Abstract

We investigate the abundance and distribution of metals in the high-redshift intergalactic medium and circum-galactic medium through the analysis of a sample of almost 600 SiIV absorption lines detected in high and intermediate resolution spectra of 147 quasars. The evolution of the number density of SiIV lines, the column density distribution function and the cosmic mass density are studied in the redshift interval 1.7 <= z <= 6.2 and for log N(SiIV) >= 12.5. All quantities show a rapid increase between z~6 and z< 5 and then an almost constant behaviour to z~2 in very good agreement with what is already observed for CIV absorption lines. The present results are challenging for numerical simulations: when simulations reproduce our SiIV results, they tend to underpredict the properties of CIV, and when the properties of CIV are reproduced, the number of strong SiIV lines (log N(SiIV) > 14) is overpredicted., Comment: 14 pages, 8 figures, accepted for publication in MNRAS

Abstract

We investigate Extremely Red Quasars (ERQs), a remarkable population of heavily-reddened quasars at redshift z$\sim$2 - 3 that might be caught during a short-lived 'blow-out' phase of quasar/galaxy evolution. We perform a near-IR observational campaign using Keck/NIRSPEC, VLT/X-shooter and Gemini/GNIRS to measure rest-frame optical spectra of 28 ERQs with median infrared luminosity $\langle$log L(erg/s)$\rangle$ $\sim$46.2. They exhibit the broadest and most blue-shifted [OIII] $\lambda$4959,5007 emission lines ever reported, with widths (w90) ranging between 2053 and 7227 km/s, and maximum outflow speeds (v98) up to 6702 km/s. ERQs on average have [OIII] outflows velocities about 3 times larger than those of luminosity-matched blue quasar samples. We show that the faster [OIII] outflows in ERQs are strongly correlated with their extreme red colors and not with radio-loudness, larger quasar luminosities, nor higher Eddington ratios. We estimate for these objects that at least 3-5 per cent of their bolometric luminosity is being converted into the kinetic power of the observed wind. Abstract

We investigate the NV absorption signal along the line of sight of background quasars, in order to test the robustness of the use of this ion as criterion to select intrinsic (i.e. physically related to the quasar host galaxy) narrow absorption lines (NALs). We build composite spectra from a sample of $\sim$ 1000 CIV absorbers, covering the redshift range 2.55 < z < 4.73, identified in 100 individual sight lines from the XQ-100 Legacy Survey. We detect a statistical significant NV absorption signal only within 5000 km s$^{-1}$ of the systemic redshift, z$\rm_{em}$. This absorption trough is $\sim$ 15$\sigma$ when only CIV systems with N(CIV) > 10$^{14}$ cm$^{-2}$ are included in the composite spectrum. This result confirms that NV offers an excellent statistical tool to identify intrinsic systems. We exploit the stacks of 11 different ions to show that the gas in proximity to a quasar exhibits a considerably different ionization state with respect to gas in the transverse direction and intervening gas at large velocity separations from the continuum source. Indeed, we find a dearth of cool gas, as traced by low-ionization species and in particular by MgII, in the proximity of the quasar. We compare our findings with the predictions given by a range of Cloudy ionization models and find that they can be naturally explained by ionization effects of the quasar., Comment: 18 pages, 13 figures, 1 table. Abstract

We describe the execution and data reduction of the European Southern Observatory Large Programme "Quasars and their absorption lines: a legacy survey of the high-redshift universe with VLT/XSHOOTER" (hereafter `XQ-100'). XQ-100 has produced and made publicly available an homogeneous and high-quality sample of echelle spectra of 100 QSOs at redshifts z~3.5-4.5 observed with full spectral coverage from 315 to 2500 nm at a resolving power ranging from R~4000 to 7000, depending on wavelength. The median signal-to-noise ratios are 33, 25 and 43, as measured at rest-frame wavelengths 1700, 3000 and 3600 Angstrom, respectively. This paper provides future users of XQ-100 data with the basic statistics of the survey, along with details of target selection, data acquisition and data reduction. The paper accompanies the public release of all data products, including 100 reduced spectra. XQ-100 is the largest spectroscopic survey to date of high-redshift QSOs with simultaneous rest-frame UV/optical coverage, and as such enables a wide range of extragalactic research, from cosmology and galaxy evolution to AGN astrophysics., Comment: In press at A&A. Abstract

Summary: Acute chest syndrome (ACS) is a frequent cause of hospitalization in sickle cell disease (SCD). Despite advances in acute care, many settings still lack knowledge about ACS best practices. After the AIEOP Guidelines were published in 2012, suggesting standardized management in Italy, a retrospective study was performed to assess the diagnostic and therapeutic pathways of ACS in children. From 2013 to 2018, 208 ACS episodes were presented by 122/583 kids in 11 centres. 73 were male, mean age 10.9 years, 85% African, 92% HbSS or Sβ°. In our hub‐and‐spoke system, a good adherence to Guidelines was documented, but discrepancies between reference centres and general hospitals were noted. Improvement is needed for timely transfer to reference centres, use of incentive spirometry, oxygen therapy and pain management. [ABSTRACT FROM AUTHOR]

Abstract

Background The aims of this study were: to investigate the capacity of the rare disease healthcare network in Campania to diagnose patients with rare diseases during the outbreak of Covid-19; and to shed light on problematic diagnoses during this period. Methods To describe the impact of the Covid-19 pandemic on the diagnosis of patients with rare diseases, a retrospective analysis of the Campania Region Rare Disease Registry was performed. A tailored questionnaire was sent to rare disease experts to investigate major issues during the emergency period. Results Prevalence of new diagnoses of rare disease in March and April 2020 was significantly lower than in 2019 (117 versus 317, P Conclusions This study describes the effects of the Covid-19 outbreak on the diagnosis of rare disease in a single Italian region and investigates potential issues of diagnosis and management during this period.

Abstract

Proper selection of the quasi-free (QF) break-up channel in a three-body reaction is a key aspect for the applicability of the Trojan Horse Method (THM). The Treiman-Yang (TY) Criterion is a model-independent experimental test for the dominance of the QF mechanism, and hence constitutes one of the strongest validity tests of the THM. An experiment was performed at LNS to apply the test to the d(10B, 7Be α)n reaction. Here, the criterion is described and some preliminary data from the experiment are shown.

Abstract

Pulse oximetry is a simple, safe and inexpensive assessment widely used for the diagnosis of cardio-respiratory disorders. However, some rare diseases cause low oxygen saturation at the pulse oximetry in the absence of cardiac or pulmonary dysfunction. The case described outlines the need to improve knowledge and take in consideration the rare causes of impaired pulse oximetry in order to avoid expensive and invasive procedures and ensure the best diagnostic approach. The case described is one of the most serious. Hence the need to know and take into account the rare causes of impaired saturimetry in order to avoid very expensive and invasive instrumental investigations and ensure the best diagnostic and therapeutic approach to these children.

Abstract

Objectives: HbS/β+ patients’ presence in Italy increased due to immigration; these patients are clinically heterogeneous, and specific guidelines are lacking. Our aim is to describe a cohort of HbS/β+ patients, with genotype-phenotype correlation, in order to offer guidance for clinical management of such patients. Methods: Retrospective cohort study of HbS/β+ patients among 15 AIEOP Centres. Results: A total of 41 molecularly confirmed S/β+ patients were enrolled (1-55years, median 10.9) and classified on β+ mutation: IVS-I-110, IVS-I-6, promoter, and “others.” Prediagnostic events included VOC 16/41 (39%), ACS 6/41 (14.6%), sepsis 3/41 (3.7%), and avascular necrosis 3/41 (7,3%). Postdiagnostic events were VOC 22/41 (53.6% %), sepsis 4/41 (9.7%), ACS 4/41 (9.7%), avascular necrosis 3/41 (7.3%), aplastic crisis 2/41 (4.8%), stroke 1/41 (2.4%), ACS 1/41 (2.4%), and skin ulcerations 1/41 (2.4%). The IVS-I-110 group presented the lowest median age at first SCD-related event (P=.02 vs promoter group) and the higher median number of severe events/year (0.26 events/patient/year) (P=.01 vs IVS-I-6 and promoter groups). Promoter group presented a specific skeletal phenotype. Treatment regimen applied was variable among the centers. Conclusions: HbS/β+ is not always a mild disease. Patients with IVS-I-110 mutation could benefit from a standard of care like SS and S/β° patients. Standardization of treatment is needed.

Abstract

Takenouchi-Kosaki syndrome (TKS) is a recently delineated syndromic form of thrombocytopenia strictly related to an hot-spot missense variant, p.Tyr64Cys, in CDC42 (Cell Division Control protein 42). Herein we report an additional patient with the p.Tyr64Cys aminoacidic substitution who showed the well-defined phenotypical TKS features and an intermittent, very mild, macrothrombocytopenia at 10.7 years of age (93,000/mL), that was only retrospectively valorized. Outside of this value the PLT count had always been higher than 100,000/mL. We also review literature data from patients carrying this recurrent variant. Our female patient presented with prenatal onset of short stature and microcephaly, camptodactyly, heart defects, typical facial gestalt, developmental delay, and not specific brain abnormalities. After several genetic investigations (karyotype, CGH-Array, targeted NGS analysis for short stature genes), by whole exome sequencing we identified the p.Tyr64Cys in CDC42, occurring de novo. The case presented here provides further evidence that macrothrombocytopenia can be intermittent and thus it might escape attention of clinicians. Without this key feature, TKS clinical presentation can overlap other syndromic forms of short stature. Immunodeficiency, autoimmunity, and malignancies were recently reported in patients with the p.Tyr64Cys substitution, making imperative an early diagnosis of Takenouchi-Kosaki syndrome to organize the most proper follow-up of these pediatric patients. The whole exome sequencing can be a solving tool in the challenge to the rare diseases.

Abstract

We investigate the abundance and distribution of metals in the high-redshift intergalactic medium and circum-galactic medium through the analysis of a sample of almost 600 Si  iv absorption lines detected in high- and intermediate-resolution spectra of 147 quasars. The evolution of the number density of Si  iv lines, the column density distribution function, and the cosmic mass density are studied in the redshift interval 1.7 ≲ z ≲ 6.2 and for log  N (Si  iv) ≥ 12.5. All quantities show a rapid increase between z ∼ 6 and z ≲ 5 and then an almost constant behaviour to z ∼ 2 in very good agreement with what is already observed for C  iv absorption lines. The present results are challenging for numerical simulations: When simulations reproduce our Si  iv results, they tend to underpredict the properties of C  iv , and when the properties of C  iv are reproduced, the number of strong Si  iv lines [log  N (Si  iv) > 14] is overpredicted. Abstract

BACKGROUND Patients with transfusion-dependent β-thalassemia need regular red-cell transfusions. Luspatercept, a recombinant fusion protein that binds to select transforming growth factor β superfamily ligands, may enhance erythroid maturation and reduce the transfusion burden (the total number of red-cell units transfused) in such patients. METHODS In this randomized, double-blind, phase 3 trial, we assigned, in a 2:1 ratio, adults with transfusion-dependent β-thalassemia to receive best supportive care plus luspatercept (at a dose of 1.00 to 1.25 mg per kilogram of body weight) or placebo for at least 48 weeks. The primary end point was the percentage of patients who had a reduction in the transfusion burden of at least 33% from baseline during weeks 13 through 24 plus a reduction of at least 2 red-cell units over this 12-week interval. Other efficacy end points included reductions in the transfusion burden during any 12-week interval and results of iron studies. RESULTS A total of 224 patients were assigned to the luspatercept group and 112 to the placebo group. Luspatercept or placebo was administered for a median of approximately 64 weeks in both groups.
