This article discusses two cases of patients with advanced neurodegenerative parkinsonism who developed repetitive movements of the upper limbs. The authors suggest that these movements demonstrate clonic perseveration, which is more pronounced than the stereotyped movements commonly seen in patients with neurodegenerative dementias. The first patient's movements were triggered by holding a newspaper, while the second patient's movements seemed to evolve from his typical parkinsonian rest tremor. The authors acknowledge that there may be differing viewpoints on the phenomenology demonstrated in the videos, but they argue that the movements are suggestive of clonic perseveration rather than stereotypies. The text explores the possibility that clonic perseveration observed in patients could be misinterpreted as re-emergent tremor or other movement disorders. It suggests that clonic perseveration may be associated with frontal-subcortical circuit malfunction and is seen in conditions such as progressive supranuclear palsy and behavioral variant frontotemporal dementia. The text emphasizes the importance of recognizing clonic perseveration to avoid misdiagnosis and inappropriate treatment. [Extracted from the article]
