories over a median follow-up of 2.7 years (interquartile range, 1.2-5.1). Clinical and histologic parameters associated with each class were determined using logistic regression. RESULTS: M, E, T, and C scores were correlated with either eGFR or proteinuria at biopsy. Two classes were identified by latent class mixed model, one with initial improvement in eGFR followed by a late decline (class 1, N =91) and another with stable eGFR (class 2, N =218). Class 1 was associated with a higher risk of an established kidney outcome (time to ≥30% decline in eGFR or kidney failure; hazard ratio, 5.84; 95% confidence interval, 2.37 to 14.4). Among MEST-C scores, only E1 was associated with class 1 by multivariable analysis. Other factors associated with class 1 were age 18 years and younger, male sex, lower eGFR at biopsy, and extrarenal noncutaneous disease. Fibrous crescents without active changes were associated with class 2. CONCLUSIONS: Kidney outcome in patients with biopsied IgA vasculitis nephritis treated with immunosuppression was determined by clinical risk factors and endocapillary hypercellularity (E1) and fibrous crescents, which are features that are not part of the International Study of Diseases of Children classificatio

Abstract

Critical species inventories provide primary biodiversity data crucial for biogeographical, ecological, and conservation studies. After six years, a second update to the inventory of the vascular flora native to Italy is presented. It provides details on the occurrence at regional level and, for the first time, floristic data for San Marino. The checklist includes 8,241 species and subspecies, distributed in 1,111 genera and 153 families; 23 taxa are lycophytes, 108 ferns and fern allies, 30 gymnosperms, and 8,080 angiosperms. The species/subspecies endemic to Italy are 1,702, grouped in 71 families and 312 genera. The taxa currently occurring in Italy are 7,591, while 545 taxa have not been confirmed in recent times, 94 are doubtfully occurring in the country, 11 are data deficient, and 236 are reported by mistake and to be excluded at national level. Abstract

Critical and updated alien species inventories are crucial to properly manage and contrast biological invasions. After 6 years, a second update to the inventory of the vascular flora alien to Italy is presented. It provides details on the occurrence at administrative regional level and, for the first time, floristic data for San Marino. The checklist includes 1782 taxa (1673 species, 28 subspecies, 69 hybrids and 12 cultivar groups not attributable to a specific binomial), distributed in 786 genera and 159 families; 2 taxa are lycophytes, 13 ferns and fern allies, 37 gymnosperms and 1730 angiosperms. Among these, 154 taxa are archaeophytes and 1628 neophytes. The alien taxa currently established in Italy are 899 (649 naturalized and 250 invasive), while 796 taxa are casual aliens, 1 is not assessed, 38 have not been confirmed in recent times, 4 are considered extinct or possibly extinct in the country, 40 are doubtfully occurring in Italy, 4 are of unknown regional distribution and 110 are reported by mistake and to be excluded at national level. Item does not contain fulltext, Living donation challenges the ethical principle of non-maleficence in that it exposes healthy persons to risks for the benefit of someone else. This makes safety, informed consent (IC) and education a priority. Living kidney donation has multiple benefits for the potential donor, but there are also several known short- and long-term risks. Although complete standardization of IC is likely to be unattainable, studies have emphasized the need for a standardized IC process to enable equitable educational and decision-making prospects for the prevention of inequities across transplant centers. Based on the Three-Talk Model of shared decision-making by Elwyn et al., we propose a model, named 3-Step (S) Model, where each step coincides with the three ideal timings of the process leading the living donor to the decision to pursue living donation: prior to the need for kidney replacement therapy (team talk); at the local nephrology unit or transplant center, with transplant clinicians and surgeons prior to evaluations start (option talk); and throughout evaluation, after having learned about the different aspects of donation, especially if there are second thoughts or doubts (decision talk). Based on the 3-S Model, to deliver conceptual and practical guidance to nephrologists and transplant clinicians, we provide recommendations for standardization of the timing, content, modalities for communicating risks and assessment of understanding prior to donation. The 3-S Model successfully allows an integration between standardization and individualization of IC, enabling a person-centered approach to potential donors. Studies will assess the effectiveness of the 3-S Model in kidney transplant clinical practice.

Item does not contain fulltext, BACKGROUND: Kidney transplant (KT) recipients of HLA identical siblings (HLAid) have lower immunological risk, but there are no specific recommendations for immunosuppression. Our aim was to analyze evidence about results from HLAid living-donor recipients under different immunosuppression in the current era of immunological risk assessment. METHODS: Systematic review of studies describing associations between outcomes of HLAid living-donor KT recipients according to their immunological risk and applied immunosuppression. RESULTS: From 1351 studies, 16 (5636 KT recipients) were included in the analysis. All studies were retrospective, ten comparing immunosuppression strategies, and six immunological risk strata. Of those ten, six studies were published in 1990 or earlier and only three included tacrolimus. The evidence is poor, and the inclusion of calcineurin inhibitors does not demonstrate better results. Furthermore, only few studies describe different immunosuppression regimens according to the patient immunological risk and, in general, they do not include the assessment with new solid phase assays. CONCLUSIONS: There are no studies analyzing the association of outcomes of HLAid KT recipients with current immunological risk tools. Abstract

Invasive alien species are among the main global drivers of biodiversity loss posing major challenges to nature conservation and to managers of protected areas. The present study applied a methodological framework that combined invasive Species Distribution Models, based on propagule pressure, abiotic and biotic factors for 14 invasive alien plants of Union concern in Italy, with the local interpretable model-agnostic explanation analysis aiming to map, evaluate and analyse the risk of plant invasions across the country, inside and outside the network of protected areas. Using a hierarchical invasive Species Distribution Model, we explored the combined effect of propagule pressure, abiotic and biotic factors on shaping invasive alien plant occurrence across three biogeographic regions (Alpine, Continental, and Mediterranean) and realms (terrestrial and aquatic) in Italy. We disentangled the role of propagule pressure, abiotic and biotic factors on invasive alien plant distribution and projected invasion risk maps. We compared the risk posed by invasive alien plants inside and outside protected areas. Invasive alien plant distribution varied across biogeographic regions and realms and unevenly threatens protected areas. As an alien's occurrence and risk on a national scale are linked with abiotic factors followed by propagule pressure, their local distribution in protected areas is shaped by propagule pressure and biotic filters. The proposed modelling framework for the assessment of the risk posed by invasive alien plants across spatial scales and under different protection regimes represents an attempt to fill the gap between theory and practice in conservation planning helping to identify scale, site, and species-specific priorities of management, monitoring and control actions. Abstract

Background Rare diseases are chronic and life-threatening disorders affecting Results To the purpose of our analysis, we considered heart, lung, liver and kidney transplants included in the Transplant Registry (TR) of the Italian National Transplantation Center in the 2002–2019 timeframe. Overall, 49,404 recipients were enrolled in the cohort, 5.1% of whom in the pediatric age. For 40,909 (82.8%) transplant recipients, a disease diagnosis was available, of which 38,615 in the adult cohort, while 8,495 patients (17.2%) were undiagnosed. There were 128 disease categories, and of these, 117 were listed in the main rare disease databases. In the pediatric cohort, 2,294 (5.6%) patients had a disease diagnosis: of the 2,126 (92.7%) patients affected by a rare disease, 1,402 (61.1%) presented with a monogenic condition. As expected, the frequencies of pathologies leading to organ failure were different between the pediatric and the adult cohort. Moreover, the pediatric group was characterized, compared to the adult one, by an overall better survival of the graft at ten years after transplant, with the only exception of lung transplants. When comparing survival considering rare vs non-rare diseases or rare and monogenic vs rare non-monogenic conditions, no differences were highlighted for kidney and lung transplants, while rare diseases had a better survival in liver as opposed to heart transplants. Conclusions This work represents the first national survey analyzing the main genetic causes and frequencies of rare and/or monogenic diseases leading to organ failure and requiring transplantation both in adults and children.

Abstract

IDPlanT is the Italian Database of Plant Translocation, an initiative of the Nature Conservation Working Group of the Italian Botanical Society. IDPlanT currently includes 185 plant translocations. The establishment of a national database on plant translocation is a key step forward in data sharing and techniques improvement in this field of plant conservation. Abstract

The COVID-19 pandemic was confirmed in Italy at the end of January 2020, when the first positive cases for the virus were identified. At the beginning of March, the virus had spread to all Italian regions and on 10 March 2020 the lockdown phase began, limiting the movement of people and prohibiting almost all commercial activities, businesses and non-essential industries. As a result, millions of people were forced to stay at home, causing a drastic drop in traffic volume, which significantly changed the acoustic environment and air quality of cities. On 4 May 2020, the lockdown was partially lifted and activities were progressively reopened. Therefore, traffic gradually started to increase and, consequently, the noise emitted by motor vehicles. This behaviour was confirmed by the data collected by the DYNAMAP system, an automatic platform developed within the LIFE DYNAMAP project, providing real time traffic noise maps in terms of sound pressure levels and impacts at receivers (people and dwellings exposed to noise level bands). In this paper traffic and non-traffic-related noise events in the cities of Rome and Milan from March to May 2020 are analysed and compared to the corresponding values in 2019 to evaluate the effects of the lockdown period.

Moreover, the widespread implementation of vaccination policies before and after transplantation, and the development of new prophylactic and treatment strategies for coronavirus disease 2019 (COVID-19) over the past 12-18 months, has raised several new issues concerning kidney transplant recipients. In this special report, the ERA DESCARTES (Developing Education Science and Care for Renal Transplantation in European States) Working Group addresses several questions related to everyday clinical practice concerning kidney transplant recipients and to the assessment of deceased and live kidney donors: what is the current risk of severe disease and of breakthrough infection, the optimal management of immunosuppression in kidney transplant recipients with COVID-19, the role of passive immunization and the efficacy of antiviral drugs in ambulatory patients, the management of drug-to-drug interactions, safety criteria for the use of SARS-CoV-2-positive donors, issues related to the use of T cell depleting agents as induction treatment, and current recommendations for shielding practices.

Item does not contain fulltext, Due to the shortage of deceased and genetically- or emotionally-related living donors, living unrelated paid donor (LURpD) kidney transplantation has been considered; however, this practice may result in medical, ethical and social dilemmas, induce organ trading (commodification), and even criminal activities. Commodification also risks undermining public trust in the transplant system and impeding the development of proper altruistic or deceased donor programs by ignoring altruism, volunteerism, and dignity. However, despite many objections by authoritative organizations, black market practices are involved in up to 10% of all transplants worldwide. The authors strongly discourage any payment or rewards for organ donation, and instead urge the governments of all countries to provide adequate and accessible kidney health care. However, it is an undeniable fact that paid-living donor transplantation is increasing despite all objections, disapprovals and regulations. We feel it as our responsibility not to ignore this uncertain and undesirable practice, but rather to underline the necessity for strict rules and prohibitions to minimize unacceptable medical, social and ethical risks as long as it exists. Furthermore, economic profit, be it direct or indirect, must not be the goal of those involved, and the employment of intermediaries must be avoided entirely. Additionally, the donor should be in a position where not donating has no detrimental effect on his/her future in any way (free agency). In our view, every country has the obligation and responsibility to provide adequate kidney health care and to make kidney transplantation accessible to those in need. This provision is key to stop transplant tourism and commercialization of kidney transplantation. 26. New case of syncytial giant-cell variant of hepatocellular carcinoma in a pediatric patient with HNF1B deficiency: Does it fit with the syndrome?

Abstract

Background Hepatocyte nuclear factor 1B (HNF1B) is a member of the homeodomain-containing family of transcription factors located on 17q12. HNF1B deficiency is associated with a clinical syndrome (kidney and urogenital malformations, maturity-onset diabetes of the young, exocrine pancreatic insufficiency) and to an underdiagnosed liver involvement. Differently from HNF1A, the correlation between hepatocellular carcinoma (HCC) and germline HNF1B deficiency has been poorly evaluated. Case report Here, we report a novel case of a syndromic HNF1B-deficient paediatric patient that developed HCC with unique histopathological features characterised by neoplastic syncytial giant cells, which was observed only in one additional case of paediatric cholestatic liver disease of unknown origin. Conclusions Our case highlights the influence of HNF1B deficiency in liver disease progression and its putative association with a rare yet specific HCC histotype. We hypothesised that HCC could be secondary to the repressive effect of HNF1B variant on the HNF1A transcriptional activity.

Abstract

Italy was the first Western country to face the COVID-19 pandemic. Here we report the results of a national survey on kidney transplantation activity in February and March 2020, and the results of a three-round Delphi consensus promoted by four scientific societies: the Italian Society of Organ Transplantation, the Italian Society of Nephrology, the Italian Society of Anesthesia and Intensive Care, and the Italian Group on Antimicrobial Stewardship. All 41 Italian transplant centers were invited to express their opinion in the Delphi rounds along with a group of seven experts. The survey revealed that, starting from March 2020, there was a decline in kidney transplantation activity in Italy, especially for living-related transplants. Overall, 60 recipients tested positive for SARS-CoV2 infection, 57 required hospitalization, 17 were admitted to the ICU, and 11 died. The online consensus had high response rates at each round (95.8%, 95.8%, and 89.5%, respectively). Eventually, 27 of 31 proposed statements were approved (87.1%), 12 at the first or second round (38.7%), and 3 at the third (9.7%). Based on the Italian experience, we discuss the reasons for the changes in kidney transplantation activity during the COVID-19 pandemic in Western countries. We also provide working recommendations for the organization and management of kidney transplantation under these conditions. Abstract

Background It is unknown whether renal pathology lesions in immunoglobulin A nephropathy (IgAN) correlate with renal outcomes over decades of follow-up. Methods In 1130 patients of the original Validation Study of the Oxford Classification for IgA Nephropathy (VALIGA) cohort, we studied the relationship between the MEST score (mesangial hypercellularity, M; endocapillary hypercellularity, E; segmental glomerulosclerosis, S; tubular atrophy/interstitial fibrosis, T), crescents (C) and other histological lesions with both a combined renal endpoint [50% estimated glomerular filtration rate (eGFR) loss or kidney failure] and the rate of eGFR decline over a follow-up period extending to 35 years [median 7 years (interquartile range 4.1–10.8)]. Results In this extended analysis, M1, S1 and T1–T2 lesions as well as the whole MEST score were independently related with the combined endpoint (P Conclusion Long-term follow-up analyses of the VALIGA cohort showed that the independent relationship between kidney biopsy findings and the risk of progression towards kidney failure in IgAN remains unchanged across all age groups and decades after the renal biopsy.

Abstract

The authors briefly discuss about the list of the taxa endemic to Campania (southern Italy) and the typification of their names. The names Santolina neapolitana and Seseli polyphyllum (and other linked names) are lectotypified on herbarium specimens kept at LY and BOLO. A new combination at subspecific rank is proposed for Globularia neapolitana, validating both a previous invalid proposal and an unpublished lectotypification.

Abstract

L’Italia è localizzata al centro del bacino Mediterraneo, uno tra i centri di biodiversità a livello globale più minacciati. La sua eterogeneità ambientale e climatica ha permesso la differenziazione di un ampio numero di specie vegetali (oltre 8,200 taxa), che tuttavia sono sottoposte ad un crescente numero di minacce principalmente legate ai cambiamenti di dinamiche socio-economiche. Pertanto, urgenti misure di conservazione sono necessarie per fermare la perdita di diversità vegetale e conservare il patrimonio naturale del nostro paese. Questo volume è il prodotto finale di un progetto iniziato nel 2012, finanziato dal Ministero dell’Ambiente e della Tutela del Territorio e del Mare, portato avanti dalla Società Botanica Italiana. Complessivamente, i criteri per la redazione delle liste rosse elaborati dalla IUCN sono stati applicati a 2.488 taxa della flora vascolare autoctona del nostro paese, al fine di valutare il loro attuale stato di conservazione e mettere in evidenza le principali minacce che incombono su di esse. Dopo la pubblicazione del primo volume (2013), che include la valutazione di tutte le specie italiane incluse nelle normative ratificate a livello nazionale (policy species), quali le specie elencate negli allegati della Direttiva 92/43/UE “Habitat” e negli allegati della Convenzione di Berna, in questo contributo è stato valutato un ulteriore gruppo di 2.191 taxa, incluse tutte le specie endemiche italiane non ancora sottoposte ad assessment. I risultati pubblicati nel presente volume hanno rilevato che 37 taxa sono estinti, estinti in natura o verosimilmente estinti a livello regionale (inclusi 11 taxa endemici), mentre 420 taxa (inclusi 228 taxa endemici) sono stati assegnati ad una categoria di rischio. Inoltre, 338 taxa (inclusi 248 taxa endemici) sono stati inseriti nella categoria DD, ovvero specie per cui i dati a disposizione sono insufficienti per una valutazione. La flora vascolare italiana è minacciata soprattutto dalla modificazione degli habitat naturali e semi-naturali, dovuta al disturbo antropico e, specialmente, ad agricoltura, turismo e sviluppo residenziale. Il numero più alto di taxa estinti o in declino è infatti localizzato nelle zone costiere e di pianura, dove gli impatti antropici e la distruzione degli ecosistemi sono più evidenti. La valutazione di circa un terzo dei taxa vegetali spontanei del nostro paese costituisce un importante passo verso la conservazione della flora italiana. Tuttavia, per mettere in atto una strategia di conservazione efficace del paesaggio naturale italiano e in particolare della flora vascolare, ulteriori ricerche ed approfondimenti sono necessari. Sarebbe infatti urgente risolvere alcune criticità tassonomiche, effettuare monitoraggi sulle specie minacciate e sviluppare strategie specifiche di conservazione in situ ed ex situ e piani di azione specie specifici.

Abstract

Primary hyperoxaluria type 1 is a rare genetic disease; the onset of symptoms ranges from childhood to the sixth decade of life and the disease may go unrecognized for several years. There is an urgent need for drugs able to inhibit the liver production of oxalate and to prevent the disease progression; lumasiran, an innovative molecule based on RNAi interference, is one of the most promising drugs. A group of leading Italian experts on this disease met to respond to some unmet medical needs (early diagnosis, availability of genetic tests and dosage of plasma oxalate, timing of liver transplantation, need for etiologic treatment), based on the analysis of the main scientific evidence and their personal experience. Children showing the characteristic symptoms of the disease usually undergo a metabolic screening and obtain an early diagnosis, while the experience is very limited in adults and the diagnosis difficult. It is therefore essential to increase the knowledge around this disease and the importance of metabolic and genetic screening to define a checklist of shared clinical and laboratory criteria and to establish a multidisciplinary management of potential patients. Oxalate is the cause of the disease: it is crucial to reduce both oxaluria and oxalemia through appropriate therapeutic strategies, able to prevent and/or reduce renal and systemic complications of primary type 1 hyperoxaluria. Lumasiran allows to significantly reduce the levels of oxalate both in blood and in urine, halting the course of the disease and preventing serious renal and systemic complications, if the therapy is started at an early stage of the disease.

Abstract

Background On the basis of findings of observational studies and a meta-analysis, proteinuria reduction has been proposed as a surrogate outcome in IgA nephropathy. How long a reduction in proteinuria needs to be maintained to mitigate the long-term risk of disease progression is unknown. Methods In this retrospective multiethnic cohort of adult patients with IgA nephropathy, we defined proteinuria remission as a $25% reduction in proteinuria from the peak value after biopsy, and an absolute reduction in proteinuria to,1 g/d. The exposure of interest was the total duration of first remission, treated as a time-varying covariate using longitudinal proteinuria measurements. We used time-dependent Cox proportional hazards regression models to quantify the association between the duration of remission and the primary outcome (ESKD or a 50% reduction in eGFR). Results During a median follow-up of 3.9 years, 274 of 1864 patients (14.7%) experienced the primary outcome. The relationship between duration of proteinuria remission and outcome was nonlinear. Each 3 months in sustained remission up to approximately 4 years was associated with an additional 9% reduction in the risk of disease progression (hazard ratio [HR], 0.91; 95% confidence interval [95% CI], 0.89 to 0.93). Thereafter, each additional 3 months in remission was associated with a smaller, nonsignificant risk reduction (HR, 0.99; 95% CI, 0.96 to 1.03). These findings were robust to multivariable adjustment and consistent across clinical and histologic subgroups. Conclusions Our findings support the use of proteinuria as a surrogate outcome in IgA nephropathy, but additionally demonstrate the value of quantifying the duration of proteinuria remission when estimating the risk of hard clinical endpoints. Item does not contain fulltext, BACKGROUND: Publications from the last decade have increased knowledge regarding long-term risks after kidney donation. We wanted to perform a survey to assess how transplant professionals in Europe inform potential kidney donors regarding long-term risks. The objectives of the survey were to determine how they inform donors and to what extent, and to evaluate the degree of variation. METHODS: All transplant professionals involved in the evaluation process were considered eligible, regardless of the type of profession. The survey was dispatched as a link to a web-based survey. The subjects included questions on demographics, the information policy of the respondent and the use of risk calculators, including the difference of relative and absolute risks and how the respondents themselves understood these risks. RESULTS: The main finding was a large variation in how often different long-term risks were discussed with the potential donors, i.e. from always to never. Eighty percent of respondents stated that they always discuss the risk of end-stage renal disease, while 56% of respondents stated that they always discuss the risk of preeclampsia. Twenty percent of respondents answered correctly regarding the relationship between absolute and relative risks for rare outcomes. CONCLUSIONS: The use of written information and checklists should be encouraged. This may improve standardization regarding the information provided to potential living kidney donors in Europe. There is a need for information and education among European transplant professionals regarding long-term risks after kidney donation and how to interpret and present these risks.

Abstract

Italy has a rich natural heritage, which is dangerously under pressure. In recent years, there is an increased awareness of the crucial role of plants in ecosystem functioning and in providing ecosystem services. Consequently, an updated Red List of the Italian vascular flora was compiled in this work, at the request of the Ministry for Environment, Land and Sea Protection, with the scientific support of the Italian Botanical Society. The IUCN Red List criteria were applied to 2,430 Italian native vascular plant taxa to assess their current extinction risk and to highlight the major threats affecting the Italian flora. Our results revealed that 54 taxa (2.2% of the assessed taxa) are extinct or possibly extinct at regional level, while 590 taxa (24.3%) were assigned to a risk category. Moreover, 404 taxa (16.6%) were categorized as Data Deficient. The Italian vascular flora is primarily threatened by habitat modifications due to anthropic disturbance and, especially, to agriculture, tourism and residential development. Coastal areas and lowlands, where anthropogenic impacts and ecosystem destruction are more pronounced, host the greatest number of extinct or declining taxa. Our results represent an important baseline to establish conservation priorities, legislative choices and intervention strategies on a national scale.
